ELEVATOR MAINTENANCE
LITTLE GOOSE LOCK AND DAM
Part 1 – General Information
1.1 General: This is a non-personal services contract to provide Elevator Maintenance, Repair, Inspection and Safety Testing/Certification at U.S. Army Corps of Engineers, Little Goose Lock and Dam, Starbuck, Washington (Little Goose Project). The Government will not exercise any supervision or control over the Contractor’s employees performing the services herein. The Contractor’s employees shall be accountable solely to the Contractor, who in turn is responsible to the Government.
1.2 Background: Little Goose Project is a multi-purpose hydroelectric facility located on the Lower Snake River. This multi-building facility houses two elevators, as described in Attachment 1, Little Goose Project Elevators.
1.3 Objectives: The objectives of this contract are to provide regular maintenance, timely repairs, and scheduled load and no-load tests (Section 5.2.3) to ensure all elevator equipment at Little Goose Project is operable and safe for daily use.
1.4 Scope:
1.4.1 The Contractor shall inspect, maintain, and repair the two Little Goose Project elevators to ensure maximum availability for safe use by Government personnel with no more than two (2) outages per month.
1.4.2 This contract shall provide for recurring and “as needed” ASME A17.1 safety code inspections and tests, technical consulting, equipment condition surveys, preventive maintenance performance evaluations and equipment for Little Goose Lock and Dam. The contractor shall furnish all labor, supervision, materials, equipment, tools, transportation and incidentals necessary to effectively and efficiently fulfill all the requirements of this contract. Except for those services listed in paragraph 4.4 as Excluded Services, the monthly fixed price includes all inspections, maintenance, repairs and replacements.
Non-Routine service calls shall be executed on an as needed basis. This contract allows the Government to budget total yearly costs and eliminates concerns relating to individual parts repair or replacement invoicing.
The Contractor assumes all responsibility and determines the amount of service visits and types of service required to keep the elevators operating safely and within the performance objectives of this PWS, including Technical Exhibit 1, Performance Requirements Summary.
1.4.3 The Contractor shall maintain elevators, equipment appurtenances, and accessories so as to comply with the requirements of the latest edition of the ASME A17.1 Safety Code for Elevators and all other applicable laws, regulations, rules, ordinances, codes, etc., including the recommendations of the original equipment manufacturer (OEM) The Elevator Inspector’s Manual ASME A17.2 shall be used as a guide to establish that equipment is operating safely.
Inspections will be scheduled every six months
Safety Test once a year
1.4.4 The Contractor shall provide all management, personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items (Section 4.2.1) and non-personal services necessary to perform Elevator Maintenance, Repair, Safety Testing, and Entrapment Rescue as defined in this Performance Work Statement (PWS), except for those items specified in Part 3 as Government Furnished Property and Services.
The work is detailed in Part 5, Specific Tasks.
1.5 Period of Performance: The period of performance shall be for one Base Period of 12 months and four 12-month Option Periods. The performance periods are as follows:
Base Period June 1, 2017 – May 31, 2018 Option Period 1 June 1, 2018 – May 31, 2019 Option Period 2 June 1, 2020 – May 31, 2021 Option Period 3 June 1, 2021 – May 31, 2022 Option Period 4 June 1, 2022 – May 31, 2023
1.6 Place of Performance: The work shall be performed at USACE Little Goose Project, 1001 Little Goose Dam Road, Dayton Washington 99328. The two elevators are located in the Powerhouse building on the Project.
1.7 Hours of Operation:
1.7.1 Hours of Operation. The Contractor is responsible for conducting routine maintenance and repairs between the hours of 6:30AM and 5:00PM, Monday thru Friday, excluding federal holidays or when the Government facility is closed due to Government-directed facility closings. In the case of passenger entrapment or other critical need or severe hazard, the Contractor must be available on call at all times and provide response as defined in section 5.4.

1.8 Contractor Personnel: The Contractor shall at all times maintain an adequate workforce for the uninterrupted performance of all routine and non-routine tasks defined within this PWS.
1.8.1 Special Qualifications. The Contractor shall obtain and maintain all licenses and permits within the federal, state, and local laws, codes, and regulations as they apply to the work under this contract. The Contractor employees must be skilled in and capable of performing the work under this contract. The Contractor shall provide state-licensed elevator mechanics to perform the work (copies of licenses must be available upon request). All Contractor employees shall be at least 18 years of age and must be able to fluently read, write, speak, and understand English.
