1. Description of Services. The Contractor shall supply refuse dumpsters, remove refuse, clean, service and maintain sanitary conditions at all refuse dumpsters in accordance with specifications, drawings and schedules contained herein. The Contractor shall remove all refuse collected from Government property and properly dispose of materials in accordance with Federal, State, and local regulations and commercial standards. The contractor’s performance will be based on the Contracting Officer’s (KO), Contracting Officer’s Representative (COR), and Quality Assurance Personnel (QA) evaluations and the results required by the Service Delivery Summary (SDS).
The evaluation of results will be based on COR based on COR performance assessment, tenant satisfaction, and customer complaints. Final results of the evaluations will be the determining factor for the success or failure of this contract. See Appendix C & D for the number and types of facilities and schedule of services for each area, which are for reference only.
1.1. Dumpster Services
1.1.1. Dumpster Placement. Refuse dumpsters shall be delivered and placed in areas specified in Appendix B by the first morning of the contract period. Dumpsters shall remain in place until the last scheduled service in the area, the end of the contract period, or until notified to be removed by the Contracting Officers Representative. Per the schedule, if a dumpster is no longer on the service schedule (Appendix D) and is not removed from an open area then the contractor shall continue services for that dumpster at their own expense.
1.1.2. Service Refuse Dumpsters. The Contractor shall remove all refuse from refuse dumpsters as well as maintain sanitary conditions in and around each dumpster.
1.1.3. Area Cleanup. The Contractor shall remove all refuse on and within ten feet (10’) of the surface of the dumpster pad and shall place it in the refuse truck each time a refuse removal service is performed.
The Contractor shall replace and center all dumpsters within the designated area and shall insure that all container lids are properly closed.
1.1.4. Landfill Receipts. Official signed and dated landfill receipts shall be submitted with the Contractors monthly submittal of invoice for payment, showing that all refuse collected from Government containers at Rivers Project Office Facilities was deposited at a legal landfill.
1.1.5. Refuse Dumpster Movement. “General Dumpster Locations” are shown in Appendix B but may be moved within service areas, with Government approval, to accommodate government requirements.
The Contractor shall insure that dumpsters remain in their proper locations. In the event dumpsters are moved, the Contractor shall return them to their proper location at each regularly scheduled service or within 24 hours after notification by the Contracting Officers Representative. The Contractor shall collect and dispose of refuse spilled on both land and water due to a dumpster’s unauthorized movement when the dumpster is returned to its proper location. Dumpsters located within the flood pool elevations of Rivers Project Office Facilities shall be moved by the Contractor to a safe location due to rising lake levels within 6 hours of being notified by the COR. The Government reserves the right to move and relocate the Contractors dumpsters during flooding conditions to accommodate requirements or construction activities. The Contractor shall return the dumpsters to their designated locations as soon as the water levels drop to safe levels when notified by the Contracting Officers Representative.
1.2. Temporary / Special Event Services
1.2.1. Temporary/ Special Event Services. The Contractor shall provide additional dumpsters, of which size will be dictated by the government, for temporary services and special events held on the project.
The Contractor will be notified of specific quantities, schedules of services, and locations no less than seventy-two (72) hours prior to delivery date of additional dumpsters. The number of required temporary/special event dumpsters will not exceed ten at any given time. NOTE: Dumpster service schedule may be daily during special events.
2. Service Delivery Summary (SDS). The contract service requirements are summarized in performance objectives that relate directly to requirements. The maximum number of defective items in a lot that can be considered satisfactory is called the performance threshold. The performance threshold for each service to be performed can be found in the chart below. When the number of defects for a specified service (or lot) is below the performance threshold, the work will be considered satisfactory. However, the Contractor is still required to correct the deficiencies if possible. Each service that is performed will be determined acceptable or deficient according to the inspection rating that is received. The SDS and the contractor’s Quality Control Plan provide information on contract requirements, the expected level of contractor performance and the expected method of government validation and confirmation of services provided.
Performance Objective Performance Threshold
Method of Assessment Remedy
See Description of Services 1.0 through 1.3.1
No more than zero (0) defects per month

QUALITY ASSURANCE PLAN
INTRODUCTION
This Quality Assurance Surveillance Plan (QASP) has been developed to evaluate contractor actions while implementing this PWS. It is designed to provide an effective surveillance method of monitoring contractor performance for each listed objective on the Performance Requirements Summary (PRS) in the maintenance contract.
The QASP provides a systematic method to evaluate the services the contractor is required to furnish.
This QASP is based on the premise the Government desires to maintain a quality standard in operating, maintaining, and repairing facilities and that a service contract to provide the service is the best means of achieving that objective.
The contractor, and not the Government, is responsible for management and quality control actions to meet the terms of the contract. The role of the Government is quality assurance to ensure contract standards are achieved.
In this contract the quality control program is the driver for product quality. The contractor is required to develop a comprehensive program of inspections and monitoring actions. The first major step to ensuring a “self-correcting” contract is to ensure that the quality control program approved at the beginning of the contract provides the measures needed to lead the contractor to success.
Once the quality control program is approved, careful application of the process and standards presented in the remainder of this document will ensure a robust quality assurance program.

Supply and Deliver Requested Dumpsters
Contractor shall supply and deliver dumpsters of requested type and size per the Government’s request.
Sec.
3.0
100% of requested dumpsters are supplied per Government’s request.
SURVEILLANCE: The Contracting Officer’s Representative (COR) will receive complaints from Government personnel and pass them to the contractor's quality control inspector (QCI) for correction.
STANDARD: 100% of all facilities are without customer complaints for the month or service period. For example, if they are 100 facilities receiving service during the month, the COR should receive no more than 5 customer complaints during the service period.
