The mission of the 103rd Airlift Wing is to train, organize, and equip personnel so they can deploy and employ in support of national objectives as directed, support the governor’s office, aid in emergencies, and participate in programs benefiting the community.

B. TANK INSPECTION COMMENTS

1. Tank Construction

Tank 1 is a welded, steel, single-wall, vertical aboveground storage tank (AST). The tank was constructed in 1993 by Witherup Fabrication and Erection IAW API 650, Appendix H, 8th Edition. The tank is 32’-9” in diameter and 20’-9” high with a nominal capacity of 2,500 BBL. The maximum fill height is 17’-0” and is restricted to the bottom of the shell overflow ports. The previous NDT records from the previous inspection and the original construction drawings were available for review and comparison. The existing nameplate for Tank 1 specifies the purchaser’s tank number as Tank 2.

2. Tank Foundation

Tank 1 rests on a 1’-6” tall concrete ring wall. Four (4) leak detection tell-tale pipes are equally spaced around the foundation. Eight (8) anchor chairs are equally spaced around the tank perimeter. Various locations of the tank chime were cut to accommodate the anchor bolts and the bottom plate projection past the tank shell does not meet the minimum requirement of 3/8”, which is specified by API 650 / 653.

It is evident that these areas are coated and from initial fabrication and do not show indications of active corrosion. Sealant is not provided at the tank chime-to-ringwall interface, anchor bolt penetrations, or grounding cable conduit penetrations to prevent water ingress beneath the tank bottom. Water in direct contact with the tank bottom can lead to accelerated corrosion and as a result, lower the service life of the tank.

3. Cathodic Protection (CP)

The tank bottom is protected by an ICCP system. CP readings taken during the inspection indicate the CP system is functioning and adequately protecting the tank bottom.

4. Tank Bottom

The tank bottom is of cone-down construction with butt-welded steel radial plates and a 29” diameter, 18” deep center sump. NDT was performed on the tank bottom including visual testing (VT), ultrasonic thickness testing (UTT), and a magnetic flux leakage (MFL) scan. Five (5) UTT readings were taken on each radial plate and center plate. One (1) UTT reading was taken every 3 feet around the tank perimeter within the tank bottom critical zone. The MFL scan and UTT prove-up identified two (2) indications of soil-side corrosion with a minimum measured thickness of 0.187” and 0.213”. Each indication had a service life greater than 20 years; therefore, no repairs are required at this time. VT was performed along the bottom plate welds and center sump welds. One (1) 3” linear indication of lack of fusion was identified in the shell-to-bottom weld. Two (2) pin-holes were identified in one (1) water draw-off and one (1) low suction pipe support bearing plate welds. VT examination was limited due to the internal coating. An internal settlement survey was performed in the tank and no indications of excessive settlement were observed. The internal receipt diffuser is welded directly to the tank bottom.

Refer to Drawing A-1, “Bottom Layout” and Table 1, “Tank Bottom Ultrasonic Thickness Readings” in

Appendix A for plate locations and thickness data.

5. Internal Floating Roof

The tank is provided with an aluminum honeycomb internal floating roof (IFR). The IFR was originally equipped with foam log primary and secondary seals which were replaced with a double wiper seal system as part of this inspection. The IFR was examined using VT methods. No gross defects nor damages were observed during the inspection. The IFR rests on six (6) 2 ½” aluminum support legs. Each support leg is provided with a stainless steel landing pad sealed to the tank bottom. Manway 2 is equipped with an aluminum filler drum to allow the IFR to move up and down freely past the nozzle neck. The tank is currently not equipped with an internal ladder to allow access to the IFR while the tank is in service.

6. Tank Shell

The tank shell is equipped with three (3) butt-welded A-36 modified steel shell courses. Shell plates and welds were examined using UTT and VT methods. Nine (9) UTT measurements were taken on the lower shell course and nine (9) UTT measurements were taken on the upper shell courses adjacent to the spiral staircase. UTT readings indicated no significant metal loss on the shell plates.

Refer to Drawing A-2, “Shell Roll-Out” and Table 5, “Shell Ultrasonic Thickness Readings” in Appendix A for plate location and thickness data.

7. Fixed Roof

The self-supported fixed roof is of cone-up construction with fully coated lap-welded steel plates. Fixed roof plates were examined using UTT and VT methods. Five (5) UTT measurements were taken on each full plate and four (4) UTT measurements were taken on each sketch plate. UTT readings indicated no significant metal loss on the fixed roof plates. The circulation vent adjacent to the spiral staircase currently has a vertical spacing of 6 feet 6 inches to the leading edge of the spiral stair tread. Occupation

Safety and Health Administration (OSHA) requires a minimum clearance of 7 feet as specified in 29 Code of Federal Regulations (CFR) 1910, Subpart D, 1910.23.10.

