SUMMARY OF CHANGES
SECTION SF 1449 - CONTINUATION SHEET
The following have been modified:
PEFORMANCE WORK STATEMENT
Performance Work Statement (PWS) for (Jar) Disturbed Sample Testing
FARGO, ND
Fall /Winter 2015/2016
GENERAL:
The US Army Corps of Engineers (USACE), St. Paul District (MVP) will be conducting soil borings in the Fargo- Moorhead metro area for the proposed Fargo-Moorhead Metro Flood Risk Management Project starting approximately 1 September 2015 and ending field work approximately 31 December 2015. During this exploration phase the USACE will collect disturbed jar samples which require laboratory tests to be completed on them. The contractor shall perform the soils tests as defined by the CLIN structure in this contract which lists the types of tests, the applicable ASTM standard associated with each test and the number of tests required.
LABORATORY REQUIREMENTS:
The laboratory shall be USACE validated for the required tests. All testing must be performed at the USACE validated laboratory and be completed within 30 days of the receipt of the samples at the laboratory. The laboratory shall store the samples a minimum of 90 days after submission of an approved invoice for the completed tests. After 90 days the laboratory may dispose of the samples. The samples are typically provided in glass, wide-mouth, 1-pint jars; 12 to a box with box dimensions approximately 15 inches by 11 inches by 4 inches high. Environmentally contaminated soils are not anticipated for this phase of exploration.
EXECUTION:
The USACE will begin the exploration on or near 1 September 2015 and it is anticipated to continue through at least 31 December 2015. The contractor shall expect to receive samples periodically throughout this period. The contractor must be capable of completing testing and providing results within 30 days of receipt of the samples. If the successful contractor’s facility is located within 70 miles (driving distance) of the Fargo-Moorhead area or within 70 miles of the Minneapolis-St.Paul metropolitan area, USACE will deliver the samples directly to the laboratory. If the successful contractor is not located within 70 miles driving distance of either above noted locations then the contractor shall pay for all shipping of samples (to, and from USACE-if necessary). Contractors outside of the 70 miles shall incorporate the shipping costs into their quote (not using a separate line item). The laboratory shall be capable of receiving samples between the hours of 0630 and 1830, Monday through Friday.
The Contractor is required to pick up the samples locally from the USACE Storage Area which is located in the Fargo-Moorhead metro area. The USACE Representative in the area will provide periodic updates to the Contractor on the number of samples available for pickup. The USACE representative will be available between the hours of 1800 – 2000, Monday through Friday for to the Contractor to pickup of the samples. The Contractor shall coordinate the pickup of the samples with the USACE Representative 48 hours in advance. Once the Contractor picks up and receives the samples, the Contractor will be responsible for packing and shipping the samples to the laboratory. The samples shall remain in their glass jars and will not be allowed to be transferred and repacked in different containers for shipping purposes. The Contractor shall photograph the sample once received at the laboratory and provide these photos the USACE representative, via email.
The contractor shall provide signed testing results to the USACE electronically, via email, when tests are completed and reviewed by the contractor. The electronic files shall be in PDF format. Results from each set of samples per individual boring shall be grouped together for efficient use. The USACE reserves the right to require the contractor to revise and resubmit test reports if errors and/or omissions are found. In addition the contractor shall provide the test results in a format that may be imported into a gINT database. A gINT project file or formatted Excel file is acceptable.
W912ES-15-T-0143
PAYMENT:
The contractor must quote a unit price for each type of test or service required along with the total estimated price (unit price x number of tests). The contractor shall be paid based on the quantity of tests completed satisfactorily.
The contractor may submit monthly billing statements for all testing completed and for which results have been received by USACE. The billing statement shall be detailed to show what samples have been received and which tests have been completed and reported.
