TOE-DRAIN REPAIR
STATEMENT OF WORK
General: The Mayfield Dike is associated with the Aylesworth Creek Lake project located in the northeastern part of Lackawanna County and provides flood risk management for the town of Mayfield in association with substantial high water events at Aylesworth Creek. The dike is located approximately 11 miles northeast of Scranton and 3 miles southeast of Carbondale (See Site Location Map).
Site Access: Access to the site is available via Old Mining Road which can be readily accessed by way of state highway route 6 at the Meredith Street exit (See Site Location Map). The old mining road is a privately owned and maintained road with keyed / gated access. Keys to the access gate will be provided to the Contractor for the duration of the inspection and construction project.
The keys may not be copied and must be returned to the COR at the conclusion of work. Old Mining Road is a gravel road that is privately maintained and is very steep and winding in places.
A site visit is highly recommended by prospective bidders to view the access conditions, and other features of work before bidding on the work described herein.
Objectives: The US Army Corps of Engineers at Aylesworth Creek Lake in Archbald, Pennsylvania requires removal of an existing 12-inch diameter Corrugated Metal Pipe (CMP) and replacement with new 8-inch diameter High Density Polyethylene (HDPE) pipe. The work shall include excavating into the existing toe-drain; replacing the existing drainage zone pervious material to the lines and grades shown on the drawings with new drainage zone material and geotextile; replacing the pipe; placing a precast inlet box; placing precast headwall and wingwalls unit at the discharge end; backfilling; grading the site as shown on drawings; and performing a video camera inspection of the toe-drain system upon completion of work. The Contractor shall provide all labor, supervision, materials, equipment, supplies, and transportation to complete the work.
Pre-Construction Survey: A topographic survey was not performed as part of the design of this project. Prior to any construction, the contractor shall hire a licensed surveyor to perform a basic survey of the area. In particular the survey shall record the invert elevation of the existing pipe outlet and general ground elevations in the area of the toe drain system. This information will be necessary for the contractor to size features such as the inlet box (i.e. to ensure the depth of the box is properly selected for the top to be flush with finished grade and the pipe penetration holes at correct elevations). Survey information shall be submitted to the government who will work with the contractor to ensure that final elevations for structures/pipes are fixed accordingly prior to ordering of materials and start of construction.
Toe-Drain Repair
1. Existing Conditions: The area of the toe drain is relatively flat and grassed with a drop in grade at the treeline in the vicinity of the outlet. Drainage/grading is poor and surface runoff frequently accumulates in the area. Due to the poor drainage the ground surface is typically soft and rutted with water standing in the ruts. Refer to the attached photos for a general illustration of conditions. The depth of pipe varies but it should be assumed that the top of existing pipe is 4’ to 5’ below existing grade on average and that an average excavation depth of 6’ will be required to perform the work. The contractor shall anticipate that there are two basic zones of materials over the existing pipe. Directly above and surrounding the existing CMP is an existing zone of gravelly/sandy material referred to herein as Pervious Fill. Above this pervious fill zone is a mixture of soil types. There is typically some amount of subsurface groundwater flowing into the toe drain system as discussed in the Dewatering paragraph.
2. Excavation: Prior to excavation the contractor shall remove existing vegetation and strip/stockpile topsoil separately from other excavation materials. Stripped topsoil may be re-used at finished grade after backfilling but the contractor shall be aware that much of the topsoil will likely be wet and require drying to facilitate placement. Contractor shall ensure that excavation does not cause any instability into the adjacent dike slopes. It may be advantageous to perform excavation in shorter reaches for preventing instability during excavation. The contractor shall excavate and fully remove the 12-inch diameter CMP and replace it with new 8-inch diameter HDPE pipe within the limits shown on the design drawings. The contractor shall remove and dispose of the excavated pipe from Government property at the contractor’s expense. Excavation in the pervious fill (for new drainage fill) shall be sloped at 1:1 as shown. Contractor shall employ safe means and methods for excavating through the random fill into the pervious fill section. Refer to Excavation Stability & Protection section for more details. Excavation into the dike embankment is not permissible. Contractor shall ensure that the existing pervious fill materials that remains on the bottom and sides of the excavation do not become coated/contaminated with other soils as the design intent is to ensure free flow of groundwater through the pervious fill into the new drainage fill. Any soils which fall onto the remaining pervious fill shall be removed prior to the placement of geotextile and new drainage fill.
