Summary: Well Installation and Well Abandonment Scope of Work

This is a Scope of Work document issued by the Virginia Department of Health (VDH) for a well installation and spring abandonment project (WIIN24-01_W&WA) located at 358 Lauck Lane, Luray, VA 22835, under Page County Department Well Construction Permit #139-WW-20658. The project requires the contractor to construct a private water well meeting Virginia Private Well Regulations, perform tree removal and site clearing as needed, install a minimum of 50 feet of casing and grout for a Class III(B) well, provide or subcontract for well pump installation with plumbing and electrical work, disinfect the well system, obtain required inspections and water quality testing, appropriately abandon the existing water supply and spring, implement erosion and sediment control measures, and comply with Department of Professional and Occupational Regulations (DPOR) requirements. The initial bid must be based on an estimated well depth of 150 feet with itemized costs for mobilization, setup, and other services, though the vendor will be paid only for actual depth drilled as verified by inspection records. Contractors must provide project start and end dates within two weeks prior to work commencement or within four weeks of purchase order issuance, notify VDH of subcontractor names within four weeks of contract signing, and submit all required documentation and invoices upon completion of inspection by the local health department.

Pricing is based on actual costs incurred for services performed, with bidders required to submit an average cost per foot of depth for the 150-foot baseline and separate per-foot rates for any additional depth beyond 150 feet. Additional services require prior written approval from the VDH Project Manager before execution, with change orders issued for approved work. Bidders must demonstrate compliance with DPOR Water Well System Provider Certification and proof of insurance, provide required documentation within 24 business hours of VDH request or face non-responsive bid status, and sign and return a Notice to Proceed form within three business days of award notification. A pre-bid site visit is recommended and can be arranged with Angelica Cipara, the designated project contact. Funding is provided through the VDH SWAP (Source Water Assessment Program) Grant program.