Refer to Drawing A-3, “Fixed Roof Layout” and Table 6, “Fixed Roof Ultrasonic Thickness Readings” in

Appendix A for plate location and thickness data

8. Tank Appurtenances

a. Tank Shell

Tank shell nozzles consist of two (2) 36” manways, one (1) 6” product receipt, one (1) 12” main suction, one (1) 3” low suction, one (1) 3” water draw-off, two (2) 1” aqueous film forming foam

(AFFF) sensors, one (1) ¾” thermometer port (removed dial gauge), two (2) 1” HLA/HHLA penetrations, two (2) 1” LLA/LLLA penetrations, two (2) 1” HLCV shutoff switch penetrations, and one (1) 8” AFFF chamber. One (1) UTT measurement was taken on each accessible nozzle with a welded connection and reinforcement pad. The main suction reinforcement pad is welded

directly to an anchor chair. Weld spacing between the lower weld toe of Manway 1 reinforcement pad and the tank chime does not meet minimum spacing requirements of 3” as specified by API 650. The HLCV tubing steel attachment plate located adjacent to Manway 1 reinforcement pad does not meet the minimum weld spacing of 6” as required by API 650.

Manway 1 and Manway 2 are not provided with davit arms to allow for easy removal during cleaning and inspection activities. Shell manways are currently provided with carbon steel bolts which are heavily coated and exhibit corrosion.

Refer to Drawing A-2, “Shell Roll-Out” and Table 7, “Nozzles and Appurtenances” in Appendix A for nozzle location and data.

b. Internal Floating Roof

The IFR is equipped with one (1) 30” manway, one (1) 24” tape level gauge float well, and one

(1) 30” manway with a vacuum breaker. The tank is currently only equipped with one (1) anti-rotation cable. A grounding strap is currently not provided at the 30” manway.

c. Fixed Roof

Fixed roof nozzles consists of one (1) 12” Varec servo probe, one (1) 24” hatch with the Barton

ATG, one (1) 2’-0” x 6’-0” IFR installation hatch, one (1) 24” center vent, one (1) 2” tape level gauge penetration, and one (1) 24” inspection hatch. One (1) UTT measurement was taken on each nozzle with a welded connection and reinforcement pad. The center vent screens are damaged with large gaps present. The Varec servo probe is equipped with a partially slotted stilling well. A fully slotted stilling well is not provided for the Barton ATG. The Barton ATG probe is routed through a funnel penetration in the IFR.

Refer to Drawing A-3, “Fixed Roof Layout” and Table 7, “Nozzles and Appurtenances” in

Appendix A for nozzle location and data.

d. Gauging

Automatic tank gauging is performed using the Barton ATG. The tank is also equipped with a

Shand & Jurs tape level gauge and Varec Servo probe for automatic tank gauging; however, these are inoperable per facility personnel. Manual gauging and sampling operations are not possible due to the ATG probes and cables being located in the manual gauging hatch nozzle.

e. Level Alarms

The tank is provided with LLLA, LLA, HLA, and HHLA. The level alarm settings are 10 3/8”, 2’ –

7 1/16”, 14’ – 7 15/16”, and 16’ – 4 5/8” respectively.

f. Stairs and Platforms

The tank is provided with a spiral staircase, work platform at Manway 2, and upper platform at the fixed roof. The clearance between Manway 2 and the handrail is 1’-4” and does not allow easy passage to the fixed roof or removal of the manway cover. An approach step of adequate size is not provided to allow easy access to the spiral staircase. There is currently no access point for the HLCV shutoff switch and HLA.

g. Grounding and Bonding

The tank is equipped with four (4) grounding lugs and cables that are in good overall condition with no signs of damage or detachment. Grounding cable conduit penetrations are not provided with sealant. The spiral staircase is not equipped with a personnel grounding station at the stair entry point.

9. Tank Coating

All exterior and internal surfaces of the tank are coated and were tested using a DFT gauge. The internal surface of the tank bottom exhibits widespread blistering with approximately 15 ft2 of coating failure and spot base metal exposure adjacent to the Varec servo probe stilling well. The coating at the center sump and adjacent plates exhibits widespread blistering and poor application. The internal shell coating is in moderate condition with indications of corrosion bleed through on the upper shell courses. The internal fixed roof is fully coated with spot locations of flaking and general surface corrosion on the associated roof rafters. The external tank shell is fully coated and exhibits widespread deterioration and surface corrosion bleed through. The external fixed roof coating is in moderate condition with widespread deterioration, corrosion bleed through, and bio-growth.

