The evaluations are limited to the actual number of pages for the evaluation (i.e., actual number of pages for the PPIRS or similar Government evaluation system printout or the actual number of pages in the PPQ form). Offeror self-evaluations will not be accepted. PPQs shall be submitted directly to the Government to Zachary Garst at zachary.a.garst@usace.army.mil and to Frank D’Andrea at frank.j.d’andrea@usace.army.mil. PPQs shall be delivered by the offer submission date and time.
In the case of an offeror without a record of relevant past performance or for whom information on past performance is not available, the offeror may not be evaluated favorably or unfavorably on past performance.
The Government reserves the right to verify past performance evaluations by reviewing PPIRS or other Government appraisal systems. The Government may also check any cited references to verify supplied information. The Government is not limited to the evaluations provided. Any additional sources of information concerning the offeror’s past performance which the Government becomes aware of can be considered in the evaluation of the contractor’s past performance.

(9) A statement specifying the extent of agreement with all terms, conditions, and provisions included in the Combined Synopsis/Solicitation, inclusive of the specifications of the Performance Work Statement and Appendix A. Offers that fail to furnish required representations or information, or reject the terms and conditions of the Combined Synopsis/Solicitation may be excluded from consideration;
(10) Offerors shall complete Blocks 17 and complete/sign Blocks 30 of the attached page 1 of the Standard Form 1449 and include it with submission of their offers.
(c) Period for acceptance of offers. The offeror agrees to hold the prices in its offer firm for 60 calendar days from the date specified for receipt of offers.
(d) Late submissions, modifications, revisions, and withdrawals of offers.
(1) Offerors are responsible for submitting offers, and any modifications, revisions, or withdrawals, so as to reach the Government office designated in the solicitation by the time specified in the solicitation.
(2) (i) Any offer, modification, revision, or withdrawal of an offer received at the Government office designated in the solicitation after the exact time specified for receipt of offers is "late" and will not be considered unless it is received before award is made, the Contracting Officer determines that accepting the late offer would not unduly delay the acquisition; and-
(A) Since being transmitted through an electronic commerce method authorized by the solicitation, it was received at the initial point of entry to the Government infrastructure not later than 5:00 p.m. one working day prior to the date specified for receipt of offers; or
(B) If this solicitation is a request for proposals, it was the only proposal received.
(ii) However, a late modification of an otherwise successful offer, that makes its terms more favorable to the Government, will be considered at any time it is received and may be accepted.
(3) If an emergency or unanticipated event interrupts normal Government processes so that offers cannot be received at the Government office designated for receipt of offers by the exact time specified in the solicitation, and urgent Government requirements preclude amendment of the Combined Synopsis/Solicitation or other notice of an extension of the closing date, the time specified for receipt of offers will be deemed to be extended to the same time of day specified in the solicitation on the first work day on which normal Government processes resume.
(4) Offers may be withdrawn by written notice received via email to the Contract Specialist and Contracting Officer at any time before the exact time set for receipt of offers.
(e) Contract award. The Government intends to evaluate offers and award a contract without exchanges with offerors. Therefore, the offeror’s initial offer should contain the offeror’s best terms from a price and technical standpoint. However, the Government reserves the right to conduct exchanges if later determined by the Contracting Officer to be necessary. The Government intends not to follow the clarification/discussion rules of FAR Part 15, but rather reserves the right to conduct exchanges with any one or more offerors that submit offers. Simplified Acquisition Procedures under FAR Subpart 13.5, Commercial Item Test Program are being used for this acquisition.
The evaluation team may determine one or a few offers have the highest potential to provide the best value to the Government. Once the offer(s) is/are identified, the Government may hold exchanges with only that one (or fewer than all offerors) to address any remaining issues and settle the final terms of the contract. The Government may reject any or all offers if such action is in the public interest; accept other than the lowest offer; and waive informalities and minor irregularities in offers received.
(f) Unique entity identifier.(Applies to all offers that exceed the micro-purchase threshold, and offers at or below the micro-purchase threshold if the solicitation requires the Contractor to be registered in the System for Award Management (SAM).) The Offeror shall enter, in the block with its name and address on the cover page of its offer, the annotation "Unique Entity Identifier" followed by the unique entity identifier that identifies the Offeror's name and address. The Offeror also shall enter its Electronic Funds Transfer (EFT) indicator, if applicable. The EFT indicator is a four-character suffix to the unique entity identifier. The suffix is assigned at the discretion of the Offeror to establish additional SAM records for identifying alternative EFT accounts (see FAR Subpart 32.11) for the same entity. If the Offeror does not have a unique entity identifier, it should contact the entity designated at www.sam.gov for unique entity identifier establishment directly to obtain one. The Offeror should indicate that it is an offeror for a Government contract when contacting the entity designated at www.sam.gov for establishing the unique entity identifier.
