STATEMENT OF WORK
FOR
REPAIR ASPHALT PAVEMENTS BY SEALING
WYTD192001
AT
180TH FIGHTER WING
GENERAL PROJECT INFORMATION
PART I GENERAL May 2, 2019
1.01 SCOPE
Contractors to furnish all labor, materials, equipment, management and other items of expense required to perform the work in strict accordance with the specifications and drawings for this project.
1.02 LOCATION
Site of the proposed work is at the Ohio Air National Guard Base, 180th Fighter Wing, 2660 S. Eber Road, Swanton, Ohio 43558-9645 (Adjacent to Toledo Express Airport).
1.03 SCOPE OF WORK: Prepare asphalt paved surfaces and sealcoat all asphalt paving base wide. More specifically the work includes but is not limited to the following:
1.03.A Preparation of pavements:
1. Remove all vegetation from asphalt surfaces to be sealed. Clean and prepare cracks and joints per manufacturer’s instructions. Preparation includes routing, widening, cleaning, drying and any other procedure required by the manufacturer.
2. Clean pavement surfaces and joints to be sealed according to the seal coat manufacturer’s requirements prior to treatment. Prime oil spots as required by the manufacturer.
3. Remove or prepare painted lines on pavements to be sealed as required by the manufacturer.
1.03.B New Work:
1. Prepare and seal all cracks and open joints in asphalt pavements greater than 1/8” wide with a hot pour joint sealer that meets the requirements of ASTM- D6690 Type 1. Ensure all cracks are dry prior to application of the crack filler.
2. Cracks wider than 2” shall be cut out and repaired at full or partial depth to sound pavement or base. Cold patching is not an acceptable repair.
3. Remove damaged asphalt at areas indicated on the drawings and replace with new asphalt in same thickness as removed. Saw cut asphalt full depth at the perimeter of the patch. Provide new 2” surface and 2” intermediate courses of asphalt per ODOT specification 448 for Type 1 Heavy Duty pavements. Fill potholes with hot mix asphalt.
4. Prepare and seal all asphalt pavements with a minimum of 2 coats of coal tar sealer as recommended by the manufacturer.
a. Coal Tar Concentrate shall be Sealmaster Coal Tar Concentrate Pavement Sealer or preapproved equal. Concentrate shall meet the requirements of ASTM D5727, ASTM D490, ASTM D3320-74T, FAA P627, FAA P628, FAA 629 and FAA EB44B specifications for coal tar pitch emulsions.
b. Coverage rate per coat of mixed coal tar sealer shall be a minimum of .13 gallons per square yard (7.77 sy/gal). Apply sealer in a uniform manner providing complete coverage. Areas that do not have complete and uniform coverage shall have an additional coat applied to the surfaces at no additional cost to the government.
c. Coal tar sealer shall be mixed with Masterseal Concentrate, water, sand and polymer additive per the manufacturers recommendations. First coat of sealer shall contain a minimum of 450 pounds of sand per 100 gallons of concentrate. Second coat shall contain not less than 300 pounds of sand per 100 gallons of concentrate.
5. Traffic markings: Remark all pavements to match existing. See attached traffic marking and parking plan for quantities. Provide traffic marking paints in the colors indicated. Traffic marking paint shall be an acrylic waterborne paint that meets the requirements of Federal Specification TT-P-1952B. Minimum thickness of markings is 8.5 mils (dry) unless otherwise noted. See pavement marking plan for existing markings to be repainted.

4. Phasing: The project shall be accomplished in phases as described below. The contractor awarded the work, may offer an alternate phasing plan for accomplishing the work. However, the OANG is under no obligation to accept the contractor’s plan.
a. Phase 1: Crack filling, pothole repair and asphalt replacement. This work shall be accomplished during normal work hours Monday through Friday.
The contractor shall provide for approval, a plan for accomplishing this work.
