SECTION SF 30 BLOCK 14 CONTINUATION PAGE
The following have been added by full text:
A. The purpose of this Amendment is to incorporate the following changes to the solicitation:
1. Total quantity of square foot for CLIN 0001, 1001, 2001, 3001, and 4001 is changed from 288,566 SF to 316,766 SF.
2. Delete CLINs 0004, 1004, 2004, 3004, and 4004 (Phase-In).
3. Revise PWS, Section 1.6.13 of the Performance Work Statement (PWS) is deleted and changed to read "Non Applicable".
4. Revise PWS, Section 1.1 Scope of the PWS to read as follows:

During the life of the contract, buildings may be added/deleted due to new buildings being built and old buildings being demolished. During the life of this contract some or all of the contract requirements may be reduced due to the planned relocation of U.S. Forces from Yongsan to Camp Humphreys (Pyongtaek) starting in 2017. This will affect the overall square footage for performance. The contract will be adjusted accordingly for any increase or decrease to square footage.
5. Revise ADDENDUM FAR 52.212-1(Factor II - PAST PERFORMANCE) and FAR 52.212-2 (Factor II - PAST PERFORMANCE) to read as follows:
(a) Addendum FAR 52.212-1:
(2) FACTOR II - PAST PERFORMANCE: Past performance shall be evaluated on an acceptable/unacceptable basis. Offerors are authorized to provide information on the problems encountered on the identified contracts and the Offeror’s corrective actions. The Government may ascertain past performance information on the Offeror from sources other than the references provided. In the case of an Offeror without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the Offeror may not be evaluated favorably or unfavorably on past performance (see FAR 15.305 (a)(2)(iv)). Therefore, the Offeror shall be determined to have unknown past performance. In the context of acceptability/unacceptability, “unknown” shall be considered “acceptable”.
The offeror shall provide recent and relevant past performance in cleaning service of food exhaust fans and ducts. Recent past performance is within the last five (5) years as of the opening date of the solicitation. (b) The Government may send a Past Performance Questionnaire to the POC listed by the Offeror for each relevant contract, who will subsequently be requested to submit information to the Government no later than 14 days after Government issuance of the questionnaire.
Note: Offerors are reminded that while the Government may elect to consider data obtained from other sources, the Government does not assume the duty to search for data to cure problems it finds in the proposals. The burden of providing thorough and complete past performance information rests with the Offeror.
(b) Addendum FAR 52.212-2:
II. FACTOR B. PAST PERFORMANCE: (1) Past performance will be evaluated on an acceptable/unacceptable basis. Offerors are authorized to provide information on the problems encountered on the identified contracts and the Offeror’s corrective actions. The Government may ascertain past performance information on the Offeror from sources other than the references provided. In the case of an Offeror without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the Offeror may not be evaluated favorably or unfavorably on past performance (see FAR 15.305 (a) (2) (iv)). Therefore, the Offeror shall be determined to have unknown past performance. In the context of acceptability/unacceptability, “unknown” shall be considered “acceptable”.
(2) The offeror shall have recent and relevant past performance in cleaning service of food exhaust fans and ducts. Recent past performance is within the last five (5) years as of the opening date of the solicitation. Relevant past performance are those that are the same or similar services based on size, scope, complexity, and contract type.

