Unlisted classifications needed for work not included within the scope of the classifications listed may be added after award only as provided in the labor standards contract clauses (29CFR 5.5 (a) (1) (ii)).
The body of each wage determination lists the classification and wage rates that have been found to be prevailing for the cited type(s) of construction in the area covered by the wage determination. The classifications are listed in alphabetical order of "identifiers" that indicate whether the particular rate is a union rate (current union negotiated rate for local), a survey rate (weighted average rate) or a union average rate (weighted union average rate).
Union Rate Identifiers
A four letter classification abbreviation identifier enclosed in dotted lines beginning with characters other than "SU" or "UAVG" denotes that the union classification and rate were prevailing for that classification in the survey. Example:
PLUM0198-005 07/01/2014. PLUM is an abbreviation identifier of the union which prevailed in the survey for this classification, which in this example would be Plumbers. 0198 indicates the local union number or district council number where applicable, i.e., Plumbers Local 0198. The next number, 005 in the example, is an internal number used in processing the wage determination. 07/01/2014 is the effective date of the most current negotiated rate, which in this example is July 1, 2014.
Union prevailing wage rates are updated to reflect all rate changes in the collective bargaining agreement (CBA) governing this classification and rate.
Survey Rate Identifiers
Classifications listed under the "SU" identifier indicate that no one rate prevailed for this classification in the survey and the published rate is derived by computing a weighted average rate based on all the rates reported in the survey for that classification. As this weighted average rate includes all rates reported in the survey, it may include both union and non-union rates. Example: SULA2012-007 5/13/2014. SU indicates the rates are survey rates based on a weighted average calculation of rates and are not majority rates. LA indicates the State of Louisiana. 2012 is the year of survey on which these classifications and rates are based. The next number, 007 in the example, is an internal number used in producing the wage determination. 5/13/2014 indicates the survey completion date for the classifications and rates under that identifier.
Survey wage rates are not updated and remain in effect until a new survey is conducted.
Union Average Rate Identifiers
Classification(s) listed under the UAVG identifier indicate that no single majority rate prevailed for those classifications; however, 100% of the data reported for the classifications was union data. EXAMPLE: UAVG-OH-0010 08/29/2014. UAVG indicates that the rate is a weighted union average rate. OH indicates the state. The next number, 0010 in the example, is an internal number used in producing the wage determination. 08/29/2014 indicates the survey completion date for the classifications and rates under that identifier.
A UAVG rate will be updated once a year, usually in January of each year, to reflect a weighted average of the current negotiated/CBA rate of the union locals from which the rate is based.
WAGE DETERMINATION APPEALS PROCESS
1.) Has there been an initial decision in the matter? This can be:
* an existing published wage determination
* a survey underlying a wage determination
* a Wage and Hour Division letter setting forth a position on a wage determination matter
* a conformance (additional classification and rate) ruling
On survey related matters, initial contact, including requests for summaries of surveys, should be with the Wage and Hour Regional Office for the area in which the survey was conducted because those Regional Offices have responsibility for the Davis-Bacon survey program. If the response from this initial contact is not satisfactory, then the process described in 2.)
and 3.) should be followed.
With regard to any other matter not yet ripe for the formal process described here, initial contact should be with the Branch of Construction Wage Determinations. Write to:

