WORK SCOPE
Scope of Work:
The scope of this project is to provide a review of contractor certifications, compliance with regulations during abatement, air monitoring throughout the asbestos abatement and to provide a summary report include clearance certification. Daily logs will be submitted along with all chains of custody and sampling results as outlined in the regulations. The Scope of this Contract will encompass all areas of the Canandaigua VA Medical Campus for both Phase I and Phase II of the CARES Renovations and New Construction at VA Medical Center Canandaigua, NY project.
A CIH or Professional IH must oversee the work. A Professional IH is an IH who meets the definition requirements of AIHA; meets the definition requirements of OSHA as a "Competent Person" at 29 CFR 1926.1101 (b); has completed two specialized EPA approved courses on management and supervision of asbestos abatement projects; has formal training in respiratory protection and waste disposal; and has a minimum of four projects of similar complexity with this project of which at least three projects serving as the supervisory IH. To afford more flexibility with clearance sample turn-around-time in instances when PCM sampling is used, the IH Project Monitor to have the NIOSH 582 equivalency course will allow for PCM samples to be interpreted immediately on-site.
The contractor is to act as the supervisory IH on site. A PIH is acceptable in meeting this intent. This role is to be filled by the contractor’s PIH and not the VA’s PIH under this contract. The PIH on site needs to be a NYS certified project monitor.
The CIH will; assure quality; adherence to the specification; resolve problems; prevent the spread of contamination beyond the regulated area; and assure clearance at the end of the project. In addition, work includes performing the final inspection and testing to determine whether the regulated area or building has been adequately decontaminated. All air monitoring is to be done utilizing PCM/TEM. The CIH will perform the following tasks:
1. Establish background levels before abatement begins by collecting background samples. Retain samples for possible TEM analysis.
2. Perform continuous air monitoring, inspection, and testing outside the regulated area during actual abatement work to detect any faults in the regulated area isolation and any adverse impact on the surroundings from regulated area activities.
3. Submit to the COR a daily log at the end of each day which includes, but is not limited to the following:
Project Name, Project number, Dates, Site activity overview, time log of events with notes, list of all personnel on site along with confirmation of certifications, list of all personnel who visit the site, and weather conditions, site pictures, and issues of note.
4. Perform unannounced visits to spot check overall compliance of work with contract/specifications. These visits may include any inspection, monitoring, and testing inside and outside the regulated area and all aspects of the operation except personnel monitoring.
5. Provide support to the USACE representative such as evaluation of submittals from the Contractor, resolution of conflicts, interpret data, etc.
6. Perform, in the presence of the USACE representative, final inspection and testing of a decontaminated regulated area at the conclusion of the abatement to certify compliance with all regulations and USACE requirements/specifications.
6. Issue certificate of decontamination for each regulated area.
7. Provide immediate (within four hours of receipt) written and phone notice to the COR(s), USACE IH, and
VA Infection Control regarding any sampling results that do not meet regulations. Work with the COR(s) and contractor to mitigate the issue and plan way forward.
7. Attendance of project meetings are required throughout the project at the CORs request.
9. Provide 50% of samples at an expedited turnaround (24 hours) and 50% of samples with a three-day max turnaround. Coordinate with the COR(s) as needed to verify which samples need to be expedited.
Code requirements:
All abatement will be in compliance with NYSCR 56. The abatement will also be in accordance with all applicable codes including Veterans Affairs standards which can be found at https://www.cfm.va.gov/til/. This will include, but not be limited to: Department of Veterans Affairs Design Manuals, VA Publications, and Construction Standards, USACE EM 385-1-1, National Fire Protection Code (NFPA), Life Safety Code, Underwriters Laboratory (UL), International Building Code IBC 2003, and the National Electric Code (NEC).

Prior to any work commencing in an area, the contractor shall notify the COR of his intention to work in the area and shall schedule a walk-through with the COR.
The contractor shall receive all deliveries of materials, equipment, tools, or other goods off station, and bring them to the job site. The Canandaigua VA shall not accept deliveries for project purposes.

The contractor shall not proceed with any on-site work until the completion of the submittal process or approval to proceed from the CO and COR without completion of the submittal process. Not less than 14 calendar days shall be planned for COR review of each submittal. Re-submittals shall be provided as required by the COR, and shall be treated as a new, separate submittal.
The CIH shall determine if work is in accordance with applicable laws and regulations and report discrepancies or deficiencies to the Contracting Officer’s Representative immediately. Should the CIH identify conditions with the abatement that endanger workers, VA Medical Center staff or patients, or other persons on the campus, the CIH shall immediately notify the COR with recommended courses of corrective actions to include shutting down the abatement effort.
