Indefinite Delivery Indefinite Quantity (IDIQ) Design-Build, Design Bid Build Multiple Aw ard Task Order Contract for repair and maintenance and construction services for the USACE Northw estern Division. The MATOC and Task Order 0001 w ill be made based on the best value to the Government using the Best Value Trade-Off source selection process in accordance w ith FAR Part 15 and the tw o-phase design-build procedures in FAR 36.3. The MATOC's w ill be aw arded for a three-year base period w ith a tw o-year option period. The total contract dollar capacity shared among f irms w ill not exceed $99,900,000. Any unused capacity in one period may be carried over into the subsequent contract period. The minimum guarantee amount for the entire life of the contract for each IDIQ aw ardee is $2,500. EVALUATION AND SUBMISSION
PROPOSAL EVALUATION AND CONTRACT AWARD
A. BASIS FOR AWARD
The MATOC and Task Order 0001 awards will be made based on the best value to the Government by using the Best Value trade-off source selection process in FAR Part 15 and two-phase design-build procedures in FAR Part 36.3. The MATOCs will be awarded for a three-year base period with a two-year option period. The total contract dollar capacity shared among firms will not exceed $99,900,000. Any unused capacity in one period may be carried over into the subsequent contract period. The minimum guarantee amount for the entire life of the contract for each IDIQ awardee is $2,500. The minimum Task Order value is $2,000 and the maximum Task Order value is $5,000,000. The Government intends to award up to five (5) IDIQ contracts to the offeror(s) whose proposal, conforming to the RFP, will be most advantageous to the Government resulting in the Best Value. Task Orders will be primarily awarded on a firm-fixed price basis;
however, task orders may be awarded on a Time and Materials (T&M) or Labor Hour basis if necessary to address unusual circumstances.
The requirement is for repair and maintenance of real property and construction services to included new construction and renovation to large civil works projects including flood control and hydroelectric dams, river navigation facilities, maintenance and office facilities, roadways and recreational facilities, and river levees. Specific work may include a combination of design, construction and/or repair within the Northwestern Division (NWD) Walla Walla District (NWW), Portland District (NWP), Seattle District (NWS), Kansas District (NWK), Omaha District (NOW) project sites.
The government will initially evaluate offerors’ proposals submitted in response to Phase One (1) of this solicitation. After evaluation of Phase One, the government will select up to seven (7) of the most highly qualified firms to submit Phase Two (2) proposals. Upon the conclusion of Phase One (1), the solicitation will be amended to include the Phase Two (2) evaluation factors including price and the scope of work for the first Task Order that will be awarded along with the MATOC IDIQ contracts. Phase One (1) submissions and evaluations will not be carried over into Phase Two (2). All offerors selected to propose for Phase Two (2) will be evaluated only on the Evaluation Factors outlined in the Phase Two RFP. PHASE ONE (1) DOES NOT INVOLVE ANY SUBMISSION OF PRICING INFORMATION. Phase Two (2) of the solicitation will include the informational plans (floor plan, cross section, and elevation), Statement of Work, and the specifications for the first Task Order. The estimated construction magnitude for the first Task Order is between $1,000,000 and $5,000,000. A site visit may be conducted during Phase Two (2). Phase Two (2) offerors will have a minimum of 30 days to submit their proposals.
Phase Two (2) evaluation factors are as follows:
Factor 1- Design Factor 2 – Work Plan Factor 3- Project Management Factor 4- Price
During Phase Two (2), the government will provide submission requirements and specific information pertaining to the Phase Two (2) evaluation factors which will be based on the requirements for Task Order 0001. Additionally, the relative importance of the Phase Two (2) factors both to each other and to price will be identified in the Phase Two (2) solicitation.
All offerors selected as most highly qualified and invited to Phase Two (2) source selection will be required to submit a proposal from all evaluation factors in order to be considered both for the seed project and to be a MATOC holder. Further, any deficiency in the Phase Two (2) source selection will render the proposal unawardable both for the seed project and the overall MATOC.
The government may award the MATOC contracts and the first Task Order on the basis of initial offers received in Phase Two, without discussions; therefore, an offeror’s proposal should contain the offeror’s best terms from a technical and price standpoint.
The price proposal will not be assigned a rating for evaluation. The government reserves the right to award to other than the lowest prices offered. Awards will be made based on the proposals found to be the most advantageous to the government considering the technical and price proposals.
