Hand carried quote submittal procedures have changed. When dropping off all hand carried proposals, the official delivery location is now the guard station in the main foyer of Federal Center South, located at 4735 E.
Marginal Way South where the official time clock is also located. Notify the guard on duty that you have a quote for Contracting Division. The point of contract will be notified and a representative from Contracting Division will sign in your package(s) and transport them to Contracting Division. 4. PROSPECTIVE OFFERORS: Contractors are required to be registered in the System for Awards Management
(SAM) for awards resulting from solicitations.
This rule more efficiently implements the Debt Collection Improvement Act of 1996, as it requires contractors to be registered in SAM for consideration of future solicitations, awards, and payment. Registration is required prior to award of any contract, basic agreement, basic ordering agreement, or blanket purchase agreement from a solicitation. ANNOUNCEMENT TO OFFERORS
Due to recent national events Seattle District US Army Corps of Engineers shall be conducting business under heightened security for the foreseeable future.
Access to Federal Center South, 4735 E Marginal Way S, Building 1202, Seattle, WA 98134 will be through the front lobby only. The building is under Federal Protective Service, which means that persons entering the facility are subject to inspection; including purses, packages, etc. All deliveries shall be thoroughly inspected. In addition, visitors may be required to be escorted by Corps personnel while in the building.

PERFORMANCE WORK STATEMENT (PWS)
Libby Dam Garage Doors and Openers Replacement
1. GENERAL: This is a non-personnel services contract to remove, provide, and install seven new garage doors and openers in the Resource Shop and Tech. section garage and one in the Warehouse and one in the powerhouse at Libby Dam. The Government shall not exercise any supervision or control over the contract service providers performing the services herein. Such contract service providers shall be accountable solely to the Contractor who, in turn is responsible to the Government.
1.1 Description of Services/Introduction: The contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personal services necessary to perform Garage Doors and Openers replacement as defined in this Performance Work Statement except for those items specified as government furnished property and services. The contractor shall perform to the standards in this contract.
1.2 Background: Libby Dam currently has five garage doors and openers located in the Resource Shop at
Libby Dam, one in the warehouse, two in the Tech. Section garage and one in the Powerhouse. These doors and openers are in poor condition and do not function properly after years of service. These Doors and Openers need to be removed and replaced with new doors and openers that function properly.
1.3 Objectives: To replace Doors and Openers with new equipment that functions properly to access the buildings.
1.4 Scope: Garage Doors and Openers Replacement services in the Resource shop and Tech section garage include:
The Contractor will remove existing garage doors, tracks, manual openers, hardware and electric openers.
The existing doors and openers will be removed in such a manor to minimize damage to panels and parts so they can be re-used to repair other doors on the project. The contractor shall provide and install seven new garage doors 12’-2” x 16’-0” (size to be field verified) and openers in existing resource shop area.
Provide and install seven new Commercial garage doors the same as or similar to Amarr Model 2700. Steel front, Steel back, 2”foamed-in-place polyurethane insulated (R-19.40), lightly ribbed panel design with no windows. Color brown. Standard Lift R/A 3" track. Solid Spring shafts with 25,000 cycle springs. Weather stripping: use Top seal mounted to top section then 2" brush seal for side seals. Provide one strut on each top section. Provide and install a Miller Electronic safety edge ME 116-4 wire (or approved equal) to bottom edge of all five new garage doors. Installer to run low voltage wire. Note: Some Duct work may be attached or hanging from horizontal tracks. Temporally support and reconnect to new track. Check each back hang support to make sure it is properly anchored to the concrete ceiling. Replace anchors or back hangs if needed (note: if new anchors are needed the locations must be approved prior to installation by government official). Remove and reinstall existing security sensors that are attached to the doors and track. *2 year warranty on all parts and labor.
Contractor will also provide and install seven new garage door openers the same as or similar to the 16' Liftmaster Model T7511L5 3/4hp Heavy duty commercial operator with brake, 3 Phase, 480V. All operators to include a manual chain hoist with quick disconnect for hand operation if needed similar to existing hoists. Note: do not use stock wall stations; upgrade too Exterior surface mount NEMA-4 Wall station. Reconnect existing Exterior Key Controls to operate new openers. Install new Safety eyes 6"off the ground. Note: Installer to run low voltage wire for safety eyes. Check all back hang supports to make sure they are properly anchored and in good condition/ replace if necessary. Replace concrete anchors or back hangs if needed. The contractor will provide all necessary lifts and equipment needed to remove and install new doors and openers. *2 year warranty on all parts and labor. Contractor will comply with EM 385-1-1 and Libby Dam standards and SOP’s.
