GENERAL NOTES
1. PVC SURFACE DRAINAGE INLETS AND MANHOLES SHALL INCLUDE THE DRAIN BASIN TYPE AS INDICATED ON THE
CONTRACT DRAWING AND REFERENCED WITHIN THE CONTRACT SPECIFICATIONS. THE DUCTILE IRON GRATES / COVERS FOR EACH OF THESE FITTINGS ARE TO BE CONSIDERED AN INTEGRAL PART OF THE SURFACE DRAINAGE INLET OR MANHOLE AND SHALL BE FURNISHED BY THE SAME MANUFACTURER. THE SURFACE DRAINAGE INLETS AND MANHOLES SHALL BE AS MANUFACTURED BY NYLOPLAST A DIVISION OF ADVANCED DRAINAGE SYSTEMS, INC., OR APPROVED EQUAL.
2. THE DRAIN BASINS REQUIRED FOR THIS CONTRACT SHALL BE MANUFACTURED FROM PVC PIPE STOCK, UTILIZING A THERMO-MOLDING PROCESS TO REFORM THE PIPE STOCK TO THE SPECIFIED CONFIGURATION. THE DRAINAGE PIPE CONNECTION STUBS SHALL BE MANUFACTURED FROM PVC PIPE STOCK AND FORMED TO PROVIDE A WATERTIGHT CONNECTION WITH THE SPECIFIED PIPE SYSTEM. THIS JOINT TIGHTNESS SHALL CONFORM TO ASTM D3212 FOR JOINTS FOR DRAIN AND SEWER PLASTIC PIPE USING FLEXIBLE ELASTOMERIC SEALS. THE FLEXIBLE ELASTOMERIC SEALS SHALL CONFORM TO ASTM F477. THE PIPE BELL SPIGOT SHALL BE JOINED TO THE MAIN BODY OF THE DRAIN BASIN OR CATCH BASIN. THE RAW MATERIAL USED TO MANUFACTURE THE PIPE STOCK THAT IS USED TO MANUFACTURE THE MAIN BODY AND PIPE STUBS OF THE SURFACE DRAINAGE INLETS AND MANHOLES
SHALL CONFORM TO ASTM D1784 CELL CLASS 12454.
3. THE GRATES / COVERS FURNISHED FOR ALL SURFACE DRAINAGE INLETS AND MANHOLES SHALL BE DUCTILE IRON MADE SPECIFICALLY FOR EACH BASIN SO AS TO PROVIDE A ROUND BOTTOM FLANGE THAT CLOSELY MATCHES THE DIAMETER OF THE SURFACE DRAINAGE INLET OR MANHOLE. GRATES / COVERS FOR DRAIN BASINS SHALL BE CAPABLE OF SUPPORTING H-10 LOADING FOR PEDESTRIAN TRAFFIC. 12” AND 15” GRATES / COVERS WILL BE
HINGED TO THE FRAME USING PINS.
4. THE SPECIFIED PVC SURFACE DRAINAGE INLET OR MANHOLE SHALL BE INSTALLED USING CONVENTIONAL FLEXIBLE PIPE BACKFILL MATERIALS AND PROCEDURES. THE BACKFILL MATERIAL SHALL BE CRUSHED STONE OR OTHER GRANULAR MATERIAL MEETING THE REQUIREMENTS OF CLASS 1 OR 2 MATERIAL AS DEFINED IN ASTM D2321. THE SURFACE DRAINAGE INLETS AND MANHOLES SHALL BE BEDDED AND BACK-FILLED UNIFORMLY IN ACCORDANCE WITH ASTM D2321. THE DRAIN BASIN BODY WILL BE CUT AT THE TIME OF THE FINAL GRADE SO AS TO MAINTAIN A ONE PIECE, LEAK PROOF STRUCTURE. NO BRICK, STONE OR CONCRETE BLOCK WILL BE USED TO SET THE GRATE / COVER TO THE FINAL GRADE HEIGHT. FOR H-20 LOAD RATED INSTALLATIONS, A CONCRETE RING WILL BE POURED UNDER AND AROUND THE GRATE / COVER AND FRAME. THE CONCRETE SLAB MUST BE DESIGNED TAKING INTO CONSIDERATION LOCAL SOIL CONDITIONS, TRAFFIC LOADING, AND OTHER APPLICABLE DESIGN FACTORS. FOR OTHER INSTALLATION CONSIDERATIONS SUCH AS MIGRATION OF FINES, GROUND WATER, AND
SOFT FOUNDATIONS REFER TO ASTM D2321 GUIDELINES.
CLEANOUT DETAIL SCALE: N.T.S.
THE BACKFILL MATERIAL SHALL BE CRUSHED STONE OR OTHER
GRANULAR MATERIAL MEETING THE REQUIREMENTS OF CLASS II
MATERIAL AS DEFINED IN ASTM D2321. BEDDING & BACKFILL FOR SURFACE
DRAINAGE INLETS SHALL BE PLACED & COMPACTED UNIFORMLY IN
ACCORDANCE WITH ASTM D2321.

MARCH 16, 2016
Using the following table, identify the type of construction project activity (Type A-D) Step One
Control Risk Assessment Matrix of Precautions for Construction and Renovation
Ty pe
A
Inspection and non-invasive activities, including, but not limited to:
A. Removal of ceiling tiles for visual inspection (1 tile per 50 square feet) B. Painting (But not sanding) C. Wall covering, electrical trim work, minor plumbing, and activities which do not generate dust or require cutting of walls or access to ceilings other than visual inspection.
Ty pe
B
Small scale, short duration activities, including, but not limited to:
A. Installation of telephone and computer cabling B. Access to chase spaces C. Cutting of walls or ceiling where dust migration can be controlled
Ty pe
C
Work that generates a moderate to high level of dust or requires demolition or removal of any fixed building component or assemblies, including but not limited to:
A. Sanding of walls for painting or wall coverings B. Removal of floor coverings, ceiling tiles, and casework C. New wall construction D. Minor duct work or electrical work above ceilings E. Major cabling activities F. Any type A, B, or C activity that cannot be completed within a single work shift.
Ty pe
D
Major demolition and construction projects, including, but not limited to:
A. Activities requiring heavy demolition or removal of complete cabling system B. New construction C. Activities which require consecutive work shifts
Step Two
Using the following table, identify the risk groups that will be affected. If more than one group will be affected, select the higher group.
Risk Group: Low Risk
- Office
Step Three
Determine the level of control classification necessary for the work by matching the construction activity with designated risk group in the matrix below. Provide the associated infection control procedures. Note each construction renovation project has several different levels depending on the location and type of activity.
See description of required infection control precautions by class chart for Class I-IV precautions.

1. Remove or isolate HVAC system in area where work is being done to prevent contamination of duct system.
2. Complete all critical barriers I.E. sheetrock, plywood, plastic, to seal area from non work area of implement control cube method (cart with plastic covering and sealed connection to work site with HEPA vacuum for vacuuming prior to exit) before construction begins.
3. Maintain negative air pressure within work site utilizing HEPA equipped air filtration units.
4. Contain construction waste before transport in tightly covered containers.
5. Cover transport receptacles or carts. Tape covering unless solid lid.
1. Do not remove barriers from work area until completed project is inspected by Cemetery CO.
2. Remove barrier materials carefully to minimize spreading of dirt and debris associated with construction.
3. Vacuum work area with HEPA filtered vacuums.
