Amendment 3

QUESTIONS/RESPONSES RFI V2

Version 2 (Inclusive of all questions to date with corrections) Issued 27 June 2017.

QUESTION 1: Is there a page limit.

RESPONSE 1: Please limit quotes to 75 pages or less.

QUESTION 2: Do you want separate volumes for Technical, Past Performance, Pricing, and

Reps/Certs?

RESPONSE 2: This is a Request for Quote (RFQ) not a request for proposal. Separate volumes will not be needed for this.

QUESTION 3: What is the government’s anticipated invoicing structure/payment structure for this effort? Will it be the annual CLIN price divided by 12, to equate to 12 fixed monthly invoice amounts? (Since FFP Jobs are listed as CLINs in Section B, but para. 34. states Invoices: The contractor will invoice in WAWF on a monthly basis following award. The Government does not define the contractors pay schedule (weekly, bi-weekly, monthly), but does define that it be timely and consistent throughout the lifecycle of the contract. Will the government clarify the invoicing schedule?)

RESPONSE 3: Since this is a service contract for a period of performance for 1 year plus option years, awarded vendor will be allowed to bill monthly per the appropriate CLIN.

Travel expenses must be billed into the travel CLIN as a fixed price CLIN and vendor will

NOT be paid for going over the exceeded quoted amount.

QUESTION 4: We are asking the government to please address the discrepancy between the

SCA occupation code (01020) which is listed as an Administrative Assistant in WD 15-5573 versus the Medical Records Technician which is listed in Schedule B versus the extract of WD

15-5573, 01020 - Administrative Assistant. More importantly, the duty description of the

Administrative Assistant (per the SCA Occupation Handbook, 5th Edition) does not match the

PWS. We recommend the statement, "Schedule B contains the minimum wage for the services to be provided under this contract", and allow the vendor to choose the occupation that best suites the PWS requirement. This is important given the wording, "minimum wage for the services..."

implies an evaluation criteria.

RESPONSE 4: The wage determination is a requirement that must be put into all solicitations and contracts. Not all occupations are listed in the Service Contract Act (SCA) webpage provided by the DOL. The wages listed are for estimating what a typical medical professional should be paid in a service contract. The DOL sets standards for what employers should be paying employees according to the DOL so employees are protected.

QUESTION 1: On page 17, para. 11, the solicitation states, “Contractor shall provide at a minimum case managers who are either a Registered Nurse (RN) with an active unrestricted license in Oregon and who have graduated from an accredited nursing school.”. Will the government accept social workers in lieu of registered nurses in case management positions?

RESPONSE 1: Correction, delete the word either, only Registered Nurses are acceptable.

QUESTION 2: Section C, Item 13, Credentialing and Insurance, states that “Further, Contractors are required to provide proof of Professional Liability Insurance for all Medical

Personnel supporting clinical requirements. Non-clinical personnel may be covered by the

Contractor’s General Liability Insurance Policy.” Each CLIN structure for Case Managers states

“1. Provide (1) Case Manager in (city), Oregon for Non-Clinical Case Management Services per the Performance Work Statement in Section C.” Please clarify if Professional Liability

Insurance is required for this contract.

RESPONSE 2: The Contractor's General Liability Insurance is acceptable.

QUESTION 3: In Section L, there is no requirement listed for a pricing volume. Can the government clarify how pricing should be submitted by an offeror?

RESPONSE 3: The pricing should be submitted in a format similar to the following:

QUESTION 4: What location does the government envision the Case Managers and Care

Coordinators working from? Paragraph 8 of the PWS on page 15 states there are four locations but only Salem and Ashland are listed. Moreover, Section B, CLINS 003, 1003, 2003, 3,003, and 4003 also do not state the location. Please provide the street address for each of the locations.

RESPONSE 4: Correction, should read two locations. One Case Manager and all the Care

Coordinators work out of the Anderson Readiness Center in Salem, Oregon. The address is: 3225 State St, Salem, Oregon, 97301.

The other Case Manager works out of the Ashland Armory. The address is: 208 Oak St, Ashland, Oregon, 97520

QUESTION 5: Does the RN Case Manager need to have an active unrestricted license in

Oregon; or will any of the 50 states suffice as long as the license is active and unrestricted?

RESPONSE 5: The Case Managers need to have an active unrestricted license in Oregon.

QUESTION 6: Please confirm that ONLY the Care Coordinators are required to travel to: Bend, Medford, Eugene, LaGrande, and Clackamas as stated in the Paragraph 9 of the PWS on page

16.

RESPONSE 6: Only the Care Coordinators are required to travel.

QUESTION 7: Are you expecting amounts to be quoted within the travel CLINs for the travel listed in paragraph 8 of the PWS or will this be determined upon award?

RESPONSE 7: It is expected that travel costs will be quoted within the travel CLINs.

QUESTION 8: Does the proposal response need to only be submitted by email or does a hard copy need to be shipped also?

RESPONSE 8: The proposal only needs to be submitted by email.
