About this file
This document is a Worker Protection and Investment Certification Form required by the Commonwealth of Pennsylvania for contractors and grantees. It certifies that the contractor/grantee is compliant with Pennsylvania's Unemployment Compensation Law, Workers' Compensation Law, and all applicable state labor and workplace safety laws. The form must be signed by an authorized official and submitted, as false certification can result in contract termination and a three-year ineligibility to bid on commonwealth contracts.
The related state and local contract opportunity is for the construction of a new 40' x 80' pole building for the Pennsylvania Game Commission (PGC) to be used for general work/research and storage of maintenance implements and tools. The project scope includes the construction of the building with a 12' wide covered canopy, installation of metal siding and roof panels, concrete floor, insulation, interior finishes, garage and man doors, windows, HVAC, electrical, plumbing, and other building appurtenances. The contract is for the Dauphin County and Harrisburg City jurisdictions in Pennsylvania.
