Request for Information

Kentucky Lock Downstream Lock Monoliths

The following information is requested from Contractors interested as a prime contractor in this project:

1. Firm's name, addresses, point of contact, phone number, and e-mail address.

2. Firm's interest in providing a bid on the pending solicitation once issued.

3. Experience: Evidence of capabilities to perform comparable work in the areas of mass concrete placements, structural concrete placements, concrete batch plant operation, fabrication of structural steel components.

4. Firm's capability to perform a contract of this magnitude and complexity.

5. Questionnaire comments concerning Project Labor Agreements (PLAs). See questionnaire below.

6. Firm's business category and business size (Large Business, Historically underutilized Business Zones (HUBZone), Women Owned, etc.)

7. Firm's Joint Venture information, if applicable.

8. Cage Code and Unique Entity Identifier (UEI, formerly known as DUNS) number

9. Bonding capacity

PLA Questionnaire

1. Do you have knowledge that a PLA has been used in the local area on projects of this kind? If so, please provide supporting documentation.

2. Are you aware of skilled labor shortages in the area for those crafts that will be needed to complete the referenced project? If so please elaborate and provide supporting documentation where possible.

3. Are you aware of time sensitive issues/scheduling requirements that would impact the rate at which this project should be completed? If so, please elaborate and provide supporting documentation where possible.

4. Identify specific reasons why or how you believe a PLA would advance the Federal Governments interest in achieving economy and efficiency in federal procurement.

5. Identify specific reasons why you do not believe a PLA would advance the Federal Governments interest in achieving economy and efficiency in federal procurement.

6. Identify any additional information you believe should be considered on the use of a PLA on the referenced project.

7. Identify any additional information you believe should be considered on the non-use of a PLA on the referenced project.

8. Have you ever used a PLA for a construction project? If so, what was the project (dates & location)?

9. Will a PLA impact the cost of submitting an offer?

10. Will the use of a PLA produce management-labor stability on this project?

11. Are you aware of examples of labor-management conflicts in the area which could affect the efficiency of this project, which a PLA could positively impact/resolve?

12. Will use of PLA be conducive to ensuring compliance with laws and regulations governing safety and health, equal employment opportunity, labor and employment standards and other relevant matters?

13. The project will require multiple construction contractors and/or subcontractors employing workers in multiple crafts or trades. How are you going to fulfill your labor needs?

14. Will the use of PLA impact the ability of potential offerors and/or subcontractors to meet small business utilization goals?
