The chiller manufacturer shall provide a representative who shall be present at the site throughout testing and startup of the chiller to ensure that the manufacturer’s recommendations and procedures for testing and startup are followed and shall provide the Government with a testing and startup report. The chiller manufacturer shall provide in-person on-site training in chiller operation and maintenance to Government personnel.
Within ten working days of issuance of a purchase order, the chiller manufacturer shall furnish the Government with chiller lifting and handling instructions and operation and maintenance manuals.
2. SEQUENCE OF WORK: The area concerned in this project is Government-owned and will be in use during the course of this project.
3. SCHEDULE:
3.1 The chiller shall be delivered and available for unloading at building 1021 by not later than noon on Friday, March 30, 2018, so long as the purchase order is issued by December 29, 2017. If the purchase order is not issued by December 29, 2017, then the mandatory delivery date will be made later by one calendar day for each calendar day from December 29, 2017 until the date when the purchase order is issued. If the mandatory delivery date thus falls on a weekend, the next weekday which is a Federal non-holiday shall be the mandatory delivery date. Written reports from the manufacturer which set forth the progress of the chiller through the manufacturing process shall be provided to the Contracting Officer on January 17, February 14, and March 14, 2018.
3.2 The Contractor shall establish a firm schedule for delivery and unloading of the chiller with the Ft. Leonard Wood Corps of Engineers Resident Office one week in advance of the scheduled delivery date.
4. CONTRACTOR ACCESS TO FT. LEONARD WOOD:
Army Directive 2014-05 – “Policy and Implementation Procedures for Common Access Card Credentialing and Installation Access for Uncleared Contractors” established Army-wide standards for the unescorted access for uncleared contractor employees to Department of Defense installations. These standards are now in effect on Ft.
Leonard Wood and apply to all contractor and subcontractor personnel, including deliverymen. Personnel must complete the application process, pass the background check and obtain their card in order to be allowed access to Ft. Leonard Wood.
The Army has made a distinction between American citizens and foreign nationals. Foreign nationals will not be allowed access to Ft. Leonard Wood unless they are accompanied by a full-time government escort. As it is not feasible for the government to provide a full-time escort for workers who are foreign nationals, contractors should ensure that all workers who will try to gain access to Ft. Leonard Wood are American citizens.
Contractor personnel who request unescorted access to Ft. Leonard Wood must have a valid need for access and must undergo a vetting process to determine their fitness for access. After successfully passing the vetting process, contractor personnel will be able to obtain a card which they will show to the gate guard each time they come on post. A website exists through which all contractor personnel must apply for unescorted access. The web address is https://vcc1.icmvcc.com/im3500.
In a case where an individual will need access to Ft. Leonard Wood only once and for a period of time that will not exceed 48 hours, the individual may elect to forego the online application process and opt instead to present himself at the Visitor Center at the front gate (Bldg. 100) upon arrival at Ft. Leonard Wood to undergo a background check and obtain a 48-hour one-time pass. If the individual does not pass the background check, he will be turned away.
Individuals who elect to forego the online application process and who opt instead to present themselves at the Visitor Center to undergo a background check to obtain a 48-hour one-time pass should be aware that effective 13 January 2016, individuals presenting a driver’s license from the states of Illinois, Minnesota, Missouri, New Mexico or Washington will also need to present one of the documents listed below to prove identity in order to be issued an installation pass. (Driver’s licenses from these five states are not in compliance with the REAL ID Act of 2005 and therefore Fort Leonard Wood requires an additional form of identification):
• United States Passport or United States Passport Card
• Certified Copy of Birth Certificate
• Social Security Card
• Permanent Resident Card/Alien Registration Receipt Card (Form I-551)
• Foreign passport with a temporary (I-551) stamp or temporary (I-551) printed notation on a machine readable immigrant visa
• Employment authorization document that contains a photograph (Form I-766)
• Foreign passport with Form I-94 or Form I-94A bearing same name as passport and containing an endorsement of the alien’s nonimmigrant status, as long as the endorsement has not expired and proposed employment is not in conflict with restrictions or limitations identified on the form.
• School identification card with photograph
• U.S. Military or draft record (Certified Copy of DD Form 214)
• U.S. Coast Guard Merchant Mariner Card
• Transportation Worker Identification Card (TWIC)
• Native American Tribal Document https://vcc1.icmvcc.com/im3500
Should the vetting process reveal derogatory information that may indicate a threat to good order, discipline, or health and safety on the installation, that person will be denied unescorted access to the post. Derogatory information includes but is not limited to the following:
• A criminal arrest that may indicate a potential threat to the good order and discipline or health and safety on the installation;
• Providing false information of the individual’s identity;
• A current arrest warrant, regardless of the offense or violation;
• Being barred from entry to a Federal installation or activity;
• Being convicted of crimes encompassing sexual assault, armed robbery, rape, child molestation, production or possession of child pornography, human trafficking, or drug possession with the intent to sell or distribute;
• Being convicted of espionage, sabotage, treason, terrorism or murder;
• Being a registered sex offender;
• Having a felony conviction within the past ten years regardless of the offense or violation;
• Having a felony conviction of a firearms or explosives violation; or
• Being identified in the Terrorist Screening Database (TSDB).