3. Excavation Stability & Protection: The contractor shall assume that shoring of the excavation may be required for worker safety. The excavation shall be shored in accordance with EM 385- 1-1 requirements. Sloping will not be allowed to extend into the dike embankment. The Contractor shall anticipate that the granular nature of the soils and the presence of groundwater will result in relatively unstable excavation sidewalls. Sidewall material which sloughs into the excavation shall be removed prior to the placement of backfill materials (i.e.
backfill shall not be placed on top of sloughed material).
4. Dewatering: Groundwater is relatively shallow from the hillsides on either side of the area and it is typical that there is flow out of the current toe drain collector pipe. The contractor shall therefore anticipate to provide a sump/pump beyond both the east and west sides of the toe drain area to capture flow and allow work to proceed on a stable dry surface. It shall be assumed that the sump pits will need to be deeper than the planned excavation to effectively lower the water. It should also be assumed that the dewatering operations will need to begin in advance as this will help drain water from the pervious fill zone and improve the stability of the excavation sidewalls. Work shall be performed so that the excavations will be continually and effectively drained and water shall not be permitted to accumulate in any excavation.
Control groundwater flowing toward or into excavations to prevent sloughing of excavation slopes and walls, boils, uplift and heave in the excavation and to eliminate interference with orderly progress of construction.
5. Satisfactory Backfill Materials:
a. New Drainage Fill Material: The new drainage fill material must be imported and must be crushed angular gravel meeting the requirements for AASHTO M-43 Number 8 coarse aggregate.
b. Random Fill: Random fill shall consist of soil material that is free of organics, trash, rubble, and cobbles/boulders larger than 4 inches in diameter. Random Fill shall consist of materials classified in accordance with ASTM D2487 as GM, GC, SM, or SC. Materials from onsite excavation that meet these requirements may be re-used as backfill but the contractor shall note that much of the excavated material will be wet and require drying prior to use as backfill. Portions of the excavation not backfilled using on site excavated materials shall be imported.
6. Riprap: Riprap shown on the design drawings shall be PENNDOT Standard Specifications
PENNDOT Rock Lining Gradations R-5 with bedding material not exceeding 4 inches.
7. Geotextile: Geotextile shall meet the requirements of AASHTO M 288. The filtration geotextile shall be a needle punched nonwoven fabric. Geotextile strength properties must meet the requirements of Class 1 listed in Table 1 of AASHTO M 288. For subsurface drainage requirements, geotextile must meet the requirements listed in Table 2 for percent in situ soil passing 0.075 mm sieve from 15 to 50 of AASHTO M 288. Geotextile must completely wrap around the new drainage fill material to ensure no direct contact can be made between the new drainage material and surrounding materials. Anchor pins shall be used along the overlaps, refer to the design drawings for geotextile overlapping details. Trenches to be lined with filtration geotextile shall be graded to obtain smooth side and bottom surfaces so that the geotextile will not bridge cavities in the soil or be damaged by projecting rock. The geotextile shall be laid flat but not stretched on the soil, and it shall be secured with anchor pins. All geotextile shall be labeled, shipped, stored, and handled in accordance with ASTM D4873. No hooks, tongs, or other sharp instruments shall be used for handling geotextile.
8. High Density Polyethylene Pipe (HDPE): Pipe for the 8-inch diameter HDPE shall conform to the requirements of AASHTO M 252 Type CP. Contractor shall supply and install a hinged, lockable grating over the outlet end of the pipe that is affixed to the headwall to prevent rodent entry. The grating shall be made of stainless steel materials and have opening sizes of approximately 1-inch x 1-inch. The Contractor shall submit certification from the material supplier that shows that the pipe supplied to the site meets the requirements given. This certification shall be submitted by the Contractor at least 15 days prior to the start of work.