(b) References will be contacted by phone or e-mail to verify contents in detail explanation and to determine record of compliance with contract requirements, accuracy of reports, technical, and quality awards/certificates.
(c) The Government reserves the right to obtain past performance information from any reasonable source available, including Government database in order to evaluate past performance.
6. Revise ATTACHMENTs No. #1 and #2 (Building List with SF).
B. The following are the responses to the Questions submitted after the Site Visit.
(Question 1): Does the "food duct cleaning service" in Sub-factor 2. Prime Contractor's Prior Experience have the same meaning as the "relevant past performance in cleaning service of exhaust fans and ducts" in Factor II - Past Performance? Is it correct?
(Response 1): NO. Exhaust fans and ducts described in Factor II is for food exhaust fans and duct system, it is not for air ventilation system.
(Question 2): Shall Sub-factor 2. Prime Contractor's Prior Experience be also "same or similar" services based on size, scope, complexity, and contract type in the same context? Is it correct?
(Response 2): NO. Prior experience for only food exhaust fans and duct cleaning service is acceptable
(Question 3): "A" company which has not same or similar prior experience to this RFP's requirement, it is not acceptable in accordance with Factor II-Past Performance provision. Is it correct?
(Response 3): YES. It's not acceptable.
C. As a result of the above changes, the closing date is extended from 01:00 PM on 18 Apr 2016 or 11:00 PM on 17 Apr 16 to 01:00 PM on 29 Apr 2016 or 00:00 AM on 29 Apr 16 (Eastern Day-Light Time/EDT)

