Computers and Blackberry phones will also be supplied by the Government. The requirement will be that there be at least one staff member in the office and available during regular business hours, allowing the other two personnel the opportunity to travel and telework. There may be occasions when all three will be out of the office at the same time, but it will be infrequent and due to concurrent travel, not telework Please see Exhibit B of the solicitation for the multiple readiness division sites.

22. Can an out-of-state service-disabled veteran-owned/minority-owned small business such as mine bid on providing the social workers as outlined? And if so, can just the social worker portion of the contract be bid on separately?
Response. No. This is a behaviour healh program which calls for oversight of the program as well, social workers and nurse case managers. It will be an all or none award.
23. Are all positions on the current contract filled? IF not, which positions are vacant and where are they?
Response: Yes, all incumbent positions are filled, please see exhibit B for the locations of the Lead DPH and DPH, as well as the nurse case managers that are being added to the requirement.

Response: Closing date has been extended to 28 June 2018
25. We noticed on pg. 92 of 209 of the solicitation, #6 States “It is the responsibility of the Contractor, both administratively and financially to obtain a Top secret facility Clearance (FCL)”. Is this still a requirement as it is not mentioned anywhere else within the solicitation?
Response: Yes and No, at this time none of the facilities they will be currently assigned to have the need for a top secret clearance. If, in the future they were going into a facility that needed a top secret clearance then they would have to get the required clearances.
26. Can the Government provide the GS equivalent for the various positions?
Response: Please see 52.222-42 page 23 of the solicitation
27. RFQ, PWS, Exhibit B does not identify a location for the National Psychological Health Program Director (Director) or the Deputy. From the description of DPH Team Lead on PWS, page 72, the DPH- Team Lead in Exhibit B would appear to be different from the Director and Deputy. Is it the Government’s intent that we propose key personnel for each of the labor categories in exhibit B?
Response: Yes, please see below for a further explanation of key personnel The National Psychological Health Program Director and Deputy Director are the vendors who will employ the Directors of Psychological Health and the Nurse Case Managers, and are usually the CEO and COO of the company that wins the award, or they may bring someone on board who qualifies. The responsibility of the National Psychological Health Program Director and Deputy will be to meet the requirements of the PWS, specifically to provide qualified personnel and all deliverables. They will be located at their corporate HQ. The Director’s role is a strategic role, while the Deputy’s role is operational.
The Director’s behavioral health experience may be purely administrative, whereas the Deputy should have the credentials and a working knowledge of the clinical aspects of the program. The Director’s requirement to have five years of experience in a national service delivery program and five years of experience in military mental health.
PLEASE NOTE: The USAR Director of Psychological Health and Deputy Director of Psychological Health (DOD personnel) are responsible for strategic and operational oversight of the program to ensure that it continues to meet the needs of the Army Reserve.

However,Exhibit B indicates one Sr. Nurse Case Manager and eight Nurse Case Managers.
Response: There are eight nurse case managers and one of the eight will be the lead nurse case manager, she will represent them to the Team lead and in communications with the DoD.
29. On pages 179 and 180 of the RFQ the Workload Data, Exhibit A shows 16 FTE while the Exhibit B – Psychological Health Program Personnel Location Summary suggests 15 FTE. Also, the Schedule B Line Items suggests 14 FTE to be provided under this contract. Could you please clarify the specific number of FTE per position and location in which the Government is requesting?
Response: There are 14 FTE’s that will need to be hired by the awardee, the other 2 key personnel National Psychological Health Program Director, National Psychological Health Program Deputy Director are employed by the Contractor who is awarded the contract, and is usually the CEO, COO of the company (please refer to question 27’s answer for a more detailed answer) which gives you a count of
16. Exhibit B list the Senior Nurse Case Manager separately but isn’t a separate FTE because these duties will be given to one of the eight nurse case managers.
