. 1 Statement of Work

ASPHALT REPAIR

64 NORTH AND 60 SOUTH PARKING LOT

I. DESCRIPTION OF THE WORK

A. Work Site Locations

U.S. Census Bureau 1201 East Tenth Street Jeffersonville, Indiana 47132 North Parking Area near Bldg. 64 E-bay on the North side of 12th street.

and South Parking Area near Bldg. 60 – 3 areas of damaged asphalt.

B. General Scope

1. This specification outlines the minimum performance level for the work outlined below and demands the use of the best industry and trade practices in all phases of the work.

2. It is strongly recommended that any prospective bidder become thoroughly acquainted with this document, the associated drawings and specifications, and make an in person site visit with a Census representative, in order to gain a complete understanding of the scope of work.

3. Problems To Be Corrected: Repair an area of approximately, 50 foot by 15 foot, 750 Square Feet (SF), of badly deteriorated asphalt in the 64 North Parking Lot opposite bay 64-E (across 12 th street). Repair three separate but closely spaced areas of totaling approximately 245 square yards (2,202 SF) in the parking lot on the south side of building 60.

4. Corrective Steps Required (listed below):

The Contractor Shall:

a) Contact Contracting Officer Representative (COR) before beginning any work. The COR is Marty Dailey, 812-218-4044, email marty.l.dailey@census.gov.

b) Contact the COR in the event an unanticipated situation arises that would materially impact the successful completion of the contract, add to the costs, damage government property, endanger safety, or compromise quality.

c) Field verify all dimensions and measurements prior to bidding and again before starting work.

These areas will be clearly marked by the Census Bureau prior to beginning any work.

d) Perform utility location to insure all gas, electric, water, sanitary sewer, storm sewer, telecommunications lines are marked prior to beginning work.

e) Insure asphalt is NOT to be placed over manholes, handholes, water valves, survey monuments, etcetera during repairs.

f) Insure repairs do NOT compromise drainage.

g) Repair an area measuring 50’ x 15’ (750 SF) of damaged asphalt as shown on the attached drawing (DWG-1) for 64 North.

h) Repair 3 areas measuring at 60 South:

i. 34’ x 13’ (442 SF) of damaged asphalt labeled area E on the attached drawing (DWG-1).

ii. 37’ x 17’ (629 SF) of damaged asphalt labeled area H on the attached drawing (DWG-1).

iii. 87’ x 13’ (1,131 SF) of damaged asphalt labeled area K on the attached drawing.

i)

j) Repairs must made in accordance with the standard specifications, methods, and materials described in the Indiana Department of Transportation (INDOT) following documents titled (incorporated by reference):

i.i. 2013 Indiana Design Manual, Chapter 304 Comprehensive Pavement Analyses, as amended/updated for Local Public Agency (LPA) paving project pavement designs on low volume roads (<= 50 trucks AADTT) as shown on page 19, paragraph 2 and the associated referenced figure 304-21F.

i.ii. INDOT standard specifications, section 200, 300, and 400.

k) Excavate damaged asphalt and enough sub-grade material necessary to install the following constructed as follows:

i.i. 165lb/yd2 QCQA HMA, 2, 64, Surface, 9.5 mm (0.38 inch) on i.ii. 275 lb/yd2 QC/QA HMA, 2, 64, Intermediate, 19.0 mm (0.75 inch) on, i.iii. 152.4mm (6 inches) Compacted aggregate No. 53 Base with lime, on i.iv. Subgrade Treatment, Type III or IIIA, or as specified in the Geotechnical Report.

l) Properly recycle, reuse, or dispose of excavated asphalt or subgrade materials.

m) Saw-cut existing asphalt edges to create straight edges and 90-degree corners.

n) Add supplemental subgrade treatment material described in 4c(iv) above, as needed, to insure effective repair.

o) Dampen stone and compact.

p) Install and compact aggregate course material as described in 4c(iii) with compactor and roller.

q) Install and roll intermediate HMA course as described in 4c(ii).

r) Install and roll surface HMA course as described in 4c(i) to insure against water ponding.

s) Seal and roll around asphalt edges as described in figure 304-21F.

C. Contractor Shall Provide

1. The Contractor shall provide all the necessary product compliance submittals, project management, supervision, site superintendence, work schedule coordination with the government, labor, equipment, tools, materials, barricades and caution devices, site restoration where contractor has caused blemishes and/or damage, total clean-up, and other related documents, and all other items and procedures necessary to successfully accomplish the work indicated and

specified in this document.

