CAUTION PAGE

CAUTION!

Notice: Several changes have been made as a result of the COVID-19 virus and its impacts on the normal course of business regarding the request for proposal (RFP) and the proposal submission process. Those changes are reflected with “red letter” font

BEFORE SIGNING AND EMAILING YOUR BID, please take note of the following, as failure to perform any one of these actions may cause your proposal to be rejected.

1. AMENDMENTS: Have you acknowledged receipt of ALL Amendments? If in doubt as to number of amendments issued, please contact our office.

2. AMENDED BID PAGES: If any of the Amendments furnished amended bid pages, the amended bid page(s) must be used in submitting your bid.

3. EMAILED BIDS: INCLUDE A BID COVERSHEET WITH THE EMAILED BID. The Coversheet will show the Contractor name and address, the solicitation number (W912DQ20B1082), title of project, amendments received, and the date and time bids are due.

Mail in bids will not be accepted.

4. MISTAKES IN BIDS: Have you reviewed your bid prices for possible errors in calculations or work left out?

5. FACSIMILE BIDS AND MODIFICATIONS: Will not be considered. Facsimile withdrawals will not be considered.

6. SUPPLEMENTAL BID SCHEDULE: In addition to the Contract Line Item Schedule, contractors should complete the Supplemental Bid Schedules provided. The Supplemental Bid Schedules are not contractually binding. The Government will use the Supplemental Bid Schedules to assist in the evaluation of prices. The individual supplemental bid schedule items should add up to the prices for the Contract Line Item Schedule. In the event that the CLIN subtotal in a Supplemental Bid Schedule does not match the CLIN total in the Contract Line Item Schedule, the Contract Line Item Schedule will be the price evaluated by the Government.

7. BID GUARANTEE: Sufficient bid guarantee in proper form must be furnished with your proposal, if your proposals exceeds $150,000.

8. BUY AMERICAN ACT: All are cautioned that, prior Government conduct notwithstanding, the Contractor's selection of a domestic construction material which would require the subsequent selection of a foreign construction material for compatibility is not a justification for waiver of the Buy American Act. It is the Contractor's responsibility to verify, prior to submitting the materials for approval that each system can be built to meet the contract specifications without the use of foreign construction materials.

9. SYSTEM FOR AWARD MANAGEMENT (SAM): In accordance with FAR 52.204-7: By submission of a bid, the bidder acknowledges the requirement that a prospective awardee shall be registered in the SAM database prior to submitting a bid, prior to award, during performance and through final payment of any contract/task order resulting from this solicitation. If the Bidder does not become registered in the SAM database in the time prescribed by the Contracting Officer, the Contracting Officer will proceed to award to the next otherwise successful registered Bidder. Bidders may obtain information on registration and annual confirmation requirements via the SAM database accessed through https://www.sam.gov/SAM/pages/public/index.jsf It is highly encouraged that all prospective Bidders start the SAM’s registration process as soon as possible.

SAM.GOV REGISTRATION IS FREE OF CHARGE

BIDDER MUST HAVE AN ACTIVE REGISTRATION IN SAM AT THE TIME OF BID

SUBMISSION OR THE CONTRACTING OFFICER MAY PROCEED WITH AWARD