The government will evaluate offerors’ proposals based on how well the proposals address each of the Evaluation Factors provided below for Phase One (1) and the Evaluation Factors that will be provided in more detail for Phase Two (2). The evaluation process includes an assessment of the offerors’ strengths, weaknesses, deficiencies, risks, and uncertainties in addressing the government’s requirements as specified in the RFP.
B. SCOPE OF WORK:
Work on civil systems may include: waterstop installation, potable & wastewater systems, underground utilities, drainage culverts, geotechnical exploration and testing, hillside stabilization stream bank stabilization, irrigation systems, paving, striping, site fencing, and river levee rehabilitation.
Work on structural/architectural systems may include: structural concrete, structural frames (including steel, aluminum, FRP), machinery supports and seismic bracing, hydraulic steel structures (primarily metal fabrication and machining), Corps of Engineers paint systems (including vinyl), fall protection systems, pre-engineered buildings, manufactured buildings, building interior renovation, elevators, roofing, waterproofing, firestop systems, and building exterior envelope renovation.
Work on electrical systems may include: Information & Communication Technology (ICT), fiber optic, motor control, instrumentation & controls (e.g. PLC, SCADA), commercial/industrial lighting, fire & security systems, low and medium voltage distribution (at most 15KV), lightning protection, and diesel generators.
Work on mechanical systems may include: construction or repair of the following systems: fire protection, HVAC, pumping (less than 100 HP), compressed air, hydraulic power systems (less than 50 HP), cranes and hoists (less than 10 tons) and general/industrial plumbing.
C. ADJECTIVAL RATINGS AND DESCRIPTIONS
The ratings and descriptions provided in the table that follows will be utilized by the Government in the evaluation of Phase One, Factor 1. Experience and Factor 2. Technical Approach. The ratings and descriptions will also be used in the evaluation of all Phase Two factors except for price. Phase One, Factor 3, Past Performance is evaluated based on a performance assessment confidence rating and is described in detail under paragraph D Past Performance Factor below.

D. PHASE ONE FACTORS TO BE EVALUATED
Evaluation Factors. The following evaluation factors will be used to evaluate each proposal. All three Phase One evaluation factors are of approximately equal importance.

FACTOR 1: EXPERIENCE
The Government will evaluate and assess the extent of an offeror’s relevant Construction and Design-Build experience and proposed team’s experience (design firm and major subcontractors) on projects completed in the last ten (10) years or at least 50% complete at the time proposals are due. The Government will evaluate the offeror’s depth and breadth of experience on different types of relevant design capability and construction types. The Government will evaluate a minimum of three projects but no more than five projects for each prime contractor and designer and a minimum of two but no more than three projects for each major subcontractor (Mechanical, Structural, Electrical, Civil) as listed by the offeror on Attachment A, Experience Overview Sheet and as further described on Attachment B, Experience Information Form.
Projects described in the below paragraph will be considered more relevant and rated more favorably if:
Projects wherein the design was facilitated for the end user/customer from design concept through 100% design completion including construction of the design; projects that demonstrate experience with multiple civil works type construction trade categories; and Projects with previous teaming experience between the offeror and proposed team.
The combined experience demonstrated by the submitted projects must address the following (a-c):
a. “Design-Build” construction is defined as a civil-works type project in which the contractor is provided less than a full design and is required to: complete the design with a state licensed Architect Engineer (A-E); participate in design review; and accomplish full construction of the designed project.
b. Projects will be considered relevant if they:
1. Are valued at $250,000 or larger; and
2. Collectively the projects submitted must include general construction and/or design service requirements similar to the following construction or repair on:

c. Experience provided for proposed design firm and major subcontractors will only be evaluated if a letter of commitment is signed by each proposed firm and submitted with the proposal. A letter of commitment is not required if the offeror is a joint-venture, and the firm is part of the joint-venture agreement.
FACTOR 2: TECHNICAL APPROACH
The government will evaluate and assess the offeror’s understanding of the requirements, feasibility of the proposed technical approach, and risks and mitigation in the execution of task order projects wherein the Government provides minimal or limited design consisting of informational drawings and functional/performance specifications. The purpose of this factor is to determine whether the offeror has appropriate understanding of the design-build process and is not intended to limit an offeror to any proposed technical solution or to allow an offeror to provide less than the minimum requirements for Phases One and Two of the solicitation.