1.8.2 Subcontracting. The Contractor shall normally perform all work under this contract with its own forces. No work may be subcontracted without written approval of the Contracting Officer. Compliance with the provisions of this contract by subcontractors shall be the responsibility of the Contractor. Vendor proposals shall be submitted with a subcontracting plan if subcontracting will be utilized during the course of this contract.
1.9 Coordination:
1.9.1 Post-award Conference. Within 15 calendar days of award, and prior to commencement of any work on the contract, the Contractor shall attend a post-award conference called by the Contracting Officer’s Representative (COR). This conference may be held in person at Little Goose Project, or telephonically, and will be scheduled at a time and date mutually agreed to by the Contractor Representative (CR) and the Contracting Officer Representative (COR).
1.9.2 Pre-work Submittals. The Contractor shall submit the following draft documents for review and acceptance by the Government prior to the Post-award Conference:

1.9.3 Assessment Site Visit. Within 15 calendar days of contract award, the Contractor shall schedule an escorted assessment site visit at Little Goose Project to verify elevator equipment conditions. During the site visit, the Contractor shall look for and note deficiencies and non-compliances issues for the existing equipment and machine rooms, and these shall be submitted in writing as an Initial Maintenance Report within 15 calendar days of inspection and assessment. The Contractor will be responsible for all maintenance, repair, and replacement of parts, etc., unless noted in the deficiency report and accepted by the Government.
1.9.4 Maintenance Control Plan (MCP). The Contractor shall develop an annual written Maintenance Control Plan for each elevator and submit to the Government at or before the Post-award Conference. A separate schedule shall be prepared for each non-identical elevator. All schedules shall describe periodic maintenance procedures and required inspections, and shall specify the frequency of performance and appropriate intervals for specific maintenance items. A maintenance schedule shall be posted in each elevator room. Posted schedules shall become the property of the Government and shall not be removed from elevator rooms upon completion of the contract. Schedules shall be kept current and changes coordinated with the COR. Routine maintenance work required by this contract shall be performed on a regularly scheduled basis but not less frequently than once each 30 days.
1.9.5 Advance Notice of Service. Elevator maintenance shall be scheduled to minimize interference with plant maintenance and operation activities. Except for Non-Routine Call-back Services, Project plant maintenance and operations will take precedence over elevator maintenance. The Contractor shall notify the COR or Control Room Operator not less than 48 hours prior to arrival for routine maintenance and repair. Prior notification is not required for non-routine call-out service; however, the Contractor employee(s) must be pre-authorized under this contract’s Project Security requirements (para 1.12).
1.9.6 Authorized Government Callers. The Government will provide the Contractor a list of names and contact information for all Little Goose Project employees who are authorized to call the Contractor’s dispatch for non-routine call-back services. The Contractor must not honor requests made by persons other than those authorized.
1.10 Quality Control Program / Quality Assurance:
1.10.1 Quality Control. The Contractor shall develop and maintain an effective Quality Control Program (QCP) to ensure services are performed according to the standards in this PWS.
1.10.2 Maintenance Report. At a minimum, the Contractor shall submit a Maintenance Report for each day work is performed, which includes, but is not limited to, the following information:
Compliance with the maintenance/repair schedule Reasons for non-compliance with the maintenance schedule All maintenance/repair work performed Parts repaired or replaced Safety problems identified Corrective actions taken to correct safety problems Elevator repairs required that are outside the scope of this contract
Maintenance Reports shall be provided to the COR or designee within two business days of performance of work.
1.11 Safety:
1.11.1 Safety Standards. The Contractor shall comply with all applicable Occupational Safety and Health Act (OSHA) Standards, as well as the Corps of Engineers’ Safety and Health Requirements Manual EM 385-1-1 in effect at time of contract award. This section calls out only a few specific requirements. Prospective offerors are encouraged to review the EM 385-1-1 to gain an understanding of the full requirement prior to developing their offer (see Part 6 for references). Contractor personnel and equipment shall be subject to safety inspections by Government personnel while on federal property.
