STATEMENT OF WORK
1. GENERAL INFORMATION
Perform required monthly testing and provide Flaming Gorge Field Division (FGFD) thorough information concerning wastewater for the purposes of meeting reporting requirements under the Utah Department of Environmental Quality
(UDEQ) discharge permit, UPDES-UT0020338.

SCOPE OF WORK. The Contractor shall provide all personnel, equipment, tools, materials, supervision and other items and services necessary to perform analysis of wastewater as described in this statement of work.
Samples of water will come from Dutch John, UT once per month. The Contractor shall perform to the standards in this contract. The sampling frequencies and quantities of work are listed in Exhibit 1.
PERIOD OF PERFORMANCE: Base plus four option years starting on August 1, 2022 and ending on July 31, 2027.
CONTRACTOR AND LABORATORY STANDARDS
1.3.1 Contractor’s employees performing analysis must have current and valid certifications in the analysis methods required under this contract. Proof of these certifications shall be submitted to the Contracting Officer prior to starting work under this contract.
1.3.2 Laboratory shall be accredited in accordance with the most recent National Environmental Laboratory
Accreditation Conference (NELAC) standards for water analysis and certified by the state of Utah. The laboratory must be in good standing with both, and not be under review or investigation. The Contractor must be participating in the EPA
Quality Assurance Program. The Contractor will submit proof of laboratory certification and applicable QA sample results prior to starting work under this contract.
2. GOVERNMENT–FURNISHED PROPERTY AND SERVICES
2.1 The government will obtain samples of the water from our wastewater system using containers and procedures provided by the Contractor.
2.2 The government will ship the wastewater samples to the Contractor using the chain of custody protocol and forms provided by the Contractor. Because of our remote location, we have found that Express Overnight shipping through the
USPS has been the only way to get samples to Salt Lake within the 28 hour timeframe required for some of the testing.
3. CONTRACTOR-FURNISHED ITEMS AND SERVICES
3.1 The Contractor shall provide clean sample containers (e.g., free of debris and residue, acid-washed, etc.) and arrange delivery of these containers via courier. Sample containers will contain applicable preservatives. In the event that it is not acceptable for sample containers to contain preservatives, the preservatives will be provided. Contractor shall also provide chain of custody forms and any other necessary processing documents. The Contractor must deliver the containers and necessary documents to the Bureau of Reclamation, Flaming Gorge Field Division at 5995 Flaming Gorge
Visitor Center, Dutch John, Utah within 14 calendar days after receipt of the Notice to Proceed, unless other arrangements are made.
The Contractor will provide the Plant Foreman of FGFD with instructions regarding how to sample, pack and ship the samples to the Contractor and any time limits that must be observed between the time of sampling and reception by the
Contractor. Instructions will include chain of custody procedures.
Exhibit 1 presents the sampling frequencies, quantities, and tests to be conducted by the Contractor.
4. DELIVERABLES
4.1 REPORTS
4.1.1 Contractor shall provide an electronic report after each monthly test. Each report will be comprised of a written narrative of detection highlights, observations and analysis.
5. SCHEDULE
5.1 Utah DWQ requires a monthly report from FGFD in accordance with UPDES permit No. UT0020338.
Wastewater samples are analyzed once per month with one monthly TSD test analyzed on the wastewater and also the culinary water.
5.2 Contractor shall coordinate with the Plant Foreman for each monthly test as to the sampling date and shipping date for shipping samples to the Contractor’s laboratory. The Contractor shall provide the government sampling containers, sampling protocols, shipping containers, and chain of custody forms within 5 days of scheduled sampling date.
Contractor will arrange and pay for shipping. Upon receipt of samples, the Contractor shall complete analysis and submit required deliverables within 14 calendar days.