Flexible watertight joints shall be made with plastic or rubber-type gaskets for HDPE pipe following the pipe manufacturer’s instructions and specifications. Contractor shall provide submittal on such materials for approval. The design of the connectors between the inlet box and pipe and the headwall and the pipe shall conform to ASTM F 2510 and pipe manufacturer’s instructions. The bedding surface for the pipe shall provide a firm foundation of uniform density throughout the entire length of the pipe. Each pipe shall be thoroughly examined before being laid; defective or damaged pipe shall not be used. Pipe shall be laid to the grades and alignment indicated. Pipe shall not be laid in water, and pipe shall not be laid when trench conditions or weather are unsuitable for such work. Perforated pipe shall have the slots in accordance with Class 2 Perforations requirements of AASHTO M 252. Slot width and length shall be 3-mm (0.12-in) by 30-mm (1.2-in) respectively, as per AASHTO M
252. Materials delivered to site shall be inspected for damage, unloaded, and stored with a minimum of handling. Materials shall not be stored directly on the ground. The inside of pipes and fittings shall be kept free of dirt and debris. The Contractor shall have a copy of the manufacturer's instructions available at the construction site at all times and shall follow these instructions unless directed otherwise by the Contracting Officer.
9. Inlet Box: The inside dimensions of the inlet box shall be 36-inches by 36-inches with 6-inch wall thickness. The height of the inlet box shall be such that top of the inlet box without the grate/cover shall sit flush to the finished ground surface. An approximate 6-foot inlet box height is expected, but the exact dimensions shall be determined after the pre-construction survey has been performed. The concrete for the inlet box shall meet 5000 PSI strength at 28 days and reinforcing shall conform to ASTM A-185. The grate/cover for inlet box shall be a solid cover made of steel or fiberglass with a lock, designed to support H-20 wheel loading.
Ladder rungs shall be provided in the box.
10. Headwall and Wingwall: General dimensions for headwall and wingwall are shown on the design drawings. Adjustments to these dimensions shall be determined after the pre-construction survey has been performed. Concrete for the headwall and wingwall shall meet 5000 PSI strength at 28 days. Reinforcing shall include #4 steel bars, grade 60, at 12 inches Over Center Each Way (O.C.E.W.). Steel placement shall be in accordance with ACI 318.
11. Compaction of Backfill Materials:
a. Compaction of Subgrade: Prepare the bottom of the excavation by having two complete passes with a plate compactor before placing any fill.
b. Compaction of New Drainage Fill: Material shall be placed in 8-inch lifts except that a minimum lift thickness of 12 inches is required above the top of the pipe. The materials shall be compacted with two passes of a vibratory plate tamper. Care must be taken to not damage the drainage pipe from compaction efforts.
c. Compaction of Random Fill: The random fill shall be placed in 8-inch loose lifts and any particle size greater than 4-inches shall be removed. Unless otherwise directed, lifts shall be compacted by using a walk-behind roller weighing between 1.5 and 2 tons to achieve 95% of maximum dry density with moisture content range within ±3% of the optimum moisture content as determined by Standard Proctor Test (ASTM D-698). Care must be taken to not damage the drainage pipe from compaction efforts. Random fill shall not be placed when wet and shall have a moisture content that allows the material to be compacted without creating a wave (pumping) or squeezing material sideways from the roller.
d. Density Tests: Perform density tests on each compacted lift of random fill material. At a minimum perform two density tests for each compacted lift. The locations of the test shall be randomly distributed and at the discretion of the COR. Density tests shall be performed using sand cone method (ASTM D1556) or nuclear method (ASTM D6938). If ASTM D6938 is used, ensure that the equipment is properly calibrated and provided equipment calibration certificates.
12. Finished Grade: The area currently is poorly graded resulting in accumulation of surface water in the area. After backfilling the trench, the contractor shall regrade the entire area between the toe of the levee and the woodline to promote positive drainage towards the woodline (note that this will require stripping of vegetation and topsoil from this full area not just over the trench). Grades should generally be within 1-foot of existing but the contractor’s plan for finished grading shall be discussed with and agreed to by the government in advance. These adjustments to grades must be considered when determining top elevations of structures and cleanouts.
