Response: Four (4).

5. How many Neurosurgeons vs Ortho?

Response: 3 Neuro Spine, 1 Ortho spine.

6. Do you have an average length for the cases?

Response: No. They are quite variable for Neuro, 2 to 8 hours for Ortho usually 4 hours.

7. What types of cases would we monitor?

Response: Cervical spine cases and lumbar fusion cases, Implant insertion (fusions and TDR's)

8. Just to clarify would you like our proposal sent via email, hard copy in a binder, or faxed?

Response: Proposals are to be emailed - please ensure that you keep in line with the required page limit as set-forth within the solicitation instructions to offerors and provide only the requested information.

9. Could you give a simple checklist of the documentation and forms we need to include in the RFP, so we are sure not to forget anything? Like the SF1449, Past Performance Questionnaires, etc.

Responses: Required documentation to be submitted is contained within the instructions to offerors contained within the solicitation.

10. Reference: CLINs 0001 and 0002 on page 3, and all associated option CLINs.

Question: We acknowledge that the hour quantities are estimated, and that the Government will pay only for actual usage. Are these the maximum hours estimated for the contract year?

Response: Yes, these are the maximum hours estimated for the contract year.

11. Reference CLINs 0003 and 0005 on pages 4-5, and all associated option CLINs. Question: Will the Government please confirm that this CLIN covers hourly charges for equipment that is limited to a laptop computer with software that will provide a functional monitoring circuit and conforms to the bulleted requirements at the top of page 83?

Response: The Government isn’t defining the equipment that your company is to provide; however, the neuromonitoring equipment and computer system used must be able to successfully carry out the required functions of this requirement. CLINS 0003 and 0005 are not limited to the a laptop computer with software. CLIN 0003 covers the charges as outlined in paragraph 1.4.1.3 of the PWS. CLIN 0005 are those hours that are above the two hours minimum as noted in the CLIN description.

12. Reference CLINs 0003 and 0005 on pages 4-5, and all associated option CLINs. Question: Will the Government provide space to store these supplies on premise? If so, how will these supplies be secured and inventory tracked?

Response: No, space to store supplies on premises is not provided, the required supplies are to be transported by the technician based upon the scheduled cases. See paragraphs 4 and 5 of the PWS. These sections speaks to how, when, and what supplies are order and equipment needed etc.

13. Reference: Deliver Information for all CLINs on pages 32-36 Question: Considering the proposal due date is now 30-MAR-2016, will the Government please confirm that the period of performance remains from 01-APR-2016 to 31-MAR-2017?

Response: The start date will shift to the right, tentatively 1 June 2016.

14. Reference: Pricing Page 3.

Question: Is the Government ultimately making an award based on a per hourly rate for Contract Providers or a flat (To not exceed) fee by ca.

Response: No. The Government will make the award in accordance with the Evaluation Criteria listed in the solicitation (See the evaluation criteria contained within the solicitation).

15. Under the Pricing Section, can you please provide more detail regarding CLIN0001 (720 hours, Tech), CLIN0002 (800 Hours, Neurologist), CLIN0003 (720 Hours, Equipment), and CLIN0004 (1800 Hours, IONM Services)? We are trying to get a better idea of how these hours equate to the case volume. If 0001 and 0002 are for the initial 2 hours of time (Tech and Neurologist), what do the 1800 Hours represent? Is this a combined Tech and Pro service?

Response: CLIN 0001 is the minimum guarantee on-call hours for the technician services see the CLIN description. More hours are required for the Technician as they must arrive to the hospital, set up and calibrate the neuromonitioring equipment (machine). CLIN 0002 isn't a minimum guarantee CLIN. The Neurologist only performs services during the actual case. CLIN 0003 is the minimum guarantee on-call hours for the equipment rental (See the CLIN description). CLIN 0004 covers regular hours for the Technician above the two hours guarantee under CLIN 0001, this also incorporates time to break down the equipment etc. (see the CLIN description). CLIN 0005 Equipment Charge is for the regulars above the two hours minimum guaranteed under CLIN 0003 for each on-call service, again more hours are required as the equipment is subject to the equipment rental charge when it's in use including breakdown of the neuromonitioring equipment (machine) after a case has been performed.

Yes this requirement is for both technical (Electroneurodiagnostic Technician) and professional (Neurologist) services.

16. Are we able to provide disposable supplies at No Charge to the facility? We typically include this in our Technical services rate and waive these fees to the facility.

Response: No. The disposable pricing should be priced as shown on the SF1449 CLIN Schedule Unit Price, Amount and Net Amount, per CLIN.

3. Under Factor 3 “Compensation Plan” our group contracts with our Neurologists on a 1099 (independent contractor) basis where they are paid a flat case rate per case, regardless of procedure type or length. Should we estimate what the hourly/annual compensation is for all of the cases they perform across the country? Or what this would look like specifically at JBLM cases only. This is much easier for Technicians as they are salary employees with our group. This further explained in the Department of Defense Instructions (DODI) 6025.5.

Response: The compensation plan should show your hourly/annual compensation for the providers as it pertains to this requirement here at Madigan Army Medical Center only. It is important to note that the providers performing services under this requirement can't make more than $208.33 per hour (total compensation).
