STATEMENT OF WORK:

Q-Switched Nd: YAG Laser (PR 0010846609)

BACKGROUND:

Army Public Health Center (Provisional), Nonionizing Radiation Program is tasked to conduct eye protection safety evaluations according to established DOD guidelines which require testing at a wavelength of 1064 nanometers (nm). APHC (Prov) does not currently have the ability to accurately measure optical density of laser safety eyewear as required by DOD standards. The laser currently utilized is no longer reliable (32 years old) and cost prohibitive to repair. Without a new laser, eye protection evaluations to meet compliance with established permissible exposure limits will not be accomplished. Failing to provide safety parameters for military and civilian personnel, the risk of possible hazards from the laser emissions could be harmful to their health.

SPECIFIC REQUIREMENT:

APHC requires the purchase of a Q-Switched (Neodymium-doped) Nd: Yttrium Aluminum Garnet (YAG) Laser System that is a brand name or equal to the model listed below:

a.) Continuum – Powerlite Model 9020 Digital Laser Source (DLS)

In order to be considered for award, vendors shall quote a model and include specifications which either meet or exceed the salient characteristics listed below.

a.) Q-Switched Nd: YAG Laser (Oscillator/two amplifiers, 20 Hz, 1064 nm) b.) Digital power supply c.) 5 meter umbilical cord d.) Second and third Harmonic Generators (for 355 nm with reduced 532 nm) e.) Wavelength separation package (dichroic mirrors and mounts for use with harmonic generators.

f.) Optical Parametric Oscillator (OPO) - Pump option PL (optimizes pump source for OPO (pumping) g.) Fiber optic coupled injection seeder for SLM operation h.) Two stage, BBO type I narrow bandwidth OPO – Sunlite EX OPO i.) 20 Hz option for Sunlite EX j.) One-time installation

ORDER SHIPMENT DETAILS: The Laser System will be shipped to Public Health Center (Provisional) to the following Receiving Warehouse address:

U.S. Army Public Health Center (PHC)(Provisional) E-5165 Blackhawk Road Aberdeen Proving Ground (APG), MD 21010

ANTI-TERRORISM AND OPERATIONAL SECURITY:

All contractor employees, to include subcontractor employees, requiring access Army installations, facilities and controlled access areas shall complete AT Level I awareness training within 60 calendar days after contract start date or effective date of incorporation of this requirement into the contract, whichever is applicable. The contractor shall submit certificates of completion for each affected contractor employee and subcontractor employee, to the COR or to the contracting officer, if a COR is not assigned, within XX calendar days after completion of training by all employees and subcontractor personnel. AT level I awareness training is available at the following website: http://jko.jten.mil

2b. For contractors that do not require CAC, but require access to a DoD facility or installation. Contractor and all associated sub-contractors employees shall comply with adjudication standards and procedures using the National Crime Information Center Interstate Identification Index (NCIC-III) and Terrorist Screening Database (TSDB) (Army Directive 2014-05/AR 190-13), applicable installation, facility and area commander installation/facility access and local security policies and procedures (provided by government representative), or, at OCONUS locations, in accordance with status of forces agreements and other theater regulations.
