QASP
PURPOSE: The government will periodically evaluate the contractor’s performance by appointing a representative(s) to monitor performance to ensure services are received.
ROLES AND RESPONSIBILITIES: The government representative will evaluate the contractor’s performance through intermittent on-site inspections of the contractor's quality control program, random sampling, and periodic inspections. The government will also investigate complaints from visiting public as they are received.
PROCEDURES: Inspections will be accomplished by Government personnel using a sampling method of inspection based on reported completed cleanings. The frequency of inspection will vary from not less than 10 % of the work accomplished to 100%.
The Government may inspect each task as completed or increase the number of quality control inspections if deemed appropriate because of repeated failures discovered during quality control inspections or because of repeated customer complaints. Likewise, the government may decrease the number of quality control inspections if performance dictates.
The Corps inspector will inspect task performance by watching actual performance, physically checking an attribute of the completed task, or otherwise inspecting the task or its results to determine whether or not the performance meets the standards contained in the contract.
All surveillance observations/inspections made by the inspector will be recorded on the Janitorial Inspection Checklist (See Section A). The completed form becomes the official USACE record of the contractor’s performance.
When a surveillance observation results in unacceptable service, the inspector will record the information on the checklist and request the contractor to initial. If the service can be re-performed, request the contractor to re-accomplish the task or service. If the service cannot be re-performed, the deficiency will be a defect. The specific reason for unacceptable performance will be recorded on the checklist.
During the month, the inspector may receive customer complaints about the quality of service, or may observe unacceptable performance by the contractor. These complaints and observations will be noted and should reinforce the accuracy of the sample results.
The inspector is required to accept contractor services and determine payment due. At the completion of each contract payment period (usually monthly), the inspector must certify the services actually received under the contract.
UNACCEPTABLE PERFORMANCE: When performance is determined unacceptable, the inspector must inform the contractor’s on-site representative that performance is unacceptable and why, and request his or her initials and date on the surveillance form. By initialing and dating the form, the contractor’s on-site representative is acknowledging that he or she has been told of the unacceptable performance. He or she is not necessarily agreeing that the performance is unacceptable. If the contractor wants to dispute the results of the surveillance, the inspector must refer them to the contracting officer for resolution of the matter.
All work shall be performed in an orderly manner in accordance with the specifications and per the terms of the contract. If services are found to be defective in workmanship or not in conformance with the specifications and/or the terms of the contract, the Government may reject such service(s).
If time permits, the rejected work shall then be re-performed in accordance with contract specifications without additional cost to the Government.
Rejected work which cannot be corrected in a timely fashion should not be invoiced to the Government. If it is, the Government may reject the invoice and return to the Contractor for correction.
Partial payments may be made on the basis of work accomplished.
The contractor may be responsible for the cost of the Government to perform the service with its personnel and equipment or that of another contractor.
Damage to Government facilities should be minimized. Any damage to Government facilities caused by the contractor shall be corrected in a timely fashion by the contractor. The contractor shall reimburse the Government if the Government corrects the damage. The Government may withhold this amount from any outstanding invoices.
The contractor shall maintain an on-site representative, designated in writing, with the authority to address daily issues concerning the contract and be responsible for on-site supervision of the contractor’s workforce at all times.
The on-site representative will have knowledge of the contract specifications, work schedules, cleaning and safety.
The on-site representative shall have sufficient English language skill to be able to communicate with the members of the Jennings Randolph Lake staff. This person will interact directly with the Contracting Officer’s Representative (COR) in matters relating to the day to day administration of the contract. A meeting will take place at each Monday in the Ranger’s office or otherwise as requested by the COR to resolve any problems with the past cycle and to review the work schedule for the current cycle. The contractor shall immediately notify the COR of any changes in the on-site representative.
The on-site representative shall complete weekly inspection check sheets and provide to the COR or their representatives no less than one time per week. The master copy will be provided by the Government. These check sheets may be used to verify contractor invoices. Contractor is responsible for making additional copies of the inspection check sheet.
The on-site representative shall attend a mandatory meeting once a week with the Supervisory Ranger or on an as needed basis with the COR. The meetings will be held at the Ranger Office. Dates and times will be set by the
COR.
SAFETY REQUIREMENTS: The contractor shall abide by all OSHA Regulations and pertinent sections of the Corps of Engineers Safety and Health Requirements Manual, EM 385-1-1, and submit all reports required therein to the manager.
Vehicles will not be driven or parked in unsafe positions or off designated roadways and parking areas. Drivers of all contractor vehicles will comply with all applicable local, state and federal laws and regulations.
The contractor shall observe all speed limits and other safety signs posted in the operation of vehicles and equipment. Trucks or other vehicles will be operated in a safe manner and will not be parked in unsafe positions.
Visitor safety is as much a concern as any other feature of the work. The contractor will exercise extraordinary and extreme care to assure their operations do not endanger the visiting public.
All wet floors shall have proper signs to alert Government employees and the public to the condition. Signs shall be professional in appearance and have the proper coloring and wording to denote such hazards.
SECURITY: Employees are subject to rules and regulation of Title 36 CFR part 327. Any loss of keys will be reported to the Operations Project Manager within 24 hours. Failure to do so may result in the contractor accepting the burden of reestablishing the integrity of the Jennings Randolph Lake lock and key system.
The contractor shall turn in to the Park Office or to a Park Ranger all items found in the Public Use Areas and Facilities.
HOURS OF DUTY: Unless otherwise directed, such as for extra cleanings, all work is to be performed between the hours of 8:00 am and 12:00 p.m.
RANDOM INSPECTION SCHEDULE
The dates will be selected using the Random Number Tables from Regulation # 1180-1-2. The numbers were selected by randomly picking a group of numbers from the table.
An extra date will be selected in the case of a change in schedule or weather that does not allow service on the selected date. If for any reason the service requires additional inspection new numbers will be selected to increase the frequency of inspection.
Random Inspection: 10% frequency. Number of inspections rounded off.
To be added when the contract is awarded in FY17.