Garage Door and Opener Replacement services in the warehouse garage include:
The Contractor will remove existing garage door, track, manual opener, hardware and electric opener. Remove and reinstall existing security sensors that are attached to the doors and track. *2 year warranty on all parts and labor.

Garage Door and Opener Replacement services in the Powerhouse include:
The Contractor will remove existing garage door, manual opener, hardware and electric opener. The existing door and opener will be removed in such a manor to minimize damage to panels and parts so they can be re-used to repair other doors on the project. The contractor shall provide and install one new garage doors 15’-2” x 18’-0” (size to be field verified) and opener in existing Powerhouse area.
Provide and install one new Commercial garage door the same as or similar to Amarr Model 2700:
Steel front Steel back Polyurethane insulated R-19.40. 1 section of 24"x12" insulated windows. Lightly Ribbed panel design. Color brown. *Full Vertical lift door*. Note* Re use existing 3" full vertical track.
Existing track is welded and anchored into place. Brace track in any spots if necessary. Note* Existing door is 2" thick. Solid Spring shafts with 25,000 cycle springs. Weather stripping use Top seal mounted to top section then 2" brush seal for side seals. * 3" strut on each section*. Door to have a Miller Electronic safety edge installed* installer to run low voltage wire. Removal and Haul of Old door. Remove and reinstall of security sensors attached to the doors and track. New Door installed. Large Electric Lift Required to reach top of track approximately 36'. *2 year warranty on all parts and labor.
Contractor will also provide and install one new garage door opener the same as or similar to the Liftmaster GH-50-43-L4 Gear Head Heavy Duty Commercial jack shaft operator with chain hoist. 460V 3 phase. Upgrade to a higher quality Nema-4 wall station. Removal and Haul of old unit. Hook up unit to existing external keyed entry. New operator installed. Note* This operator is for the 15'2"x18' Full vertical lift door. *2 year warranty on all parts and labor. Check all back hang supports to make sure they are properly anchored and in good condition/ replace if necessary. Replace concrete anchors or back hangs if needed. The contractor will provide all necessary lifts and equipment needed to remove and install new doors and openers. Contractor will comply with EM 385-1-1 and Libby Dam standards and SOP’s.
1.5 Period of Performance: The contractor shall complete all phases of work at Libby Dam within 7 days of notice of award.
1.6 General Information:
1.6.1 Quality Control: The contractor shall develop and maintain an effective quality control program to ensure services are performed in accordance with this PWS. The contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. The contractor’s quality control program is the means by which he assures himself that his work complies with the requirement of the contract. After acceptance of the quality control plan the contractor shall receive the contracting officer’s acceptance in writing of any proposed change to his QC system.
1.6.2 Quality Assurance: The government shall evaluate the contractor’s performance under this contract in accordance with the Quality Assurance Surveillance Plan. This plan is primarily focused on what the Government must do to ensure that the contractor has performed in accordance with the performance standards. It defines how the performance standards will be applied, the frequency of surveillance, and the minimum acceptable defect rate(s).
1.6.3 Recognized Holidays: Contractor shall NOT be permitted to work on these days.
Thanksgiving Christmas New Year’s Day Martin Luther King Day Presidents Day Independence Day (July 4th)
1.6.4 Hours of Operation: Contractors shall be permitted to be onsite between the hours of 0630 and
1645 Monday thru Thursday. Extensions or changes to these timeframes may be requested in written format and provided to the QAR (Quality Assurance Representative).
1.6.5 Place of Performance: The work to be performed under this contract will be performed at Libby
Dam, 17877 Montana Highway 37, Libby, MT 59923.
1.6.6 Type of Contract: The government will award a firm fixed price services contract.
1.6.7 Security Requirements: Contractor personnel performing work under this contract must have an
FBI background check prior to working unescorted at Libby Dam. All personnel that will be working at Libby Dam will be required to fill out the attached background check forms (See Technical Exhibits 3 and 4).