30. Regarding Line Items 0002 and 0003, the positions will be located at geographically dispersed locations which may result in different hourly rates per location. Is it the Government’s intent to receive a blended rate to cover all positions at all locations for each of these line items? If not, would the Government consider revising the Schedule B to include individual line items for each position at each individual location?
Response: CLIN 0002 is a social worker and does not fall under the DOL Wage Determination rate;
however the government realizes there will be different locality rates and will accept a blended rate. CLIN 0003 for the nurse case managers that are going to be placed in SC, CA, WI and NJ do fall under the DOL Wage Determination rate and those rates vary among the states; therefore, the government will accept a blended rate for the nurse case managers as well. However, you must submit compensation for each of the DOL positions, to include option years. In addition, you will need to indicate how you determined the rate. If you wish to submit pricing for each of the states then add these to your schedule B when submitting proposal.
31. The instructions on page 180 of 209 only state to complete and submit Clause 52.212-3 Alt 1 Offeror Representations and Certifications for Commercial Item; are contractors required to complete and submit the following clauses?
・ 52.209-2 PROHIBITION ON CONTRACTING WITH INVERTED DOMESTIC CORPORATIONS--
REPRESENTATION (NOV 2015)
・ 52.209-7 INFORMATION REGARDING RESPONSIBILITY MATTERS (JULY 2013)
・ 52.209-11 REPRESENTATION BY CORPORATIONS REGARDING DELINQUENT TAX
LIABILITY OR A FELONY CONVICTION UNDER ANY FEDERAL LAW (FEB 2016)
Response: Please complete and submit the above clauses.
32. Attachment 13 – Sample Compensation Plan” (page 169 or 209) states that the compensation plan should be submitted as part of the pricing proposal. Page 180 of 209 states that compensation plan is to be submitted as part of the technical proposal. Can the government please clarify which volume the compensation is to be include within?
Response: Both
33. Section 52.212-1, Instructions to Offerors, pages 180-181 identifies 7 Subfactors: Subfactor 1 - Recruitment Plan, Subfactor 2 - Retention Plan, Subfactor 3 - Training and Immunization Plan, Subfactor 4 - Compensation Plan Subfactor 5 - Travel Plan Subfactor 6 - Transition Plan Subfactor, and 7 - Management Plan. However, under paragraph 7 of the same section on page 182, identifies the following technical subfactors: Recruitment; Retention;Training and Immunization Plan; Compensation Plan, Quality Control Plan, Transitional Plan, and Management Plan. Does the government want Subfactor 5 Travel Plan or Quality Control Plan or both?
Response: 52.212-1 will be corrected on the solicitation amendment to have the quality control plan with the Management plan as well.
34. Will the government confirm receipt of past performance questionnaire upon offerors request when the government has received from Past Performance from the POC?
Response: No
35. Will the government consider extending the due date for the solicitation by additional 7 working days after the release of government response to the questions, to allow offeror to address government responses to the question?
Response: Closing date has been extended to 28 June
36. Would the Government consider setting this requirement aside as a SDVOSB?
Response: No
37. In support of our recruiting efforts, we will need to notify candidates of work location. When is it estimated that the Government will determine the location for the Senior Nurse Case Manager?
Response: 90 days after award, one of the nurse case managers will be chosen for the lead nurse case manager, which they will perform those duties at their current site.
38. Are you able to post the bill rates for the current positions by the incumbents?
Response: No, all bids submitted to the government are kept confidential and are within FOIA exemption 4.
39. Will the Government allow us to propose our own level of effort or are we expected to use the FTE estimates in the RFP? If we are allowed to propose a different level of effort, will the Government provide historical work load data for each labor category in each location for a basis of effort?
Response: No, Please adhere to the governments FTE estimates for the level of effort in the RFP.
40. In Attachment 10, the Government provided wage determinations (WD) for VA, CA, SC, NJ, and CA.
There was no WD provided for FL. The counties for the provided WDs do not match the counties for place of performance within each of the states. Could the government please confirm if they will be incorporating the WDs provided in the solicitation as part of the contract or if the contractor should update them to reflect the proper county for place of performance?
