DELAWARE ARMY NATIONAL GUARD

126th MEDEVAC – Yellow Ribbon Reintegration Program New Castle, DE 19720

STATEMENT OF WORK FOR:

Yellow Ribbon Event Delaware Army National Guard Post Deployment 1 & 2 November 9th, 10th, & 11th

STATEMENT OF WORK

SCOPE OF WORK

We require 65 rooms to be available for our members, families, and staff. We require a buffet breakfast Saturday and Sunday morning; a buffet lunch Saturday; and drinks throughout Saturday and Sunday; three (3) breakout rooms and ballroom to include AV support (screens, projectors, cabling and microphones); and parking.

CLIN0001: Meeting Space/Breakout Room - 5

GENERAL AREA. General area that will be used for welcoming and registration of guests. Can be near entrance doors, in a hall-way, lobby area, foyer.

ROOM 1. Welcome reception and vendor fair from 1900 to 2100, 09 Nov 2018 for approximately 90 people.

General session/breakfast from 0700 to 0850 and lunch from 1200 to 1300, 10 Nov 2018. General session/breakfast from 0700 to 0830, 11 Nov 2018. Room for approximately 90 attendees. Room set up with round tables for 8 to 10 people each.

Continuous self-service ice water in room.

LCD Projector, Screen and Support Package: Audio Visual service contact during event complete ability to show PowerPoint slides/video Includes LCD projector, display screen, extension cable, cord, power strip Mixer to produce sound and volume for video presentations Microphone – wireless preferred

ROOM 2. Breakout room from 0700 to 1700, 10 Nov 2018, and from 0700 to 1300, 11 Nov 2018. Room set up in theater style for approximately 60 people. Continuous self-service ice water in room.

ROOM 3. Youth activity room from age 6 on up from 0700 to 1700, 10 Nov 2018, and from 0700 to 1300, 11 Nov 2018. Room for approximately 10 attendees with round tables for 5 to 8 people. Continuous self-service ice water in room. Room must be in close proximity to restrooms. If available please provide time for set up in the afternoon or evening of 09 Nov 2018.

Set up:

4 round tables (seating at 2, open for activities 2) TV and DVD Player and/or Wii Old tablecloths if available for crafts

ROOM 4. Child care room for ages 0 to 5 from 0700 to 1700, 10 Nov 2018, and from 0700 to 1300, 11 Nov 2018. Room for approximately 10 attendees with roundtables for 5 to 8 people. Continuous self- service ice water in room. Room must be in close proximity to restrooms. If available please provide time for set up in the afternoon or evening of 09 Nov 2018.

Set up:

4 round tables (seating at 2, open for activities 2) Open-floor space TV and DVD Player Old tablecloths if available for crafts

ROOM 5. Counseling/Staff room from 1200 to 2100, 09 Nov 2018, from 0700 to 1700, 10 Nov 2018, and from 0700 to 1300, 11 Nov 2018.

Vendor area. Space must be available to accommodate no more than 40 vendor tables. Tables must be 6 to 8 feet, draped, with two chairs each. There must be enough space for the vendors to sit behind the table or to sit to the side of the table for foot traffic through the hallway not to be impeded.

3 Support Tables: Tables to be 6 to 8 feet in length and skirted with two chairs each. One table to be next to the venue registration desk on 09 Nov 2018 for Event registration and one table to be at the general session’s room for late registration 10 and 11 Nov 2018. The childcare registration table will be outside the child care room for 10 and 11 Nov 2018.

CLIN0002: Lodging:

Approximately 65 rooms for 09 and 10 November 2018. 65 rooms will be at the Government rate and on contract. If possible, request for additional 40 rooms to be blocked off for vendors, will be paid individually by vendors and will not be on contract. Final numbers and rooming list will be provided NLT 5 days prior to event. Request late checkout time of 1300.

CLIN0003: Meals

All meals will not exceed the Government Per Diem rate.

Hotel should provide sample of proposed menu with estimated cost that takes into consideration the following factors; cost, nutrition, suitability for mixed population of adults, teenagers and children, and ability to respond to special dietary requirements.

Friday schedule: 09 Nov 2018 from 1900 to 2100, Reception for 90 people

(1) Cash bar

(2) Horderves

Saturday schedule: 10 Nov 2018, for 90 people

(1) Breakfast (0730 to 0830): Juices, coffee, tea, eggs, bread selection, cereal and two meats

(2) Lunch (1200 to 1300): Pasta with meat and sauce selection, breads, rolls etc., tossed salad, pasta salad, cakes and cookies

(3) Beverage availability during event hours: coffee, tea, water, soda

Sunday schedule: 11 Nov 2018, for 90 people

(1) Breakfast (0730 to 0830): Juices, coffee, tea, eggs, bread selection, cereal and two meats

(2) Beverage availability during event hours: coffee, tea, water, soda

Minimum of two buffet lines- double sided Note: If two large rooms are not available, one room would be acceptable for both general session and dining.

Total: 90 hors d’oeuvre snacks, 09 Nov 2018 180 Breakfast meals, 10 and 11 Nov 2018 90 Lunch meals, 10 Nov 2018 180 all day beverage snacks, 10 and 11 Nov 2018

CLIN0004: Parking

Parking passes for 120 cars and 40 passes for vendors.

Contract Manpower Reporting Application (CMRA): The Office of the Assistant Secretary of the Army (Manpower & Reserve Affairs) operates and maintains a secure Army data collection site where the contractor will report ALL contractor manpower (including subcontractor manpower) required for performance of this contract. The contractor is required to completely fill in all the information in the format using the following web address: https://cmra.army.mil. The required information includes: (1) Contracting Office, Contracting Officer, Contracting Officer’s Technical Representative; (2) Contract number, including task and delivery order number; (3)

Beginning and ending dates covered by reporting period; (4) Contractor name, address, phone number, e-mail address, identity of contractor employee entering data; (5) Estimated direct labor hours (including subcontractors); (6) Estimated direct labor dollars paid this reporting period (including subcontractors); (7) Total payments (including subcontractors); (8) Predominant Federal Service Code (FSC) reflecting services provided by contractor (and separate predominant FSC for each subcontractor if different); (9) Estimated data collection cost; (10) Organizational title associated with the Unit Identification Code (UIC) for the Army Requiring Activity (the Army Requiring Activity is responsible for providing the contractor with its UIC for the purposes of reporting this information; (11) Locations where contractor and subcontractors perform the work (specified by zip code in the United States and nearest city, country, when in an overseas location, using standardized nomenclature provided on website);

(12) Presence of deployment or contingency contract language; and (13) Number of contractor and subcontractor employees deployed in theater this reporting period (by country). As part of its submission, the contractor will also provide the estimated total cost (if any) incurred to comply with this reporting requirement. Reporting period will be the period of performance not to exceed 12 months ending September 30 of each government fiscal year and must be reported by 31 October of each calendar year. Contractors may use a direct XML data transfer to the database server or fill in the fields on the website. The XML direct transfer is a format for transferring files from a contractor’s systems to the secure website without the need for separate data entries for each required data element at the website. The specific formats for the XML direct transfer may be downloaded from the website.

AT Level I Training. Contractor employees with an area of performance within an Army/Air National Guard controlled installation, facility or area. All contractor employees, to include subcontractor employees, requiring access to Army/Air National Guard installations, facilities and controlled access shall complete AT Level I awareness training. The contractor shall submit certificates of completion for each affected contractor employee and subcontractor employee, to the Contracting Officer’s Representative (COR). SW-4: TOUR OF JOB SITE

4.1: A site visit will be conducted at a date to be determined.
