Bid Schedule – Government template provided.
EVALUATION CRITERIA
The Government will award a firm-fixed-price (FFP) contract to the responsible Offeror whose offer conforms to the requirements as stated in this request for quotation (RFQ). The award will be made using the Lowest Price Technically Acceptable (LPTA) method based on the following three (3) evaluation factors:
mailto:Dean.R.Whitmore@usace.army.mil
1- Technical
2- Past Performance
3- Price.
A. To receive consideration for award, a rating of no less than “Acceptable” must be achieved for the Technical factor.
B. The Government intends to evaluate quotes and make award without questions to contractors. When combined Technical and Past Performance factors are approximately equal to cost or price. Therefore, the initial quote should contain the contractor's best terms from a Technical and Cost standpoint.
C. Unclear quotes may not result in questions for clarifications. Offers with an omission or unclear quote may be judged to mean the contractor does not fully understand the requirements or understand what it takes to meet the requirements, regardless of the proposed cost/price and will be rated unacceptable. In that case, the Offeror may be found unacceptable, and ineligible for award. The Government reserves the right to ask questions of offerors if the Contracting Officer determines them to be necessary.
1. Technical –
To be technically acceptable, the Offer must provide information as specified in the attached
Scope of Work. Each submittal will be evaluated to ensure requirements are met. The responsiveness and quality of information delivered by the Technical Service Department contacts provided shall also be considered in evaluating technical acceptability.
Technical Rating
ADJECTIVAL RATINGS
ACCEPTABLE Offer clearly meets the minimum requirements of the solicitation.
UNACCEPTABLE Offer does not clearly meet the minimum requirements of the solicitation
2. Past Performance (see attached questionnaire below) –
Past performance refers to the quality of recent project experience from the customer’s perspective.
The Offeror shall arrange for the reference customer point of contact for two (2) projects to fill out the Questionnaire and return to Gentian Mulla, Purchasing Agent, e-mail at
Gentian.Mulla@usace.army.mil. Past Performance for similar projects within the last (5) years. Past
Performance will be evaluated utilizing the information provided by the Contractor, the Past
Performance Questionnaires and two (2) references and by checking other sources such as the
Contractor Performance Assessment Reporting System (CPARS).
Any Offeror who has demonstrated satisfactory past performance will be considered eligible for award. Any Offeror who demonstrated unsatisfactory past performance will not be considered eligible for award. If the Government does not obtain past performance information for the projects identified by the Offeror and cannot establish a past performance record for the Offeror through other sources, or if the Offeror has no past performance record and cannot tie significant, relevant experience of key personnel proposed for this project to cited experience, past performance will be evaluated neither favorably nor unfavorably.
Past Performance Rating Definition
PAST PERFORMANCE RATING DEFINITION

3. Price –
Price will be evaluated in accordance with FAR 15.402. The government may require additional pricing information to assist in determining the extent to which the price is fair and reasonableness and the pricing information shall be provide if requested. The Government will evaluate quotes for award purposes by adding the total of all CLIN prices to ensure that pricing is not unbalanced. Offeror must complete all line items to be considered for award. All line items must be priced. Quotes providing partial pricing shall be considered non-responsive

CONTRACT INFORMATION (Contractor to complete Blocks 1-4)
RATING DEFINITION NOTE
(E) Exceptional Performance meets contractual requirements and exceeds many to the Government/Owner’s benefit.
The contractual performance of the element or sub-element being assessed was accomplished with few minor problems for which corrective actions taken by the contractor was highly effective.
An Exceptional rating is appropriate when the
Contractor successfully performed multiple significant events that were of benefit to the
Government/Owner. A singular benefit, however, could be of such magnitude that it alone constitutes an Exceptional rating. Also, there should have been NO significant weaknesses identified.
(VG) Very Good Performance meets contractual requirements and exceeds some to the Government’s/Owner’s benefit.
