6 Detailed Description of Work
6.1 Detailed Description of Work – Base Bid Item(s)
6.1.1 Base Bid Item 1
Contractor to mobilize to the site and demobilize upon completion of all awarded base and option items.
6.1.2 Base Bid Item 2
Work for Service Gates in Conduits 7-9 shall include:
(1) Positioning and securing sluice gates to accommodate all work and maintaining the work area.
(a) Lower the emergency (upstream) gate and secure in position
(b) Raise the service (downstream) gate and secure in position
(c) Any dewatering and/or water control necessary to allow work in the dry and to provide suitable conditions per the repair material manufacturer’s recommendations.
(2) Clean and prepare the bottom sealing surface of sluice gates.
(a) Prepare the steel bottom surface of the gate in order to allow for a sufficient thickness, per manufacturer’s recommendations, of the repair material to be applied. Note that there are brass “shoes” at the edges of the gate that are not to be surfaced and can be used to establish a profile for the finished surface once the repair is complete.
(b) Roughen the bottom of the gate surface and an area upwards from the bottom on both the upstream and downstream sides in order to establish a surface profile that will insure proper adhesion of the repair material to the gate. This area will extend a minimum of 10 inches upwards from the bottom on the downstream side and 20 inches upwards from the bottom on the upstream side, but can extend further in order to provide sufficient bonding of the repair material to the gate.
(3) Rehabilitate babbitt gate sill on sluice gate seat.
(a) Smooth out any damaged areas by scraping or localized heating and puddling of the babbitt material.
Damaged areas are defined as anything showing physical deformation, such as any debris getting caught between the gate and sill causing impact damage.
(b) Special precautionary measures shall be taken for work on babbitt metal. These measures are as indicated in Section 7.5. The babbitt material is a Royal Babbitt #8 specification, a lead alloy. The government will furnish ingots of the material to the contractor when necessary.
(4) Perform surface preparation, apply material and lower service gate in order to allow the material to mold and match the babbitt sill surface.
(a) Clean the surface of the gate that is to have the repair material applied to the material manufacturer’s requirements. (e.g. SSPC SP-10) Power tool cleaning, abrasive blasting, etc may be used. The cleaned surface shall be wiped down with a cleaner and the repair material applied.
NOTE: If steel grit is used as the blast media, the expended blast media can be allowed to wash downstream. If other blast media is used, prior approval shall be obtained from the Contracting Officer.
NOTE: Insure that the cleaner or any other environmentally hazardous materials do not enter the water column of the river.
(b) The existing gate sealing surface is approximately 2 inches wide. The sill in way of the gate shall be cleaned and a release agent applied in order to prevent bonding of the repair material to the sill.
(c) The gate shall then be lowered in order for the brass “shoes” to make contact with the sill. The gate will then remain in the down position while the repair material cures.
(5) Once the repair material has fully cured and the repair work is complete, inspect for proper seal along bottom of the gate and proper adhesion of the repair material to the gate.
6.2 Detailed Description of Work – Optional Bid Item(s)
6.2.1 Optional Bid Item 3
Work for Service Gates in Conduits 1-6 shall include the same work as indicated above for gates 7-9.
7 Project Information
7.1 Basis for Award
The government shall award the contract to the offeror who provides the lowest priced technically acceptable offer.
The evaluated price shall include the base work plus all option(s). In order to be considered technically acceptable, the contractor/subcontractor shall:
• Have a minimum of five (5) years experience on projects of similar nature, specifically dealing with metal work using Belzona or equal repair materials as well as work with pumps or other large machinery. The contractor shall supply the names, locations, and client contact information for two (2) projects of similar size and scope that have been accomplished within the previous five (5) years as part of the bid submittals.
• Points of contact provided may be selectively contacted and their comments will affect the evaluations of proposals.
• A brief statement (less than 100 words) how each project illustrates your performance capabilities.
• Manufacturer’s cut sheets or similar for the products to be used
• This information can be transmitted electronically or in hard copy to the USACE Contracting Officer.
7.1.1 Award Period for Options
The government reserves the right to award any and/or all optional bid items at any time starting from the award of the base bid items to fifteen (15) days after completion of the first repair. Options may be awarded using multiple awards, e.g. an option for 2 additional gates may be awarded with the base bid award, 2 more may be awarded prior to the start of on-site construction and 2 more may be awarded after the start of on-site construction.
7.2 Period of Performance
The contractor shall be required to commence work under this contract within ten (10) calendar days after the date of receipt by the contractor of the Notice to Proceed, to prosecute said work diligently, and to complete the base bid and option work in accordance with the milestone schedule below. The time stated for completion shall include final clean-up of the premises.