Response: The government will correct the Wage Determination within the solicitation to reflect the correct counties; however, the nurse case managers will only be in four states CA, SC, NJ, and WI, and the amendment to the solicitation will reflect the revised Attachment 10 , which will be incorporated into the contract.
41. The Government has included Clause 52.222-62 Paid Sick Leave Under Executive Order 13706 (JAN 2017), however, the CLIN structure is set at 1920 hours per FTE which does not account for paid sick leave. Would the Government allow for 1880 hours per FTE to partially account for mandatory sick leave benefits?
Response: No, the FTE will remain at 1920. There are 52 weeks in a year, 40 hours/wk is a full time work week and 40 hours times 52 weeks (40*52 = 2080) the 1920 FTE allows for 10 holidays and 10 days of vacation (160 hours), which are not reimbursable by the government. The government only pays for hour worked.

ADDENDUM 52.212-2
Paragraph (a) is hereby supplemented with the following:
a. The Award will be made using best value Lowest Priced Technically Acceptable (LPTA) procedures. The government will initially list proposals from lowest to highest price. The government will evaluate the Technical Acceptability of the five (5) lowest priced proposals first. If one of those proposals are rated Technically Acceptable, the government will not evaluate any other proposals. If all of those proposals are determined technically unacceptable, the government will evaluate the next five (5) lowest price proposals and continue in that manner until a proposal is rated technically acceptable. If a proposal is rated Technically Unacceptable , the government will not evaluate any other factor. USING THIS PROCEDURE, THE GOVERNMENT MIGHT NOT
EVALUATE ALL PROPOSALS.
Offers will be evaluated to determine compliance with all requirements of the solicitation, including any attachments and exhibits. The evaluation criteria will be applied to all offers in the same manner. Each proposal will be evaluated strictly in accordance with its content and the Government will not assume that performance will include areas not specified in the offeror's proposal.
Volume 1 - General - The Government shall review the following:
Solicitation and any Amendments for signatures;
Representation and Certifications;
Volume II - TECHNICAL EVALUATION. The proposal technical factors and subfactors will be evaluated to determine the adequacy of the offeror’s proposed compensation to individual health care providers HCPs for the local area. The primary determinant will be the base salary and fringe benefits paid to the individual HCP. Offerors must propose a base salary and fringe benefits which is at least commensurate with the salaries of similar health care providers in the local area in order to be technically acceptable. This salary in almost all instances exceeds the prevailing wage rates in the Department of Labor’s wage determination for a locality. The Government has developed an independent government estimate (IGE) based upon its assessment of base salaries paid in the local area as well as salaries paid on current contracts. A low, or no Compensation Plan, may be viewed as evidence of failure to comprehend the complexity of the contract requirements or appropriate compensation levels for geographic area where performance will take place. Plan must fully address how the offeror intends to perform the requirement Offerors which propose low base salaries and fringe benefits, particularly below rates paid in the local area, will have the burden of establishing that their plans will result in the recruitment and retention of quality personnel and the uninterrupted performance of services throughout the life of the contract.
Technical factors as follows:
Subfactor 1: Recruitment Plan - Submit a plan how they will recruit qualified personnel in time to work on day one (1) of the performance period. There are currently incumbents for the Lead Psychological Health Director and the Directors of Psychological Health; however, there are no incumbents for the Nurse Case Managers.
Subfactor 2: Retention Plan - Submit a plan how they will retain qualified personnel to provide continuity of care throughout the performance period.
Subfactor 3: Training and Immunization Plan - Submit their plan how they will track training and immunizations.
Subfactor 4: Compensation Plan - For each provider type for a base and two option periods.
Subfactor 5: Travel Plan - Explain how your company will handle the travel expenses and reimburse the employee. For example does your company issue credit cards for travel. Or does the employee have to pay for travel up front and then be remimbursed.