1.11.2 Training & PPE. The Contractor shall ensure that all personnel are trained to the level of expertise required for proper performance of the task, general first aid procedures, and use of safety equipment. Personal protective equipment (PPE) shall be provided by the Contractor and must be appropriate to the task.
1.11.3 Safe Clearance / Hazardous Energy Control / Lockout/Tagout. All Contractor work on site shall meet the requirements of Walla Walla District "Safety and Occupational Health Office; Safe Clearance Program for the Control of Hazardous Energy,” NWWP 385-1-20 (see Part 6 for reference). 1.11.3.1 In accordance with Chapter 10 of NWWP 385-1-20, the Contractor may perform work under its own Lockout/Tagout procedures for work on elevator equipment that can be completed in one work shift.
To do so, the Contractor must provide its Lockout/Tagout procedures to the Government for review and acceptance, as well as provide NWW HECP Awareness certificates of completion for each authorized employee, at least one week prior to the start of the Period of Performance. Upon acceptance by the Government’s Responsible Official, the Contractor shall implement its Lockout/Tagout procedures on the elevator equipment downstream of the Government isolation point.
1.11.3.2 If the Contractor’s Lockout/Tagout procedures are not submitted or accepted, or if elevator equipment must be left under a clearance longer than one shift, the Contractor shall be required to operate under the Walla Walla District Safe Clearance Program requirements. See NWWP 385-1-20 for training requirements.
1.11.3.3 All Contractor and subcontractor employees shall be trained in and abide by the accepted Lockout/Tagout or Safe Clearance Program procedures, and Project Control Room personnel shall be notified of all actions involving clearing and switching energy sources.
1.11.4 Protection of Government Property. The Contractor shall protect all existing Government structures, utilities, equipment, and work of any kind against damage or interruption of service, which may result from its operations. Persons engaged under this contract may be working in areas containing control systems vital to the operation of the project. The Contractor shall protect against accidental and/or intentional operation of any control systems.
1.11.5 Contractor Exposure Hours. In accordance with EM 385-1-1 paragraph 01.D.05.c, the Contractor shall submit project work hours to the COR by the 5th business day of each month for work completed in the month prior. Work hours include all hours on the Project where a Contractor employee is in an on-duty pay status.
1.11.6 Incident/Accident Reporting. An accident constitutes an incident or act involving the Contractor, which may or may not have caused obvious damage to persons or property. This includes injuries to Contractor employees, members of the public, or damage to Government or personal property. The Contractor shall notify the COR by telephone within 30 minutes of any incident or accident, followed by a written report. The report shall include, but not be limited to, location, nature of the injury or accident, name(s) of injured party(ies), conditions/circumstances surrounding the occurrence, cause of the occurrence, parties notified, and actions taken. The report shall be accompanied by sketches, graphs, drawings, and photographs, as needed, and forwarded to the COR within one working day following the occurrence of accident or injury.
1.12 Project Security: The Contractor shall be responsible for safeguarding all Government equipment, information, and property provided for Contractor use.
1.12.1 Authorized Personnel. Only persons directly involved with accomplishment of the Contractor’s operations on this contract shall be allowed on the Project. The Contractor shall provide and maintain updated information on all Contractor and subcontractor employees authorized to work on the Project under this contract. Employee information shall include:
Full name Date of birth Driver’s license number Elevator mechanic license documentation Statement of citizenship (special authorization is required for foreign nationals and may take up to four weeks to acquire)
1.12.2 Contractor Employee Identification. Contractor and subcontractor employees must wear official clothing that identifies them as employee of the Contractor, OR each must wear, visibly on their person at all times, a Contractor-provided laminate badge with employee name, photo, and Contractor’s company identification. In addition, all employees must have valid photo identification on their person at all times while on the Project.
1.12.3 Orientation and Refresher. Each Contractor employee performing work on the site shall receive an initial Project Access Policy (security) orientation before being authorized unescorted access to perform work on the Project. The time required for this orientation varies between 30 and 60 minutes, depending on current threat conditions or policy updates. Each employee shall receive a refresher of this orientation at least annually and any time the employee violates the Project Access Policy.
1.12.4 Contractor Vehicles. All Contractor vehicles shall have permanent or temporary identification on both sides, of a size sufficient to provide visual identification from at least 20 feet from the vehicle. Vehicles not so identified will not be allowed inside gated areas. All vehicles are subject to search. The Contractor shall provide a list of all company vehicles its employees will regularly use for work on this contract, to include make, model, color(s), year, and license plate of each vehicle. The Contractor must contact the COR in advance of arrival when an unlisted vehicle is expected.