DESIGN REQUIREMENTS FOR
AIR TRAFFIC CONTROL TOWER
1. DESCRIPTION OF WORK
1.1 PROJECT DESCRIPTION
The Air Traffic Control Tower (ATCT) Facility will serve as a stand alone facility. The Air Traffic Control Tower (ATCT) facility houses the equipment and personnel for control of aircraft approaching, departing, and transiting on aircraft movement areas (runways, taxiways, aprons, etc.). The project requires the construction of a new facility and demolition of existing buildings to locate the functions for the ATCT Facility. The existing ATCT and adjacent simulator building as well as associated equipment pads, stormshelter, and utility connections will be removed in their entirety. The new facility will be a modern, efficient, and appropriately-sized facility that will house system components directly supporting the mission.
The new facility for the ATCT will consist of but not limited to the following rooms: Control Cab w/ accessible roof area, ATCT Simulator Room, Ready/Break Room, Training Room, Administrative Room, Controller Office, Deputy Controller’s Office, ADA compliant Toilets, Locker/shower room, Janitorial Closet, Electrical Room, Telecommunications Room, Mechanical Rooms (Fire Pump Room for the Control cab and Tower Shaft, Entrance/Elevator Lobby, and vertical circulation consisting of stairs and elevator (with elevator equipment closet or room).
The Project also includes underground utilities, paving, parking, sidewalks access drive around the building, storm drainage, exterior and site lighting, site improvements, landscaping, underground communications infrastructure to connect the new facility, and all applicable AT/FP (Antiterrorism/Force Protection) measures.

2. TINKER GENERAL
2.1 CODES, STANDARDS AND CRITERIA
2.1.1 The Southwestern Architectural and Engineering Instruction Manual, (SWD-AEIM) prescribes standard procedures for the preparation of design documents for construction projects under the direction of the US Army Corps of Engineers (COE), Tulsa District. The design manual and other useful criteria are available on the Tulsa District home page at http://www.swt.usace.army.mil/
In addition to the specific Codes, Standards and Criteria referenced throughout this RFP, the designer shall prepare all design and construction documents in accordance with the most current edition of the following:

2.1.2 In this contract and associated documents, where the word “designed” is used the agent performing the design is the “Design-Build (DB) contractor.” The Designer shall also follow all current national and state codes, regulations and specifications involving civil, architectural, interior design, structural, mechanical, electrical, and environmental disciplines which are applicable to the engineering design and construction industry, including OSHA Safety and Health Standards and Air Force policy on Ozone Depleting Chemicals (ODC’s).
2.1.3 Use of National Fire Codes and Life Safety Codes, published by the National Fire Protection Association (NFPA), the International Building Code (IBC), the Americans with Disabilities Act (ADA), and the Uniform Federal Accessibility Standards (UFAS) is mandatory on all projects.

2.3 EXISTING CONDITIONS
TEXAS HORNED LIZARD (THL)
The Texas horned lizard is a small reptile, ranging in size from 3/4-to 4-inches long. It has horn-like projections on its head and spiny scales on its body, legs, and tail.
If a Texas horned lizard is encountered, dead or alive, during construction, the Contractor shall notify the Tinker Natural Resources office immediately and ensure the lizard is protected so it may be relocated.
The Contractor shall notify the Tinker AFB Natural Resources office as soon as it is known when construction activities will begin. At that time, Tinker Natural Resources staff will conduct visual ground searches of the construction site prior to any construction activities.
Any lizards found will be temporarily captured and relocated to suitable habitat away from the construction site. Once the site has been inspected and cleared, the Tinker Natural Resources staff will inform the Contractor that construction activities may start.
All contractor and subcontractor employees shall be briefed on the requirements outlined in the information included in the Appendices prior to beginning work on the site.
The Contractor shall post the horned lizard information sheet on the job site bulletin board or other conspicuous location where it can be viewed by all field crew members. The Contractor shall contact the Tinker Natural Resources staff to obtain the latest information sheet prior to starting construction.
ASBESTOS CONTAINING MATERIALS (ACM)
1. Asbestos is expected to be encountered within the existing ATCT on pipe insulation.
2. DB Contractor will be required to notify the Contracting
Officer immediately if Asbestos is encountered.

LEAD-BASED PAINT

2. Information regarding fire prevention, welding and cutting, or fire protection systems shall be obtained from and coordinated with the Base Fire Department (405-734-7964).