Maximum Calendar Days After Receipt of Notice to Proceed (or Milestone Start/Completion Date) Commence Work (Submittals, Administration, etc.) 10
Pre-Construction Submittal Completion 30 April 2017
Commence Actual On-Site Operations 15 May 2017
Complete Base and Awarded Options 30 September 2017
The Notice to Proceed will be issued on or about 30 September 2016. Work may not begin until government approval of all submittals required in Section 20.2 of this Statement of Work.
The time period associated with the completion of the base bid work includes:
1. An optional no-work period of 150 days, from 1 December 2016 through 30 April 2017, where the Contractor may choose to work but is not required to do so. If the Contractor chooses to work, any adverse weather days occurring during the optional no-work period shall not be included in the total number of adverse weather days for the purpose of obtaining an extension.
For any and/or all optional bid items awarded within one hundred and fifty (150) days following the notice to proceed, no additional days will be given to complete said bid items since this work is covered in the base bid period of performance. For optional items awarded after the 150 day period, the period of performance will be extended by a mutually agreed period. In the event the contractor is unable to meet the project completion date, requests for extensions must be made in writing. The request must explain, in detail, why the original project completion date will not be met. The request shall be sent to the DCO along with an informational copy to the Project Manager (PM). The request shall be made as soon as possible, but not later than ten (10) days prior to the original completion date. This request shall not negate penalties for liquidated damages unless an extension is granted by the DCO in writing.
7.3 Pre-Construction Meeting
Following the award of the contract and review and acceptance of the contractor’s pre-construction submittals, the prime contractor must attend a mandatory pre-work conference. The meeting shall be scheduled by the government, and conducted on-site, at the Mount Morris Dam, at a time and date to be announced following the award of the contract.
7.4 Work Schedule
All scheduled work is to be performed Monday through Friday 0830-1700 hours, excluding federal holidays. If the contractor desires to work beyond the previously stated hours and days, written permission to do so shall be obtained from the government. Schedule changes shall be requested a minimum of 48 hours in advance, unless there is an emergency situation.
7.5 Special Job Requirements
A Certified Industrial Hygienist is required in accordance with Federal regulations and the Corps Safety Manual (EM 385-1-1) to provide initial monitoring and evaluation of the confined space work conditions and babbitt resurfacing process. This will include a respiratory protection plan and selection of respiratory devices for any melting of the babbitt material. Additional monitoring and/or changes in the respiratory program will be performed any time a major change to the work process occurs.
Sealing water out and handling any leakage through the emergency gates shall be the Contractor’s responsibility, see Section 7.6 for information regarding water control.
Providing adequate ventilation and conforming to any applicable confined space requirements shall also be the responsibility of the Contractor. There is a 12” vent shaft that can be used and is located inside the conduit and outlets approximately 150 feet up to Level 3 of the dam.
No more than three (3) gates can be out of service at one time.
The government will provide 120V outlets as well as NEMA 14-30R 220V outlets at each gate bay.
7.6 Water Management During Construction
The emergency gate upstream of service gate will be closed to prevent water from entering the conduit during construction although some leakage is expected and the contractor should account for this. Under normal dam operating conditions, the tailwater elevation ranges between Elevation 581 and 583 (NGVD 29). In order to facilitate ease of construction, dam operators will be able to maintain the tailwater elevation at approximately no higher than
585.0 (NGVD 29) during the periods of April to December except during flood conditions as defined below. For reference the service gate sill elevation is 580.25 (NGVD 29) therefore the contractor will need to prevent the tailwater from entering the work area while working in the conduit. The slope of the conduit is 1 on 10.
Water releases through the dam will impact the Contractor’s operations. During heavy storm events, the number of gates open will, either be reduced, or completely closed to prevent additional damaging flows downstream. After the event has passed, releases will be made based on downstream conditions and channel capacity, to drain the reservoir.
Normal releases will maintain a tailwater elevation no higher than approximately 585.0 (NGVD 29). In very rare instances, releases may result in tailwater levels above 585.0 (NGVD 29) which may last from several days to several weeks. Every effort will be made to give the Contractor a 24-hour notification of gate changes. The Contractor shall constantly monitor weather conditions and forecasts and be prepared for potential emergency conditions. The Contractor shall constantly be prepared to secure and protect current working areas and personnel from flood releases.