1.12.5 Key Control. The Contractor shall be required to check out door keys, elevators keys, and an access badge at the Powerhouse Control Room upon arrival to the Project. All checked out keys must be returned to the Control Room each day before the Contractor leaves the Project. The Contractor shall ensure all keys are safeguarded and are not used by, or to allow entry for, unknown or unauthorized persons. The Contractor shall immediately report any occurrences of lost or duplicated keys/badges to the COR. If keys/badges are lost or duplicated by the Contractor or its employees, and in the opinion of the Government it is necessary to replace or re-code any or all locks for reasons of security, the direct cost of such replacement or re-coding will be charged to the Contractor.
1.12.6 POST-AWARD REQUIREMENTS: The Contractor must pre-screen Candidates using the E-verify Program (http://www.uscis.gov/e-verify) website to meet the established employment eligibility requirements.
The Vendor must ensure that the Candidate has two valid forms of Government issued identification prior to enrollment to ensure the correct information is entered into the E-verify system. An initial list of verified/eligible Candidates must be provided to the COR no later than 3 business days after the initial contract award
1.13 Environmental:
1.13.1. Conservation. The Contractor must ensure employees practice utility conservation. The Contractor is responsible for operating under conditions that prevent the waste of utilities, to include turning off lights when not necessary for work and turning off water faucets and valves when not in use.
1.13.2. Environmental and Hazardous Materials. The Contractor must establish systems and controls to minimize use of hazardous materials and generation of hazardous wastes, maximize recycling of materials and comply with local, state and federal guidelines for hazardous material and waste storage and disposal. Where applicable, the Contractor shall use environmentally safe products in the course of completion of their project.
The Contractor shall maintain, in company vehicle on site, Material Safety Data Sheets (MSDS) for all chemicals. In addition, all requirements in the EM 385-1-1 manual apply.
Part 2 – Definitions & Acronyms
2.1 Definitions:
Acceptable Quality Level (AQL). A predetermined value selected and used by the Government Quality Assurance inspector to distinguish between satisfactory and unsatisfactory performance.
Alteration. An alteration is also on existing elevator equipment but is not included under the full maintenance service arrangement. Alteration/modernization will be covered on an over and above Contract Line Item or it may result in a modification to the contract. The typical alteration results in a betterment to the safe operation of the equipment. In the case of elevators, when an alteration is made, all affected safety requirements must be complied with. All work as part of an alteration is required to comply with specific requirements of the current ASME A17.1 Code edition. See ASME A17.1 for requirements.
Contractor. A supplier or service provider awarded a contract to provide specific supplies or service to the Government. The Government works directly with the prime Contractor; however, all references to and requirements of the Contractor in this document apply equally to any subcontractor.
Contracting Officer (KO). A person with authority to enter into, administer, and or terminate Government contracts and make related determinations and findings on behalf of the Government. Only a Contracting Officer is authorized to legally obligate the Government and to make any changes to this contract in terms of scope, time, or cost.
Contracting Officer’s Representative (COR). An employee of the Government who is appointed by the Contracting Officer to administer a contract after award. The COR monitors all technical aspects of the contract and assists in contract administration. A letter of designation issued to the COR, a copy of which is sent to the Contractor, states the responsibilities and limitations of the COR, especially with regard to changes in cost or price, estimates, or changes in delivery dates. The COR is not authorized to change or to waive any term of the contract, particularly with respect to scope, time, or cost.
Contractor Representative. The Contractor employee authorized by the Contractor to manage Contractor operations under this contract.
Defective Service. A service output that does not meet the standard of performance associated with the Performance Work Statement or Performance Requirements Summary.
Deliverable. Any item that can be physically delivered, including written documents/information.
Maintenance. The term Maintenance includes all maintenance, repair, and replacement activities on existing elevator equipment as stated in the contract scope of services. Maintenance activities are actions to maintain or restore equipment to a state in which it may safely perform its required operations as installed in accordance with the manufacturer’s recommendations. See ASME A17.1 for requirements. All maintenance, repair, and replacement of damaged, broken, or worn parts are to be done in a manner that ensures that the equipment may be operated safely.