Based on your understanding of the MATOC scope of work, describe your technical approach for design- build task order projects wherein the Government provides minimal or limited design consisting of informational drawings and functional/performance specifications. This narrative shall not exceed fifteen (15) pages. Pages in excess of this limitation will be removed and not evaluated by the Source Selection Evaluation Board.
Offerors shall address the following areas in your technical approach to include any risks and the proposed approach for risk mitigation:
Organization. Provide an organizational chart showing the reporting structure and lines of authority for the proposed team key functions such as project manager, superintendent, lead designer, safety manager, Design Quality Control Manger, Contractor Quality Control Manager, and others. Without identifying by name, describe the role of the key functional members during design and construction.
Cost effectiveness and expedited completion. Describe your process for providing a task order proposal that is cost-effective and will allow you to expedite completion of the project. Describe your approach to streamlining design and construction resulting in reduced costs and achievement of aggressive schedules with consideration to operational or environmental restraints.
Communication and coordination. Describe your communication and coordination approach within the design-build team, and with the government during the design and construction phases. Describe your approach to facilitate design and construction submittal reviews. Describe in detail the Designer of Record’s involvement throughout the entire project period.
Quality control. Describe your approach for quality control for both design and construction. Include: quality control organization; review of design; verification of construction materials; construction inspection; permit compliance; project documentation; and change request process.
Technical Factor Ratings for Factors 1 and 2: ratings outlined below focus on the strengths, deficiencies, weaknesses, risks, and uncertainties of the offeror’s proposal.
The rating depicts how well the offeror’s proposal meets the technical factor requirements.

FACTOR 3: PAST PERFORMANCE
The government will evaluate and assess the offeror’s (and proposed team’s) record of performance on relevant projects to ascertain the probability of successfully performing the efforts of the MATOC scope of work. Past performance is the degree of success, in terms of quality, timeliness, customer satisfaction, management effectiveness, safety and other items important for success on prior relevant projects. From this information, the government will determine an overall performance confidence assessment regarding the offeror’s likelihood of success on the task orders to be issued under the MATOC. In the event an offeror lacks any record of relevant past performance, the offeror’s past performance will be evaluated neither favorably nor unfavorably and will receive an Unknown Confidence (neutral) rating. Data submitted for evaluation of past performance shall not extend past ten (10) years prior to the issue date of the solicitation but may include performance data generated during the past six years without regard to the contract award date. The government will consider Past Performance Questionnaires, interviews, CPARS, and any other sources available to it regarding past performance for this factor. CPARS data is limited to the past 6 years. Offeror’s should provide additional performance data for projects beyond 6 years.
There are three aspects to the past performance evaluation: Recency, Relevancy (including context of data) and Quality (including general trends in Contractor performance and source of information.) assessment and performance confidence assessment.
a. RELEVANCY ASSESSMENT. The government will utilize the four levels of relevancy identified in the table below to determine how relevant a recent effort accomplished by the offeror is to the scope of anticipated work under this MATOC. For this procurement, relevancy is defined as projects that are valued at $250,000 or larger; and included general construction and/or design service requirements similar to the listing of relevant jobs under Factor 1, section B, item 2 of this solicitation.

Based on this assessed quality, the government will arrive at a performance confidence assessment for the offeror, selecting the most appropriate rating from the chart below.

c. QUALITY: The third aspect of the past performance evaluation is to establish the overall quality of the offeror’s past performance. The quality past performance evaluation conducted gathers information from the offeror’s customers to determine how well the offeror performed those past contracts. This quality assessment will be reflected in the overall confidence assessment described below.
E. PROPOSAL SUBMISSION REQUIREMENTS
1. Introduction. The offerors’ proposal shall be submitted in hard copy, with accompanying digital copies, as set forth below. The RFP shall provide the Government address and receipt date for proposal submittal.
The offerors’ proposal shall consist of two (2) volumes. The Volumes are:
Volume I – Technical Proposal (Technical Approach, Experience and Past Performance)
Volume II – Price & Administrative Proposal (Price, Solicitation, Offer and Award Documents and Certifications/Representations.)
Files shall not contain classified data. The use of hyperlinks in proposals is prohibited.
