This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued.
(ii) The solicitation number is W912CN-19-Q-SHRP and this solicitation is issued as request for quotation (RFQ).
(iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2019-01, effective 22 January 2019 and Defense Far Supplement Publication Notice DPN 20190215, effective 15 February 2019.
(iv) This is a small business set-aside and the associated NAICS code is 611430.

ADDENDUM TO 52.212-1: INSTRUCTIONS TO OFFERS-COMMERCIAL ITEMS
QUOTE SUBMITTAL INSTRUCTIONS
1. The request for quotation (RFQ) and evaluation of quotes will be conducted in accordance with FAR Part 12, Acquisition of Commercial Items, and FAR Part 13 Test Program for Certain Commercial Items. Each interested contractor shall complete and submit the following forms, clauses, and statements with their quote. Failure to do so may cause a quote to be considered unacceptable.
b) Amendments or acknowledgements (if applicable), signed and dated
c) Submittal of quote in accordance with FAR 52.212-1(b)
2. Submission of quotes will be received through email to the contract specialist, Sebrina Lewis, and the Contracting Officer, Melanie Phaniphon, via email at Sebrina.A.Lewis.civ@mail.mil and PMelanie.D.phaniphon.civ@mail.mil .
a) Solicitation and amendment - Issued solicitation packages will be posted at the FBO (fbo.gov) website. Once posted, interested contractors are responsible to monitor the website until the closing date for any issued amendments.
b) It is the responsibility of the contractor to ensure quotations are received and acknowledged by the contracting specialist, Sebrina A. Lewis, email: Sebrina.A.Lewis.civ@mail.mil on or before the deadline as stated.
3. Interested contractors shall submit questions in writing regarding the solicitation to the contract specialist via email at Sebrina.A.Lewis.civ@mail.mil. Final questions must be submitted no later than May 2, 2019. A summary of questions received and Government responses will be provided via amendment and posted on the FBO website. Questions received after May 2, 2019 may not be answered.
4. Materials not directly related to the information requested shall not be included in the quote. Any information not requested will be disregarded in the evaluation process. Unnecessarily elaborate quotes are neither necessary nor desirable.
ADDENDUM TO 52.212-2: EVALUATION- COMMERCIAL ITEMS:
A. Basis of Award. The request for quotations uses streamlined simplified acquisition procedures pursuant to FAR Subpart 12.6 and FAR Subpart 13. The Government intends to award a single firm fixed price contract resulting from this solicitation to the responsible vendor whose quote conforms to the solicitation, meets the minimum requirements, and will be the most advantageous to the Government.
The following factors shall be used to evaluate vendors:
Factor 1 – NOVA Victim Advocacy Certifications Factor 2 – Price Basis for award will be to the vendor providing the lowest quote determined to meet the minimum requirements stated in the Statement of Work. Responsibility determination will be conducted on apparent successful vendor only. Past performance will not be formally evaluated, but will be reviewed during the responsibility determination pursuant to FAR Part 9.
As set forth in FAR 52.212-1, the Government intends to evaluate offers and award a contract without discussions with vendors. Therefore, the Vendor’s initial offer should contain the Vendor’s best terms from a price and technical standpoint. The Government reserves the right to conduct discussions if later determined by the Contracting Officer to be necessary.
B. Evaluation Criteria.
Vendors are required to submit the following factors upon submission of quote.
Factor 1 – Proof of NOVA Victim Advocacy Certifications.
Minimum Requirement: The offeror shall submit National Organization for Victim Assistance (NOVA) certificate for each employee performing this requirement.
Factor 2 - Price Price will not be scored or rated and should be provided on the quote. The Government expects competitive quotes to be submitted for this procurement and will use these competitive quotes to determine price reasonableness in accordance with FAR 13.106-3. The lowest price quote acceptable for award will be evaluated based on the combined total. If competitive quotes are not received, the Government reserves the right to utilize other methods identified in the FAR to determine price reasonableness. The Government intends to evaluate quotes and award without discussions; therefore quotes should be the contractor’s best pricing. Regarding calculation of pricing, please refer to FAR 52.212-2 (b).
