We have received clarification on the A+ and Security+.
The requirement is for one position to hold an A+ certification and one position to hold a Security + certification.
The network IT certified position requires a Security + certification.
The helpdesk technician and managing work orders, requires an A+ certification.
Sorry for the confusion.

There are no page limitations/requirements for the submitted technical proposal
Is a security clearance required? must be able to pass the security check for base access and be a US citizen.
Past performance information that was submitted in the first amendment: Submit past performance information regarding contract work with government (Federal and State) entities for the past two years, then non-governmental entities for the past two years. This information should include organization, how long and POC contact information.
Please clarify the evaluation criteria: IAW FAR 52.212-2 Evaluation of the solicitation, last two sentences. Award shall be based on All or None and Best Value to the Government. Best value can include but is not limited to price, delivery date, and technical capability.
We plan to make award as close to 1 Oct 2013 as possible start date may slip to a later date depending on the outcome of the evaluations and start date.
Any format to email responses such as MS word or PDF versions is acceptable
Cut off dates for questions (email and phone calls) shall be 1200 MT 20 September 2013.
It is acceptable to have two people with Security + certification.

FA4855-13-T-0012 Amendment #3
In response to questions submitted:
1. This is a Firm Fixed Price contract requirement.
2. Proposals do not have to be submitted in two volumes.
3. The Basic Life Support certification is required of all who will be working in the Medical Treatment Facility.
4. Amending FAR Clause 52.217-8 to add: "As part of the price evaluation, the Government will evaluate the Option to Extend Services under FAR Clause 52.217-8 by adding one-half of the offeror's final option period prices to the offeror's total price. Thus, the offeror's total price for the purpose of evaluation will include the base period and all option periods. Offerors are required only to price the base and option CLINs. Offerors shall not submit a price for the potential one-half extension of services period."
5. In para 1.1. The sentence reads. "The contractor Personnel shall perform services and support compatible with the medical facilities operating capacity and equipment within a fifty (50) mile radius.
The support is only on base, the med group and the health and wellness center.
6. In para 1.3.5.2.4. What clinical licenses and certifications is this requirement referring to?
It is a DOD Reg 8570 requires Security + certification for IT staff working on the network and other certification for helpdesk that is required, is A+. However, we need the contractor to ensure their IT staff/contractor be hired with certification and training before start date at MTF.
7. In para 1.3.5.1.1. Personnel shall have three (3) years experience within the last five (5) years. Includes one year of specialized experience related to Military Treatment Facility information systems Help Desk Operations.
What is meant: "Personnel shall have three years IT and/or LAN Network work experience within the past five years. Preferred one year experience related to the MTF Info systems help desk operations.

Amendment to Solicitation FA4855-13-T-0012 #2
1. Resumes are recommended to submit qualifications of the contractor/contractor employees. Under paragraph 1.3.5 of the SOW, resume requirements are implied.
2. This is a new requirement and are no incumbents.
3. This requirement will be funded with FY14 funds.
4. The Combo Synopsis/Solicitation includes the option years with the base year and indicates a requirement of hourly rate per hour. Please submit quotes as $ XX.XX per hour X 1920 hours x 2 FTEs.
5. Reference paragraph 1.3.2.3. The contractor shall ensure that scheduled and unscheduled absences do not interrupt service performance. It will be assumed that during scheduled absences one FTE will cover the workload.
6. A Standard Form 1449 will not be posted.

In response to some questions regarding past performance:

There are no actual security clearance requirements for this contract other than Contractor Employees must be able to receive access to the installation as referenced in AFFARS 5352.242-9000 "Contractor Access to Air Force Installations" as well as comply with paragraph section 1.3.4. Conduct Requirements.

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-69, Defense Federal Acquisition Regulation Supplement Change Notice (DPN) 20130808, and Air Force Acquisition Circular (AFAC) 2013-0827. This announcement constitutes the only solicitation. Quotations are being requested and a written solicitation will not be issued. The solicitation number for this procurement is FA4855-13-T-0012 and is a Request For Quotation (RFQ). Submit written offers only, oral offers will not be accepted.