The procedure for being vetted and obtaining a card begins with each person going to the above website and filling out the online “visit request.” The visit request form contains a window where the applicant must enter the email address of his government sponsor. For this project, the sponsor is Travis Lynch, (travis.a.lynch@usace.army.mil). When the applicant has correctly completed the visit request, he will receive a system-generated email notifying him that his visit request has been received and advising him to notify his sponsor separately of the submission of his visit request in order to help ensure that the sponsor signs the request. The sponsor will receive a system-generated email notifying him that the visit request has been received and asking him to review it and either sign it if it meets with his approval or decline to sign it if it does not. After the sponsor signs the visit request, it is automatically forwarded to the Ft. Leonard Wood Directorate of Emergency Services (DES).
The applicant will receive a system-generated email notification of this as well. DES will then perform the background check of the applicant. The background check should take 24 hours. If the applicant passes the background check, DES will notify the sponsor that the visit request has been approved. It will be up to the sponsor to notify the applicant via email that his visit request has been approved. The applicant will then print a copy of the sponsor’s email and take it to the Visitor Center at the front gate of Ft. Leonard Wood (Bldg. 100) along with their identification documents that were used to apply for the pass (most likely this would have been his driver’s license).
NOTE: If the applicant used a driver’s license from Illinois, Minnesota, Missouri, New Mexico or Washington to fill out the online application, he will need to bring with him to the Visitor Center an additional form of identification from the list above, because his driver’s license is not compliant with the REAL ID Act of 2005). There, DES will conduct one more background check which should take only a couple of minutes. If everything is in order, the applicant will be photographed and will receive their card. This process should take about 15 minutes and can be done anytime from 8:00 AM to 4:00 PM Monday through Friday. For visits lasting less than six months, a paper card will be issued. For visits lasting six months to a year, a plastic card will be issued.
When the applicant fills out the visit request online, the following information will be among that which is required:
• The sponsor’s email address (see above).
• The applicant’s own email address.
• The applicant’s drivers license number and the state where it was issued.
• The “visit start date.” This is the date when the applicant wishes to begin accessing Ft. Leonard Wood. If the applicant is already working on post, then he should fill in the date that is one week after the date on which he is filling out the application. The reason for this is that the system will not allow the sponsor to approve the applicant’s visit request if the “visit start date” has passed.
• The “visit end date.” This is either the date when the applicant expects to no longer need access to Ft.
Leonard Wood or one year from the date of the “visit start date,” whichever is sooner.
• The sponsoring organization. This is the Corps of Engineers.
If the applicant will be driving a privately-owned vehicle onto Ft. Leonard Wood, the online visit request form also requires the following information (no information is required for a company-owned vehicle):
• VIN number
• License plate number.
• The state where the plates were issued.
• The vehicle year, make and model, the vehicle color and the type of vehicle.
In order for the Corps of Engineers’ sponsors to be certain that an applicant has a valid need for access to Ft.
Leonard Wood, it will be necessary for the contractor to notify the Corps sponsor of the names of his personnel who will be submitting a visit request. If the Corps sponsor receives notification of a visit request having been submitted but the applicant’s name does not appear on the prime contractor’s list, the visit request will be denied.
In the case where an applicant’s visit request is denied based upon derogatory information obtained through the background check, the applicant may request a waiver. Denial letters will include instructions about the waiver process. The waiver application would be submitted to the applicant’s sponsor at the Corps of Engineers. The applicant would have to:
• Obtain a certified copy of his criminal history that includes all arrests and convictions.
• Obtain a letter of support from the Government sponsor.
• Provide current physical address and email address to enable the Garrison Commander to notify the individual of the determination of the waiver request.
• The waiver request must include all offenses listed, explaining why the conduct should not result in denial, and the impact of the denial on the applicant and/or the mission at Ft. Leonard Wood. Factors to be addressed and considered are the nature and seriousness of the conduct, the circumstances surrounding the conduct, the length of time elapsed since the conduct, the applicant’s age at the time of the incident or conduct, and proof of efforts toward rehabilitation.
The sponsor will review the waiver request for completeness. If he endorses the waiver request, he will forward it to the Garrison Commander who will review the request and render a determination. If the waiver request is denied, the applicant may be able to request reconsideration one year from the date of the original decision.
