Questions & Answers

W91YTZ24Q0018

can a staffing agency apply so long as they can find someone within 50 miles per the solicitation requirements to do the job?

Yes, a staffing agency can apply.

Is the place of performance Richmond or Fort Knox KY? Solicitation has 2 different locations

The BPA is for Ireland Army Health Clinic with services needed at Bluegrass Army Depot in Richmond KY. BGAD is one of our locations that we service.

The services needed are, New patient office visit, Eye exam, Rx only and Scuba inserts, did I miss anything?

That is correct. New Patient Office visit, Eye exam, RX only & SCBA Inserts (included in Exam or Rx price only). The MTF will order SCBA and inserts we just need them to do exam and complete paperwork we provide

Do you need any products? If yes, is there a specific brand you prefer? [I see some information about importing products, or this should be ignored!]

SCBA inserts – when needed, they are required for all of our firefighters/EMS/civilian police.

Since the Optometrist is an eye specialist, why is the cardiologist license needed which is I guess required to treat the heart.

Statement of Work corrected and RFQ will be updated for Amendment 3.

For the products shipments, do you have a delivery team in place or we should take care of that?

BGAD will order all supplies. We just need the examination completed by the vendor

The price per test on the solicitation, is that an example or that is the price you looking to pay?

This is the price that was quoted during our market research.

I don't see form SF30 for amendments approval, where should that be done?

Please sign the latest Amendment on the SAM.gov website

For the place of manufacture of the products approval, do you need these to be done now or after we get awarded the solicitation?

BGAD staff will order the SCBA inserts when needed. The vendor need not worry about the inserts.