Request for information. If an offeror requests information on an award that was based on factors other than price alone, a brief explanation of the basis for the contract award decision shall be provided (see 15.503(b)(2))..

52.212-1 (4) INSTRUCTIONS TO OFFERORS--
COMMERCIAL ITEMS (SEP 2021)
A full resume for each of the proposed personnel to meet the labor category requirements of Appendix A of the Performance Work Statement” is required. This section is limited to 15 pages.
However, Appendix A cites 16 labor categories. Is it the requirement that the offeror provides a resume to meet qualifications for each of these 16 categories within the 15-page limitation? Or, may offerors provide a selected group of resumes for staff whom they believe will be key to performance of this contract? Please clarify how many resumes are required, as well as the distribution of resumes based on the Appendix A listing.
Appendix A states that it can be any mix of the listed disciplines for the Environmental Professional I-III to meet the resume requirement. There are a total of 8 required resumes; one each of an Environmental Professional at each level for a total of three, a resume for each programmer level II and III for a total of two, a resume for each system administrator level II and III for a total of two, and an administrative support position.
52.212-1 (4) INSTRUCTIONS TO OFFERORS--
COMMERCIAL ITEMS (SEP 2021)
Is it allowed to provide subcontractor resumes? Yes
52.212-1 (4) INSTRUCTIONS TO OFFERORS--
COMMERCIAL ITEMS (SEP 2021)
Are commitment letters to be signed by the individual whose resume is being submitted? Or should commitment letters be signed by the organizational entity/employer?
This requirement will be removed. Only requirement is that when performance begins any key personnel must meet the minimum qualifications the PWS requires.
52.212-1 (5) INSTRUCTIONS TO OFFERORS--
COMMERCIAL ITEMS (SEP 2021)
Is there a page limit for the past performance section?
52.212-1(b)(5) states 3 projects at three pages maximum per project; so nine pages total. Also required are three completed PPQs which are 5 pages each; PPQs should total 15. PPQs template will be provided with this amendment.
52.212-1 (5) INSTRUCTIONS TO OFFERORS--
COMMERCIAL ITEMS (SEP 2021)
Is it allowed to provide subcontractor past performance projects?
yes if they are a key subcontractor and the project meets the required minimums.
52.212-1 (5) INSTRUCTIONS TO OFFERORS--
COMMERCIAL ITEMS (SEP 2021)
Regarding “The projects must have been completed within the past 5 years from the date of issuance of this solicitation for similar Government environmental projects…”. What is considered complete?
Complete refers to a environmental data management system that has been designed, implemented, has been/is being populated with data and is being/has been used by a Government project.
52.212-2 EVALUATION--COMMERCIAL ITEMS (OCT
2014); Price
In order to provide software system set up and fees please provide the following: Total number of users for the system;
Breakdown of total users by system administration and set up role vs general use role; Number of field teams for data collection; Size of the database in record count and volume; and any other information you can provide regarding software specifications/requirements.
Total numbers of users is unknown as this time as this system will be accessable to: all USACE personnel working on FUSRAP projects, all contractors working on FUSRAP projects, laboratories who are required to import EDD's into FEDMS, etc.
System admins will be assigned by each district and while ideally there would only be one admin a backup (or two) is expected, so estimating based on that approximately 20 admins. Estimated database record count cannot be accuratly determined at this point as currently there are well over 6mil records of analytical data alone, this is not including litlological, well construction, locational, groundwater elevation data, environmental monitoring data, valid values, etc. Again we are unable to provide a accurate number of total records.
1.0 INTRODUCTION Would REST API be considered as an alternative to ODBC? Unfortunately not.
1.0 INTRODUCTION
Regarding "snapshots" of final electronic data to the USACE on a quarterly basis, and at the conclusion of the period of performance.” Please define what is expected here? Is the expectation the snapshot will be in the form of an electronic data deliverable?
The database will be exported to an ascii, or equivalent, format so that a sql database that is located behind the USACE firewall can be populated and maintained. Yearly exports of data for inclusion into the database behind the USACE firewall will be too large.
2.0 DESCRIPTION OF SERVICES TO BE PERFORMED
Regarding “No system software will be installed on USACE hardware, but instead will be located remotely using an Application Service Provider (ASP) model.” We intend to offer a cloud-hosted Commercial Off-the-Shelf (COTS) application. Please clarify if that is acceptable?
As long as it is a secure connection, yes.
2.0 Data Management Services
We see references to ERPIMS, please clarify the following statement, “The Contractor will enforce project specific system rules, based on ERPIMS database specifications.”
