However, the signed copy of the notarized letter must be sent to the GSA Federal Service Desk (FSD) within 30 days of activation or the vendor risks no longer being active in SAM.

Vendors can check whether an account is active by performing a query by their CAGE or Unique Entity Identifier (formerly known as DUNS). The new registration process may now take several weeks, so vendors are highly encouraged to begin registering as soon as possible to avoid any possible delays in future contract awards.

Remember, there is no cost to use SAM.

To find out additional information about the changes of the SAM registration process, contractors should visit the Frequently Asked Questions (FAQ) link located at the top of the SAM homepage (www.sam.gov).

Performance Work Statement (PWS):

1. General. The work will involve stabilizing the reservoir bank by re-sloping an area of shoreline, installing a toe barrier, adding, and compacting topsoil, drilling seed, spreading straw, overlaying and placing new sections of rip rap, and creating pedestrian accesses to the water. This work shall be accomplished though different line items and will be split between two separate sections of the same bank named Area #1 and Area #2 in Axtel Campground.

A. Area #1 (Exhibit A):

1. Re-sloping approximately 250’ of shoreline with low impact equipment to no more than a 2:1 slope:

a. Re-slope and grade an area approximately 250 ft x 50 ft to remove current cutbank and large loose rocks to create a smooth grade. Only tracked equipment will be used to reduce soil disturbance in the area.

b. Riprap toe anchor 2 ft in depth x 4 ft in width will be installed along the shoreline. This toe anchor will be approximately 200 ft. in length to catch any sedimentary material and retain it on the bank. This toe anchor will run along the summer pool line with the bottom edge of the toe being elevation 495 ft. The rip rap toe shall tie into the existing rock features upon the termination at the beginning and the end. See Exhibit C for an example of a rock toe drawing.

c. The contractor will add 24 inches of topsoil and compact the area to bring the slope back to an adequate grade that blends into existing topography. Once the topsoil is in place and compacted, all disturbed areas will be slightly agitated to a maximum depth of ½ inch, cleaned of all clumps and debris, and ready to seed.

d. Drill a 50/50 mixture of Kentucky 31 fescue combined with a shade tolerant grass seed mix into newly prepared seed bed and all other disturbed areas. Cover all disturbed areas with straw mat to reduce erosion. Straw mat must be biodegradable.

e. Refresh existing riprap areas shown on Exhibit A by adding new riprap to create a smooth uniform surface. The riprap in these areas should not exceed 18 inches in depth.

B. Area #2 (Exhibit B):

2. Refresh and extend current sections of Rip rap below sites 112, 113, and 114 in Axtel Campground to stabilize the shoreline.

a. Re-slope and grade an area approximately 200ft x 10ft to remove current cutbank and large loose rocks to create a smooth grade. Only tracked equipment will be used to reduce soil disturbance in the area.

b. Add riprap to the same section of shoreline mentioned in (a) above, approx. 200ft long and 25ft wide.

c. Create 4ft wide pedestrian access through the riprap to the water from campsite 113 and 114. This area should be constructed from 2-4 inches of compacted dense grade gravel. Install treated 4x4 water bar every 20 ft to reduce erosion. 4x4 will be anchored to the ground with 18-inch cross tied rebar pins on both ends. Water bars will be flush with the final surface of the compacted dense grade and will extend across the entire width of the pathway. The slope of the newly created access will match slope of the existing topography and the pathway will be built in a manner which is suitable for easy walking and will not easily erode.

TP-3.1 GENERAL SPECIFICATIONS:

d. Spread or drill native grass seed mix into all disturbed areas. Cover all disturbed areas with straw or straw mat to reduce erosion. Straw mat must be biodegradable.

Considerations:

1. The Technical POC must be contacted prior to beginning work to allow access into the area.

A minimum of 24 hours’ notice must be given to the technical POC to gain access to the area.

2. All staging areas must be left in the same condition prior to use and delivery of any products.

Any apparent degradation from use because of this project will be restored back to the previously disturbed condition.

3. Disturbed Areas Outside of Shoreline Stabilization Work Area: All disturbed areas shall be seeded with a 50/50 mixture of Kentucky 31 fescue combined with a shade tolerant grass.

Upon completion of the seeding the entire disturbed area shall be covered with straw.

4. All work shall be done at or below elevation 485 MSL.

5. All rock utilized shall be sound, dense, durable, and free of toxic pollutants in accordance with the KY DOT.

6. All work must be completed prior to December 31 2023.

2. Dimensions. The contractor should field verify dimensions, etc. prior to submitting a quote. The dimensions listed in the specification herein are approximate and should not be considered completely accurate.

3. Submittals. The Contractor submittals shall be furnished in accordance with the specifications. All listed items shall be submitted for Government approval by the TPOC. Associated work items shall not be performed until receiving approval from the TPOC. The TPOC shall provide a response to the submittals within 15 calendar days of receipt.

a. Accident Prevention Plan (APP)

b. Activity Hazard Analysis (AHA)

c. Project Schedule

4. Waste Disposal. The contractor will remove all demolition debris and left-over construction debris to a site licensed to receive/recycle construction debris. The contractor is responsible for any waste disposal and any permits and fees associated with disposal.

5. Permits. The contractor is responsible for obtaining any construction permits, conducting any tests or inspections, and paying any associated fees necessary for this contract. All work shall be coordinated through the TPOC, Adam Taylor, 270-257-2061.
