QUALITY ASSURANCE SURVEILLANCE PLAN (QASP) for Deluge System Bldg.699 at U.S. Army Aberdeen Test Center (ATC)

1. PURPOSE

This Quality Assurance Surveillance Plan (QASP) is developed in accordance with FAR 46.103 to establish the procedures for monitoring, reporting and evaluating the performance of the contractor under W91CRB-15-P-XXXX. The intent is to ensure that the Contractor performs in accordance with the Performance Work Statement (PWS) set forth in the contract, that the Government receives high quality of services, and that the Government only pays for the acceptable level of services received.

2. AUTHORITY

Authority for issuance of this QASP is provided under FAR Part 46, “Quality Assurance” and FAR 52.212-4, “Contract Terms and Conditions-Commercial Items”, which provides for inspections and acceptance of the articles, services, and documentation called for in this contract. This acceptance is to be executed by the Contracting Officer or his duly authorized representative.

3. ROLES AND RESPONSIBILITIES

The following personnel shall oversee and coordinate surveillance activities.

Contracting Officer (KO) – The KO will ensure performance of all necessary actions for effective contracting, ensure compliance with the contract terms, and shall safeguard the interests of the United States in the contractual relationship. The KO will also ensure that the contractor receives impartial, fair, and equitable treatment under this contract. Determine the final assessment of the contractor’s performance.

Assigned KO: Tom Marousek Organization or Agency: CCAP-SCD Telephone: 410-306-2187 Email: Thomas.j.marousek.civ@mail.mil

Contracting Officer’s Representative (COR) The COR is responsible for providing continuous technical oversight of the contractor’s performance. The COR uses the QASP to conduct the oversight/surveillance process. The COR will keep a Quality Assurance file that accurately documents the contractor’s actual performance. The purpose is to ensure that the contractor meets the performance standards contained in the contract. The COR is required to provide a performance assessment to the KO at the end of the period of performance, which will be used in documenting overall performance. The QASP is the primary tool for documenting contractor performance. The COR is not empowered to make any contractual commitments or to authorize any contractual change on the Government’s behalf.

Assigned COR: Richard Dolly Organization or Agency: ATEC

6. METHODS OF QA SURVEILLANCE

The below listed methods of surveillance may be used in the administration of this QASP. The 100% inspection method will be the most frequent method used as there is a direct comparison between the inspection report provided by the contractor and the work performed. In addition to specific instructions that are mentioned, the appropriate and standardized form that is to be used for documentation of QA surveillance is the “Surveillance Checklist for Contractor” at the end of this

QASP.

a. 100% Inspection - This level of inspection will be accomplished by monitoring and documentation. Upon completion of maintenance and/or repair services, the COR will inspect designated performance tasks, review the generated documentation and enter summary results into the Surveillance Activity Checklist.

b. Periodic Inspection – The COR will conduct periodic inspections on a routine basis (daily/weekly).

7. LEVEL OF PERFORMANCE EVALUATION.

The contractor shall be evaluated in the areas listed below (the checklist table of performance evaluation is provided at the end of this plan):

a. Customer Service. Contractor personnel shall be courteous, responsive, and responsible at all times.

b. Technical Performance. The contractor will be evaluated as to the quality of the output of their work and results will be documented in the COR’s file. The contractor's personnel shall be technically competent in the tasks identified in the PWS. Included in the technical performance is the contractor’s overall quality of the technical support provided.

c. Schedule and Timeliness of Deliveries. The contractor shall submit their technical report and Piping & Instrumentation Diagram as required by the contract.

g. Safety, Security, Environmental. The contractor shall be in compliance with Federal laws, DOD, APG and ATC SOP and regulations.

8. FEEDBACK TO CONTRACTOR. The COR will conduct meetings as necessary with the contractor to provide feedback on their performance and/or or about any other contractual matter. The COR will summarize and document QASP results and provide to the contractor during the meeting.

The COR will document everything in writing and report unacceptable performance to the ATC Contracts branch. Prompt feedback is essential so the contractor can develop and implement a corrective action plan. Issues and concerns that cannot be resolved at the working level or which impact on contractual requirements shall be reported to the KO for resolution and contractual action.
