SCOPE OF WORK

Zuni-Ramah Service Unit – Dental Program

Dental Laboratory Services

In an effort to provide the highest quality of care to our patients, the Zuni Dental Department requires the use of many dental laboratories to fabricate certain aspects of basic dental procedures including but not limited to crowns, bridges and removable dentures. These are essential aspects of all dental operations and without which no dental office will be functional.

A. General requirements:

As a minimum such dental laboratories shall provide the following:

1. Fabrication of all needed dental devices requested by the Zuni Dental Clinic.

2. Timely and accurate completion of all required dental prostheses ordered by the Zuni

Dental Clinic.

3. Communicate regularly with the dental staff to ensure accuracy of prosthetic devices.

B. Responsibility Factors:

1. The Dental department at the Zuni Indian Health Service Hospital shall be responsible for ordering and setting all specifications regarding dental prostheses fabricated at assigned dental laboratories.

2. The department shall also be responsible for ensuring proper storage and distribution of all completed dental prosthesis.

C. Place of Performance:

All dental laboratory prostheses shall be distributed to patients at the Zuni I.H.S. Hospital dental clinic.

E. Reporting Requirements:

Eligibility and distribution management shall be conducted by the dental staff of the Zuni Dental Clinic. The functioning and adjustment of dental prostheses shall be managed and monitored by clinic staff.

F. Government’s Responsibility:

The government shall bear reasonable cost of all prostheses ordered on behalf of patients.

G. Evaluation:

The evaluation of quality and quantity of dental laboratory performances will be conducted by the Chief Dental Officer in conjunction with the clinic dentists as well as the auxiliary staff.

H. Quality Assurance:

The quality of all dental laboratory prostheses will be monitored by the Chief Dental Officer in conjunction with the clinic dentists.

I. List of All Fabrications Lab is Qualified to do.

This list at a minimum should include but is not limited to:

Flippers(single and multiple teeth), Acrylic Partial dentures, Partial Frame Works ( to include but not limited to completion, any attachments, acrylic clasps, set ups, and processing), Complete Dentures (to include but not limited to base plate with wax rings, pour up and mounts, processing to finish, wax ups, reset, set ups for interim/immediate/Full dentures, ) , Surgical guides (with CAD/CAM capability), space maintainers, custom trays, bleaching trays, Mouth guards both soft and hard, crown and bridge ( this includes but not limited and must show/list all types crowns/bridges including all types of metals, porcelains), Diagnostic wax ups, temporary crowns, Implants (what systems they can and cannot restore and certifications of use of requested materials for restoration of implants), Lab denture relines, Lab denture solders, Addition of tooth to existing fabricated dentures (acrylic, metal, full and partial dentures), Rebasing. (ANY AND ALL

PROCEDURES LAB HAS ABILITY TO DO).

J. Turn Around Times List of all lab fabrication turn around times for all procedures lab does.

Must certify that all requested services have not been subcontracted to other facilities for fabrication.

K. List of all fee schedules.

List should include fee schedule for all fabrications List should include materials used for fabrications. (examples: gold pricing for PFM or FCC /Denture teeth prices).

List should also include “rush” order services

L. Redo / Refabrication Policy Should include but not limited to policies on redo/refabrication of crowns/bridges/dentures, possible shade changing of dentures/crowns etc. etc.

M. Cad/Cam ability Lab should have CAD/CAM software ability to fabricate all fixed and removable prosthodontics and dental devices as needed.

N. Certification of Materials Lab must be able to certify all materials used to fabricate a fixed or removable prosthesis( including but not limited implant prosthesis) are of materials requested by Dental Practitioner and verify no substitute materials were used.

O. Verification of ability to service case work from 4 to 6 new dentist in timely manor.
