1.0 GENERAL INFORMATION
The Don Clausen Fish Hatchery is located in northern Sonoma County, approximately 15 miles northwest of Healdsburg, California. The hatchery is part of the U.S. Army Corps of Engineers’ (San Francisco District) Warm Springs Dam / Lake Sonoma Project. The fish hatchery is situated near the base of the dam and is operated by the California Department of Fish & Game (CDFG). A site plan is provided on Plate 1.
The purpose of the work is to replace a damaged structural glu-lam beam and appurtenances within the main building to the Don Clausen Fish Hatchery. The beam, situated within the southeast entrance area, serves as a support member for the building’s upper (2nd) floor. The upper floor houses an interpretive center open for public education, while the lower floor is where Fish and Game personnel conduct fish rearing activities for the endangered Steelhead Trout and Coho Salmon. Directly below the worksite are a series of fish rearing tanks, which will need to be protected during work activity.
A damage assessment by the government found significant water damage (rot) to the glu-lam beam as well as flooring (plywood sheeting) and floor joists. Due to the unique working conditions present, it is strongly recommended that the contractor conduct a site visit before submitting any bid proposals.
2.0 DESCRIPTION OF WORK AND SERVICES
Repair work by the Contractor shall include site preparation and removal of all damaged components. The Contractor shall purchase, deliver, and install all necessary materials to essentially restore damaged/disturbed areas back to an as-built condition. Proposed design repairs are shown on Plate 2 - “FH-BEAM-REPAIR-328,” dated 11 November 2008.
Accordingly, the existing glu-lam beam (5” wide x 25” depth x 27’-4” long) shall be replaced with a new hot-dipped galvanized steel-tube beam (4” wide x 20” depth x 5/16” thick) of equal mailto:Shirley.A.Turnbo@usace.army.mil mailto:Fernando.Garcia@usace.army.mil length. Because the depth and width of the new steel beam are significantly less than the old wooden beam, the new beam shall be shimmed with steel spacers at the existing steel hangers.
Because of the presence of the fish rearing tanks on the lower floor, which are utilized year-round, the support beam will need to be replaced from the upper 2nd floor level. This effort will include the replacement of a triangular shaped exterior stud wall with double glass doors, damaged interior and exterior flooring, and support joists. Refer to Plates 3 thru 6 for as-built conditions.
2.1 Stud Wall. The stud wall shall be reconstructed with 2”x 4” Douglas Fir (No. 2 grade or better) members at 16” on center and include R-11 rated standard insulation between members. Secure stud wall sill plate to steel beam with drill/tap type anchors. Ram-set type anchors shall not be allowed. The interior shall be faced with ½” thick sheet rock, and the exterior faced with a stucco type finish. Surfaces will be trimmed and painted to match existing trim and finished surfaces.
2.2 Floor Joists. Replace all damaged floor joists with 4” x “10 size Douglas Fir members – No. 2 grade or better. Secure to new steel beam, as shown on design drawing (Plate 2), with drill/tap type anchors. Ram-set type anchors shall not be allowed..
2.3 Flooring and Floor Covering. Replace all damaged flooring with exterior rated 1- 1/8” tongue & groove pressure treated plywood sheeting. Replace damaged interior carpeting to match existing carpeting and reseal exterior flooring with a latex mastic deck covering to match existing deck covering.
As the total amount of weather damage is not known precisely, the US Army Corps of Engineers retains the right to further refine tasks as well as add new ones, and orders for additional needed materials and services.
3.0 SCHEDULE
The Contractor shall be required to a) commence work under this contract within 5 working days following official notice to proceed, b) prosecute the work diligently, and c) complete all work within 21 calendar days after receipt of notice to proceed. The time stated shall include final cleanup of premises. The contactor shall not begin the on-site work day earlier than 7:00 am and end later than 5:30 pm. Construction activity shall not unnecessarily hinder fish hatchery facility operations.
All work shall be coordinated with the Federal Government’s representative, or Contracting Officer’s representative (COR) – in the case of this job the project’s Facility Engineer. The Contractor shall fully inform the COR as to progress of work and dates when site work will occur. The Contractor shall notify the COR at least 48 hours prior to commencing work.
The Contractor shall provide a qualified on-site representative to perform quality control (QC). Quality assurance (QA) shall be the responsibility of the COR.
It is the Contactor’s responsibility to make all necessary arrangements with the COR for access to grounds and mobilization / storage of equipment and materials, as required.
4.0 GENERAL CONTRACT REQUIREMENTS
The Contractor shall take no direction from any government employee or any person other than the Contracting Officer that changes the terms and conditions of this contract action, the scope, or any change that impacts the price or schedule. Changes authorized by the Contracting Officer will be in the form of a written, official, signed modification to the contract action received by the Contractor before the Contractor will act upon the changes. The Contractor will comply with the changes clause of this contract when the Contractor believes direction has been given from persons other than the Contracting Officer that equate to a change by notifying the Contracting Officer. Any direction given by any government employee or any other person outside their authority must be reported to the Contracting Officer. Contracting Officer Representatives (CORs) are limited to the authority stated in COR appointment letters.
5.0 SAFETY