The plan shall incorporate the following requirements.
i. The approved plan shall incorporate a work sequence that does not shut down more than one lane of traffic on main base roads at any one time and allows for at least one uninterrupted entrance/exit between the parking apron and the main base.
ii. The contractor shall provide all required cones, barricades, personnel, etc… as required to provide protection of new work and government personnel and equipment until new materials are sufficiently cured so as not track or remove materials from the repaired area.
iii. Provide barricades/cones to block off parking areas and roads as needed to keep traffic off the affected areas so the work can be accomplished.
b. Phase 2: Seal coat Southgate road from Eber Rd to the north side of the driveway to bldgs. 155 & 158 (Base Bid and OBI #1), and Triple Nickel Rd. (OBI #3). This work shall occur on a Friday, Saturday or Sunday.
c. Phase 3: Seal coat the Munitions Storage Area and the drives and parking lot into the MSA from Southgate Road. This work shall occur on a Friday, Saturday or Sunday. OBI #2 may also be accomplished during this phase at the contractor’s discretion.
c. Phase 4: Seal coat Cannonball Run Road (Base bid) and the north portion of Southgate road (OBI#2). This work can be accomplished during the week with a seven day notice to the contracting officer.
Provide barricades of the roads at all intersections, parking lot and drives until sealer is cured sufficiently to permit traffic.
d. Phase 5: Striping/Pavement Markings: This work shall be accomplished weekdays during normal working hours. Coordinate shutdown of parking areas with the contracting officer or his representative. Pavement markings can be accomplished as soon as the sealcoat in any area has been properly cured.
1.03.C General:
1. Field verify all dimensions and conditions.
2. Provide all barricades necessary to isolate work areas from general traffic.
3. Provide hazardous material listing and MSDS per MATOC specifications.
4. Environmental Conditions: Adhere to manufacturer’s specifications for all products.
5. Provide a minimum 7 day notice prior to closing off any roads, parking lots or drives.
6. Bid items: The contractors bid shall be broken out as described below and as shown on the drawings and includes all required cleaning, preparation, crack filling, asphalt repairs, sealcoating and pavement markings.
a. Base bid: Southgate Road, Munitions Storage Area including drives and parking lot and Cannonball Run Road.
b. Optional Bid Item # 1 (OBI#1): Southgate Road past the intersection at Triple Nickle Rd. to the north side of the driveway to Building 155 drive to Northgate Rd (Worked along with Phase 2).
c. OBI#2: Southgate road from the drive to building 155 to Northgate Rd. (This will not be worked as part of phase 2)
d. OBI#3: Triple Nickel Road (Worked along with Phase 2).
e. OBI#4: Provide a unit cost for additional removal and replacement of 4” of asphalt as described in paragraph 1.03.B.3. The price shall be based on an 80 square yard section (12’ wide x 60’ section). This work may be separate or continuous with other indicated sections shown on the drawings. Additional repairs shall be accomplished at the same time as those indicated with the base bid. Price shall be in $_____/per square yard (SY).
f. OBI#5: Provide a unit cost to prepare and seal pavements according to this statement of work. Price shall be in $_____/per square yard (SY).
Minimum quantity to be added is 200 SY. Additional quantities will be done concurrently with phase 2, 3 or 4 or as a separate phase if a large enough area is added.
g. OBI#6: Provide a unit cost for additional crack filling. Price shall be $_____/lineal foot. Assume an average crack is ½” wide by ½” deep.
Minimum quantity to be added is 1000 lf. Additional quantities will be done concurrently with phase1, 2, 3 or 4 or as a separate phase if a large enough area is added.
1.03.D. Submittals: Submit for approval 3 copies of the listed items.
1. Crack filler
2. Seal coat materials
3. Asphalt mix designs for asphalt repairs.
4. Traffic marking paint
1.03.E. Drawings:
1. Phasing and Pavement Repair Plan
2. Pavement Marking Plan.
1.04 TOURS OF THE PROJECT SITE
Site visits will be scheduled in advance with Base Civil Engineering during the bidding period at (419) 868-4007, between the hours of 8:30 AM and 4:00 PM, Monday through Friday.