The Contractor’s on-site superintendent will be notified of gate changes by the Government. Voice mail notification will be made when Government personnel are unable to contact the superintendent in person. The Government will make a concerted effort to completely avoid using conduits in which contracted work is occurring. However, should river flow, river basin conditions, and existing operating procedures necessitate, all nine conduits could be used to make releases from the reservoir. The Contractor is advised that rains far upstream from Mount Morris Dam may impact construction operations even though no rain in the immediate vicinity has fallen.
Mount Morris Dam reservoir data is available upon request. Any conclusions drawn by the Contractor from any of this data are done so at the Contractor’s risk. The point of contact for this information is Keith Koralewski at (716) 879-4358.
*NOTE: Only government personnel will operate the emergency and service gates and the contractor personnel will lock and tag out emergency gates in the conduits that are being worked in. Each person in the conduit shall have a lock on the system.
7.7 Material/Equipment Information
Contractor shall provide all labor, tools, ladders, scaffolding, equipment, supplies and materials needed to complete the work described herein, unless otherwise noted herein that any of these items will be provided by the government.
7.8 Dimension/Quantity Verification
Contractor shall independently measure and verify all dimensions and quantities given herein prior to ordering materials and supplies for the project. This information shall be provided to the government QA person as part of the pre-work submittals.
7.9 Material Storage
If temporary storage of project material is requested, the contractor shall make a written request to the government’s QA person and receive written approval from the government. If material is found to be hazardous, flammable, combustible, etc, the contractor will be required to provide the appropriate hazardous material storage locker. Storage locations will be discussed at the pre-bid site visit and during the pre-con meeting. See Section 18 of this SOW for further information.
7.10 Site Cleanup
All excess, waste, and removed material related to this project shall become the contractor’s property and shall be promptly removed from the site and disposed of or reused/recycled in accordance with applicable local, state, and federal regulations. Documentation describing disposal of material shall be furnished to the government as part of the project closeout files. The contractor shall not dispose of any project generated waste in government receptacles or on government grounds.
8 Quality Assurance (QA)
8.1 Overview
8.1.1 Government Project Manager (PM)
The government has appointed a Project Manager (PM) for this project. The PM shall ensure all questions of a technical nature concerning the contractual work are answered.
8.1.2 Government Contracting Officer Representative (COR)
The government will appoint a Contracting Officer Representative (COR) person to oversee the contract.
8.1.3 Government Quality Assurance (QA)
The government will appoint a Quality Assurance (QA) person to ensure all work is accomplished in accordance with contractual documents. The QA person for this contract will be identified after the contract is awarded. The
QA person serves as the liaison between the contractor, PM/COR, and the DCO. The QA person does not have the authority to modify contractual agreements.
8.1.4 Government Inspections
Contractor shall allow inspection by the government PM, QA person, DCO personnel, and District Safety and Health personnel, at any time during the life cycle of the project. The contractor shall provide safe access for government personnel visiting the project.
8.1.5 Product Data Sheets
The contractor shall provide to the government product data sheets from the manufacturer for all materials used on this job, as part of the pre-construction submittals. All materials shall be delivered to the project site in new condition, in their original containers and wrapping material, with labels clearly showing all product information.
8.2 Contractor Quality Control (CQC)
Contractor Quality Control (CQC) is the means by which the contractor ensures that the construction, to include that of subcontractors and suppliers, complies with the requirements of this contract.
8.2.1 Quality Control Program (QCP)
Contractor shall refer to ‘Quality Control’ Specifications Section 01 45 00.00 10, Attachment #2 of this Statement of Work.
9 Communication
During the execution of the contract, the PM shall receive weekly status reports from the contractor on issues such as milestones met and/or missed, and issues standing in the way of project completion. The DCO shall be the communication hub for the government QA Person, COR/PM, and the contractor regarding clarification of contractual issues. By adhering to the established lines of communication, it will reduce or avoid generation of duplicate requests for information, which may inadvertently result acceptance of contractual changes without the approval of the DCO.
10 Repair/Restoration of Government Property Contractor, at no expense to the government, is required to protect all government facilities, grounds, equipment, sidewalks, and pavement during construction. All government structures, grounds, and equipment disturbed by contractor during the performance of work shall be returned to a pre-work condition prior to request of final payment.
Damages to government property that were caused by contractor and/or sub-contractor will be repaired at no expense to the government by the prime contractor.
11 Work Stoppage, Contract Cancellation Actions The Mount Morris Dam Park Manager, PM, or government QA person may issue temporary “Stop Work” notification if they find activities or job site conditions are in violation of Federal, Department of Defense, State or local health, safety and/or environmental standards, laws and regulations. The District Safety Officer (DSO), the DCO, and the PM shall be contacted immediately after a Stop Work notification order is issued or rescinded. Violation of health, safety or environmental regulations committed by the contractor shall not be grounds for compensation by the government to the contractor. Refer to contractual documents provided by the District Contracting Office regarding additional work stoppage and/or contract cancellation actions.