All contractor and all associated sub-contractors employees shall comply with applicable installation, facility and area commander installation/facility access and local security policies and procedures (provided by government representative). The contractor shall also provide all information required for background checks to meet installation/facility access requirements to be accomplished by installation Provost Marshal Office, Director of Emergency Services or Security Office. Contractor workforce must comply with all personal identity verification requirements
(FAR clause 52.204-9, Personal Identity Verification of Contractor Personnel) as directed by DOD, HQDA and/or local policy. In addition to the changes otherwise authorized by the changes clause of this contract, should the Force Protection Condition (FPCON) at any installation or facility change, the Government may require changes in contractor security matters or processes.
All contract employees, including subcontractor employees who are not in possession of the appropriate security clearance or access privileges, will be escorted in areas where they may be exposed to classified and/or sensitive materials and/or sensitive or restricted areas.
The Contractor must pre-screen Candidates using the E-verify Program (http://www.uscis.gov/e-verify) website to meet the established employment eligibility requirements. The Vendor must ensure that the Candidate has two valid forms of Government issued identification prior to enrollment to ensure the correct information is entered into the E-verify system. An initial list of verified/eligible Candidates must be provided to the QAR no later than 3 business days after the initial contract award.

1.6.8 Post Award Conference/ Periodic Progress Meetings: The Contractor agrees to attend any post award conference convened by the contracting activity or contract administration office in accordance with Federal Acquisition Regulation Subpart 42.5. The contracting officer, Quality Assurance Representative (QAR), and other Government personnel, as appropriate, may meet periodically with the contractor to review the contractor's performance. At these meetings the contracting officer will apprise the contractor of how the government views the contractor's performance and the contractor will apprise the Government of problems, if any, being experienced. Appropriate action shall be taken to resolve outstanding issues. These meetings shall be at no additional cost to the government.
1.6.9 Quality Assurance Representative (QAR): The (QAR) will be identified by separate letter. The
QAR monitors all technical aspects of the contract and assists in contract administration. The QAR is authorized to perform the following functions: assure that the Contractor performs the technical requirements of the contract: perform inspections necessary in connection with contract performance: maintain written and oral communications with the Contractor concerning technical aspects of the contract: issue written interpretations of technical requirements, including Government drawings, designs, specifications: monitor Contractor's performance and notifies both the Contracting Officer and Contractor of any deficiencies: coordinate availability of government furnished property, and provide site entry of Contractor personnel. A letter of designation issued to the QAR, a copy of which is sent to the Contractor, states the responsibilities and limitations of the QAR, especially with regard to changes in cost or price, estimates or changes in delivery dates. The QAR is not authorized to change any of the terms and conditions of the resulting order.
1.6.10 Key Personnel: The following personnel are considered key personnel by the Government: Josh
Erickson – Libby Dam Chief of Tech., Josh Baltz – Libby Dam Natural Resource Manager. The contractor shall provide a contract manager who shall be responsible for the performance of the work. The name of this person and an alternate who shall act for the contractor when the manager is absent shall be designated in writing to the Contracting Officer. The contract manager or alternate shall have full authority to act for the contractor on all contract matters relating to daily operation of this contract.
1.6.11 Identification of Contractor Employees: All contract personnel attending meetings, answering
Government telephones, and working in other situations where their contractor status is not obvious to third parties are required to identify themselves as such to avoid creating an impression in the minds of members of the public that they are Government officials. They must also ensure that all documents or reports produced by contractors are suitably marked as contractor products or that contractor participation is appropriately disclosed. When onsite, Contractor personnel shall wear and display the appropriate “visitor badges” that will be issued daily by the Administrative Section at Libby Dam. Contractor personnel will be required to sign in to the Visitor Log Book upon entry to the project and sign out of the Visitor Log Book prior to leaving.
1.6.12 Contractor Travel: All travel costs and coordination will be covered by the contractor and included in the final quote.
1.6.13 Data Rights: The Government has unlimited rights to all documents/material produced under this contract. All documents and materials, to include the source codes of any software, produced under this contract shall be Government owned and are the property of the Government with all rights and privileges of ownership/copyright belonging exclusively to the Government. These documents and materials may not be used or sold by the contractor without written permission from the Contracting Officer. All materials supplied to the Government shall be the sole property of the Government and may not be used for any other purpose. This right does not abrogate any other Government rights.