D. Materials & Products

1. The Contractor is responsible for purchasing all new (unused) materials and products, their delivery to the sites, and their installation as indicated and specified in this document. New (unused) products and materials are those made of either virgin materials or recycled materials as defined in CFR 40, Part 247. Materials must meet the INDOT standard specifications referred to in section 4 above.

E. Contractor Work Hours

1. This work shall be performed during the Census Bureau’s normal hours of operation, which are Monday through Friday, 7:00 AM through 4:30 PM.

2. The work shall be closely scheduled and coordinated with the Government in order to not interfere with ongoing operations at this facility.

F. NOTES TO CONTRACTORS & PROSPECTIVE BIDDERS

1. The Contractor shall field verify all measurements, dimensions and quantities, and throughly investigate site conditions for the purpose of bid preparation and potential installation of this work prior to formulating and submitting a bid, and more importantly, prior to commencement of any of this work.

G. Site Visit

1. It is strongly encouraged the prospective bidder attends the site visit to confirm measurements, dimensions, and quantities; to personally inspect the site conditions prior to formulating and submitting a bid.

2. The Government shall furnish a technical representative who is familiar with this work to accompany interested parties to the job sites. The site visit is scheduled on 9 A.M. EST on 11 Dec

15. The site visit is limited to two individuals per company. All company’s wishing to attend the site visit must notify Tina Jorae, email tina.m.jorae@census.gov, phone (812)218-3575, with the names of the individuals attending the site visit. These names must be submitted by 9 A.M. EST on 10 Dec 15.

II. PROJECT COORDINATION WITH GOVERNMENT

A. All communication of a technical (specifications) or project coordination (scheduling, phasing, etc.)

related nature shall be conducted primarily through the Contracting Officer’s Representative (COR) and at times through the Contract Specialist.

B. All communications of an administrative or contractual related nature shall be conducted through the Contract Specialist for the Contract Officer (CO).

C. The Contractor is advised that the work under this contract shall be performed in close proximity to fully occupied and operational Government buildings, parking lots, and streets. The work shall be closely staged, phased, and coordinated with the COR in order to avoid interruptions of ongoing Government operations.

D. Work Site Superintendent

1. In addition to the administrative support, supervisory personnel, and general work force required for performance of the work, the contractor shall provide an on site general superintendent.

2. The superintendent shall be a responsible person who has the authority of the contractor to communicate with the COR, the Contract Specialist or the CO; and speak and act on behalf of the contractor.

3. The superintendent shall have sufficient knowledge of all crafts and trades associated with this work to the extent that this knowledge affords him the ability to inspect, interface, and direct the work being performed by the prime Contractor as well as all sub-contractors, for the purpose of assuring contract compliance as well as non-interference with ongoing Government operations.

4. The site superintendent shall be on site at all times in which work is being performed under this contract.

5. Prior to commencement of work, each day that work is performed, the Contractor’s on-site project superintendent shall inform the COR by means of telephone or an in-person visit (as directed by the COR) of the number and location of contract workers for that day. The superintendent shall be prepared to describe the work to be performed for that day and the number of workers.

6. At the end of each day, each day that work is performed, the Contractor’s superintendent shall inform the COR by means of telephone or an in-person visit (as directed by the COR) of the departure of all contract workers.

7. It shall be the responsibility of the superintendent to maintain at the work site at all times that work is being performed a clean, legible copy of this document, which includes all written specifications, all drawings and all modifications that have been made part of this document, as well as all Government approved compliance submittals.

8. The site superintendent shall also be responsible for posting at the job site, in unobstructed view for all contracted workers to see, the Wage Schedule which is contained in this document.

E. Any utility interruptions must be coordinated and scheduled with the Government’s COR.

F. If unanticipated elements which conflict with intended function or design are encountered, investigate to determine the nature and extent of the conflict. Pending receipt of directive from the COR, rearrange selective demolition and/or installation schedule as necessary to continue overall work progress without delay.

III. COMPLIANCE SUBMITTALS

A. Compliance submittals include, but not necessarily limited to, shop drawings, product data, safety data sheets (SDS), and samples which are prepared by the contractor, subcontractor, manufacturer, or supplier and submitted by the prime contractor to the COR.

B. Submittal documents shall be clearly legible. Illegible documents will be returned without review to the Contractor for re-submittal.