13. Topsoil and Seeding: The contractor shall place a minimum 4” thick layer of topsoil at the ground surface. Seed shall be a drought and heat tolerant variety suitable for the region and soil conditions. Provide seed of the latest season's crop delivered in original sealed packages, bearing producer's guaranteed analysis for percentages of mixtures, purity, germination, seed content, and inert material. Label in conformance with AMS seed act and applicable state seed laws. Wet, moldy, or otherwise damaged seed will be rejected. Field mixes will be acceptable when field mix is performed on site in the presence of the government. Seed shall not be planted when the ground is frozen, snow covered, muddy or when air temperature exceeds 90 degrees Fahrenheit. The seed application method shall be hydroseeding. Water and fiber shall be mixed first. Wood cellulose fiber, paper fiber, or recycled paper must be applied as part of the hydroseeding operation. Fiber must be added at 1,000 pounds, dry weight, per acre. Then add and mix seed and fertilizer to produce a homogeneous slurry. Seed must be mixed to ensure broadcasting at a rate that is appropriate for the seed type used to ensure a suitable vegetative cover. When hydraulically sprayed on the ground, material must form a blotter like cover impregnated uniformly with grass seed.
Spread with one application with no second application of mulch. Hydroseeding fertilizer shall include controlled release fertilizer including pills coated with plastic resin to provide a continuous release of nutrients for at least 6 months. Start watering areas seeded as required by temperature, wind and soil conditions. Apply water at a rate sufficient to insure thorough wetting of soil to a depth of 2 inches without run off. During the germination process, seed is to be kept actively growing and not allowed to dry out. Watering shall be performed for a minimum of 4 weeks from seeding. Final turf acceptance by the government will be based on adequate vegetated groundcover.
14. Video Inspection
After the completion of all subsurface activities the Contractor shall perform a video inspection of the completed pipes and inlet box. The results and video of this inspection shall be submitted to the Government for approval. Any damaged areas revealed by the video inspection shall be fixed at no additional cost to the Government. The contactor shall perform this inspection in accordance with the latest NASSCO guidance. The CCTV inspection shall be performed by CCTV personnel trained and certified in the use of NASSCO's Pipeline Assessment and Certification Program (PACP). All CCTV inspections shall be performed in accordance with PACP standards including the specific date and time of inspection. The contractor shall also document their findings in accordance with NASSCO guidance and provide the Contracting Officer with the documentation. The robotic inspection system shall record the distance along the pipe on the video and provide a plan view diagram of the pipe system labeled to correlate with the distances on the video. The self-propelled robotic camera used for the inspection shall be appropriately sized for such inspection. Lighting for the camera shall be suitable to allow a clear picture of the entire periphery of the pipe. The camera shall be operative in 100% humidity conditions. The camera, television monitor, and other components of the video system shall be capable of producing picture quality sufficient to clearly document the pipe condition and any defects. The Contracting Officer should be contacted to observe the start of the video inspection to ensure the video quality is satisfactory to the Government.
15. Final Record Drawings (As-Builts): Submit the As-Builts drawings after completion of work to correctly show the features of the project as it has been constructed by bringing the contract set into agreement with approved working as-built prints, and adding such additional drawings as may be necessary. These working as-built marked prints must be neat, legible and accurate. Provide both PDF and Autocad 2018 version files for As-Builts.
16. Emergency Requirements: Throughout the work, the contractor shall maintain equipment and sufficient excavated material on site to perform emergency backfilling of all open excavation within 24 hours of notification by the COR. This requirement is intended to ensure that the integrity of the dike system is maintained in an extreme flood event where Aylesworth Lake extends to the dike area. A written plan for emergency backfilling shall be submitted prior to excavation and shall be discussed between the Contractor, COR, Dam Operator, Geotechnical Branch and Water Management Section.

2. Additional Submittals: Digital copies of the following shall be submitted. The pipe inspection reports and video shall be provided on an external hard drive.
A. Daily Quality Control Reports B. As-built modifications to the shop drawings C. Report and Video files from the pipe inspection.
SPECIAL CONDITIONS
1. Period of Performance: The contractor shall complete all work included herein within 180 days of receiving notice to proceed from the Contracting Officer. The start date shall be coordinated with the Contracting Officer’s Representative (COR) and the Head Dam Operator. Prior to the start of work it should be confirmed that the Head Dam Operator has coordinated the schedules with the Baltimore District Water Management Section and Geotechnical Branch. (Note: Unless otherwise indicated the COR and Operations Project Manager shall be considered as the same).