1.6.14 Organizational Conflict of Interest: Contractor and subcontractor personnel performing work under this contract may receive, have access to or participate in the development of proprietary or source selection information (e.g., cost or pricing information, budget information or analyses, specifications or work statements, etc.) or perform evaluation services which may create a current or subsequent Organizational Conflict of Interests (OCI) as defined in FAR Subpart 9.5. The Contractor shall notify the Contracting Officer immediately whenever it becomes aware that such access or participation may result in any actual or potential OCI and shall promptly submit a plan to the Contracting Officer to avoid or mitigate any such OCI. The Contractor’s mitigation plan will be determined to be acceptable solely at the discretion of the Contracting Officer and in the event the Contracting Officer unilaterally determines that any such OCI cannot be satisfactorily avoided or mitigated, the Contracting Officer may affect other remedies as he or she deems necessary, including prohibiting the Contractor from participation in subsequent contracted requirements which may be affected by the OCI.
DEFINITIONS & ACRONYMS
2. Definitions and Acronyms:
2.1 DEFINITIONS:
2.1.1 CONTRACTOR. A supplier or vendor awarded a contract to provide specific supplies or service to the government. The term used in this contract refers to the prime.
2.1.2 CONTRACTING OFFICER. A person with authority to enter into, administer, and or terminate contracts, and make related determinations and findings on behalf of the government. Note: The only individual who can legally bind the government.
2.1.3 QUALITY ASSURANCE REPRESENTATIVE (QAR). An employee of the U.S. Government appointed by the contracting officer to administer the contract. Such appointment shall be in writing and shall state the scope of authority and limitations. This individual has authority to provide technical direction to the Contractor as long as that direction is within the scope of the contract, does not constitute a change, and has no funding implications. This individual does NOT have authority to change the terms and conditions of the contract.
2.1.4 DEFECTIVE SERVICE. A service output that does not meet the standard of performance associated with the Performance Work Statement.
2.1.5 DELIVERABLE. Anything that can be physically delivered, but may include non-manufactured things such as meeting minutes or reports.
2.1.6 KEY PERSONNEL. Contractor personnel that are evaluated in a source selection process and that may be required to be used in the performance of a contract by the Key Personnel listed in the PWS. When key personnel are used as an evaluation factor in best value procurement, an offer can be rejected if it does not have a firm commitment from the persons that are listed in the proposal.
2.1.7 PHYSICAL SECURITY. Actions that prevent the loss or damage of Government property.
2.1.8 QUALITY ASSURANCE. The government procedures to verify that services being performed by the Contractor are performed according to acceptable standards.
2.1.9 QUALITY ASSURANCE SURVEILLANCE PLAN (QASP). An organized written document specifying the surveillance methodology to be used for surveillance of contractor performance.
2.1.10 QUALITY CONTROL. All necessary measures taken by the Contractor to assure that the quality of an end product or service shall meet contract requirements.
2.1.11 SUBCONTRACTOR. One that enters into a contract with a prime contractor. The Government does not have privity of contract with the subcontractor.
2.1.12 WORK DAY. The number of hours per day the Contractor provides services in accordance with the contract.
2.1.13 WORK WEEK. Monday through Thursday, unless specified otherwise.

GOVERNMENT FURNISHED PROPERTY, EQUIPMENT, AND SERVICES
3. GOVERNMENT FURNISHED ITEMS AND SERVICES:
3.1 Facilities: The Government will provide all necessary workspace for the contractor to complete the services outlined in the PWS. Facilities include: limited telephone use, and use of the restroom facilities.
***NOTE: There is no cellular service available at Libby Dam so the contractor shall plan accordingly.
CONTRACTOR FURNISHED ITEMS AND SERVICES
4. CONTRACTOR FURNISHED ITEMS AND RESPONSIBILITIES:
4.1 General: The Contractor shall furnish all supplies, equipment, facilities and services required to perform work under this contract that are not listed under Section 3 of this PWS.
