Contractors must have an authorized official execute the certification, attesting to full compliance with all applicable Pennsylvania labor laws and confirming understanding of mandatory reporting requirements should the contractor's compliance status change.
The certification carries significant legal consequences for false submission, including immediate contract termination and three-year debarment from bidding on future Commonwealth contracts under 62 Pa. C.S. § 531. Signatories acknowledge that the certification is subject to penalties under 18 Pa. C.S. § 4904 for unsworn falsification to authorities. The form requires the certifying official's signature, printed name, title, date of execution, and the contractor/grantee's printed name. This certification applies to the limestone Driving Surface Aggregate delivery and placement services to Francis Road in Potter County, which is under the purview of the Pennsylvania Department of Conservation and Natural Resources.
