The certification must be executed by an official duly authorized to represent the contractor or grantee, who must acknowledge understanding of their obligation to report any changes in compliance status to the purchasing agency immediately.

The Commonwealth of Pennsylvania imposes significant penalties for false certifications, including contract termination and three-year debarment from bidding on state contracts under 62 Pa. This form represents a mandatory compliance requirement for all contractors seeking to provide roadside pesticide and herbicide spraying services for Pike and Wayne County Roads under PennDOT guidelines.
