DESCRIPTION: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Proposals are being requested and a written solicitation will not be issued. Solicitation
VA-250-11-RP-0111 is issued as a Request for Proposal (RFP). The solicitation document and incorporated provisions, clauses are those in effect through Federal Acquisitions Circular 05-50.This synopsis is a streamlined solicitation of offers for commercial items for use where appropriate. The intent of these is to simplify the process of preparing and issuing solicitations/synopsis for evaluating offers for commercial items/services consistent with commercial practices. Only written requests received directly from the requester are acceptable.
The Louis Stokes Cleveland Department VA Medical Center known as LSCDVAMC, Wade Park Campus, 10701 East Boulevard Cleveland OH 44106; proposes to contract for Professional Art Therapy services for VA Spinal Cord Injury patients who statues are either in-patient or out-patient. The Art Therapist must have a Master's Degree in Art Therapy. It is highly desired, but not mandatory for the Art Therapist to have Art Therapy credentialing through the Art therapy Credentialing Board.
Performance period is Date of Award through September 30 2011, with four (4) one-year options. NAICS code assigned to this procurement is 624190 (other individual and family services) with a size standard of $7 million.
Offerors should submit proposals based on the work requirements specified herein. Service Disabled Veteran Owned small businesses are encouraged to contact their nearest Procurement Technical Assistance Center (PTAC) if assistance is required in preparing a proposal. Offerors shall thoroughly review the specifications and include a delivery time for specified requirements. Offerors responding to this synopsis shall also provide information included in the Federal Acquisition Regulation (Far 52.212.2 Evaluation - Commercial Items) NOTE: The Government reserves the right to award at BEST VALUE. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer; conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Please note; the following factors used to evaluate offers:
Technical capability of the services offered to meet the Government requirement, Price
Past performance, the relative order of importance of the evaluation factors are in ascending order of importance. Technical Capability and Past Performance, when combined, are greater than Price. Evaluations may be made without discussion with the offeror(s). Federal Acquisition Regulations require that all contractors submitting offers/bids to obtain contracts with agencies of the Federal Government; including the Department of Veterans Affairs, shall be registered in the Central Contractor Registration (CCR) database prior to receiving an award, during performance and through final payment of any contract or award resulting from a solicitation.