Access and general protection policy and procedures.
a) Commercial businesses must possess and present a contractor IAP to gain entry at any access control point (ACP). Installation Access Passes are processed Visitor Control Center/Lyman Gate-Kunia Rd at Lyman Rd, Schofield Barracks, HI 96786.
b) In order to obtain a contractor IAP, the contracting office representative must submit a signed and approved IAP application along with a personnel information sheet to the Installation Access Office. Printed proof of social security numbers will need to be presented with all substantiating documents to the IAP Office. After the office has processed the paperwork (typically two days), the contractor may pick up his or her pass at the office. The contractor must bring some form of identification with proof of his or her social security number
c) Vendors and contractors with commercial vehicles who do business on Army installations on a regular basis can receive extended passes for individual and fleet vehicles, for periods stipulated by the COR/sponsor, by registering their vehicles at the IAP Office. Company “Multiple Operator” memorandums will be presented to IAP personnel upon requesting a vehicle IAP.
d) The Contractor shall provide to the Contracting Officer Representative the names and Social security Numbers of individuals requiring access in performance of the contract no later than 14 days prior to start of performance. The Army Organization or Directorate for whom the contractor is working is responsible for processing the paperwork to obtain passes.
e) Contractors and vendors requiring long term access (over 30 days) will require a government sponsor. Visitors seeking short term access (less than 30 days) do not require a government sponsor. In either case all applicants shall undergo a background check. By submitting the installation access request form, individuals are agreeing to a background check, which includes the National Crime Information Center Interstate Identification Index, or NCIC III, a Federal Bureau of Investigation database that provides criminal histories. The Contractor is responsible for assuring personnel required to access any installation in performance of the contract obtain the required identification. Costs for employees to travel to the Government location will be borne by the contractor. Failure to obtain required identification will not relieve the contractor from contract performance. Contractor shall be responsible for the return of identification cards upon the expiration of the contract or when an employee is no longer performing work under the contract.
f) The Contractor will provide a list of the names of all employees and subcontractor employees who will access any Army facility in the performance of this contract. Contractor will provide written document used to verify employees and subcontractor's employees compliance with FAR 52.222-54, Employment Eligibility Verification (clause that requires contractor to do background check.) The contractor will provide written proof from the Immigration and Naturalization Service that all employees listed are approved for employment within the US.
g) At any time contractor employees are operating contractor-owned vehicles on an Army Installation, they shall have in their possession a letter signed by a corporate officer authorizing the individual to drive the vehicle.
h) The Contracting Officer and the Provost Marshal office shall be notified of any changes in vehicles within three business days of the change.
i) Lost vehicle passes shall be reported immediately in order to obtain new passes. All vehicle passes issued shall be returned to the Installation Access Office upon completion of the contract, termination of an employee or discontinued use of the registered vehicles.
j) Failure to follow the procedures outlined above may result in delays in entering Army Installations. The Government is not responsible for any adverse impact on the contractor or its operation as a result of delays due to the failure to register vehicles.
k) Contractors operating vehicles on an Army post will not use cell phones unless used with a hands-free device. This applies to ALL personnel driving privately-owned and government vehicles on Army installations and housing areas. Failure to comply will result in the loss of on-post driving privileges:

If caught driving while privileges are suspended, your privileges may be further suspended for a 5-year period.
l) The Provost Marshall has the authority to change the above procedures without notice. Contractors shall adhere to any changes in security procedures About the background check In accordance with Homeland Security Presidential Directive 12 and Army Regulation 190-13, all visitors and un-cleared contractors desiring un-escorted access to USAG-HI Installations, must be vetted through NCIC III, the Army’s baseline background check for entrance onto Army installations for non-CAC or non-DOD ID card holders. Upon successful completion of a background check the individual(s) may be eligible to be issued an installation access badge. This badge is only good for access onto the installation and to be available for presentation to Installation Security personnel. It is not to be used as a form of identification for other purposes. The installation commander may, in the absence of an approved waiver, deny un-cleared contractors, subcontractors and visitors un-escorted access to the installation based on the results of the NCIC III check that contains credible derogatory information indicating the individual may present a threat to good order, discipline, or health and safety on the installation.