Quality Assurance. The Government’s procedures to verify that the Contractor’s Quality Control Plan is effective in ensuring the services being performed by the Contractor meet the contract requirements, applicable regulations and operations manuals.
Quality Assurance Surveillance Plan (QASP). A written document specifying the surveillance methodology to be used by the Government for surveillance of Contractor performance. The QASP is a “living” document and may be changed through the life of the contract, as needed.
Quality Control. All necessary measures taken by the Contractor to assure that the quality of an end product or service conforms to contract requirements, applicable regulations and operations manuals.
Subcontractor. A supplier or service provider that enters into a contract with a prime Contractor to provide service under a Government contract. The Government does not have privity of contract with the subcontractor.
All requirements of the contract are binding on the subcontractor. The prime Contractor must ensure its subcontractor(s) understand all contract requirements.
Valid Repair Service Request. A service call to the Contractor from an individual formally named as authorized to call for service.

Part 3 – Government Furnished Property, Equipment, and Services
3.1 Services: The Government will provide schematic and structural drawings, if available, for the Contractor’s use when performing the work. All drawings shall be retained on Government premises. The Contractor must note any changes to the drawings and notify the COR that changes have been made, to ensure drawings are maintained up-to-date.
3.2 Utilities: The Government will provide utility services electrical outlets, water, and restroom facilities where available, that may be required in the performance of the contract. Utilities will be provided “as is,” and all extensions, adapters, and other necessary items shall be Contractors responsibility.
Part 4 – Contractor Furnished Items and Services
4.1 General: Contractor shall provide all management, personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personal services necessary to perform work under this contract that are not listed under Part 3 of this PWS.
4.2 Materials:
4.2.1 Parts and Materials. The Contractor shall be responsible to furnish the following parts and supplies: all replaceable type contacts, relays, switches, contact insulators, contact springs, copper braids, circuit boards, power supplies and all oils and greases, actuating gates, call buttons, car lamps/lenses and cleaning materials.
Contractor parts and supplies may be stored at a location designated by the Government; however, most spare parts will be carried in Contractor service vehicles.
4.2.2 Component Replacement. When a component in a labeled product is replaced, it must be replaced with an identical component manufactured under the original labeling service (certifying agency). Substitution of a different component is only permitted where it is equivalent to that which was tested, as determined by the certifying agency. The parts used for replacement are required to comply with all the requirements that the old parts originally complied with. The old replaced part shall be considered government property and disposed of, recycled or retained at the Governments discretion.
4.3 Warranty of Materials and Services: All materials provided and services performed under this contract shall be free from defects and conform to ASME A17.1. The Contractor shall be responsible for all materials delivered and work performed until completion and acceptance of the entire work, except for any completed unit of work which may have been accepted under the contract. The Contracting Officer or COR will give written notice of any defect or nonconformance within 120 days from the date of acceptance by the Government.
4.4 Excluded Services: The following are not covered in the full maintenance service arrangement and are considered over and above services:
4.4.1 Alterations (a.k.a., modernizations) are not covered in the full maintenance service arrangement.
4.4.2 Acts by parties other than the Contractor of vandalism, abuse, negligence, damage resulting from rescue and recovery operations, acts of God (including damage resulting from emergency power generator power spikes and low quality) are not included in the contract
4.4.3 Not included in routine maintenance are hoist enclosures, total replacement of hoist way control cables, door panels, car enclosures, frames, sills, hoist ropes, and complete replacement of motor assemblies unless damaged as a result of Contractor negligence.
4.4.4 Additional items excluded in the full maintenance coverage are cosmetic, construction, ancillary components of the elevator system, including the finishing, repairing, or replacement of the cab enclosure, ceiling frames, hoist way door panels, door frames, sills, car flooring, floor covering, main line power switches, main power breaker(s), hydraulic elevator jack outer housing, buried piping, smoke and fire sensors, fire service reports, main communication feeders to controller, security systems, batteries for emergency lighting and lowering that are not solely dedicated to the elevator, air conditioners, and heaters.
4.4.5 Routine cleaning and refinishing of the interior of cars and the exterior of the hoist way doors and frames is not included in the basic full maintenance service.