2. Performance of Work: The Contractor shall perform work only during normal Corps of
Engineers duty hours (7:00 a.m. to 3:30 p.m., Monday through Friday, excluding Federal holidays). Exceptions to this condition shall be coordinated with and approved in advance by the Operations Manager, Susquehanna River Project.
3. Accident Prevention: In performing this contract the Contractor shall comply with all current Federal, State, and Local safety regulations, including Corps of Engineers Safety and Health Requirement Manual EM 385-1-1 and shall comply with any subsequent changes.
Information regarding acquiring the Corps of Engineers Safety and Health Requirement Manual EM 385-1-1 may be found on the USACE Headquarters website at https://www.usace.army.mil/Missions/Safety-and-Occupational-Health/Safety-and-Health- Requirements-Manual/. Prior to commencement of work under this contract the Contractor shall furnish to the Government an Accident Prevention Plan in accordance with Section I, Article 01.A.12 of the EM 385-1-1, dated 30 November 2014.
4. Accident Reporting: In the event of an accident or injury involving contractor personnel or equipment in performance of work, the Contractor shall immediately notify the Government representative by the most expedient means feasible. The Contractor shall complete forms furnished and/or provide a written description of the incident within 24 hours of notification to do so.
5. Pre-screen Candidates using E-Verify Program: The Contractor must pre-screen Candidates using the E-Verify Program (http://www.uscis.gov/e-verify) website to meet the established employment eligibility requirements. The Vendor must ensure that the Candidate has two valid forms of Government issued identification prior to enrollment to ensure the correct information is entered into the E-Verify system.
6. Operations and Storage Areas: The Contractor shall confine all operations (including storage of materials) to areas authorized or approved by the Head Dam Operator.
7. Housekeeping: The Contractor shall keep the work area, including storage areas, free from accumulations of waste materials. Before completing the work, the Contractor shall remove from the work site and premises, any rubbish, tools, equipment, and materials that are not property of the Government. Upon completing the work the Contractor shall restore the work area to the original condition, to be approved by the Contracting Officer.
8. Environmental Protection: The Contractor shall comply with all Federal, State, and Local environmental regulations.
9. Permits and Licenses: The Contractor shall, at his/her own expense, obtain any license or permits required to perform the contract. The Contractor shall comply with all Federal, State, and Local laws and regulations and shall comply with any subsequent changes. Contractor shall be responsible for developing the Erosion and Sediment (E&S) plan and acquiring E&S permit from local governing authorities. The contractor shall be responsible for all design efforts and application fees associated with obtaining the permits.
10. Records and Reports: In addition to the administrative requirements dictated elsewhere in this contract, the Contractor shall complete a Daily Construction Quality Control Report form to maintain record of contract performance, accident/injury incidence, and observance of project deficiencies. Forms will be distributed at pre-work conference.
11. Superintendent: The Contractor shall act as or provide a Superintendent and quality control manager whenever the work specified herein is being performed. The Superintendent shall conduct overall management coordination and be the central point of contact with the Government for performance of all work under this contract. The Superintendent and any individual designated to act for him/her shall have full authority to contractually commit the Contractor for prompt action on matters pertaining to work performed under this contract.
Such authorization shall be submitted in writing to the Government representative. The Superintendent shall deal directly with the Government representative for normal day-to-day administration of the contract provisions. The Superintendent will be required to attend pre-work conference(s) prior to commencing work under this contract.
INVOICES AND PAYMENT
1. Invoices: The Contractor shall submit itemized invoices no sooner than 5 days from the completion of the contract activity and no later than 15 days from the completion of the total contract activity. The invoice shall accurately reflect work completed. Each invoice at a minimum shall contain the following information:
Contractor name and address, exactly as it appears on the contract Contract number Date of invoice submission Invoice or Call Order number Invoice total
2. Corrections: Incorrect invoices will be returned to the Contractor for correction before processing payment. Corrected invoices shall be re-dated to reflect the new submission date.
3. Remittance: Invoices shall be submitted via post-mail and/or electronic mail to the following addresses for payment. Should changes occur to the mailing addresses the Contractor shall be notified of the correct recipient.