CONTAMINATED SOIL
Contaminated soils are not expected to be encountered around Building 935 (existing air traffic control tower to be demolished). Similarly, the area within this project location, at the new air traffic control
SECTION 01 10 10 Page 11 tower site, is not expected to encounter any contaminated soils. The DB Contractor shall notify Tinker Environmental (405)-724-3058 if vapors during any excavation are encountered. See Appendix C for a map of potential areas of effect.
If contaminated soil is found during construction, it will be handled and disposed of in accordance with all applicable Federal, State and local government laws and regulations. A list of sites to be used for disposal must be submitted to the Contracting Officer. In case of fuel/chemical spills that do damage and/or enter the sewers, the spills must be reported to the Contracting Officer and the Base Fire Department.
Excavation encountering contaminated soil, based on field screening, will not be enlarged to remove contamination beyond the proposed excavation dimensions. The suspect soil left in place at these locations must be sampled for chemical laboratory analysis. A representative soil sample will be taken from the bottom of the excavation where suspected contamination is left in place. For areas encountering apparent contamination for more than 20 linear feet of excavation, a representative sample will be taken from the excavation bottom for every 20-feet. Appropriate decontamination procedures will be performed between each individual sampling episode to prevent cross contamination of the individual samples. Appropriate sample containers and preservation techniques will be used to assure quality chemical data. Appropriate chain-of-custody documentation will accompany laboratory samples. A sampling and analysis plan will be prepared in accordance with EM 200-1-3 that details the proposed sampling procedure. Contaminated locations will be located in the field by a professional surveyor and clearly documented on the as-built drawings so they can be revisited in the future.
Contaminated soils found during excavation of soils must be segregated from clean soil, consolidated, and tested for disposal. All soil suspected of being contaminated will be placed on a minimum of 6-mil plastic (or equivalent), and covered by a minimum of 6-mil plastic.
Measures should be taken to prevent any surface runoff from entering the stockpile, or washing away the excavated materials. These measures should include constructing an earthen berm or placing silt fence around the perimeter of the stockpile to prevent contamination of the surrounding surfaces. The Contractor shall utilize separate stockpiles for contaminated and clean soil storage. Stockpiles of suspected contaminated materials shall be completely covered with the appropriate plastic at all times and secured accordingly. Parameters and specifications for testing will be dictated by the disposal facility.
For estimation purposes, the Contractor is not required to assume any contamination in their base bid proposal. When any suspect contamination is identified, the Contractor shall notify the Contracting Officer immediately.

SPILLS
Any type of spill shall be reported to the Contracting Officer and Tinker Fire Department. Any spill, regardless of type, that threatens or enters the sewer system should be reported immediately to the Contracting Officer and Tinker Fire Department. These spills can be categorized in two (2) groups: hazardous chemicals and other materials (e.g., paint, tar). The other materials, as well as the hazardous materials, may have potential negative impact to the environment, as well, specifically to the storm water system.
GEOTECHNICAL ENGINEERING DATA
1. All aspects of the geotechnical design for this facility shall be in accordance with SWD-AEIM Chapter 13 and Paragraph 16 of TAFBFS.
2. The DB Contractor shall provide subsurface investigations and a geotechnical report for all new foundations and pavements. The DB Contractor shall take soil samples, make laboratory tests and write geological reports which include an evaluation of test results and recommendations for foundations. The DB Contractor shall determine the amount and types of sampling and testing required for the foundations and below grade items the DB Contractor intends to use. The Contractor’s foundation design shall be based on the recommendations contained in the geotechnical report. Geotechnical site investigation shall be conducted in accordance with UFC 3-260-02, Chapter 5. DB Contractor shall required to perform plate bearing test as required within the UFC in order to determine the design “k” modulus of subgrade reaction.
3. The subsurface sampling and testing shall be performed by a firm equipped, qualified and licensed to perform such services.
4. All geotechnical reports, investigations, specifications, and calculations shall be performed, checked and stamped by a registered Professional Engineer in the State of the project site. The geotechnical report shall be submitted to the Contracting Officer with the other design submittals.
2.4 SCHEMATIC AND REFERENCE DRAWINGS
2.4.1 Existing drawings do not exist for Building 935, existing ATCT.
The concept Drawings and schematics shown for the new ATCT are to be used only as a guide for the new design. The DB Contractor is solely responsible for the design of these systems and structures and shall take full responsibility for any design and the associated construction costs based on the use of these schematics.