Refer to Table 8, “Dry Film (Coating) Thickness Measurements”, for internal and external coating thickness measurements.

10. Tank Piping System

The external piping consists of 6” stainless steel for the receipt, 8” stainless steel for the main suction, and 3” stainless steel for the low suction. A ball valve is provided on the low suction at the tank shell. One

(1) U-bolt support within the containment dike is welded directly to the receipt piping for Tank 1.

External piping is supported by a mixture of adjustable and fixed wall mounted supports. One (1) fixed wall mounted support is not supporting the receipt piping. Rubber pads at each of the supports are damaged or missing and allowing metal-to-metal contact. The external and internal stainless steel piping is welded directly to the tank shell. Internal piping consists of stainless steel piping provided with fixed supports which are not adequately supporting the internal piping. Internal pipe supports are provided with square corner bearing plates. Internal flanges are not provided on the main suction, low suction, and receipt internal piping at the tank shell. The internal receipt diffuser is equipped with four (4) supports, which are welded directly to the tank bottom and diffuser pipe. The internal piping for the main suction currently extends to the center of the tank and terminates directly over the center sump.

Double-block and bleed (DB&B) valves are provided on the main suction and receipt piping at the tank shell and the low suction is equipped with a ball valve. Facility personnel state that the DB&B valves get stuck during the hotter months making operations difficult. The main suction and receipt piping are equipped with thermal relief valves (TRVs) at their respective skin valves. Two (2) TRVs are rated for vapor service and the associated thermal relief piping is threaded and equipped with sight flow indicators.

11. Fire Protection System

The tank is equipped with two (2) sensors for the AFFF system and one (1) AFFF chamber. A fully contact aluminum IFR also serves as an interior fire protection system for Jet-A fuel. The AFFF piping adjacent to the upper shell courses exhibits coating failure with widespread flaking.

12. Secondary Containment

Tank 1 is located within a 63’-0” x 63’-0” x 4’-0” concrete containment dike. Approximately 550 linear feet of sealant at the concrete dike floor control joints are missing or compromised and filled with sand and debris. Dike wall piping penetrations are equipped with link seals. The secondary containment dike drain was hydrostatically tested during the inspection. The drain was filled three (3) different times, and each time the liquid level deviated approximately 1 ½” over 30 minutes. Facility personnel indicate that the containment drain valve does not fully seal, allowing the dike to drain freely into the oil-water separator.

C. FINDINGS AND RECOMMENDATIONS

The following subsections outline the itemized tank repairs/upgrades. Repairs and upgrades shall be completed

D. INSPECTION INTERVALS

Intervals between tank inspections (both internal and external) should be determined by the tank service history, corrosion rates, and regulatory requirements. UFC 3-460-03 provides maximum internal inspection intervals for military installations. UFC guidelines should be used since they are more stringent. Listed below are the recommended inspection intervals:

1. Routine In-service Inspection

Facility personnel shall perform a visual inspection of the tank exterior surfaces and associated piping.

Gross defects, leaks, shell distortions, signs of settlement, and corrosion should all be monitored along with the foundation condition (including tank chime-to-foundation interface), coating, and appurtenances.

Interval: Monthly

2. External (In-Service) Inspection

An API certified inspector shall perform this inspection every five (5) years or RCA/4N years (where RCA is the difference between the measured shell thickness and the minimum required thickness and N is the shell corrosion rate) whichever is less. This inspection includes inspection requirements of the routine in-service inspection as well as a visual inspection of the tank grounding system (shunts and cables) for damage.

Interval: Five (5) years; 25 August 2021 (API)

3. Ultrasonic Thickness Inspection

An external UTT inspection of the tank shell and roof (if applicable) shall be performed by an API certified inspector every fifteen (15) years or RCA/4N, whichever is less. UT inspections may be performed in conjunction with the external inspection.

Interval: Fifteen (15) years; 25 August 2031 (API)

4. Internal (Out-of-Service) Inspection

An API certified inspector shall perform the internal inspection every 20 years or ½ remaining life of the tank bottom, whichever is less. This inspection includes a visual inspection of the tank interior surfaces and associated piping and an evaluation of the tank bottom using NDT methods. Since UFC requirements are more stringent, the internal inspection shall be performed every ten (10) years.

Interval: Ten (10) years; 25 August 2026 (UFC)

Twenty (20) years; 25 August 2036 (API)

Staten: New UFC 3-460-01 will do away with 10 year requirement.