1.05 PERFORMANCE
Submission of a bid by a Contractor shall be accepted as prima facie evidence that he/she has examined the specifications and drawings and has satisfied him/herself as to the nature and location of the work and all matters which can in any way affect the work or cost thereof under this contract.
1.06 CONTRACT DOCUMENTS
Drawings and specifications cited herein are available from the Contracting Officer. Record drawings of existing facilities and utilities are available for review at the Office of Base Civil Engineer.
a. The intent of the drawings and specifications is to provide the Government with a complete and usable facility as indicated.
b. Any item mentioned in the specifications and not shown on the drawings or shown on the drawing and not mentioned in the specifications, shall be of like effect as if shown or mentioned in both.
c. In the case of differences between the drawings and specifications, the specifications shall govern. In the case of discrepancy in the figures in the drawings, or the specifications, the matter shall be promptly submitted to the Contracting Officer's Authorized Representative, who shall make a determination in writing. Any adjustments by the Contractor without such determination shall be at his/her own risk and expense.
The drawings are intended to define an outline of the concept of the work to be accomplished and provide general locations of all items. The Contractor is ultimately responsible for field verifying the exact location.
1.07 APPLICABLE PUBLICATIONS
a. Various references are listed throughout these specifications. Such publications are incorporated into these specifications by reference. Materials and installations are to conform to the requirements of the specified applicable publications. The edition of each publication to apply is to be the most recent edition published prior to the date of the bid opening, unless stated otherwise where the actual reference is made. In case of conflict between the referenced document and the project specifications, the specifications shall govern.
b. Contractor shall insure that work performed is in full compliance with all federal, state and local laws and ordinances applicable to the construction of or the finished product of this contract.
1.08 MANUFACTURER'S INSTRUCTIONS
Where required in the specifications, the materials, products, processes, equipment or the like to be installed or applied, in accordance with manufacturer's instructions, directions, or specifications, or words to this effect, it shall be construed to mean that said installation or application shall be in strict accordance with printed instructions furnished by the manufacturer for use under conditions similar to those at the project site.
1.09 MATERIALS AND PRODUCTS
Locations where products are listed by brand names or equal (equivalent), are for informational purposes only, establishing a general standard of quality, and the Government in no way is making a warranty of availability or fitness of the product for use.

1.10 DIVISION OF WORK
a. The Contractor is responsible for all work. Should the Contractor award certain portions of the work to subcontractors, the division of work, the supervision of work, coordination of the work, and resolution of subcontractor disputes is entirely the Contractor's responsibility.
b. The Contractor shall locate the principal lines and grades for all construction and establish bench marks based on the elevations shown on the drawings.
c. The Contractor shall be responsible for the proper laying out of all work and for any damage which may accrue as a result of his/her inaccuracies.
1.11 CONTRACT PERIOD
The duration of this project shall be 45 days. However, work shall proceed in a continuous manner once started.
1.12 CLEANING REQUIREMENTS
a. Maintain the project site in an orderly condition that allows maximum access, does not impede traffic insures safety, and protects materials and equipment.
b. On a daily basis the Contractor shall remove all scrap, debris, and wastes from the project site or place it in the Contractor's approved containers daily and removed from the job site at least once each week. Properly dispose of all waste to an off-site location.
In no case shall construction waste be placed in Government leased waste containers.
c. Mud, clay, stone, or other materials dropped by vehicles on paved surfaces shall be removed promptly and shall not be allowed to accumulate. Pavement areas that have been dirtied shall be swept broom-clean.
1.13 QUALITY ASSURANCE
a. Workmanship shall be in accordance with the best practices of the trade. All work shall be installed under direct supervision of a competent foreman.
b. Material and equipment shall be the standard products of a manufacturer regularly engaged in the Manufacture of the products. Items of equipment shall essentially duplicate equipment that has been in satisfactory use at least 2 years prior to bid opening.
1.14 ITEMS AND UTILITIES FURNISHED BY THE GOVERNMENT
Reasonable amounts of electrical power, water and sanitary sewer service shall be provided by the Government at no charge to the Contractor. The Contractor shall be responsible for the installation and removal of any connections required to accomplish this work.

