The Centers for Disease Control and Prevention intends to award on a sole source basis a purchase order to the American Pharmacists Association Foundation, 2215 Constitution Avenue, Washington DC 20037-2907.
The purchase order is for the purpose to identify and engage a consortium of key stakeholders who have knowledge of, experience in, or can facilitate the adoption and implementation of collaborative medication therapy management policy. The consortium shall also be engaged in strategic outreach to a range of policy makers to educate them on the collaborative model. The expected outcomes of this project are establishment of a consortium and development of a model collaborative medication therapy management policy and associated translational tools. The American Pharmacists Association Foundation has access to the resources including online resource library for medication therapy management and toolkit.
The North American Classification System Code is 813211. This requirement is not set aside for small businesses. It is being procured under the simplified acquisition procedures, found in the Federal Acquisition Regulations (FAR) Part 13. The authority for this sole source is FAR 6-302-1- Only one responsible source and no other supplies or services will satisfy agency requirements. The period of performance shall continue for 12 months after award date.
This notice is not a request for competitive proposals and no solicitation is available. However, firms that believe they can provide all items listed for procurement need to submit a written notification of interest and capability to the Contracting Specialist within 15 days from the publication date of this notice. A determination by the Government not to compete this proposed action based on responses to this notice is solely within the discretion of the Government. Information received will be considered solely for the purpose of determining whether or not to conduct a future competitive procurement.