Only Phase 1 (Volumes I & II) Should Be Submitted During Phase 1. Proposals shall conform to the solicitation requirements to include all stated terms, conditions, representations, certifications, and all other information required of this solicitation.
Proposals shall be submitted in hard copy and electronic (CD/DVD). The electronic files submitted with hard copy files shall contain identical information.
Offerors are cautioned that “parroting” of the technical requirements with a statement of intent to perform does not reflect an understanding of the requirement or capability to perform. Offerors are responsible for including sufficient details to permit a complete and accurate evaluation of each proposal. Proprietary information shall be clearly marked.
Any costs related to proposal preparation and submission will not be reimbursed by the Government.
2. Proposal Submission Requirements
Each volume shall be submitted in a separate three-ring binder. Any pages that are changed (as a result of negotiations or proposal revisions) should be of a different color and have changed information clearly marked by a vertical line in the right margin of the page. The revised pages shall be dated. Each binder shall be clearly labeled with its Title and a copy number (e.g., copy 1 of 4).
Printing should be single spaced. Each paragraph shall be separated by at least one blank line. A standard, 12-point minimum font size applies. Arial or Times New Roman Fonts are required. Tables and illustration may use a reduced font size no less than 8-point and may be landscape.

3. Proposal Files
a. Format. The submission shall be clearly indexed and logically assembled. Each volume shall be clearly identified and shall begin at the top of a page. All pages of each volume shall be appropriately numbered and identified by the complete company name, date and RFP number in the header and/or footer. All files created in MS Word (doc) files shall use the following page setup parameters:
Margins – Top, Bottom, Left, Right – 1” From Edge – Header, Footer 0.5” Page Size, Width – 8.5” Page Size, Height – 11”
NOTE: 11 X 17 folded pages are acceptable for tables/graphic representations.
4. The following additional restrictions apply:
a. File Packaging. All of the proposal files shall be compressed (zipped) into one file entitled proposal zip using WinZip version 6.2 or later, or as separate uploads in their native format, i.e. doc, xls, ppt, etc., and provided on CDs or DVDs. Zip disks are not permitted. Files shall be in read-only format, using PDF files. All price breakdown information to aid in the Price evaluation shall be submitted in Microsoft Office Excel Read/Write format. Each disc shall be externally labeled with the volume number, date, and the offeror’s name. **Please note – Self extracting exe files are not acceptable. **
b. Content Requirement. All information shall be confined to the appropriate file. The offeror shall confine submissions to essential matters, sufficient to define the proposal in a concise manner, to permit a complete and accurate evaluation of each proposal. Each file of the proposal shall consist of a Table of Contents, Summary Section, and the Narrative discussion. The Summary Section shall contain a brief abstract of the file.
Proprietary information shall be clearly marked.
(i) VOLUME I, TAB 1 –Experience
(ii) VOLUME I, TAB 2 – Technical Approach
(iii) VOLUME I, TAB 3 – Past Performance. This tab shall contain current and past performance information regarding similar contracts. It shall not exceed 15 pages plus five pages for each major subcontractor, including Past Performance Questionnaire Forms. Offerors shall submit Government and/or commercial contracts for the prime offeror and each major contract currently in performance or awarded during the past six
(6) years, from the issue date of this solicitation, which are relevant to the efforts required by this solicitation. Relevant efforts are defined as services/efforts that are the same as or similar to the effort required by the RFP. Data concerning the offeror shall be provided first, followed by each proposed major subcontractor, in alphabetical order. The Offeror shall also submit the written consent of its major subcontractors to allow the disclosure of its subcontractor’s past performance information to the Offeror. In addition, signed letters of commitment shall be included for all major subcontractors and fabricators for their past performance to be considered. This tab shall be organized into the following sections:
1. Section 1 – Contract Descriptions. This section shall include the following information in the following format. Ensure all information is current and accurate.
(a) Contractor/Subcontractor place of performance, CAGE Code and DUNS Number. If the work was performed as a subcontractor, also provide the name of the prime contractor and Point of Contact (POC) within the prime contractor organization (name, and current address, e-mail address, and telephone and fax numbers).
(b) Government contracting activity, (or private firm) and current address, Procuring Contracting Officer’s name, e-mail address, telephone and fax numbers.
(c) Technical representative/COR and current email address, telephone and fax numbers.
(d) Government contract administration activity and the Administrative Contracting Officer’s name, current e-mail address, telephone and fax number.
