SECTION SF 30 BLOCK 14 CONTINUATION PAGE

The following items are applicable to this modification:

QUESTIONS

Questions for: W91YTZ21Q0106:

1. What is the projected breakdown or distributions of cadavers by major location (i.e Fort Bragg, Fort Campbell, Fort Stewart)?

Response: 16 total cadavers requested per year for three sites. Training will be conducted separatly for all three sites at different times. Roughly 50% of the training will take place on Fort Bragg, NC, the additional training will be split between Fort Campbell and Fort Stewart as requested.

2. What is the maximum number of labs executed quarterly?

Response: The maximum number of labs will be up to four quarterly.

3. Is refrigeration required for storage of the cadavers?

Response: Per the PWS the storage for cadavers will be the responsibility of the vendor. Refrigeration may be available with prior approvals.

4. Will the cadaver move locations once training begins?

Response: The cadaver may move locations once training begins but once assigned to one location, it will not leave that post (i.e. Fort Bragg, Fort Campbell, Fort Stewart)

5. How much notice will be provided to vendors prior to execution of any lab?

Response: The BDE will provide at least 30 days’ notice prior to the training event.

6. Within the PWS it states “1.6.1 Other Direct Costs: The Contractor must have the ability to provide cadaver perfusion technology on an as needed basis”. What is the Governments expectation on the perfusion requirement? Is the government intent for only water perfused cadavers or is the government looking for re-animate cadaveric tissue that provides a pulsatile pump system mimics heart and circulatory functions as well as a heat exchanger that perfuses the cadaver with bovine whole blood or our patented blood substitute?

Response: There is no perfusion requirement, however it is preferred that the cadaver perfuses with bovine blood. This extends the ability to use the cadaver longer then the blood substitute.

7. This solicitation is for a total of 16 whole body cadavers annually to be delivered to FT. Bragg, NC.

Will you please let me know if the amount on this line is supposed to reflect the entire cost of the 16 cadavers and the price of our services for the entire year?

Response:

Please note cadaver requirement locations is: Fort Bragg, NC; Fort Campbell KY, and Fort Stewart GA.

Contractor will provide pricing for each line item on the solicitation. Note – Pricing for each line item, for each year. Line items 0001 - 0002; 1001 - 1002; 2001 – 2002; 3001 – 3002.

See section 1.3 of the PWS that “The Government will not pay for the cadaver itself; however, the Government will pay a reasonable amount for the removal, processing, preservation, quality control, transportation, implantation, or disposal of a part.”

SUMMARY OF CHANGES

SECTION SF 1449 - CONTINUATION SHEET

The following have been modified:

ADDENDUM TO 52.212-2

Award will be made using the lowest price technically acceptable (LPTA) source selection process. Award will be made to the responsible offeror on the basis of the lowest evaluated price of quotes meeting or exceeding the acceptability standards for non-cost factors. Award may be made without discussions with offerors (except communications conducted for the purpose of minor clarification). Therefore, each initial offer should contain the offeror's best terms from a technical and price standpoint. However, the Government reserves the right to conduct discussions if it is later determined by the contracting officer to be necessary.

1. Contractors may not qualify if any of the following are not met:

1. Quote does not reasonably address the requirements in the Request For Quote (RFQ).

2. Quote includes major deficiencies or omissions which discussions could not be expected to cure without a major revision or a new quote.

3. Quote is so technically inferior or so out of line in price as compared to other quotes as to preclude it from having a reasonable chance for award.

4. The Government reserves the right to conduct discussions, if necessary.

1. All quotes submitted in response to this solicitation shall answer the requirements outlined in the Performance Work Statement.

FACTOR I: TECHNICAL:

Technical evaluation will be based on the following:

(a). The quote shall be clear, concise, and shall include sufficient detail for effective evaluation for substantiating that the items offered meet the requirements of the solicitation. The quote shall not simply rephrase or restate the Government’s requirements, but rather shall provide convincing rationale to address that the products offered clearly meet the Government’s requirements. Offerors shall assume that the Government has no prior knowledge, and will base its evaluation solely on the information presented in the offeror’s quote.

(b). Technical capability shall include how the Contractor will meet the requirement of the PWS.

(c) Offeror shall provide comments after each section of the PWS how they will provide the service.

The following ratings will be used in evaluating each offeror’s technical quote.

Acceptable – To receive this an acceptable rating, the offeror shall present a quote that clearly and comprehensively addresses the technical factor. The offeror shall provide sufficient details for the Government to determine that the items offered satisfactorily meet the requirements of the PWS. The quote shall clearly demonstrate that the offeror fully understands and is capable of performing the requirement for the total proposed price indicated in its quote.

Unacceptable – An unacceptable rating will be assessed on any offeror that presents a quote that does not clearly meet the requirements of the PWS.

FACTOR II: PAST PERFORMANCE:

Offeror is required to submit information, on a single page, identifying federal, state and local Government and private contracts performed within the past three years, which are similar in scope and complexity to the effort described in this solicitation. If none, offeror should so state and provide information on references. Information provided by the Contractor shall state the type of service provided with information to verify references: Names;

telephone numbers; Points of Contact; amount of contract; and email addresses.

The following ratings will be used in evaluating each offeror’s Past Performance:

Acceptable: Based on the offeror’s performance record, the Government has a reasonable expectation that the offeror will successfully perform the required effort, or the offeror’s performance record is unknown.

Unacceptable: Based on the offeror’s performance record, the Government has no reasonable expectation that the offeror will be able to successfully perform the required effort.

Note: In the case of an offeror without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the offeror may not be evaluated favorably or unfavorably on past performance (see FAR 15.305 (a) (2) (iv)). Therefore, the offeror shall be determined to have unknown past performance. In the context of acceptability/unacceptability, “unknown” shall be considered “acceptable.”

Evaluation of past and present performance will be subjective based on consideration of all relevant factors and circumstances. The currency, content, general trends, and relevance of the information in Contractor’s performance will be considered. Providing incorrect references and phone numbers may render your proposal as unacceptable. The Government may also consider and use any additional information from its own files, databases such as the Contractor Performance Assessment Reporting, references provided by the offeror, or from any source it deems appropriate. The Government may survey references to verify information provided and may survey other sources known to the Government.

If adverse past and present performance information is obtained, the respective point of contact may be contacted to obtain additional information about the circumstances surrounding the situation. Offerors will be given an opportunity to address unfavorable reports of past and present performance to which the offeror has not had a previous opportunity to respond. The offeror’s response, or lack thereof, will be taken into consideration. If an offeror does not have a past and present performance history relating to the solicitation, the offeror will not be evacuated favorably or unfavorably, and a neutral rating will be given for this factor. Offerors receiving a rating of neutral will be considered for award.

FACTOR III: PRICE

Offeror to provide breakdown of pricing for services at each location.

This list is not all inclusive of services you provide:

a. Pricing of Laboratory Technician and how many

b. All travel and lodging for how many people

c. Lab Support and how many.

d. Cremation pricing and how many.

e. Any shipping.

f. Please provide the total pricing for each location.

Fill in the prices for all CLINS on all contract periods. Award will be made to the lowest priced technically acceptable offor with a favorable or neutral record of past performance. Price will be evaluated on a fair and reasonble basis.