The contractual performance of the element or sub-element being assessed was accomplished with some minor problems for which corrective actions taken by the contractor were effective.
A Very Good rating is appropriate when the
Contractor successfully performed a significant event that was a benefit to the
Government/Owner. There should have been no significant weaknesses identified.
(S) Satisfactory Performance meets minimum contractual requirements. The contractual performance of the element or sub-element contains some minor problems for which corrective actions taken by the contractor appear or were satisfactory.
A Satisfactory rating is appropriate when there were only minor problems, or major problems that the contractor recovered from without impact to the contract.
There should have been NO significant weaknesses identified. Per DOD policy, a fundamental principle of assigning ratings is that contractors will not be assessed a rating lower than Satisfactory solely for not performing beyond the requirements of the contract.
(M) Marginal Performance does not meet some contractual requirements. The contractual performance of the element or sub-element being assessed reflects a serious problem for which the contractor has not yet identified corrective actions. The contractor's proposed actions appear only marginally effective or were not fully implemented.
A Marginal is appropriate when a significant event occurred that the contractor had trouble overcoming which impacted the
Government/Owner.
(U)
Unsatisfactory
Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance of the element or sub-element contains serious problem(s) for which the contractor's corrective actions appear or were ineffective.
An Unsatisfactory rating is appropriate when multiple significant events occurred that the contractor had trouble overcoming and which impacted the Government/Owner. A singular problem, however, could be of such serious magnitude that it alone constitutes an unsatisfactory rating.

SCOPE OF WORK (SOW)
Scope of Work
Raystown Lake Project Office Hesston, PA.
The Raystown Lake Project, Baltimore District, U. S. Army Corps of Engineers will contract the repair of multiple stone walls throughout the Raystown Lake Project. Work shall be completed as identified in the specifications. The Raystown Lake Project Office is located in Hesston PA.
Raystown Lake is located in Huntingdon and Bedford counties in South Central Pennsylvania, approximately 95 miles west of Harrisburg and 175 miles east of Pittsburgh. It is approximately
70 miles from the Pennsylvania - Maryland border. The dam site is on the Raystown Branch of the Juniata River with the main access to the project via Rt. 26 from Rt. 22 to the North and the
Pennsylvania Turnpike to the South. Huntingdon, Pennsylvania, approximately thirteen (13) miles to the North, has a population of 8,000 persons and contains numerous groceries, shopping facilities, doctors, and a hospital.
All activities described in the Scope of Work will occur within the Raystown Lake Project Areas.
1.Scope of Work
1.1. The Contractor shall furnish all personnel, vehicles, equipment, materials, and supplies necessary to execute work as described within the scope of work.
1.2. GENERAL
The contractor shall provide all necessary supervision, labor, personnel, materials, supplies, products, parts, tools, equipment, vehicles, and transportation required to complete the following actions related to the items listed below: The work to be performed is the repair, repointing and capping of stone walls and pillars in various locations around the project area. The walls are of various uses. Some are utilized as retaining walls, others are for aesthetics and signage. The stone varies in some walls. The largest portion of the walls needing repair are cut sandstone. Several other areas are random field stone. In some areas, the wall repair will be repointing only. In areas were the wall stone has become misplaced, bulged or has pushed out of proper alignment, the wall shall be dismantled beyond the point of misalignment or bulging and reinstalled in proper alignment. All work shall be priced by the square foot.
1.3. SUMMARY
A. Section Includes
1. Raking out of all unsound mortar from exterior stone faces
2. Careful salvaging and cleaning of bulged, displaced or misaligned wall stone
3. Removal of mortar excess from stone faces
4. Resetting salvaged and cleaned stone in proper wall position
5. Repointing all exterior stone joints.
6. Capping of the 3 stone walls at Ridenour Overlook and the Beach
1.4 Execution of the Work
The contractor shall be experienced in repointing and repair of stonework.
