The Thomas J. O’Brien (TJO) Lock and Dam is located on the Calumet River in the southeastern region of Chicago, and is operated by the US Army Corps of Engineers (USACE), Chicago District. The facility is a unit of the Inland Waterway Navigation System, and consists of a navigational lock, fixed dam, and controlling works. The address to the facility is 1831 East 130th Street Chicago, IL 60633. Figure 1 provides a general overview of the TJ O’Brien facility.

The general location of the required services within the facility is outlined in red.

Figure 1 –TJ O’Brien Lock & Dam Facility and general location of fuel tank storage system (red outline)

This service project consists of two detailed components (line items): (0001) includes the excavation, removal, cleaning, and disposal of an underground storage tank (UST) system from the parking lot at the facility, and (0002) includes the backfilling and resurfacing the pavement as required. All work shall be accomplished in accordance with all applicable codes, laws and regulations pertaining to this type of work within the jurisdiction area. This including state and federal regulations. A current state tank removal business license is required, including SAM.gov registration to perform these services.

4.A. Line Item 0001 (UST System Removal)

4.A.1. Summary. The UST system is located in the center of the paved parked lot, adjacent to the control house. The system includes the UST, the dispensing equipment to the generator, and all supporting components connecting the tank to the control house. The UST is a 1000 gallon, double walled, fiberglass tank, previously used to store diesel fuel the generator ISO facility operations. The UST was decommissioned in 2019. The tank is most likely empty of diesel fuel.

However, the Contractor must verify before beginning any excavation work by taking appropriate procedures to pump and dispose of any remaining fuel content throughout the system. The Contractor must have a valid CDPH Tank Removal Permit from the local regulatory offices to perform this specific service.

4.A.2. Excavation Area. The approximate area of excavation in the parking lot must include a minimum of 3 feet of offset from the approximated dimensions of the UST, to ensure the proper slope is maintained to support the removal of the entire UST system. The estimated area of excavation is 100 sf (+/- 20 ft.) up to a depth of 8 feet, or as dictated by existing site constraints.

The Contractor will validate the assumed area and volume of excavation, based on the verification of the location of the existing utilities and confirmation of regulated safety procedures. Specific procedures for excavation remain with the Contractor as Ways and Means.

Excavated materials and fill will be disposed of offsite by the Contractor and not used for purposes of refill. The excavation services will comply with all local, state, and federal regulations – as summarized in Section 2 of this SOW. These include relevant sections of 29 CFR 1910, 29 CFR 1926, and USACE EM 385-1-1.

Refer to Exhibit 1 - general location of the UST system and area of excavation in the parking lot.

4.A.3. Dimensions & Components. The approximated dimensions of the cylindrical UST are

4.5 feet in diameter and 12 feet in length. The base of the tank is approximately 7.5 feet below grade (road surface), and is supported by an estimated 500 cf of pea gravel backfill. The tank is secured to an underlying 72 sf reinforced, concrete pad (6 feet by 12 feet by 6 inch width) via tie down straps. Additional components of the UTS system for removal include the following parts:

a cast iron spill containment manhole with cover (24 inch diameter), FRP secondary containment pipes, the vent line, all connections/adapters, the monitoring pipe, and a 26 ft. fuel line running to the control house. The fuel line at the control house will be appropriately capped and/or sealed.

Refer to Exhibit 2 -UST Plans and Details (AS-BUILT-10 OCT 1989) for more specific dimensions and components of this system.

4.A.4. Cleaning and Disposal of Debris (UST and components). All excavated material will be safely cleaned and legally disposed of, in accordance with API-1604 and of Illinois Red Book Code, Title 41, Chapter #1, Part 175.83. The Contractor will provide the TPOC with documentation (proof) that all such materials are legally disposed of prior to the beginning of resurfacing work.

4.A.5. Required Coordinating Actions. Before beginning any work affiliated with this line item, the Contractor shall take all necessary precautions to ensure against damage to existing structures and facilities that are to remain in place. The Contractor will notify the Local Fire Administration Office at least 48 hours before UST removal work begins. The Government will not provide any utility details within the area work limits. The Contractor is responsible to locate all private utilities - which can include electric, water lines, and existing sewer lines. No excavation work shall be performed until existing utilities within the work limits are identified and marked, or determined to be non-present. Any damage to existing utilities, structures and facilities resulting from the Contractor’s operations shall be repaired by the Contractor at no cost to the Government by a licensed professional of that trade.

