The 144 Fighter Wing Fresno, Air National Guard - is seeking Garage Door Replacement for Fire House IAW the SOW. This solicitation W912LA-18-T-6052 is 100% set aside for small business IAW DFARS 219.502-2(a)(i). If there is an after hour point of contact, please include in quote. The North American Industry Classification System (NAICS) code is 238290 and the small business size standard is $15M. The Disclosure of Magnitude for this project is betw een $100,000 and $250,000 IAW FAR 36.204(c).

DESCRIPTION OF SERVICES

This statement of work describes contractor requirements for the project titled “Building 145 – Fire Department – Replace Fire Department Doors”. All work will occur at the Fresno Air National Guard Station, Fresno, California. The contractor is responsible to furnish and install all labor and materials to complete the items listed in this Statement of Work. The contractor shall provide the government a complete and usable product that will be inspected/accepted by the government at the completion of the project. All information included in this document, along with the instructions and standards referenced in the document, should be considered as part of the Statement of Work.

SITUS OF WORK

All work will occur at the

STATEMENT OF WORK

1. Objectives: The purpose of this project is to remove and install new operational Roll up Doors at the 144th FW Fire Department.

2. Directions:

2.1. Install 6 White Thermally‐Broken Insulated Steel Sandwich Garage Doors with an R‐Value

17.54. Doors will include 1 row of windows to match existing lower set of windows on existing doors.

2.2. Install 6 new Commercial Openers T75, or approved equal to operate the new doors.

Openers to be mounted above one of the tracks not centered on the door. Keep the operators uniformed, either left or right side of new door.

2.3. Install all new hardware to make all 6 new doors to function properly

2.4. Contractor shall measure the openings of the new doors to fit properly.

2.5. Hook‐up and connect necessary electrical for doors to operate.

2.6. Hook‐up and connect electrical to control box in the Alarm Center located next to the Fire Department Bays.

3. Demolition

3.1. Remove 6 existing roll‐up doors, all hardware and operating equipment.

3.2. Contractor must remove all materials completely off 144th FW property.

4. Finishing

4.1. Ensure that the new doors are properly installed to fit the existing openings.

4.2. Ensure that all hardware and equipment is working in good order upon completion.

CONTRACTOR LICENSE REQUIREMENTS

1. The contractor will possess a valid California “B” license and be able to provide proof of competence in the area of work. The contractor may self‐perform sections of the project.

2. All listed sub‐contractors working for the contractor will possess the applicable California Contractors license. Refer to the Project Specific Requirements section for the required license from the Contractor.

PROJECT SPECIFIC REQUIREMENTS

1. The contractor will possess a valid California “B” license and be able to provide proof of competence in the area of door, gate and activating devices. The contractor may selfperform sections of the project.

2. All subcontractors working for the contractor will possess the applicable California Contractors license. The contractor must be able to provide a Door, Gate and Activating Device contractor D‐28 licenses.

3. The project will be completed no later than 30 working days post Notice to Proceed (NTP).

The contractor and government will negotiate the final phasing (if applicable), and schedule during a review of the project at the pre‐construction conference.

4. Materials. The following lists out materials requirements for this project. Contractor may request substitutes for an approved equal during the pre‐construction conference. All substitution requests shall be made in writing on the AF Form 3000. Requests for material substitutions made after the pre‐construction conference may not be granted by the Government.

GENERAL REQUIREMENTS

1 When required, the Contractor will attend a Pre‐Construction Conference. The PreConstruction Conference will generally be scheduled after award but prior to the NTP.

At a minimum, the contractor shall have their project manager and site superintendent attend the meeting. The Contractor will provide the following documents at the Pre‐ Construction Conference

a. Project Schedule

b. Contract Schedule of Values

c. Safety Plan

d. Quality Control Plan

e. Material Submittals

f. Material Safety Data Sheets

g. Draft DD 1354 – Final draft to be submitted at conclusion of project

2 Government Furnished Property and Services. The government will provide water, sewer and electrical connections. Unsecure outdoor storage area at the base is available and must be requested through the project manager prior to utilizing. Secured storage is the responsibility of the contractor.

3 Quality Control. Contractor shall develop and maintain a quality program to ensure work is performed as described in this Statement of Work and in accordance with all local, state and federal rules.

4 Quality Assurance. The Government will maintain a Quality Assurance Program to ensure work is performed as described in this Statement of Work and in accordance with all local, state, and federal rules. The Project Manager will manage the Government’s QA program.

5 Hours of Operation. The hours of work shall be between 7:00 a.m. and 5:00 p.m. Monday through Friday unless otherwise prescribed by this statement of work. Work shall not be performed on Weekends and Federal Holidays and scheduled day off without prior coordination from the Project Manager.

6 Base Access. Base security personnel require that all personnel entering the installation are cleared for access. The contractor will provide first name, last name, driver’s license number and applicable vehicle data for all personnel accessing the base. The contractor shall submit this information to the base Contracting Office NLT two working days prior to the date of required access. Personnel must maintain a valid driver’s license, vehicle registration and proof of insurance at all time while on the base.

7 The Contractor, and all listed sub‐contractors, must be licensed within the State of California. Refer to the Project Specific Requirements section for the required license from the Contractor.

8 Technical Competence and Ability. If requested by the Government, the Contractor shall provide proof of technical competence and experience in the area of expertise required by this Statement of Work. The contractor shall be prepared to submit documents providing proof of successful competence such as training certificate or accreditations from recognized industry leaders. Contractors shall also be prepared to provide proof of successful project experience through references of former clients in related project types.

9 Dimensions and Quantity. All dimensions and quantities provided in the Statement of Work are estimates only. They are provided as a courtesy for contractors to estimate the magnitude of work or materials necessary to complete a task. They are not intended to be final measurements. The Contractor is responsible for verifying all dimensions and quantities in the field prior to starting work.

10 Submittals. All submittals will be transmitted with an AF Form 3000. When submitting on a finish, texture or other material characteristic where a photograph is unacceptable, the Contractor will provide a sample of the material for review and approval along with the product cut‐sheet.

11 Warranty. The contractor shall guarantee all workmanship and materials covered under this statement of work for a period of no less than one (1) year from the date of project acceptance. This contractor’s warranty shall be included with any manufacturer’s warranties that accompany materials or systems installed as part of this statement of work.

"Contractor Manpower Reporting

The Contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the Air Force collection site. The Contractor is required to completely fill in all required data fields using the following web address: http://www.ecmra.mil/.

Reporting inputs will be for the labor executed during the period of performance during each Government fiscal year (FY), which runs October 1 through September 30. While inputs may be reported any time during the FY, all data should be reported no later than October 31 of each calendar year. SATURATION DIVING:

The standby rate shall apply until saturation starts. The saturation diving rate applies when divers are under pressure continuously until work task and decompression are complete. The diver rate shall be paid for all saturation hours.

DIVING IN ENCLOSURES:

Where it is necessary for Divers to enter pipes or tunnels, or other enclosures where there is no vertical ascent, the following premium shall be paid: Distance traveled from entrance 26 feet to 300 feet: $1.00 per foot. When it is necessary for a diver to enter any pipe, tunnel or other enclosure less than 48" in height, the premium will be $1.00 per foot.

WORK IN COMBINATION OF CLASSIFICATIONS:

Employees working in any combination of classifications within the diving crew (except dive supervisor) in a shift are paid in the classification with the highest rate for that shift.