2.4.2 Reference drawings of existing site, systems, structures and conditions are provided. The information on these drawings may or may not reflect exact conditions at the current site and should not be considered “as-built”.
2.5 FIELD MEASUREMENT
2.5.0 The DB Contractor shall become familiar with all details of the work, verify all dimensions in the field and shall advise the Contracting Officer of any discrepancy before work begins. This pertains to all site conditions and any information presented in the project documentation.
2.6 GENERAL RFP TERMINOLOGY
2.6.1 DB Contractor: This is all work completed under this RFP by this
Contractor.
2.7 PERMITS
2.7.1 The Contractor is responsible for determining the applicability and obtaining all necessary Federal, State, county, local permits and fees required to complete the construction of this project.
The Contractor is responsible for paying all fees associated with obtaining the permits. All required permits will have to be coordinated with the Base Civil Engineering office and the Oklahoma Department of Environmental Quality (DEQ) offices.
2.7.2 The acquisition of permits and Base waivers shall be included in the construction schedule submitted and shall be the complete responsibility of the Contractor with no time extension given for delays due to permits.
2.7.3 The permitting process may include, but not be limited to, the submission of the following types of permits: stormwater management; water pollution; floodplain management; discharge permit; solid and hazardous waste; hazardous material tracking;
clearing; digging; traffic and closures; air quality; dust control; air emissions; drinking water; fuel tanks; historic, cultural, and archaeological preservation; noise abatement;
endangered species; airport hazard/airfield safety clearances;
generator; temporary airfield construction waiver; FAA Form 7460-

2.7.4 The DB Contractor shall be responsible for the preparation of the environmental and construction permit applications listed below.
The DB Contractor shall prepare the listed permits, providing all information and supporting documentation in a form ready for

Should the permitting agency require additional clarification or information during the permit review process, the DB Contractor shall provide all necessary assistance to resolve the outstanding issue.
2.7.5 Base Air Quality Permit: The DB Contractor will provide the information required for the permit to the Contracting Officer.
2.7.6 Notice of Intent (NOI):The DB Contractor will provide the information required for the permit to the Contracting Officer.
2.7.7 DB Contractor is responsible for all permits and coordination as required for the project. The table below shows approximate lead times and POCs for permits on Tinker AFB.