4.2 Materials: The Contractor shall furnish 5 garage doors, tracks, mounting hardware, manual openers, safety edges and seals. The Contractor will also supply 5 openers, chains, controller, brakes, safety eyes, and controls for each door and all necessary incidentals for installation of doors and openers.
4.3 Equipment: The Contractor shall mobilize and come to Libby Dam with all equipment necessary to complete the task outlined in the PWS. This equipment may include but is not limited to scissor lifts, ladders, fall protection equipment, scaffolding, Personal Protective Equipment, and all (hand and specialized) tools necessary.
4.4 Training. The Contractor shall be responsible for the following training requirements:
(a) AT Level I Training: All contractor employees, to include subcontractor employees, requiring access to Army installations, facilities and controlled access areas shall complete AT Level I awareness training within 30 calendar days after contract start date or effective date of incorporation of this requirement into the contract, whichever is applicable. The contractor shall submit certificates of completion for each affected contractor employee and subcontractor employee, to the QAR or to the contracting officer, if a QAR is not assigned, within 5 calendar days after completion of training by all employees and subcontractor personnel. (b) Access and General Protection/Security Policy and Procedures. All contractor and all associated subcontractors employees shall comply with applicable installation, facility and area commander installation/facility access and local security policies and procedures (provided by government representative). The contractor shall also provide all information required for background checks to meet installation access requirements to be accomplished by installation Provost Marshal Office, Director of emergency Services or Security Office. Contractor workforce must comply with all personal identity verification requirements as directed by DOD, HQDA and/or local policy. In addition to the changes otherwise authorized by the changes clause of this contract, should the Force Protection Condition (FPCON) at any individual facility or installation change, the Government may require changes in contractor security matters or processes.
(c) iWATCH and/or CorpsWatch Training. The contractor and all associated sub-contractors shall brief all employees on the local iWATCH, Corps Watch, or See Something, Say Something program (training standards provided by the requiring activity ATO). This locally developed training will be used to inform employees of the types of behavior to watch for and instruct employees to report suspicious activity to the QAR. This training shall be completed within 30 calendar days of contract award and within 30 calendar days of new employees commencing performance with the results reported to the QAR NLT 5 calendar days after contract award."

5.1 Basic Services. Garage Doors and Openers Replacement services in the Resource shop and Tech section garage include:
The Contractor will remove existing garage doors, tracks, manual openers, hardware and electric openers.

The contractor shall provide and install seven new garage doors 12’-2” x 16’-0” (size to be field verified) and openers in existing resource shop area.
Provide and install seven new Commercial garage doors Amarr Model 2700 (or approved equal): Steel front, Steel back, 2”foamed-in-place polyurethane insulated (R-19.40), lightly ribbed panel design with no windows. Color brown. Standard Lift R/A 3" track. Solid Spring shafts with 25,000 cycle springs. Weather stripping: use Top seal mounted to top section then 2" brush seal for side seals. Provide one strut on each top section. Provide and install a Miller Electronic safety edge ME 116-4 wire (or approved equal) to bottom edge of all five new garage doors. Installer to run low voltage wire. Note: Some Duct work may be attached or hanging from horizontal tracks. Temporally support and reconnect to new track. Check each back hang support to make sure it is properly anchored to the concrete ceiling. Replace anchors or back hangs if needed (note: if new anchors are needed the locations must be approved prior to installation by government official). Remove and reinstall existing security sensors that are attached to the doors and track. * 2 year warranty on all parts and labor.
Contractor will also provide and install seven new garage door openers: 16' Liftmaster Model T7511L5 3/4hp Heavy duty commercial operator with brake, 3 Phase, 480V (or approved equal). All operators to include a manual chain hoist with quick disconnect for hand operation if needed similar to existing hoists.
Note: do not use stock wall stations; upgrade too Exterior surface mount NEMA-4 Wall station. Reconnect existing Exterior Key Controls to operate new openers. Install new Safety eyes 6"off the ground. Note:
Installer to run low voltage wire for safety eyes. Check all back hang supports to make sure they are properly anchored and in good condition/ replace if necessary. Replace concrete anchors or back hangs if needed. The contractor will provide all necessary lifts and equipment needed to remove and install new doors and openers. * 2 year warranty on all parts and labor. Contractor will comply with EM 385-1-1 and Libby Dam standards and SOP’s.
