Amendment 0001 to Solicitation W912J726RA001

This is Amendment 0001 to Solicitation W912J726RA001, issued September 2, 2026 by the Army's W7/US FORSCOM Activity at Boise, Idaho (Contact: Natasha Spratt, 520-853-8226). The amendment incorporates government responses to vendor Requests for Information (RFIs), with the Q&A period closing September 6, 2026 at 4:00 PM MST. The period for receipt of proposals has not been extended. The amendment clarifies that NAICS code 621399 applies, specifies that estimated 8 soldiers monthly will receive Follow-on-Care determined case-by-case with IDARNG coordination, and confirms Friday is the expected day of service. Prospective offerors are reminded that this is a dental/medical professional support services requirement where the labor mix and service hours have been adapted to meet current organizational needs; proposals must be based strictly on the current Performance Work Statement, not on historical contracts or previous incumbent staffing levels.

The Performance Work Statement (PWS) requires the contractor to provide comprehensive dental examination and treatment services supporting Idaho Army National Guard (IDARNG) Service Member dental and medical readiness from September 27, 2026 through September 26, 2027, with one 12-month option year. Services include periodic dental examinations with radiographic studies and oral health education conducted at pre-coordinated locations (primarily Gowen Field Dental Treatment Facility in Boise on Fridays), case management of DRC3 (deployment-limiting) dental conditions with treatment plans approved by the State Dental Officer, and limited dental treatment at the GFDTF. The contractor must employ a licensed Idaho dentist and dental support staff, maintain access to military medical systems (MODS, eProfile, MEDPROS, MEDCHART/DenClass), comply with OSHA and infection control protocols, and upload all documentation to HIPAA-compliant systems. Authorized services include diagnostic imaging, restorative and endodontic procedures, oral surgery, and prosthodontic services. Government furnishes equipment, instruments, anesthetics, and dental materials. The contract is set aside for Economically Disadvantaged Women-Owned Small Businesses (EDWOSB) with a 50-percent limitation on subcontracting for services. Award will be made to the lowest-priced technically acceptable offer without negative past performance.

Sponsor (MPAS), the procedures for which are described below. Although there is no requirement for the contractor to designate a "Corporate Facility Security Officer" (FSO) to serve as its single point of contact, the MP ICAM application process and other CAC and security-related matters, such designation facilitates these processes. If a Corporate FSO is not established, all contractor employees requiring a CAC will be required to process their own applications. The submission process for CAC applications is as follows:

The contractor's FSO or contractor employee shall submit requests for a CAC via email to the designated MPAS. The MPAS for this requirement will be MAJ Wanty or Representative.

The Government will establish a MP ICAM application account for each CAC Request and will provide each contractor employee a user ID and password, via email, to the contractor's FSO or contractor employee. The FSO or contractor employee shall access the MP ICAM account and complete the CAC application (entering/editing contractor information as applicable) at: https:// mpartnerregweb.dmdc.osd.mil/mpartnerregweb/app/. The contractor's FSO or contractor employee shall follow up to ensure that the MPAS is processing the request.

a. The Government will inform the contractor's applicant, via email, of one of the following:

i. Approval.* Upon approval, the information is transferred to the DEERS database and an email notification is sent to the contractor with instructions on obtaining their CAC. The contractor proceeds to a RAPIDS station (RAPIDS Site Locator: http://www.dmdc.osd.mil/rsl/).

ii. Rejection.* The Government, in separate correspondence, will provide reason(s) for rejection.

iii. Return. Additional information or correction to the application required by the contractor employee.

*The contractor shall maintain records of all approved and rejected applications.

1.7.7.4. A CAC cannot be issued without evidence that the FSO has initiated a National Agency Check with Written Inquires (NACI).

1.7.7.5. At the RAPIDS station, the RAPIDS Verification Officer will verify the contractor by SSN and two forms of identification. Identity source Documents must come from the list of acceptable documents included in Form I-9, OMB No. 1615-0047, "Employment Eligibility

Verification." Consistent with applicable law, at least one document from the Form I-9 list shall be a valid (unexpired) State or Federal Government-issued picture identification (ID). The Identity documents will be inspected for authenticity and scanned and stored in the DEERS upon issuance of an ID. The photo ID requirement cannot be waived, consistent with applicable statutory requirements. The Verification Officer will capture primary and alternate fingerprints, picture, and updates to DEERS and will then issue a CAC.

1.7.7.6. Issued CACs shall be for a period of performance not longer than three (3) years or the individual's contract end date (inclusive of any options), whichever is earlier.

1.7.7.7. The contractor shall return issued CAC's to the DEERS office upon departure or dismissal of each contractor employee. Obtain a receipt for each card and provide to the TA
