ryandale.r.fernandez@usace.army.mil.

SUBMISSION REQUIREMENTS

The Government will award a firm-fixed-price (FFP) contract to the responsible offeror whose offer conforms to the requirements as stated in this request for quotation (RFQ). The award will be made based on the following three

(3) evaluation factors: Technical, Past Performance, and Price.

The Government intends to evaluate quotes and make award without questions to offerors. Therefore, the offeror's initial quote should contain the offeror's best terms from a Technical and Price standpoint. Unclear quotes may not result in questions for clarifications. Offers with an omission or unclear quote may be judged to mean the offeror does not fully understand the requirements or understand what it takes to meet the requirements, regardless of the proposed price, and will be rated unacceptable. In that case, the offeror may be found unacceptable and ineligible for award. The Government reserves the right to ask questions of offerors if the Contracting Officer determines them to be necessary. In order to be considered for award, all items labeled “MUST” are required. Include a cover letter on offeror’s letterhead, and information such as Cage Code, Tax ID#, and DUNS #. Letter shall be signed by the representative managing the account

Evaluation Criteria Factors/Ratings:

Factor I Technical –

The offer will be evaluated and MUST ensure all the items below are provided. This information consists of the offeror’s intended approach to executing the contract. The submission for this section shall not exceed 5 pages in length, excluding resumes.

Equipment Plan: The proposed equipment list should only include the equipment which will be used on this contract. Equipment listed should be identified for use on roadways. Include size, make, model, and ownership (owned or rented) of equipment.

Staffing Plan: The proposed staffing plan shall include both numbers of personnel proposed and position titles. Resumes for proposed key personnel, consisting of experience and other pertinent information, inclusive of superintendents, quality control inspectors, and foremen must also be submitted.

Capabilities Plan: The proposed capabilities should include adequate documentation to illustrate the

Offeror’s capabilities to successful complete the work identified within the solicitation.

The Government will evaluate the offeror’s equipment, staffing, and capabilities plans for acceptability. A complete and reasonable equipment, staffing and capabilities plan will receive an Acceptable rating. Incomplete or unreasonable plans, or missing any plans will receive a rating of Unacceptable. The Government will evaluate the clarity and strength of the overall organization and how well it is organized and structured to execute the entire scope of work. The Government will evaluate the capability of the firm to execute the project. The Government will evaluate the offeror’s resources to determine if it has the capability to self-perform the requirements of the project, in accordance with public policy to assure adequate interest in and supervision of all work.

RATING DESCRIPTION

ACCEPTABLE

Offer clearly meets the minimum requirements of the solicitation with the Equipment Plan, Staffing Plan, and Capabilities Plan submitted.

UNACCEPTABLE

Offer does not clearly meet the minimum requirements of the solicitation and/or neither the Equipment Plan, Staffing Plan, or Capabilities Plan were submitted.

Factor II Past Performance –

The offer will be evaluated and MUST demonstrate recent, relevant experience on similar projects. If the offeror is a joint venture, each firm shall demonstrate experience.

Submit at least three detailed examples of relevant projects that are currently well underway or performed within the past 10 years. Work experience shall be demonstrated for at least 5 years.

Each project shall include a reference and/or Past Performance Questionnaire. The offeror shall arrange for the reference point of contact to complete the questionnaire and return directly to Ryan Fernandez, Contract Specialist, by email at ryandale.r.fernandez@usace.army.mil. The questionnaires shall be emailed directly from the references. The Government will not release the questionnaire forms to the offeror at any time, in order for the Government to solicit candid, unbiased responses and comments.

The Government will evaluate how well the offeror demonstrated their recent (within five years of the issue date of the RFQ), relevant experience with similar projects. If the offeror is a joint venture, each entity must demonstrate qualifying experience comparable to its role on the project. The Government reserves the right to verify the experience record of cited projects or other recent projects or to interview owners or references. The Government reserves the right to check any or all cited references to verify supplied information. A rating of Unacceptable will be given to an offeror who cannot demonstrate a history of recent, relevant experience. The Government will consider extent of recent experience, degree of relevance of such experience to this project, demonstrated familiarity with similar sites and local conditions. Some examples of relevancy to this project may include, but not be limited to: (1) number, size, type work, location; (2) dates (completion within last 10 years); and (3) firm's role and extent of work self-performed (subcontracting out all work is an example of less relevant experience).

RATING DESCRIPTION

ACCEPTABLE

Past performance references and /or PPQ has satisfactory ratings, and meets the minimum requirements of the solicitation with three detailed examples of relevant projects submitted.

UNACCEPTABLE

Past performance references and/ or PPQ has unsatisfactory ratings, or does not meet the minimum requirements of the solicitation with three detailed examples of relevant projects submitted.

NEUTRAL No past performance submitted.

Factor III Price –

The Government may require additional pricing information to assist in determining the extent to which the price is fair and reasonable, and the pricing information shall be provide if requested. The Government will evaluate quotes for award purposes by adding the total of all CLIN prices to ensure that pricing is not unbalanced.

All line items must be priced. Quotes providing partial pricing may be considered non-responsive.

⦁ Bid Schedule – Completed Government template MUST be provided.

ADJECTIVE RATINGS AND DEFINITIONS TO BE USED TO BEST REFLECT

YOUR EVALUATION OF THE CONTRACTOR’S PERFORMANCE

RATING DEFINITION NOTE

(E) Exceptional Performance meets contractual requirements and exceeds many to the Government/Owner’s benefit. The contractual performance of the element or sub-element being assessed was accomplished with few minor problems for which corrective actions taken by the contractor was highly effective.

An Exceptional rating is appropriate when the Contractor successfully performed multiple significant events that were of benefit to the Government/Owner. A singular benefit, however, could be of such magnitude that it alone constitutes an Exceptional rating. Also, there should have been NO significant weaknesses identified.

(VG) Very Good Performance meets contractual requirements and exceeds some to the Government’s/Owner’s benefit. The contractual performance of the element or sub-element being assessed was accomplished with some minor problems for which corrective actions taken by the contractor were effective.

A Very Good rating is appropriate when the Contractor successfully performed a significant event that was a benefit to the Government/Owner. There should have been no significant weaknesses identified.

(S) Satisfactory Performance meets minimum contractual requirements. The contractual performance of the element or sub-element contains some minor problems for which corrective actions taken by the contractor appear or were satisfactory.

A Satisfactory rating is appropriate when there were only minor problems, or major problems that the contractor recovered from without impact to the contract. There should have been NO significant weaknesses identified. Per DOD policy, a fundamental principle of assigning ratings is that contractors will not be assessed a rating lower than Satisfactory solely for not performing beyond the requirements of the contract.

(M) Marginal Performance does not meet some contractual requirements. The contractual performance of the element or sub-element being assessed reflects a serious problem for which the contractor has not yet identified corrective actions. The contractor's proposed actions appear only marginally effective or were not fully implemented.

A Marginal is appropriate when a significant event occurred that the contractor had trouble overcoming which impacted the Government/Owner.

(U) Unsatisfactory Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance of the element or sub-element contains serious problem(s) for which the contractor's corrective actions appear or were ineffective.

An Unsatisfactory rating is appropriate when multiple significant events occurred that the contractor had trouble overcoming and which impacted the Government/Owner. A singular problem, however, could be of such serious magnitude that it alone constitutes an unsatisfactory rating.