12 Material Change Orders Once equipment and materials are approved and accepted by the government, the contractor shall not substitute equipment or materials without providing a detailed explanation for the substitution. Changes in materials will require full justification and approval from the DCO. Modifications to contractual documents must be reviewed by the contractor, government QA person and PM, and approved by the government’s DCO. Requests for modifications must be in writing explaining the reasons for the modification, detailed description of the modification and what the benefits are and an estimated cost broken down by materials, labor and equipment.
13 Liquidated Damages Liquidated damages of $800 per calendar day will be assessed if the work is not completed within the time period described in Section 7.2 of this Statement of Work. Liquidated damages shall be assessed beginning the day following the end of the initial award period of performance.
14 Inspection/Testing
14.1 Materials Testing/Inspection
The contractor shall, at his/her own expense, perform analysis and tests following industry standards to demonstrate that materials are in conformity with the specifications. The government may check laboratories, equipment, and procedures for compliance with requirements. Signed testing reports shall be submitted to the PM.
14.2 Photo/Video Documentation of Work
The contractor will photo document all work before, during and after as well as video recording the gate to document/test the repaired seal prior to closing up and re-sealing the conduit. The photos and video shall be supplied to USACE construction representative in CD/DVD format after each week of work.
14.3 Pre-Final/Final Inspection
Upon completion of the work, the government will perform a pre-final inspection to verify that the project is complete. Cover all construction operations, both onsite and offsite, and be keyed to the proposed construction sequence. The project superintendent will be held responsible for the quality of work and is subject to removal by the
Contracting Officer for non-compliance with the quality requirements specified in the contract. In this context the highest level manager responsible for the overall construction activities at the site, including quality and production is the project superintendent. The project superintendent must maintain a physical presence at the site at all times and is responsible for all construction and related activities at the site, except as otherwise acceptable to the Contracting Officer.
3.2 QUALITY CONTROL PLAN
Submit no later than 14 days after award, the Contractor Quality Control (CQC) Plan proposed to implement the requirements of the Contract Clause titled "Inspection of Construction." Construction will be permitted to begin only after acceptance of the CQC Plan or acceptance of an interim plan applicable to the particular feature of work to be started. Work outside of the accepted interim plan will not be permitted to begin until acceptance of a CQC Plan or another interim plan containing the additional work.
3.2.1 Content of the CQC Plan
Include, as a minimum, the following to cover all construction operations, both onsite and offsite, including work by subcontractors, fabricators, suppliers, and purchasing agents:
a. A description of the quality control organization, including a chart showing lines of authority and acknowledgment that the CQC staff will implement the three phase control system for all aspects of the work specified. Include a CQC System Manager who reports to the project superintendent.
b. The name, qualifications (in resume format), duties, responsibilities, and authorities of each person assigned a CQC function.
c. A copy of the letter to the CQC System Manager signed by an authorized official of the firm which describes the responsibilities and delegates sufficient authorities to adequately perform the functions of the CQC System Manager, including authority to stop work which is not in compliance with the contract. Letters of direction to all other various quality control representatives outlining duties, authorities, and responsibilities will be issued by the CQC System Manager. Copies of these letters must be furnished to the Government.
d. Procedures for scheduling, reviewing, certifying, and managing submittals, including those of subcontractors, offsite fabricators, suppliers, and purchasing agents.
e. Control, verification, and acceptance testing procedures for each specific test to include the test name, specification paragraph requiring test, feature of work to be tested, test frequency, and person responsible for each test. (Laboratory facilities approved by the Contracting Officer must be used.)
f. Procedures for tracking preparatory, initial, and follow-up control phases and control, verification, and acceptance tests including documentation.
g. Procedures for tracking construction deficiencies from identification through acceptable corrective action. Establish verification procedures that identified deficiencies have been corrected.
h. Reporting procedures, including proposed reporting formats.
i. A list of the definable features of work. A definable feature of work is a task which is separate and distinct from other tasks, has separate control requirements, and may be identified by different trades or disciplines, or it may be work by the same trade in a different environment. Although each section of the specifications may generally be considered as a definable feature of work, there are frequently more than one definable features under a particular section. This list will be agreed upon during the coordination meeting.
j. Any special inspection requirements as required in accordance with
ICC IBC