4.4.6 In the event that the Contractor encounters an item of work included in paragraph 4.4.1 through 4.4.5, the Contractor will report the situation to the COR. The report will include the work location, a detailed scope of the required work, justification for Contractor determination that the work was not included in the contract, and an itemized cost estimate (including time to repair, all required parts, and a breakdown of labor hours). NOTE:
Any part, component, or assembly unavailable from the manufacturer due to obsolescence remains the Contractor’s responsibility to obtain an engineered equivalent product from a commercial source.
Part 5 – Specific Tasks
5.1 Equipment Acceptance:
5.1.1 Acceptance. The Contractor shall accept all parts of the elevators in contract commencement condition and shall not prorate any of the parts for replacement purposes, other than items agreed upon in the Initial Maintenance Report.
5.1.2 Initial Assessment. Within 30 days of contract award, the Contractor, with the COR or their representative, shall determine the working order and condition of all elevators listed in Attachment 1 and prepare a written Initial Maintenance Report (see paragraph 1.9.3). The Government will either replace missing items or repair all items not in working order or serviceable condition, or the Contracting Officer will direct the Contractor to replace the missing items, or accomplish the repair, and reimburse the Contractor in accordance with the contract. The Contractor and the Contracting Officer shall certify their agreement as to the working order of the equipment.
5.2 Maintenance: The Contractor shall perform regular maintenance on the two Little Goose Elevators in accordance with the Maintenance Control Plan (MCP) and not less than once each month.
5.2.1 Preventive Maintenance. The maintenance work shall be in accordance with commercial practices and manufacturer’s specifications*, if available, and shall be intended to maintain each elevator in continuous safe and reliable operating condition. Maintenance includes, but it not limited to, cleaning, maintenance, repair, inspection, servicing, parts replacement, and adjustments and calibrations to the elevator equipment. The Contractor shall provide all elevator maintenance services for the two elevators described in Attachment 1, Little Goose Project Elevators.
5.2.2 Equipment. Equipment to be serviced includes, but is not limited to, hoist machinery, motor brushes, commutators, brake shoes and linings, relays, contacts, coils, wiring circuits, hoist way control cables, leveling devices, car ventilating equipment, car and hoist way door or gate hangers, tracks, cams, guides, infra-red door curtain, operating devices, interlocks, push buttons, hall lanterns, signal and accessory equipment., and all other equipment except as listed in paragraph 4.4 and the Contractor’s accepted Initial Maintenance Report.
5.2.3 Inspections & Tests. The Contractor shall perform necessary inspections and tests as required under ASME A17.1 Appendix N, including annual no-load tests and 5-year load tests using a Qualified Elevator Inspector (QEI). Inspections and tests shall conform to all federal, state, and local laws, regulations, and codes, regardless if the federal government is excluded from these requirements. The Contractor is responsible for scheduling and paying for an annual inspection that complies with Washington Elevator Rules, laws and policies. Copies of inspection and test reports, along with a record of all remedial action taken, shall be provided to the COR. See Attachment 1, Little Goose Project Elevators, for the next required 5-year load test for each elevator.
5.2.4 Housekeeping. The Contractor shall clean equipment spaces / machine rooms, cabinet filters and perform daily cleanup of job sites in conjunction with maintenance, inspections and tests, and repairs.
5.2.5 Maintenance Record. The Contractor shall maintain and annotate the maintenance records for each elevator in its respective machine room immediately following completion of preventative maintenance. This record shall not be removed from the elevator machine room and remains the property of the Government.
5.2.6 Safety Shut-downs. If an elevator is determined to present a threat to personal safety or property, the Contractor shall notify the COR immediately, providing information regarding the nature of the problem(s), corrective action(s) required, in order to have the elevator taken out of service according to Government protocols and plan initiated for appropriate action.
5.3 Routine Repair Service Calls: The Contractor shall provide routine repair service calls under the full maintenance service agreement. All repair service calls shall be considered routine unless identified as non-routine at the time of service request.
5.3.1 Response Time. The Contractor shall respond in-person to the Project on routine repair service calls not later than 1200 Pacific Time on the weekday following receipt of a valid repair service request (see Part 2, Definitions). If the Contractor does not arrive within the specified time, the Government reserves the right to call an alternate contractor to provide necessary services.