(e) Government contract administration activity’s Pre-Award point of contact name, and current e-mail address, telephone and fax numbers.
(f) Contract Number and, in the case of Indefinite Delivery type contracts, GSA contracts, and Blanket Purchase Agreements, include Delivery Order/Task Order Numbers.
(g) Contract Type (specific type such as Fixed-Price (FP), Cost Reimbursement (CR), Time & Material (T&M), etc.). In the case of Indefinite Delivery contracts, indicate specific type (Requirements, Definite Quantity, and Indefinite Quantity) and secondary contract type (FP, CR, T&M, etc.).
(h) Awarded price.
(i) Final or projected final price.
(j) Original delivery schedule, including dates of start and completion or work.
(k) Final or projected final, delivery schedule, including dates of start and completion of work.
2. Section 2 – Performance. Offerors shall provide a specific narrative explanation of each contract listed in Section 1 - Contract Description, describing the objectives achieved and detailing how the effort is relevant to the requirement of this RFP.
(a) For any contracts that did not/do not meet original schedule or technical performance requirements, provide a brief explanation of the reason(s) for the shortcomings and any corrective action(s) taken to avoid recurrence. All Requests for Deviation and Requests for Waiver shall be addressed with respect to causes and corrective actions. The offerors shall also provide copy of any Cure Notices or Show Cause Letters received on each contract listed and a description of any corrective action implemented by the offeror or proposed subcontractor. The offerors shall indicate if any of the contracts listed were terminated and the type and reasons for the termination.
(b) For all contracts, the offeror shall provide data on all manufacturing warranty returns. Data shall delineate total number of warranty returns, number of Could Not Duplicate (CND), number of failures attributable to GFE component failures, and number and nature of failures attributable to the offerors delivered product.
3. Section 3 – Subcontracts. Offerors shall provide an outline of how the effort required by this RFP will be assigned for performance within the offerors corporate entity and among the proposed subcontractors. The information provided for the prime offeror and each proposed major subcontractor must include the entire company name, company address, CAGE Code, DUNS Number and type of work to be performed by citing the applicable Government technical specification. This includes all subcontractors, fabricators, and suppliers who will be providing critical hardware/services or whose subcontract is for more than 10% of the total proposed Price.
4. Section 4 – New Corporate Entities. New corporate entities may submit data on prior contracts involving its officers and employees. However, in addition to the other requirements in this section, the offeror shall discuss in detail the role performed by such persons in the prior contracts cited to demonstrate the relevancy of experience and past performance with this solicitation. Information should be included in the files described in the sections above. Signed Letters of Commitment shall be included in the proposal for these employees in order to be considered. Letters of Commitment must be signed both by the company official and the employee.
5. Section 5-Past Performance Questionnaire. For all contracts identified in Section 1 - Contract Descriptions, complete a Past Performance Questionnaire for any project that has not received a formal Contractor Performance Assessment Reporting System (CPARS) evaluation. The offeror shall complete Part I of the Past Performance Questionnaire and e-mail the questionnaire to the technical representative responsible for the current and past contract. The Point of Contacts (POCs) shall be instructed to electronically complete Part II of the questionnaire and e-mail the entire questionnaire to the Contracting Office no later than the proposal due date, to Leanne.r.walling@usace.army.mil.
The offeror shall submit, with its proposal, a list of all the POCs who were sent questionnaires. The POC List shall be submitted in Word for Windows Table Format to include the following fields:
Solicitation Number; Company Name; Contract Number; Government Agency; POC Last Name, First Name; POC Title; POC Telephone Number; POC E-mail Address; and
Date E-Mail to POC (month/day).
6. Section 6-Submissions. Offerors are discouraged from providing points of contact with another contractors’ facility, i.e., in case an offeror (or one of his/her team members) is in a subcontract with another contractor who has submitted a proposal on the same requirement. Offerors shall provide and submit the prime contract number and all Governmental agency points of contact (POC) in lieu of subcontract numbers or prime contract POCs in situations as described above.
(iv) VOLUME II, TAB 1– Solicitations, Offer and Award Documents and Certifications/Representations.
Certifications and Representations – Each offeror shall complete (fill-in and signatures) the solicitation sections indicated below using the file (without modification to the file) provided with the RFP. An authorized official of the firm shall sign the SF33 and all certifications requiring original signature. An Acrobat PDF file shall be created to capture the signatures for submission.