E. All other terms and conditions remain the same.

-------- End of ADDENDUM to FAR 52.212-1 INSTRUCTION TO OFFERORS – COMMERCIAL ITEMS (JUN 2008)----- (End of provision)
52.212-2 EVALUATION--COMMERCIAL ITEMS (OCT 2014)
(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers:
ADDENDUM TO FAR 52.212-2, EVALUATION-COMMERCIAL ITEMS (JAN 1999)
Award will be made to the responsible Offeror submitting the Lowest Price Technically Acceptable (LPTA) offer that satisfies all terms and conditions of the solicitation. Offers will be evaluated for acceptability, but not ranked using the non-price factors/subfactors. All non-price factors and subfactors will be rated as either “acceptable” or “unacceptable”. In order to be considered for award, there must be an “acceptable” rating in every non-price factor/subfactor. In other words, a rating of “unacceptable” for any factor or subfactor will result in the entire offer being rated “unacceptable”.
· The following ratings will be used in evaluating all non-price technical factors and subfactors except Past Performance:
ACCEPTABLE
Proposal submitted with all required documentations including all three (3) subfactors listed in the solicitation.
UNACCEPTABLE
Proposal NOT submitted with all required documentations including all three (3) subfactors listed in the solicitation.
· The following ratings will be used in evaluating Past Performance:
ACCEPTABLE
Based on the Offeror’s performance record, the Government has a reasonable expectation that the Offeror will successfully perform the required effort, or the Offeror’s performance record is unknown.
UNACCEPTABLE
Based on the Offeror’s performance record, the Government has no reasonable expectation that the Offeror will be able to successfully perform the required effort.
NOTE: In the case of an Offeror without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the Offeror may not be evaluated favorably or unfavorably on past performance [see FAR 15.305 (a)(2)(iv)]. Therefore, the Offeror shall be determined to have unknown past performance. In the context of acceptability/unacceptability, “unknown” shall be considered “acceptable.
I. FACTOR A - TECHNICAL CAPABILITY: Each factor and sub-factor will be rated either acceptable or unacceptable. A rating of unacceptable for any factor or sub-factor will result in an overall unacceptable rating. If an offeror fails to meet the minimum requirements outlined in one of the below subfactors, and overall proposal will be rated “unacceptable” and will not be eligible for award.
Sub-factor 1. Prime Contractor’s Licenses: The offeror shall provide i) a copy of valid/current Hygiene management permission license under Management of Public Health Act (공중위생영업 중 위생관리 용역업 신고증) issued by the Republic of Korea (ROK) Government and ii) a valid Annual Hygiene Training Certificate (정기위생교육필증) issued by Korean Facility Hygiene Management Association in CY15 (한국건물위생관리협회) to provide cleaning service for food exhaust fans and ducts at dining facilities. The Subcontractor’s license cannot be substitute for the Prime Contractor’s license.
Sub-factor 2. Prime Contractor’s Prior Experience: The offeror shall provide evidence of at least two (2) years of experience in performing food duct cleaning service within the last five (5) years as of the opening date of the solicitation. The experience of a Subcontractor cannot be substituted for the Prime Contractor’s experience. The offeror shall provide evidence of prior experience as required below:
- The title of the contract/project
- A description of work completed
- Amount of Contract
- Length of contract/project (year/month/date – year/month/date)
- Complete name and phone number of Point of Contact (POC) at Client Organization
Sub-factor 3. Contract Manager (CM): The offeror shall provide a resume of the CM with at least two (2) years of experience in food duct cleaning service as a CM within the last five (5) years. CM’s resume shall clearly identify the information required below:
- CM’s Name
- A description of qualifying work experience
- Company Name/Employer, email/phone number of validating point of contact (POC),
- Length of work experience (year/month/date – year/month/date)
II. FACTOR B. PAST PERFORMANCE:
(1) Past performance will be evaluated on an acceptable/unacceptable basis.
(3) The past performance will be evaluated for all offeror’s proposals by the Contract Specialist/Contracting Officer in the following manner:
(d) Review of the detailed explanation submitted by offerors demonstrating the relevance of the references to the requirements of the solicitation.
(e) References will be contacted by phone or e-mail to verify contents in detail explanation and to determine record of compliance with contract requirements, accuracy of reports, technical, and quality awards/certificates.
(f) The Government reserves the right to obtain past performance information from any reasonable source available, including Government database in order to evaluate past performance.
III. FACTOR C - PRICE:
(1) Price analysis will be conducted for each proposal to ensure completeness, realism, reasonableness, and balance IAW FAR 15.404-1(b). The following applies to that evaluation:
(a) Completeness: The offeror shall complete the pricing information for all CLINs/Sub-CLINs identified for base period plus four (4) options (total five (5) years). Any underlying assumptions including proposed escalation rates between the 1st and subsequent periods shall be clearly explained.
(b) Price Reasonableness: Price should also reflect fair market value and be reasonable when compared to the independent Government estimate, current market prices for same or similar services and/or when compared among other technically acceptable offerors.
(c) Price is not scored: The Government will review the price proposal to determine the extent to which it is reasonable, realistic, and consistent with the technical proposal. The lowest price for award purposes will be the combined total of the effective period of five (5) years from the date of award. Price shall include all CLINs as listed on the Price Schedule. The Government intends to evaluate proposals and award a contract without discussions. Therefore, Offerors are instructed that submitted proposals should contain their best pricing.
(d) FAR Clause 52.217-8, Option to Extend Services: As part of price evaluation, the Government will evaluate its option to extend services (FAR Clause 52.217-8 - Option to Extend Services) by adding one-half of the Offeror’s final option period prices to the Offeror’s total price. Thus, the Offeror’s total price for the purpose of evaluation will include the base period, 1st option, 2nd option, 3rd option, 4th option, and one-half of the 4th option. Offerors are required only to price the base and four (4) options. Offerors shall not submit a price for the potential one-half year extension of services period.
(e) Efficient Competition: Non-Price factors, when combined, are more important than price. However, vendors are advised that the Government may NOT evaluate the technical proposals of all vendors under this RFP. The Government will first review the total evaluated price of all proposals received. The technical proposals of those vendors whose pricing is determined by the Contracting Officer to be most competitive may be reviewed prior to, or instead of, other technical proposals received. Based on the initial review of these technical proposals, the Government may not evaluate the technical proposals of other Vendors, whose total evaluated pricing was higher than that of one already evaluated and already assigned the highest possible technical rating. This would occur, when the Contracting Officer determines that any possible technical superiority of an unevaluated (and higher priced) technical proposal, over (a lower priced) one that was already evaluated and assigned the highest possible technical rating, would not warrant any additional price premium.
------------------------- End of Addendum to FAR 52.212-2, Evaluation-Commercial Items (Jan 1999) ------------------------
(b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).
(c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
(End of provision)
The following have been modified:

During the life of this contract some or all of the contract requirements may be reduced due to the planned relocation of U.S. Forces from Yongsan to Camp Humphreys (Pyongtaek) starting in 2017. This will affect the overall square footage for performance. The contract will be adjusted accordingly for any increase or decrease to square footage.