The database schema utilized for the project is required to be ERPIMS. The contractor is required to force the ERPIMS specifications to FUSRAP project contractors including laboratories so that data can be imported properly into the planned FEDMS, so this refers to data import of historical data as well as laboratories EDD's uploaded into FEDMS by contracted laboratories.
Is there an incumbent on this contract? If so, please provide the incumbent name, current contract number, duration, historical level of effort and value of the contract?
No
If new then please provide the estimated level of efforts ? N/A is a new requirement or if it is a renewal of an existing (similar) mechanism.
New
If it is a follow on of sorts could you provide us with the name of the incumbent company and the corresponding contract number.
52.212-1(b) Can the government provide clarification on whether the proposal submission will be one document or will it be separate files (e.g.
past performance, resumes, pricing, etc.)?
The cost file should be separate from the non-cost factor. At minimum 2 separate files should be sumbmitted. PPQs may be submitted as its own file for each project.
52.212-1(b)e.(4)
Key Personnel – Can a subcontractor’s personnel be used to fill the required key personnel positions? If so, will the subcontractor’s personnel be equally scored to that of the offeror?
Yes, and Yes.
52.212-1(b)e.(5)
Past Performance Projects – will a project performed by a non-government (commercial) entity be considered relevant? And/or scored equally as a project performed by the Federal Government?
Yes, and Yes.
52.212-1(b)e.(5)
Past Performance Projects – Can a subcontractor’s past performance projects be used in addition to the offeror’s projects? If so, will the subcontractor’s projects be equally scored?
Yes, and Yes.
52.212-1(b)e.(5) The PPQ template was not included in the RFP. Can the government please provide the PPQ template for us to send to our customers?
Template will be provided with amendment.
NAVFAC / USACE Past Performance Questionnaire (PPQ) Form PPQ -0 (9/30/11)
INSTRUCTIONS FOR IMPLEMENTATION OF
NAVFAC/USACE PAST PERFORMANCE QUESIONNAIRE (PPQ) FORM
1. Background: In many source selections and MATOC task order evaluations, the RFP encourages the Offeror to have their clients provide a completed PPQ directly to the contract specialist. In addition, throughout USACE, various formats of PPQs are being utilized. Industry expressed concern regarding the process by which PPQs are submitted, that completing multiple PPQs for the same type projects over time places a significant burden on their clients, and seeking a standard process and format to be implemented across USACE and NAVFAC.
2. Discussion: Based on input from industry, NAVFAC and USACE will still allow Offerors to submit PPQs with their proposal but will not require them to be sent directly from the client to the contracting office. In addition, NAVFAC and USACE will utilize a standard PPQ format for all requirements, when required as part of an evaluation. The standard PPQ form and process will allow Offerors to obtain clients’ completed PPQ at the completion of a contract or task order, and the Offeror may retain PPQs in its files for submission on future procurements. In cases where the validity of the completed PPQ is questioned, the contract specialist/contracting officer may contact the reference for verification or additional information. The standard PPQ for NAVFAC and USACE is Form PPQ-0 (9/30/11). NAVFAC/USACE does not intend to create and maintain a database of PPQs submitted by Offerors. Therefore, it will be the Offerors responsibility to insure the submission of the relevant PPQs in its files for each solicitation. Alternatively, the Offeror may obtain an updated/new PPQ from its client(s) for submission. This new process does not preclude the Government from utilizing previously submitted PPQ information in the past performance evaluation as a source of information.
Also, as the completion of Contractor Performance Assessment Reports (CPARS) is required, Contracting Officers should search CPARS to determine if a relevant and recent evaluation is on file when one is not provided by the offeror. Recognizing CPARS evaluations are just one tool for assessing past performance, these evaluations are expected to be considered by the Contracting Officer. Discrepancies between CPARS evaluations and information provided in PPQs will be resolved by the Contracting Officer during the source selection evaluation process.
In completing the PPQ form, representatives that have direct knowledge of the Offerors past performance are encouraged to complete the PPQ (i.e., Administrative Contracting Officers, Contracting Officer Representatives, Project Engineers, etc., who had direct oversight/involvement with the project).
ACTIONS: Effective immediately, for all procurements that are using PPQs in the evaluation process:
1. Form PPQ-0 shall be utilized for all evaluations that require a PPQ. Offerors shall submit the PPQ with their proposal submission to the Contracting Office designated proposal submission location. The contract specialist or Contracting Officer shall not require the PPQ to be sent directly from the client. Solicitations that have previously been issued do not need to be amended to include this requirement.