C. Quality Requirements

1. Minimal requirement is three (3) copies of each submittal.

2. One copy of approved submittal shall be maintained by the Government.

3. Two (2) copies of approved submittal shall be returned to the Contractor by the Government. The

Contractor shall maintain one (1) copy of each submittal on the job site at all times work is being performed. One (1) copy of approved submittal is for the Contractor’s own use.

4. If the Contractor has need of more than the one (1) copy of approved submittals it will maintain, the Contractor shall submit additional sets of submittals as required.

D. Compliance Submittal Action Stamp/Notation: The COR’s review action stamp and/or notation will appear on all compliance submittals returned to the Contractor. The COR shall notify the Contractor of review status designations within seven (7) calendar days of receipt of the submittals. The contractor shall take action as noted on submittals. If submittals are to be resubmitted, the contractor shall accomplish this within seven (7) calendar days.

E. The Contractor shall not install any materials or products without the prior compliance submittal approval.

F. The Contractor shall provide to the COR compliance submittals for all products and materials indicated and specified in this solicitation document and well as the Safety Data Sheet (SDS) on all products used on this job. Among the items for which submittals are required are as follows:

a. Asphalt mixes, i.e., HMA, #73 aggregate – include receipt from asphalt plant site.

b. Subgrade materials

c. Dilutants

IV. ACCESS TO WORK SITE & USE OF PREMISES

A. Access To The Work Site

1. No contracted worker shall be allowed to start work prior to receiving a security clearance. The Prime Contractor shall be responsible for making required access arrangements for his work force which includes all sub-contractors, suppliers, and delivery personnel.

2. The procedures will be elaborated upon by the government representatives at a pre-award conference/pre-work conference (to be scheduled at a later date) that the prime contractor shall follow to register his work force and obtain security clearance, security ID badges, and parking permits. In the mean time, a general description of this process is listed below.

3. To begin the security clearance process, all contracted individuals shall be required to fill out various forms and have fingerprints taken for the purpose of an official Government security background check. This initial stage typically takes up to two (2) hours (typically, this time is governed by how efficient the individual is in filling out the forms and the work load of the security office). The results of the background check typically takes from two (2) hours to two (2) weeks to get a status determination by the Census security office. After an individual has received a “favorable” background check status, the individual will be given three (3) simplistic tests relating to Security, will be “sworn in”, and will then be given a security ID badge. This security ID badge will permit the contracted individual to work on the facility without the need for a Government escort. The second stage of this process (test, swearing in, receiving badge) can be taken care of on the first morning of work just prior to going to the job site.

4. The Prime Contractor shall insure contracted individuals commence the security clearance procedures as soon as there is a signed contract.

5. SPECIAL NOTE In order to maintain the integrity of the Census Bureau’s Contractor Badge Security System, the contractor ID badges must be constantly accounted for by the contractor. Failure of the contractor to account for and return ALL ID badges to security at the end of the job may result in the Contractor being assessed the amount of $1,500.00 per badge not returned to Security. A good way to handle this is to have individuals turn in badges to Security at the end of each work day and pick up said badges at the beginning of the next work day.

6. EVERYONE shall wear their Government issued Security ID badge all day, every day while on Government property. Badges shall be worn ABOVE the waist. For example, the badges may be clipped to one’s shirt collar or shirt pocket.

7. Security personnel have the right to inspect parcels, packages, personal vehicles, and contractor vehicles etc., and may do so on occasion to help protect Government property, Government and Contractor employees, as well as personal belongings.

B. Use Of The Premises

1. The Contractor will ensure that he and all his employees as well as all sub-contractor employees become familiar with the information contained on Form DP-1050 General Instructions for Contractors At The Census Bureau. This information contains necessary information about the following subjects: (1) Parking of private vehicles, (2) Use Of The Site, (3) Camera/Picture Taking,

(4) Creating Dust, Etc., (5) Emergency Exits, (6) Audible Alarms, (7) Working Past 5:30 PM, Or Any Time Saturday or Sunday, (8) Badge Return, (9) Services For Contractors Involved In An Accident/Emergency. Each individual receiving a security ID badge will receive a copy of this form.

Form DP 1050 is made a part of this contact by reference.

2. The prime contractor, all sub-contractors, suppliers, and delivery personnel shall limit use of the premises to the immediate area of the work site/s indicated in this document, so as to allow for undisturbed Government occupancy and use at all times.

3. The contractor shall insure that portions of the facility beyond the areas in which work is specified or indicated are to not be entered and are to remain undisturbed.

4. Under no circumstances will a contracted worker be permitted to stray away from the immediate work site area into an area not affected by this work.