3. GEOTECHNICAL AND FOUNDATIONS
3.1 INTRODUCTION
Geotechnical requirements for the ATCT Facility and associated site features located at Tinker Air Force Base include, but are not limited to, the preparation of the foundation design, foundation design analysis, pavement design, pavement design analysis, construction specifications, and all pertinent subsurface investigation, foundation design, and pavement design drawings for the project including plans, sections, and details. The Contractor is fully responsible for acceptable non-airfield pavement designs and building foundation design.
The Contractor shall submit the names, addresses and qualifications of the geotechnical firms, drilling firms, and material testing laboratories intended for subcontracting, for approval by the Contracting Officer for review and approval in coordination with the USACE Tulsa District Geotechnical Engineering Section. The materials testing laboratory and the personnel performing testing, shall meet all applicable requirements of ASTM C1077, ASTM D3740, and ASTM E329. The materials testing laboratory shall possess a certificate of accreditation by a nationally recognized authority described in ASTM C1077, ASTM D3740, and ASTM E329. After contract award, validation of the materials testing laboratory by the USACE Materials Testing Center (MTC) will be required for the soils testing being performed (http://www.wes.army.mil/SL/MTC/mtc.htm). The Contractor is responsible for obtaining all permits and performing all correlation and calibration testing.
Approval of the aforementioned firms or laboratories does not release the Contractor from responsibility for preparation of an approved Foundation Design Analysis (FDA) and Pavement Design Analysis (PDA), nor should it be construed as approval of the FDA/PDA. In addition, approval of the aforementioned firms or laboratories does not release the Contractor from responsibility for the adequacy of geotechnical specifications, sections, details and all pertinent foundation and pavement design drawings, and related geotechnical portions of the project.
All required submittals and deliverables shall be submitted to the Contracting Officer for review and approval in coordination with the USACE Tulsa District Geotechnical Engineering Section. The Contractor’s Geotechnical Engineer shall prepare a final geotechnical evaluation report, to be submitted along with the first foundation design submittal
The Preliminary Geotechnical Engineering Report can be found in the Appendix.
3.2 EXISTING SUBSURFACE INFORMATION
Limited subsurface information for the general area of the project is included in Preliminary Geotechnical Investigation that is attached as an Appendix. The Contractor shall use the Government-supplied Preliminary Geotechnical Investigation as a basis for bid and for the preparation of their proposal. It shall be the Contractor’s responsibility to review, interpret, and evaluate the subsurface investigation data, including boring logs, field data, and laboratory test data. Subsequent to award, an
SECTION 01 10 10 Page 16 additional subsurface investigation and analysis will be required and shall be the responsibility of the Contractor.
The Contractor shall submit the proposed exploration plan to the Contracting Officer for review and approval in coordination with the USACE Tulsa District Geotechnical Engineering Section prior to mobilization of drill rig(s) and start of drilling. The Contractor will be required to drill and sample the proposed borings in accordance with EM 1110-1-1804 and Chapter XIII of the USACE SWD-AEIM. All subsurface investigation samples shall be performed in accordance with “Laboratory Soils Testing” (EM 1110-2-1906) dated 30 November 1970 (including Change 2 dated 20 Aug 86).
Geotechnical designs shall be based on technical criteria provided and soil data obtained by subsequent drilling or soil data provided herein; and shall be in accordance with Chapter XIII of the SWD-AEIM.
Drilling and laboratory testing for the Contractor's subsurface investigation shall be performed in accordance with EM 1110-2-1907. Reference Chapter XIII of the USACE SWD-AEIM for further requirements concerning geotechnical field investigations. The drilling and sampling techniques must conform to USACE criteria. Soil Laboratory tests shall be performed in accordance with "Laboratory Soils Testing" (EM 1110-2-1906), dated 30 November 1970. The Contractor is responsible for all applicable drilling and testing costs and acquisition of all clearances and permits from the Base Civil Engineering Office.
(A) The Contractor shall submit with the fee proposal, in writing, two copies of a proposed itemized exploration and testing program to the Contracting Officer for review and approval in coordination with the USACE Tulsa District Geotechnical Engineering Section. The submittal will consist of the backup data sheets and the quantity and cost summary sheets.
(B) The Contractor shall inform the Contracting Officer, in writing, at least 10 days prior to the start of any field investigations. This notification will include the starting date and location of the work and the names of the drilling and geotechnical firms. A site visit by a representative of the firm who prepares the geotechnical report is required.
(C) If field conditions discovered during drilling reveal that the submitted geotechnical investigation and testing program are inadequate or inappropriate, the Contractor will notify the Contracting Officer immediately, followed by a letter, recommending changes, additions, or deletions to the agreed-to program and time schedule.
(D) The Contractor is also responsible for the protection of all underground utilities from damage by the field investigations, and for coordination of drilling with regards to airfield safety requirements.
The Contractor is responsible for all applicable clearances and permits from the Base Civil Engineering Office and for the protection of all underground utilities from damage by the field investigations.

If fill, waste, or contaminated materials are encountered during drilling, the USACE Tulsa District Geotechnical Engineering Section and the Contracting Officer shall be notified immediately.
3.3 TECHNICAL CRITERIA
The geotechnical specifications, sections, details, and pertinent foundation and pavement design drawings, and related geotechnical portions of the project shall comply with, but not be limited to, the following referenced material and criteria. Note: the latest editions in effect as of the date of this Request for Proposal are to be used unless noted otherwise.