5.3.2 Excluded Services: The Contractor shall report to the work location to survey the needed repair. If necessary repairs are not covered in the full maintenance service contract, the Contractor shall provide the COR description of the needed repair and an estimate of time and cost of repairs. In such cases, the Contractor shall not commence repair work without notification from the COR. Note: Not to Exceed and Ceiling Prices associated with any estimate provided do not imply that the Government is obligated to pay the total estimated amount.
5.4 Non-Routine Repair Service Calls: The Contractor shall provide non-routine repair service calls on an as needed bases. Repair service requests may be made when the Government determines elevator failure may endanger life or property, have an adverse effect upon personnel or equipment (including entrapment), and/or may inhibit mission accomplishment. Non-Routine calls may require work outside normal duty hours. All repair service calls shall be considered routine unless identified as non-routine at the time of service request.
5.4.1 Response Time. The Contractor shall respond in-person to the Project and begin work on non-routine repair service calls not later than three hours from receipt of a valid non-routine repair service request. If the authorized caller receives a pre-recorded phone message, the Contractor shall have one hour to respond back with a phone call. If the Contractor does not arrive within the specified time, non-routine calls shall only be billed at the rate contracted in the non-routine call out contract line items regardless of number of trips required to repair/replace the covered in section 5.2.2.
5.4.2 Non-Routine Services: The Contractor shall report to the work location to survey the needed repair and shall immediately proceed with repairs necessary to eliminate the condition. If necessary repairs cannot be completed in response to the call at the time, the Contractor shall provide the COR a description of the repair and an estimate of time and cost of repairs as soon as practicable and not later than the next business day following the callout. Note: The vendor shall only proceed with the additional work once the COR provides authorization and funding is applied to the contract.
5.5 Reporting: For all service activities, the Contractor shall prepare a written Maintenance Report and submit the report to the COR within two business days of the work. The Maintenance Report shall identify each elevator, date of inspection or service, name of inspector, time in and time out of the facility, maintenance or repair work performed, repairs needed, development of any defects, and overall condition of the elevator. If repairs are identified that require work considered over and above routine maintenance work, the Contractor shall notify the COR for appropriate action.
5.6 Special Conditions:
5.6.1 Out of Scope Work.
5.6.1.1 The Contractor shall not perform any work not covered by this performance work statement that will add to the cost of the contract, unless authorized in writing by the Contracting Officer. If the
Contractor performs any additional work or provides parts not covered in this contract, without prior approval of the Contracting Officer, it will be done at the Contractor’s own risk and expense.
5.6.1.2 The Government reserves the right to obtain repair services for the excluded equipment appurtenances and accessories from other sources. The Government may undertake or award contracts for other work. The Contractor shall fully cooperate with such other Contractors and Government employees and carefully fit his own work to such additional work as may be directed by the Contracting Officer. The Contractor shall not commit or permit any act which will interfere with the performance of work by any other Contractor or by Government employees.
5.6.2 Changes to Government Property. The Contractor shall make no alterations to Government property or changes to the existing circuits, circuitry wiring and sequencing or otherwise alter the original circuit or wiring design of the elevators except with the written permission of the Government. The Contractor may submit a proposal for any recommended modification or change to the COR in a complete, neatly prepared wiring diagram, accompanied by description of the proposed change.
5.6.3 Corrective Actions. The Government may perform, or cause to be performed by a qualified third party, inspections of the elevators, at no cost to the Contractor, for purposes of capital asset management and quality assurance. If discrepancies are discovered during these inspections, the Contractor will be notified in writing of any determination and may be responsible for the corrective actions.

QUALITY ASSURANCE AND SURVEILIANCE PLAN
The Contractor service requirements are summarized into performance objectives that relate directly to mission essential items. The performance threshold briefly describes the minimum acceptable levels of service required for each requirement. These thresholds are critical to mission success. The Government will evaluate the Contractor’s performance under this contract in accordance with the Quality Assurance Surveillance Plan. This plan is primarily focused on what the Government must do to ensure the Contractor’s QCP is effective in ensuring performance is in accordance with the specification. It defines how the performance standards will be applied, the frequency of surveillance, and the minimum acceptable defect rate(s) The Contractor, not the Government, is responsible for management and quality control to meet the terms of the contract. The role of the Government is to monitor quality to ensure that contract standards are achieved.