5. The Contractor shall keep all streets, driveways, parking areas, walkways, isles, entrances, and exits serving the premises clear and open for Government use (as agreed upon with the Government).

Do not use these areas for parking of vehicles or storing of materials unless prior arrangements for such use has been made with the COR.

6. Confine stockpiling of materials to locations approved by the COR. Materials storage space on Government property is limited. If available, some storage space may be provided. If additional storage is necessary beyond that which is available on Government property, obtain and pay for such storage off of Government property at the Contractor’s expense.

7. Lock automotive type vehicles, such as passenger cars and trucks and other mechanized or motorized construction equipment when parked and unattended, so as to prevent unauthorized use. Do not leave such vehicles or equipment unattended with the motor running or the ignition key in place.

C. Electric & Water Service

1. Electric The premises are supplied with electrical service which may be used in this work.

a. Typically, electrical service that is available will be from a standard 20-amp, 3-wire receptacle.

The Contractor shall be responsible for providing electrical cords and connection devices that comply with the National Electric Code.

b. If an electric supply is required by the Contractor other than what is available from the Government, the Contractor shall supply a COR approved generating unit.

2. Water The premises are supplied with water.

a. Typically, the water supply that is available will be from a standard hose bib.

b. Fire hydrants may be used if a hose bib is not available, although, fire hydrants may not be used during months when the temperature may drop below freezing.

c. In either case, the Contractor shall be responsible for providing connection devices and hoses.

d. When a hose bib or a fire hydrant are not available, the Contractor shall provide any water that may be required for this work.

D. Construction Debris

1. Demolished materials and installation debris resulting from this work shall be considered to be the property of the Contractor.

2. The Contractor shall, on a daily basis, remove from the work site all demolition and construction debris. One method of satisfying this requirement is to store debris in a Contractor provided (at Contractor’s expense) dumpster located on site.

3. The trash cans and trash receptacles which are located within the buildings and on the docks/ramps and the dumpsters which are located on Government property shall not be utilized by the Contractor.

V. SAFETY & HEALTH

A. The publications listed below form part of this specification to the extend referenced.

1. OSHA General Industry Safety and Health Standard (29 CFR 1910), Publication V2206; OSHA Construction Industry Standard (29 CFR 1926). One source of these regulations is OSHA Publication 2207, which includes a combination of both parts 1910 and 1926 as they relate to construction safety and health.

VI. PROTECTION OF GOVERNMENT PROPERTY

A. The Contractor shall provide protections, as deemed necessary by the Contractor and more so, as deemed necessary by the COR, for the buildings, the building’s occupants and contents, vehicles parked adjacent to the buildings, the grounds surrounding the buildings, and pedestrians. This may include, but is not limited to, the Contractor providing and setting in place such items as barracades, yellow caution tape, and orange traffic cones. If the Contractor is requested by the COR to set such

items in place, the contractor shall immediately take and all steps as are necessary to comply with this request.

B. The Contractor shall provide barriers to prevent any and all seal coat over spray from such items as concrete curbs, concrete side walks, wheel stops, landscaping, grassed areas, vehicles, mechanical equipment such as A/C units, etc.

C. Upon completion of the work, or as directed by the COR, the Contractor shall remove all “caution” and protection items.

D. The Contractor shall obtain a Burn Permit prior to brazing, sweating, welding or any use of open flame, ow when sparks are likely to be produced while on Government property. A Burn Permit may be obtained from the Facilities Management Office, Bldg. 66, Rm. 156. The COR will assist the Site Superintendent in obtaining a Burn Permit.

VII. QUALITY ASSURANCE

A. Maintain quality control over suppliers, manufacturers, services, site conditions, and workmanship to produce work of indicated and specified quality, and in any event, work conforming to the Best Standards of the related industry or trade.

B. Comply in full with industry standards, codes, and regulations as they apply to this work, except when more restrictive tolerances or specified requirements indicate more rigid Standards or more precise workmanship. In such case, comply with the more stringent.

C. In instances where size, tolerance, material, method, and/or procedure are not specified nor indicated, it shall be the Contractor’s responsibility to comply with the Best Standard of the related industry and/or trade.

D. All work shall be accomplished in a workmanlike manner to the satisfaction and approval of the Contracting Officer (CO) and the Contracting Officer’s Technical Representative (COR).

VIII. WARRANTY

A. The Contractor shall guarantee all Contractor provided products, materials, installation, and workmanship for a minimum period of one (1) full year from the time of Government acceptance of the successful completion of the work.
