SCOPE OF WORK
HEYBURN LAKE
RELIEF WELL/TOE DRAIN CONNECTIONS
1. General. The Contractor shall perform all engineering services, including all personnel, transportation, equipment, and materials required to connect the relief wells to the toe drain system at Heyburn Dam, Kellyville, Oklahoma.
2. Scope of Services. All work is subject to the availability of funding. The Contractor shall connect individual PVC relief wells to the 12-inch CMP toe drain collector pipe. Each installation will include an excavation approximately 12 feet deep requiring a protective shoring box, the installation of a 6-inch PVC lateral between the well and collector, compacted backfill, and final grading. A representative of the Corps of Engineers Dam Safety Section will be on site to monitor and advise the Contractor on aspects of this scope. A minimum of 2 and a maximum of 9 installations will be required.
3. Character and Extent of Service.
a. Excavation. A trench type excavation approximately 12 feet in depth will be required for each installation. The excavation will expose sufficient CMP pipe and relief well riser to complete the required connection. The average distance between the collector pipe and well is approximately 10 feet. An approved protective shoring box will be required. Inadequate space is available to “lay-back” the excavation. Materials to be excavated will include silts, clay, and sand. No rock excavation will be required.
b. Connection. Upon completion of the required excavation, an approximate 6-inch diameter hole shall be cut in the CMP as shown on the drawings to accept a CMP saddle Tee. Tee shall be secured to the pipe with metal connecting strap bands, by sufficient metal screws, or approved method suggested by the Contractor. The PVC relief well riser shall be cut off as shown to receive a 6-inch PVC Tee. To assure gravity flow, the flow line for the well Tee shall be at least 6 inches higher in elevation than the hole cut in the CMP. 6-inch PVC pipe and required fittings shall be used to make the lateral to the CMP collector. A 6-inch Fernco rubber coupling shall be used to make the connection between the PVC lateral and CMP saddle Tee. The average distance of the required lateral between the collector pipe and well is approximately 10 feet.
c. Backfill. Sand shall be placed under and around all PVC lateral pipe and approximately 6 inches of sand shall cover the PVC. Sand shall be compacted by flooding, plate vibrator, or by approved method in the field. Random excavated materials shall be placed in 9-inch lifts and compacted by 2 complete passes of a wacker tamper or approved method. Final grade of the backfill shall conform to the existing surrounding ground.
4. Time of Completion. The Contractor shall complete all work covered by this scope by 30 September 2008.
5. Access to Worksite. Access to the installation sites shall be from the access road along the downstream toe of Heyburn Dam.
6. Work Hours. All work shall be performed during normal work hours, 0745 to 1630, Monday through Friday, or as approved by the Contracting Officer.
7. Project Manager. The Government project coordinator for the administration of this contract will be Mr.
James P. Granwehr, Dam Safety Section, telephone (918)669-7452. All questions regarding the work to be accomplished under this scope will be directed to Mr. Granwehr.
8. Payment. Payment for all work accomplished under this scope of work shall be made at the unit price per installation. A minimum of 2 and a maximum of 9 installations are covered within this scope, subject to the availability of funds.

(2) The Contractor shall not change the name or address for EFT payments or manual payments, as appropriate, in the CCR record to reflect an assignee for the purpose of assignment of claims (see FAR Subpart 32.8, Assignment of Claims). Assignees shall be separately registered in the CCR database. Information provided to the Contractor's CCR record that indicates payments, including those made by EFT, to an ultimate recipient other than that Contractor will be considered to be incorrect information within the meaning of the “Suspension of payment” paragraph of the EFT clause of this contract.