Section 00 21 00 – Standard Form 1442 Solicitation, Offer and Award
Section 00 45 00 – Representations, Certification and Other Statements of Offerors
Solicitations, Offer and Award Documents and Certifications/Representations shall not be submitted separately from that submitted in VOLUME II – Solicitations, Offer and Award Documents and Certifications/Representations.
Please make allowances for delays in transmittal.
SUBMISSION DEADLINE - Proposals shall be received by the USACE Contracting Activity no later than the time and date specified in Block 13 of Standard Form 1442. Official time will be established by the clock located in the area where proposals are received. Late proposals may not be accepted.
The Government will not be responsible for proposals delivered to any location or to anyone other than those designated to receive proposals on its behalf. The Offeror is responsible for ensuring that the proposal is submitted so as to reach the designated recipient and is responsible for allowing sufficient time for the proposal to be received in accordance with the information provided.
PHASE ONE PROPOSAL SUBMISSION REQUIREMENTS
Factor 1: EXPERIENCE, Submission Requirements:
Offerors shall submit a completed Attachment A, Experience Overview Sheet listing Construction and Design-Build project type work that best represents the offeror’s experience. Project experience shall be identified for the offeror, design firm, and major subcontractors. The listing shall include relevant experience for projects completed in the last ten (10) years or at least 50% complete at the time proposals are due. If a project is currently under construction, annotate percent of completion on the form. In selecting projects to submit, offerors should pay attention to the evaluation parameters identified for this Factor 1 in Section II.
For each project listed on Attachment A, the offeror shall provide an Experience Information Form (EIF) found at Attachment B. Additional lines may be added as necessary not to exceed one additional page total per EIF. Offerors are responsible for providing project descriptions in sufficient detail to permit evaluation of project relevancy and may provide additional descriptive information on each form beyond what is required in the attachment. One page of photographs per project may be provided in order to provide clarity to the project description and do not count against the page limit per EIF. Information provided on the EIF will be evaluated for this Factor 1 and will also be used to support the evaluation of Factor 3, Past Performance. EIFs shall be provided for the categories that follow. Note that it is not necessary to submit more than one EIF for projects that are the same for two or more of the following categories:
a. OFFEROR (PRIME CONTRACTOR): Provide no more than five (5) examples of Construction and Design-Build projects within the last ten (10) years that are relevant to the requirements identified in the MATOC scope of work. Provide an explanation of how these projects are similar in scope and magnitude to the work required in this solicitation.
If the offeror is a joint venture, the offeror shall submit the Design-Build construction experience of each of the joint venture partners, providing no more than five
(5) projects in total.
b. DESIGN FIRM: Provide no more than five (5) examples of Design and Design- Build projects within the last ten (10) years that are relevant to the requirements identified in the MATOC scope of work. Provide an explanation of how these projects are similar in scope and magnitude to the work required in this solicitation. Provide a letter of commitment signed by the design firm.
c. MAJOR SUBCONTRACTORS: Major subcontractors are defined as members of an offeror’s overall team who are expected to perform Mechanical, Structural, and Electrical work. Identify by name your major (one each) Mechanical, Structural, and Electrical subcontractors that you intend to utilize under this MATOC. If work will be performed in- house, rather than subcontracted out, provide a statement to that effect. Provide no more than three (3) examples total of projects within the last ten (10) years for which you, if performed in-house, or any of these subcontractors played a significant role on work that is relevant to the requirements identified in the MATOC scope of work. Provide an explanation of how these projects are similar in scope and magnitude to the work required in this solicitation. Provide a signed letter of commitment for each proposed major subcontractor.
Factor 2, Technical Approach, Submission Requirements:
(The Technical Approach proposal may not exceed 15 pages.)
Based on your understanding of the MATOC scope of work, describe your technical approach for design-build task order projects wherein the Government provides minimal or limited design consisting of informational drawings and functional/performance specifications. Address the following areas in your technical approach to include any risks and the proposed approach for risk mitigation:
Organization. Provide an organizational chart showing the reporting structure and lines of authority for the proposed team key functions such as project manager, superintendent, lead designer, safety manager, Design Quality Control Manager, Construction Quality Control Manager, and others. Without identifying by name, describe the role of the key functional members during design and construction.