Factors that would constitute an access denial:
· The NCIC III contains criminal arrest information about the individual that causes the Installation Commander to determine that the individual presents a potential threat to the good order, discipline, or health and safety on the installation.
· The installation is unable to verify the individual’s claimed identity based on the reasonable belief that the individual has submitted fraudulent information concerning his or her identity in the attempt to gain access.
· The individual has a current arrest warrant in NCIC, regardless of the offense or violation.
· The individual is currently barred from entry or access to a federal installation or facility.
· The individual has been convicted of crimes encompassing sexual assault, armed robbery, rape, child molestation, production or possession of child pornography, trafficking in humans, drug possession with the intent to sell, or drug distribution.
· The individual has a US conviction for espionage, sabotage, treason, terrorism or murder.
· The individual is a registered sex offender.
· The individual has a felony conviction within the past 10 years, regardless of the offense or violation.
· The individual has been convicted of a felony firearms or explosives violation.
· The individual has engaged in acts or activities designed to overthrow the U.S. Government by force.
· The individual is identified in the Terrorist Screening Database (TSDB) as known to be or suspected of being a terrorist or belonging to an organization with known links to terrorism or support of terrorist activity.
Individuals who are not cleared and denied access, will be given a denial access waiver packet to be completed and returned to the VCC. The denial waiver committee will review the packet and determine suitability for access to the installation. As a reminder, all vehicle operators are required to have a valid state driver’s license and valid motor vehicle insurance.
AT Level I training. This standard language is for contractor employees with an area of performance within an Army controlled installation, facility or area. All contractor employees, to include subcontractor employees, requiring access Army installations, facilities and controlled access areas shall complete AT Level I awareness training within 30 calendar days after contract start date or effective date of incorporation of this requirement into the contract, whichever is applicable and annually thereafter. The contractor shall submit certificates of completion for each affected contractor employee and subcontractor employee, to the COR or to the contracting officer, if a COR is not assigned, within 05 calendar days after completion of training by all employees and subcontractor personnel. This standard language is for contractor employees with an area of performance within Army controlled installation, facility, or area. Contractor and all associated sub-contractors employees shall provide all information required for background checks to meet installation access requirements to be accomplished by installation Provost Marshal Office, Director of Emergency Services or Security Office. Contractor workforce must comply with all personal identity verification requirements (FAR clause 52.204-9, Personal Identity Verification of Contract or Personnel) as directed by DOD, HQDA and/or local policy. In addition to the changes otherwise authorized by the changes clause of this contract, should the Force Protection Condition (FPCON) at any individual facility or installation change, the Government may require changes in contractor security matters or processes.
For contractors that do not require CAC, but require access to a DoD facility or installation. Contractor and all associated sub-contractors employees shall comply with adjudication standards and procedures using the National Crime Information Center Interstate Identification Index (NCIC-III) and Terrorist Screening Database (TSDB) (Army Directive 2014-05/AR 190-13), applicable installation, facility and area commander installation/facility access and local security policies and procedures (provided by government representative), or, at OCONUS locations, in accordance with status of forces agreements and other theater regulations iWATCH Training. This standard language is for contractor employees with an area of performance within an Army controlled installation, facility or area. The contractor and all associated sub-contractors shall brief all employees on the local iWATCH program (training standards provided by the requiring activity ATO). This locally developed training will be used to inform employees of the types of behavior to watch for and instruct employees to report suspicious activity to the COR. This training shall be completed within 30 calendar days of contract award and within 5 calendar days of new employees commencing performance with the results reported to the COR NLT 30 calendar days after contract award.
