HAZARD TREE REMOVAL

ALBENI FALLS PROJECT

IDAHO

1. Objective. The purpose of this plan is to provide a quality assurance surveillance plan for

Hazard Tree Removal Services at the Albeni Falls Dam recreation areas. This plan provides a basis for the CPS and/or the quality assurance evaluator (QAE) to evaluate the quality of the contractor’s performance. The oversight provided for in the contract and in this plan will help ensure that service levels reach and maintain the required levels throughout the contract term.

2. Performance Standards.

A. Quality Level. By monitoring the contractor, the CPS will determine whether the performance levels set forth in the contract and have been attained. Performance standards for all tasks are specified in the Performance Work Statement, Performance Requirements Summary, and contract clauses.

B. Frequency. During performance of this contract, the CPS and/or the QAE will perform inspections of the work and will analyze whether the work is being performed in accordance with the PWS and PRS. The CPS and/or QAE will confirm whether the contractor has satisfactorily met all contract requirements.

3. Evaluation Methods. The CPS and/or QAE will conduct performance evaluations to ensure performance levels identified in the contract are met. The following techniques will be used to perform surveillance:

A. 100% Inspection. The CPS will perform 100% inspections during the contract. Sites will be inspected to ensure that all performance standards identified in the PWS have been completed to the satisfaction of the CPS. Any discrepancies noted during the inspections will be discussed with the contractor as soon as practicable. The CPS will document any findings, discrepancies, and discussions as necessary.

B. To minimize unnecessary mobilizations and demobilizations, the contractor will keep the CPS informed of schedules and locations throughout the course of the contract. This will allow specific areas to be inspected after the work is performed, ensuring that inspections are not performed prior to the contractor’s entry to the work area.

C. Any findings, discrepancies and discussions will be identified and documented on a form like the attached inspection report as is necessary. The report will be filed with all other pertinent contract documents and will be used to determine compliance with all contract requirements.