Cost effectiveness and expedited completion. Describe your process for providing a task order proposal that is cost-effective and will allow you to expedite completion of the project. Describe your approach to streamlining design and construction resulting in reduced costs and achievement of aggressive schedules with consideration to operational or environmental restraints.
Communication and coordination. Describe your communication and coordination approach within the design-build team, and with the government during the design and construction phases. Describe your approach to facilitate design and construction submittal reviews. Describe in detail the Designer of Record’s involvement throughout the entire project period.
Quality control. Describe your approach for quality control for both design and construction. Include: quality control organization; review of design; verification of construction materials; construction inspection; permit compliance; project documentation; and change request process.
Factor 3: PAST PERFORMANCE, Submission Requirements:
The offeror shall provide the NAVFAC/USACE Past Performance Questionnaire
(Form PPQ-0) at Attachment C to clients for each project the offeror (and proposed team) included in response to Factor 1, Experience. Ensure correct phone numbers and email addresses are provided for the client point of contact. Completed PPQ-0s should be submitted with your proposal therefore, offerors should follow-up with clients to ensure timely submittal of questionnaires.
If the offeror is unable to obtain a completed PPQ-0 from a client before proposal closing date, the offeror should complete and submit with the proposal the first page of the PPQ-0, which will provide contract and client information for the respective project.
If the client requests, questionnaires may be submitted directly to the Government’s point of contact, Ms. LeAnne Walling, via email at Leanne.R.Walling@usace.army.mil prior to the proposal closing date. The offeror must submit, with its proposal, a list of all the POC’s who were sent questionnaires.
The POC List shall be submitted in Word for Windows Table Format to include the following fields: Solicitation Number; Company Name; Contract Number;
Government Agency; POC Last Name, First Name; POC Title; POC Telephone Number;
POC E-mail Address; and Date E-Mail to POC (month/day). Offerors shall not incorporate by reference into their proposal PPQs previously submitted for other RFPs. However, this does not preclude the Government from utilizing previously submitted PPQ information in the past performance evaluation.
LOCAL
REQUIRED LIABILITY INSURANCE PURSUANT TO FAR 28.306
In accordance with the clause 52.228-5 Insurance – Work on a Government Installation, the Contractor shall procure and maintain during the entire period of his performance under this contract the following minimum insurance:
(i) Coverage complying with state laws governing insurance requirements pertaining to Workmen’s Compensation and Employers” Liability Insurance.
(ii) Bodily injury liability insurance with minimum limits of $500,000 per occurrence shall be required on the comprehensive form of policy.
(iii) Automobile bodily injury and property damage liability with minimum limits of $200,000 per person and $500,000 per occurrence for bodily injury liability; and $20,000 per occurrence for property damage liability shall be required.
(iv) The coverage required under Longshore and Harbor Workers Compensation Act, if applicable.
(v) The Insurance Certificate will name the Walla Walla District, U.S. Army Corps of Engineers, 201 N. Third Avenue, Walla Walla, Washington 99362 as the Certificate Holder.
BUY AMERICAN CONSTRUCTION MATERIALS AND CONSTRUCTION MATERIALS UNDER
TRADE AGREEMENT 52.225-9 AND 52.225-11
Clause 52.225-11 is only applicable to task orders that exceed $7,032,000 but less than $12,001,460. Clause 52.225- 9 is applicable for all other task orders under $7,032,000.
CONTRACTOR RESPONSIBILITY, PREAWARD SURVEY
In order to determine a contractor’s responsibility for purposes of contract award the contractor is required to provide a statement regarding previous experience and past performance in performing comparable work, information related to the business organization, financial resources, and/or plant to be used in performing the work.
(See the Attention to Offerors form included in this solicitation.) In accordance with FAR 9.104-1, to be determined responsible, a prospective contractor must –
(a) Have adequate financial resources to perform the contract, or the ability to obtain them;
(b) Be able to comply with the required or proposed delivery or performance schedule, taking into consideration all existing commercial and governmental business commitments;
(c) Have a satisfactory performance record. (A prospective contractor shall not be determined responsible or non-responsible solely on the basis of a lack of relevant performance history except as provided in FAR 9.104-2);
(d) Have a satisfactory record of integrity and business ethics;
(e) Have the necessary organization, experience, accounting and operational controls, and technical skills, or the ability to obtain…
