ADDENDUM TO 52.212-2

ADDENDUM TO 52.212-2

Paragraph (a) is hereby supplemented with the following:

Contract Evaluation and Award:

Basis of Award - This solicitation will use lowest price technically acceptable evaluation procedures to make a best value award decision. Award will be made to the responsible offeror that provides the lowest price and a technically acceptable offer. Award will result in a firm fixed contract.

Except for communications conducted for the purpose of minor clarification, the Government intends to evaluate offers and award a contract without discussions.

Therefore, each initial offer should contain the offeror's best terms from a technical and price standpoint. However, the Government reserves the right to conduct discussions if it is later determined by the Contracting Officer to be necessary.

Evaluation Factors. The following factors will be evaluated:

Factor 1. Technical Capability reflects the Government’s confidence in each offeror’s ability, as demonstrated in its offer, to provide the Medical Appointment Services as stated in this solicitation. The Government will evaluate whether the offeror has demonstrated that the quote for the Medical Appointment Services meets the requirements of the Performance Work Statement. Limited to five pages.

The following adjectival ratings will be used in evaluating each offeror's technical offer.

TECHNICAL

EVALUATION

RATING DESCRIPTION

Acceptable Offer clearly meets the minimum requirements of the solicitation.

Unacceptable Offer does not clearly meet the minimum requirements of the solicitation.

Note: Any offeror receiving a rating of “Unacceptable” for Technical Capability will no longer be considered for award.

Factor 2. Past Performance. Offeror is required to submit information, on a single page, identifying federal, state and local Government and private contracts performed within the past three years, where your company provided relevant services that indicates your company’s ability to satisfactorily perform the magnitude of this requirement.

Past Performance will be assessed from the perspectives of an Acceptable/Unacceptable Rating.

Past Performance Evaluation Ratings

Rating Description Acceptable Based on the offeror’s performance record, the Government has a reasonable expectation that the offeror will successfully perform the required effort, or the offeror’s performance record is unknown. (See note below)

Unacceptable Based on the offeror’s performance record, the Government has no reasonable expectation that the offeror will be able to successfully perform the required effort.

Note: In case the offeror does not have a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the offeror may not be evaluated favorably or unfavorably on past performance (see FAR 15.305 (a)(2)(iv)).Therefore, the offeror shall be determined to have unknown past performance. In the context of acceptability/unacceptability, “unknown” shall be considered “acceptable.”

Note: The government may also consider any additional past performance information available, to include the Past Performance Information Retrieval System (PPIRS).

Note: A Past Performance Rating of “Acceptable” must be obtained in order to be considered for award.

Factor 3. Price will be evaluated separately from Technical and will be evaluated for fairness and reasonability in terms of the Government’s requirement. Price must be determined to be fair and reasonable.

Technical and Past Performance when combined are equal to Price.

Award will be made to the lowest-priced offeror provided its technical proposal conforms to the requirements of the solicitation; it has an acceptable record of past performance; its price is determined to be fair and reasonable by the contracting officer; and the contracting officer determines the offeror is responsible.

Paragraph (b) is hereby supplemented with the following: Options are applicable to procurement and will be evaluated.