Subfactor 6: Transition Plan: Submit their plan how to adequately provide services on day one of the period of performance to meet Womack's Army Mission. Currently there are incumbents for the Lead Director of Psychological Health, and the Directors of Psychological Health.
Subfactor 7: Management Plan & Quality Contol Plan: The subfactor will describe how the company plans to manage the program in accordance with the PWS and control the quality of the program.
I f any technical subfactor received an unacceptable rating, the combined technical will be unacceptable.
The recruitment and retention plan will evaluate the various incentives the company will offer the HCPs.
The Technical Evaluation Subfactors shall not exceed 50 pages; additional pages will not be reviewed or evaluated.
Technical merit will be evaluated as either Acceptable or Unacceptable.
Definitions are as follows:
Technical Acceptable/Unacceptable Ratings Rating Description Acceptable Proposal clearly meets the minimum requirements of the solicitation.
Unacceptable Proposal does not clearly meet the minimum requirements of the solicitation.
Note: To receive consideration for award, a rating of acceptable must be achieved for all technical factors. The technical subfactors are of equal importance. To be rated technically acceptable, offerors must receive a rating of acceptable for each factor. A rating of unacceptable in either subfactor will result in an unacceptable rating for the technical factor. For those proposals rated technically unacceptable the government will not evaluate any other evaluation factors.
Volume III: PAST PERFORMANCE. Offeror is required to submit information, on a single page, identifying federal, state and local Government and private contracts performed within the past three years, which are similar in scope and complexity to the effort described in this solicitation. If none, offeror should so state and provide information on references. Note - past performance will be accepted from a sub-contractor / teaming partner if the prime contractor does not have relevant past performance.
Information provided by the Contractor shall state: The type of service provided with information to verify references: Names; telephone numbers; Points of Contact; amount of contract; and email addresses.
The information which will be used for evaluation will be obtained from those presented in the Offer’s past performance documentation, the Contractor Performance Assessment Reporting Systems (CPARS) http://www.cpars.csd.disa.mil/cparsmain.htm, and any other sources. The Government may reject a proposal if it is found that any information that has a negative impact on the offeror’s past performance record has been deleted, misrepresented, or withheld. Past performance surveys should be based upon performance of like services to those in this solicitation.
Past performance will be rated on an “acceptable” or “unacceptable” basis using the ratings below:
Past Performance Evaluation Ratings Rating Description Acceptable Based on the offeror’s performance record, the
Government has a reasonable expectation that the offeror will successfully perform the required effort, or the offeror’s performance record is unknown. (See note below.)
Unacceptable Based on the offeror’s performance record, the Government has no reasonable expectation that the offeror will be able to successfully perform the required effort.
Note: In the case of an offeror without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the offeror may not be evaluated favorably or unfavorably on past performance (see FAR 15.305 (a) (2) (iv)). Therefore, the offeror shall be determined to have unknown past performance. In the context of acceptability/unacceptability, “unknown” shall be considered “acceptable.”
Volume IV: PRICE. Price will be evaluated separately from the technical and past performance factors and will be evaluated for fairness and reasonability in terms of:
A. That the prices are consistent with and reflect the special pricing for staffing requirement.
B. All vendors’ prices must be deemed to be fair and reasonable to qualify for award and within the guidelines of the special pricing.
C. In Accordance With FAR 15.404-1, price fair and reasonableness will be used to evaluate an offeror’s proposal, but price will not be scored. Pricing will be evaluated to determine if fair . The Government is interested in proposals that meet the requirements with acceptable risk, at a fair and reasonable price.
D. Given that FAR 52.217-8 provides that the Government may exercise the Option to Extend Services at the rates specified in the contract (i.e., the rates in effect when the Extension of Services option is exercised), for purposes of evaluating the Option to Extend Services, the Contracting Officer will consider the prices submitted for the base year and each option year, since those are the binding prices should the Option to Extend be exercised.
