HOTEL SERVICES

The contractor shall provide all required personnel, equipment, tools, materials, supervision, and quality control necessary to perform requirements in accordance with this Performance Work Statement and Quality Assurance Plan.

1. PERIOD OF PERFORMANCE-

PREFERRED DATE: 26-28 July 2019 ALTERNATE DATE: 19-21 July 2019

2. Place of Performance- Services shall be provided at the following location: HOUSTON, TEXAS

3. Service Contract Act- The service contract act applies to this contract. The following Department of Labor (DOL) Wage Determinations are applicable to this contract, see attachment. This determination supersedes previous wage determination versions: Wage Determination No: 98-0156 (Rev.-37) 01/01/2019

4. Lodging- The contractor will provide 25 rooms with two queen beds or two king beds for the period of performance. The Government will not be responsible for room service charges, phone calls, movie rental fees, internet service charges, or any other incidental charge incurred by the individual staying in the room. These charges/extra services must be charged to the individual guests. The Government will not be charged for any unused rooms.

The Chaplain Corps staff will provide the contractor with details of any special requests in a timely manner. The contractor will be provided a final room list at least 48 hours in advance of the event start time. Military members will be provide the opportunity for late check out on the final day of this event. .

5. Meeting Room- The contractor shall provide a general meeting room large enough for at least 25 rectangular tables that sit two people per table. Meeting room must also provide ample open floor space in the rear of the room for experimental activities. Meeting room must be available to the Chaplain Corps staff at least two hours before the event start time to set up. The sessions for this event will run from 8:00 am Saturday to 12:00 (noon) Sunday. The contractor will provide pens and notepads for each person in the meeting room.

6. Food Service- The contractor will provide coffee, juice, water and snack service during Saturday and Sunday mornings meeting session in or near the meeting room. Contractor will provide breakfast Saturday and Sunday and lunch on Saturday only.

7. Audio-Visual- The contractor shall provide multi-media projector, screen, large flip chart and multi-channel sound system for use during the entire meeting session.

8. Parking- Overnight parking requirements are shall be provided for attendees. Parking costs / fees should be included in the total cost of each room and the total cost for each room must not exceed government per diem rate.

9. Contractor Manpower Reporting Application (CMRA) - The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the Colorado Army National Guard via a secure data collection site.

The contractor shall completely fill in all required data fields using the following web address: http://www.ecmra.mil/. Reporting inputs shall be for the labor executed during the period of performance during each Government fiscal year (FY), which runs October 1 through September 30. While inputs may be reported any time during the FY, all data shall be reported no later than October 31 of each calendar year, beginning with 2013. Contractors may direct questions to the help desk at http://www.ecmra.mil.

Quality Assurance Plan

10. Quality Assurance (QA) - The Government shall evaluate the contractor’s performance in accordance with the Quality Assurance Surveillance Plan (QASP) to ensure that the contractor has performed in accordance with the performance standards.

11. Performance Requirements Summary- The Performance Requirements Summary includes performance standards to determine contractor performance. The chart below lists the contracts primary service requirements, their acceptable quality levels and the methods of government surveillance. While there are several contract requirements not listed in the chart below, failure to list a requirement below does not negate the contractor’s obligation to perform all contract requirements.