(NOTE: A good repointing job is meant to last at least 30 years. Shortcuts and poor craftsmanship diminish the natural beauty of the stonework, and will require future repointing sooner than if the job had been done correctly.)
1.4.1. Joint Preparation
A. Old mortar should be removed to a minimum depth of 2 to 2-1/2 times the width of the joint to insure adequate bond. Any loose or disintegrated mortar beyond this depth also should be removed where possible.
B. Care shall be used in removing the old mortar to prevent any damage to the stone units. The test panel work will help establish acceptability of tools to remove mortar with no stone damage.
C. Mortar should be removed cleanly from the stone, leaving square corners at the back of the cut. Before filling, the joints should be rinsed with a jet of water to remove all loose particles and dust. At the time of filling, the joints should be damp, with no standing water present.
1.4.2. Bulged and Misaligned Walls
A. Any location where the wall has bulged, sagged or become misaligned shall be removed beyond the damaged area and re-laid in proper wall alignment. All reset stone shall be placed in a full bed of mortar, and all voids filled as the wall advances.
1.4.3. Mortar Type and Preparation
A. Color of the mortar shall be approved by the Park Superintendent and should be approved with the test panel work prior to continuing with wall work.
B. Mortar shall be Type N mixed with river or rounded sand.
C. Mortar components should be measured and mixed carefully to assure the uniformity of visual and physical characteristics. Dry ingredients are measured by volume and thoroughly mixed before the addition of any water. Sand must be added in a damp, loose condition to avoid over sanding. Re-pointing mortar is pre-hydrated by adding water so it will just hold together, thus allowing it to stand for a period of time before the final water is added.
Half the water should be added followed by mixing for 5 minutes. The remaining water should then be added in small batches until a mortar of the desired consistency is reached.
(NOTE: It is important to keep the water to a minimum for two reasons: first, a drier mortar is cleaner to work with, and can be compacted tightly into the joints;
second, with no excess water to evaporate, the mortar cures without shrinkage cracks.)
D. Mortar should be used within 30 minutes of final mixing, and “re-tempering”, or adding more water will not be permitted.
1.4.4. Curing Conditions
A. Wall work should be performed when the temperature is at or above 40 degrees Fahrenheit, but not exceeding 90 degrees Fahrenheit.
B. Work shall not be performed in the rain unless approved cover and protection is provided.
C. Walls shall be misted and covered with wet burlap for a minimum of 3 days after completion of pointing work.
1.4.5. Filling the Joint
A. Where existing mortar has been removed to a depth greater than 1 inch, the new mortar shall be compacted in at least 3 layers. The back of the joint should be filled to approximately 1/3 of the joint depth, packing it well into the back corners and recesses. This application may extend along the wall for several feet. As soon as the mortar has reached thumb-print hardness, another third of the joint depth may be applied. When the second layer reaches thumb-print hardness, the third layer may be applied. In extremely deep areas, additional layers should be considered. It is important to allow each layer time to harden before the next layer is applied; most of the mortar shrinkage occurs during the hardening process and layering minimizes overall shrinkage.
1.4.6. Tooling
A. When the final layer of mortar is thumb-print hard, the joint should be tooled.
(NOTE: Proper timing of tooling is important for uniform color and appearance.
If tooled when to soft, hairline cracks may occur; if tooled when too hard, there may be dark streaks called “tool burning,” and good closure of the mortar against the stone will not occur.)
B. The final joint shall, in most cases, be recessed from the face of the stone.
After tooling, excess mortar can be removed from the edge of the joint by brushing with a natural bristle or nylon brush. Metal bristle brushes shall not be used.
1.4.7 Cap Blocks
Cap blocks shall have sculptured edges, must over hang wall width between 1 and
2 inches, and must be a minimum of 24 inches long and a minimum of 2 inches thick but no more than 2.5 inches thick. Color will be a shade of tan and color and style sample will be approved by the Government