The EM is available at http://www.usace.army.mil/usace-docs/eng-manuals/em385-1-1/toc.htm. The Contractor should also be aware that work tasks which are deemed potentially hazardous to life or property, require the completion of activity hazard analysis (AHA) forms. Examples of AHA are available in EM 385-1-1. Work cannot begin until the Contractor’s safety plan and AHA(s) have been reviewed and approved by the Corps’ District Safety Officer or COR. The government reserves the right to stop all work activities at no additional cost to the government for work activities deemed unsafe by the COR or any other government official.
6.0 ENVIRONMENTAL PROTECTION

Central Contractor Registration (CCR) database means the primary Government repository for Contractor information required for the conduct of business with the Government.
Data Universal Numbering System (DUNS) number means the 9-digit number assigned by Dun and Bradstreet, Inc.
(D&B) to identify unique business entities.
Data Universal Numbering System +4 (DUNS+4) number means the DUNS number assigned by D&B plus a 4-character suffix that may be assigned by a business concern. (D&B has no affiliation with this 4-character suffix.)
This 4-character suffix may be assigned at the discretion of the business concern to establish additional CCR records for identifying alternative Electronic Funds Transfer (EFT) accounts (see the FAR at Subpart 32.11) for the same parent concern.
Registered in the CCR database means that--
(1) The Contractor has entered all mandatory information, including the DUNS number or the DUNS+4 number, into the CCR database; and
(2) The Government has validated all mandatory data fields, to include validation of the Taxpayer Identification Number (TIN) with the Internal Revenue Service (IRS), and has marked the record ``Active''. The Contractor will be required to provide consent for TIN validation to the Government as a part of the CCR registration process.
(b)(1) By submission of an offer, the offeror acknowledges the requirement that a prospective awardee shall be registered in the CCR database prior to award, during performance, and through final payment of any contract, basic agreement, basic ordering agreement, or blanket purchasing agreement resulting from this solicitation.
(2) The offeror shall enter, in the block with its name and address on the cover page of its offer, the annotation “DUNS” or “DUNS +4” followed by the DUNS or DUNS +4 number that identifies the offeror's name and address exactly as stated in the offer. The DUNS number will be used by the Contracting Officer to verify that the offeror is registered in the CCR database.
(c) If the offeror does not have a DUNS number, it should contact Dun and Bradstreet directly to obtain one.
(1) An offeror may obtain a DUNS number--
(i) Via the Internet at http://fedgov.dnb.com/webform or if the offeror does not have internet access, it may call Dun and Bradstreet at 1-866-705-5711 if located within the United States; or

(d) If the Offeror does not become registered in the CCR database in the time prescribed by the Contracting Officer, the Contracting Officer will proceed to award to the next otherwise successful registered Offeror.
(e) Processing time, which normally takes 48 hours, should be taken into consideration when registering. Offerors who are not registered should consider applying for registration immediately upon receipt of this solicitation.
(f) The Contractor is responsible for the accuracy and completeness of the data within the CCR database, and for any liability resulting from the Government's reliance on inaccurate or incomplete data. To remain registered in the CCR database after the initial registration, the Contractor is required to review and update on an annual basis from the date of initial registration or subsequent updates its information in the CCR database to ensure it is current, accurate and complete. Updating information in the CCR does not alter the terms and conditions of this contract and is not a substitute for a properly executed contractual document.
(g)(1)(i) If a Contractor has legally changed its business name, “doing business as” name, or division name (whichever is shown on the contract), or has transferred the assets used in performing the contract, but has not completed the necessary requirements regarding novation and change-of-name agreements in Subpart 42.12, the Contractor shall provide the responsible Contracting Officer a minimum of one business day's written notification of its intention to (A) change the name in the CCR database; (B) comply with the requirements of Subpart 42.12 of the FAR; and (C) agree in writing to the timeline and procedures specified by the responsible Contracting Officer. The Contractor must provide with the notification sufficient documentation to support the legally changed name.
(ii) If the Contractor fails to comply with the requirements of paragraph (g)(1)(i) of this clause, or fails to perform the agreement at paragraph (g)(1)(i)(C) of this clause, and, in the absence of a properly executed novation or change-of-name agreement, the CCR information that shows the Contractor to be other than the Contractor indicated in the contract will be considered to be incorrect information within the meaning of the “Suspension of Payment” paragraph of the electronic funds transfer (EFT) clause of this contract.
(2) The Contractor shall not change the name or address for EFT payments or manual payments, as appropriate, in the CCR record to reflect an assignee for the purpose of assignment of claims (see FAR Subpart 32.8, Assignment of Claims). Assignees shall be separately registered in the CCR database. Information provided to the Contractor's CCR record that indicates payments, including those made by EFT, to an ultimate recipient other than that Contractor will be considered to be incorrect information within the meaning of the “Suspension of payment” paragraph of the EFT clause of this contract.
(h) Offerors and Contractors may obtain information on registration and annual confirmation requirements via the internet at http://www.ccr.gov or by calling 1-888-227-2423, or 269-961-5757.