4.B. Line Item 0002 (Backfilling and HMA Resurfacing)

4.B.1. Backfill. The Contractor will provide clean soil material for backfill, and crushed stone aggregate as the base layer for resurfacing, in accordance with IDOT CA 6, class B classifications. For filling the excavation pit, the soil must be certified as clean fill, and will be compacted in six (6) inch lifts, from the bottom of the excavation, up to 18 inches below grade (defined as the existing road surface).

(a) A geo-fabric liner will be used to separate the excavation fill material from the base layer of the new road surface. The woven geotextile matting shall meet AASHTO / M288 standards for roadway separation and stabilization. The Contractor shall keep in possession, proof that matting meets such standards to provide upon request by Government Representative.

(b) The subsequent 18 inches will be used as the base layer for the asphalt road surface. This will consist of a uniformly, vibratory compacted layer of commercial grade crushed #53 limestone that meets AASHTO’s density standards to a minimum depth of eight (12) inches post compaction.

4.B.2. Resurfacing. Resurfacing pavement will be Hot-Mix Asphalt (HMA). The Contractor will ensure that the slope of the finished surface conforms to the surrounding parking lot, and maintains the minimum drainage/surface runoff requirements. The resurfacing design consists of the following components: three (3) inches of Hot Mix Asphalt (HMA) Intermediate, and three

(3) inches of Hot Mix Asphalt (Surface) on top of the compacted commercial grade crushed #53 Limestone base layer. The Contractor will use mill butt joints in order to tie into and be same height as adjacent asphalt surfaces. The Contractor shall seal all butt joints with liquid asphalt.

All joints between existing asphalt surfaces shall be smooth transitions. The Contractor will adhere to weather limitations set be the Illinois Dept of Transportation, as specified in Division 400, IDOT Standard Specifications for Road and Bridge Construction (2012). The Contractor is responsible for the removal all construction debris upon completion of the work associated with the line item.

Prior to beginning work, each employee will be trained to perform the task he or she has been hired to perform. This training will consist of a review and thorough explanation of the contract specifications and requirements, followed by ‘on-the-job’ training to ensure that the contract requirements are understood. All members of a crew will be cross-trained, so that they understand all aspects of a particular type of work. Following training, each employee will be provided with a checklist of work that they are required to perform, which will serve to ensure that all aspects of a particular job are completed on time. The employee will mark each item as it is completed (with completion time), and will sign and date each sheet to ensure accountability for completed work.

Prior to start of the contract, the Contractor will designate a Quality Control Inspector (QCI) in writing. The designated QCI will perform inspections of completed work as soon as possible to ensure that the work meets contract specifications. The inspection schedule will depend on the type of work being completed; items of a routine nature will be inspected based on a random or planned sampling schedule. 100% of unusual items, items which are more important, or highly visible items will be inspected.

Allowable reject levels for all items of work will be zero.

Upon discovery of any deficiency, corrective action will be initiated immediately. The QCI will immediately either correct the deficiency personally, direct another employee to correct the deficiency, or direct the employee who marked the item as ‘complete’ to correct the deficiency.

The QCI will meet with the employee who was responsible for initial completion of the work and review contract specifications for the work which was inadequately performed. The QCI will determine what actions caused the inadequate work and take immediate steps to ensure that the work is correctly performed the next time it is scheduled.

A Quality Control Inspection Report will be completed by the QCI for each day’s work. This report will document what work was completed, what facilities were inspected, the time the inspections were performed, and the results of these inspections. This report will also document any deficiencies noted during inspections and corrective actions taken on deficiencies. This report will be provided to the Corps of Engineers the same day the work is scheduled, or the next morning prior to 9:00 a.m. by e-mail, fax, or in person.

If a Contract Discrepancy Report (CDR) is issued by the government, the Contractor will review the CDR with the QCI within one day of receipt of the CDR, and will meet with the Corps of Engineers representative as soon as possible. Prior to meeting with the Corps representative, the Contractor will meet with the QCI and the individual/crew responsible for completing the work to determine the cause of the contract discrepancy and what actions will be required to prevent any reoccurrence. The Contractor will document these actions on the CDR for submittal to the government.

The contractor will modify these procedures as required to ensure that contract specifications are met.
