Q5. Page 15 #4, do you have a specific questionnaire for this or would we be asking them these questions in person? This is not routine but it can be done A5. No specific form/questionnaire. It's really just meant to be a brief query as to whether or not an employee has any concerns regarding their potential workplace exposures, including reproductive. If they do, the contracted healthcare provider should provide basic education on prevention strategies, based on his/her scope of practice, and refer the employee to the AKARNG OHN for more specific workplace guidance. Routine visits and interaction is not possible for employees in the OHN’s Medical Surveillance program. This is merely just a way to provide these employees with basic prevention education and referral to the OHN, should they feel the need for further, more specific info.

Q6. Page 15, #5 g. What do they consider Stage 3 Hypertension? AHA does not have this in their guideline.

A6. > 180 (systolic) and/or > 120 (diastolic) mm Hg.

Q7. Page 16 #6‐. Do you have a company specific form or do we need one?

A7. If the vendor has one, they can send to the OHN for review. Alternatively, the form I use is attached herein (Q&A_Att_1_Back Flexibility Test Kraus Weber).

Q8. X‐Ray tech‐ Can we confirm if the Tech truly needs to be certified, or if they can be trained on the job to conduct X‐rays?

A8. Certification is preferable, but if a certified technician is not available in‐house, on‐the‐job training is a consideration.

Q9. Who would be performing the x‐rays, and who would be training/supervising?

A9. At a minimum, individuals performing the x‐rays would need to receive training on radiation safety for the particular equipment being used, how to conduct quality control tests on it, and receive regular continuing education related to the machines and radiation safety.