1.15 SUBMITTALS
Submittals shall be in the form of data sheets, drawings, and/or samples as appropriate for the item. Three copies of each submittal shall be supplied to the BCE or his representative for approval. One Copy will be returned to the Contractor once reviewed by the government, and approved or disapproved.
1.16 WARRANTY
a. Materials, which do not have a guarantee stated elsewhere in these specifications, shall be guaranteed for a period of one year or as manufacturer's warranty states, whichever is longer.
b. Installation shall be guaranteed for a period of one year. Defective work shall be repaired or replaced as necessary during the guarantee period at the Contractor's expense.
1.17 DELIVERY, STORAGE, AND HANDLING
a. Materials shall be received, checked, and handled by the Contractor. The Government will not receive shipments of materials for the Contractor.
b. The Government is not responsible for security or loss of tools, equipment, or materials stored at the project site. The Contractor shall be responsible for the security and protection from damage of tools, equipment, and materials, which are used in the project.
c. Storage of materials will be in an approved area adjacent to the job site.
1.18 COORDINATION
Notify the Base Civil Engineer or his representative, 72 hours prior to the desired time for performing the work.
1.19 INSTALLATION
a. Work at the site shall be accomplished during normal day working hours of 7:00 AM to 4:00 PM, Monday through Friday. Hours other than normal shall be requested in writing and approved by the Base Civil Engineer or his representative.
b. When it is required to interrupt any service in order to make connections thereto, or for any other reasons, a written request to interrupt service shall be submitted to the BCE at least 2 working days prior to the anticipated interruption. The request shall state the date, time of day, and duration of the time of interruption.

1.20 PROTECTION OF PROPERTY
a. The Contractor shall not damage any area other than those so designated for this project.
The Contractor shall protect the buildings and adjacent equipment from any physical damage, which may be caused by the performance of work under this contract and shall repair all damages so caused at no cost to the Government.
1.21 FIRE PROTECTION
a. Any use of open flames shall be requested through the BCE for approval and to get an open flame permit issued.
b. Contractor personnel will adhere to posted "No Smoking or Open Flame" areas.
c. The Contractor shall provide appropriate type and size fire extinguisher at the job site during all working times.
1.22 BARRICADES AND TRAFFIC CONTROL
The Contractor will be responsible for barricading of the work area and shall follow all restrictions posted on base. A continuous barricade will be replaced around all unattended excavation.
1.23 SAFETY AND ENVIRONMENTAL PROTECTION
a. The Contractor will perform in a safe and health conscious environment which, within the limits of controllable hazards will: Protect the life, health, and physical well being of ANG and Contractor personnel during their work on ANG projects. Prevent contamination of property, supplies, and equipment.
b. The Contractor shall immediately notify and promptly report to the BCE or his representative any accident, incident or exposure resulting in fatality, disabling occupational injury or disease, contamination of property, or property loss arising out of work performed under this contract.
c. The Contractor shall take all necessary precautions for the prevention of environmental pollution as a result of construction operations under this contract. For the purpose of these specifications, environmental pollution is defined s the presence of chemical, physical, or biological elements or agents which adversely affect human health and welfare.
d. The Contractor is subject to all applicable Federal, State and Local Laws, regulations, ordinances, codes, and orders relating to safety and health, and environmental. All references shall be the latest edition, amendment, and/or revision of such reference standard in effect as of the date of these specifications.

1.24 CONFINED SPACE
a. This requirement applies to work in confined or enclosed space such as storage tanks, process vessels, vacuum chambers, bins, boilers, vent or exhaust ducts, tunnels, pipe lines, sewer and utility manholes, and open top spaces, such as trenches, pits and vaults.
b. The Contractor shall instruct his/her employees as to the hazards and the precautions to be taken.
c. The Contractor shall notify the BCE of his plan and schedule for working in a confined space.
d. The Contractor shall employ the buddy system. No person shall be allowed to work alone in a confined space, without a buddy in close proximity outside of the work area.
A lifeline shall be attached to the person inside when required by specific environmental conditions.