2. RFPs utilizing PPQs shall incorporate language from the “Sample Solicitation Submittal Requirement” (below) stating, at a minimum, the following: “Completed Past Performance Questionnaires should be submitted with your proposal as required by RFP Section XXXXX. Offerors shall not incorporate by reference into their proposal PPQs previously submitted for other RFPs. This does not preclude the Government from utilizing previously submitted PPQ information in the past performance evaluation. While the Government may elect to consider data from other sources, the burden of providing detailed, current, accurate and complete past performance information rests with the Offeror. ”
3. When Past Performance Questionnaires are being permitted/sought, the following sample language will be tailored to the specific acquisition and included in the solicitation:
Sample Solicitation Submittal Requirement:
“The Past Performance Questionnaire (PPQ) included in the solicitation is provided for the offeror or its team members to submit to the client for each project the offeror includes in its proposal for Factor ____ (insert the applicable factor number, usually as “Factor 4” and insert the factor title, usually “Past Performance for the Prime Contractor”). Ensure correct phone numbers and email addresses are provided for the client point of contact. Completed Past Performance Questionnaires should be submitted with your proposal. If the offeror is unable to obtain a completed PPQ from a client for a project(s) before proposal closing date, the offeror should complete and submit with the proposal the first page of the PPQ (Attachment _____), which will provide contract and client information for the respective project(s). Offerors should follow-up with clients/references to ensure timely submittal of questionnaires. If the client requests, questionnaires may be submitted directly to the Government’s point of contact, ______________ via email at ________________________ prior to proposal closing date. Offerors shall not incorporate by reference into their proposal PPQs previously submitted for other RFPs. However, this does not preclude the Government from utilizing previously submitted PPQ information in the past performance evaluation.
Also include performance recognition documents received within the last ____ (insert the number of years) such as awards, award fee determinations, customer letters of commendation, and any other forms of performance recognition.
In addition to the above, the Government may review any other sources of information for evaluating past performance. Other sources may include, but are not limited to, past performance information retrieved through the Contractor Performance Assessment Reporting System (CPARS), using all CAGE/DUNS numbers of team members (partnership, joint venture, teaming arrangement, or parent company/subsidiary/affiliate) identified in the offeror’s proposal, inquiries of owner representative(s), Federal Awardee Performance and Integrity Information System (FAPIIS), Electronic Subcontract Reporting System (eSRS), and any other known sources not provided by the offeror.
While the Government may elect to consider data from other sources, the burden of providing detailed, current, accurate and complete past performance information rests with the Offeror.”

ADJECTIVE RATINGS AND DEFINITIONS TO BE USED TO BEST REFLECT
YOUR EVALUATION OF THE CONTRACTOR’S PERFORMANCE
RATING DEFINITION NOTE
(E) Exceptional Performance meets contractual requirements and exceeds many to the Government/Owner’s benefit.
The contractual performance of the element or sub-element being assessed was accomplished with few minor problems for which corrective actions taken by the contractor was highly effective.
An Exceptional rating is appropriate when the Contractor successfully performed multiple significant events that were of benefit to the Government/Owner. A singular benefit, however, could be of such magnitude that it alone constitutes an Exceptional rating. Also, there should have been NO significant weaknesses identified.
(VG) Very Good Performance meets contractual requirements and exceeds some to the Government’s/Owner’s benefit. The contractual performance of the element or sub-element being assessed was accomplished with some minor problems for which corrective actions taken by the contractor were effective.
A Very Good rating is appropriate when the Contractor successfully performed a significant event that was a benefit to the Government/Owner. There should have been no significant weaknesses identified.
(S) Satisfactory Performance meets minimum contractual requirements. The contractual performance of the element or sub-element contains some minor problems for which corrective actions taken by the contractor appear or were satisfactory.
A Satisfactory rating is appropriate when there were only minor problems, or major problems that the contractor recovered from without impact to the contract. There should have been NO significant weaknesses identified. Per DOD policy, a fundamental principle of assigning ratings is that contractors will not be assessed a rating lower than Satisfactory solely for not performing beyond the requirements of the contract.
(M) Marginal Performance does not meet some contractual requirements. The contractual performance of the element or sub-element being assessed reflects a serious problem for which the contractor has not yet identified corrective actions. The contractor's proposed actions appear only marginally effective or were not fully implemented.
A Marginal is appropriate when a significant event occurred that the contractor had trouble overcoming which impacted the Government/Owner.
(U) Unsatisfactory Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance of the element or sub-element contains serious problem(s) for which the contractor's corrective actions appear or were ineffective.
An Unsatisfactory rating is appropriate when multiple significant events occurred that the contractor had trouble overcoming and which impacted the Government/Owner. A singular problem, however, could be of such serious magnitude that it alone constitutes an unsatisfactory rating.
(N) Not Applicable No information or did not apply to your contract Rating will be neither positive nor negative.
