1 Introduction.
While the Sample Project is similar to previous project levee rehabilitation work, it is not a real project and is used for illustrative purposes only. A Cost Plus Fixed Fee Task Order will be utilized to perform the work described in this sample scope of work. The customer is USACE Omaha District.
2 Scope of Work
This Scope of Work describes Task Order requirements for the repairs to the levee at Missouri River Levee Unit R-123 between Station 0+00 and Station 50+50 in Atchison County, Iowa. The contractor shall construct a setback levee to include seepage berms, both on landside and riverside, in order to mitigate damages to the existing levee from flooding. The contractor shall mobilize within 7 calendar days after NTP and all repair work must be completed within 120 calendar days after issuance of NTP.
See Enclosure 1 for site and cross section plans.
The Contractor shall provide all necessary plant, equipment, operators, and laborers for levee construction operations in accordance with applicable federal, state, and local requirements. The Contractor shall perform work in a manner to prevent damages. Certified cost or pricing data is not required for the sample problem. DO NOT provide hard/firm quotes from subcontractors and vendors.
The contractor shall develop the technical approach and associated cost proposal per the following Work Breakdown Structure (WBS):

3 Assumptions
The contractor shall use the following assumptions in developing a technical approach, schedule, and proposal:
1. No work is required to remove the existing levee.

2. No materials will be provided from the existing levee.
3. All personnel and equipment are outside local commuting areas.
4. Adverse weather will not impede progress of work throughout duration of Task Order.
5. Borrow source material is readily available at grade. Further, quality and quantity of borrow material meets specification requirements.
6. Construction shall be performed during daylight hours, seven (7) days per week, including holidays.
7. A depth of 6 inches is required for vegetative stripping.
8. Davis-Bacon Wage Decisions in Enclosure 2 shall be utilized in the cost proposal.
9. The following per diem rates shall be utilized in the cost proposal:
Lodging (per day): $96.00 Meals & Incidentals (per day): $55.00
4 Borrow Areas
Cohesive, Random and Sand Fill Material shall be obtained from within Government owned lands and are available at provided staging area. Please see the referenced Enclosure 1 for locations. The location of material, and approximate distance to the borrow areas are shown in the table below.

5.1 Fill Materials Specifications and Testing
The following fill materials and placement requirement shall be adhered to.
• Cohesive Fill Material: Cohesive Fill Material shall include soil classified as CL and CH per
ASTM D 2487. Testing for classifying materials shall be in accordance with ASTM D 4318 and ASTM D 6913. Classification testing shall be performed initially, and every 20,000 CY of

Cohesive Fill Material placed. Cohesive Fill Material shall be non-frozen, free of particles (rocks) greater than 2 inches in diameter, placed in loose horizontal lifts not to exceed eight inches in thickness, and compacted to a minimum of 95% of the materials maximum dry density as determined in accordance with ASTM D 698 – Standard Proctor. Moisture content shall be between -2% and +3% of optimum moisture. Standard Proctor tests shall be performed initially and for each material change noted during classification testing. (Submittal Required – Soil Test Results). Provided material in fill areas meet these requirements.
• In place density testing shall be conducted once for every 500 cubic yards of Cohesive Fill
Material placed in accordance with ASTM D 1556 or ASTM D 6938. Determination of water content shall be done in accordance with ASTM D 2216. When ASTM D 6938 is used, check the calibration curves and adjust using only the sand cone method as described in ASTM D 1556 at a frequency of at least one sand cone per 50 nuclear density tests. Check the calibration curves furnished with the moisture gauges along with density calibration checks as described in ASTM D 6938. Check the calibration of both the density and moisture gauges at the beginning of a job on each different type of material encountered and at intervals as directed by the Contracting Officer. Submit calibration results for all Nuclear Gauges proposed for use on the site showing acceptable calibration within 12 months of the start of construction. (Submittal Required – Nuclear Gauge Calibrations).
• Random Fill Material: Random Fill Material shall include material / soil classified as SC, SM, and ML per ASTM D 2487. Testing for classifying materials shall be in accordance with ASTM D 4318 and ASTM D 6913. Classification testing shall be performed initially, and every 20,000 CY of Random Fill Material placed. • Sand Fill Material: Sand Fill Material shall include material classified as SW and SP per ASTM
D 2487. Testing for classifying materials shall be in accordance with ASTM D 6913.
Classification testing shall be performed initially, and every 20,000 CY of Sand Fill Material placed. Sand Fill Material shall be non-frozen, free of particles (rocks) greater than 2 inches in diameter, placed in loose horizontal lifts not to exceed eight inches in thickness in a near saturated condition, and compacted. Provided material in fill areas meet these requirements.

• In order to determine the appropriate number of passes to achieve adequate compaction of Sand Fill Material, a test section shall be analyzed in the field. The test section shall be a minimum of 65 feet long by 30 feet wide. Two passes of the compaction equipment shall be made. A dry density of the compacted sand shall be obtained and recorded. An additional pass of the compaction equipment shall be made. The dry density of the compacted sand shall again be obtained and recorded. This procedure shall be repeated until a maximum dry density of the compacted sand is determined. The Corps of Engineers Geotechnical Representative shall review the test results in order to determine the required number of passes in order to achieve adequate compaction. (Submittal Required – Proposed Compaction Equipment).
Construction QC Testing: The COR and Engineering Division Representative will scrutinize the reference Standard Proctor test results for placement. The contractor’s Quality Control personnel must be competent in oversight of soil testing and in visual identification of basic soil types.
Standard Proctor moisture-density shall be performed on borrow material every 10,000 CY or when a significant material change occurs.
In-place density and moisture content tests for production work shall be performed once for every 500 CY of material in a random pattern with regard to footprint and lifts. The location of each test shall be documented along with the reference Proctor test being used, and whether the test passes or fails based on density and/or moisture content. Areas that fail for density shall be reworked not less than one-half the distance to the nearest passing location and re-tested to verify that the density meets specification. Areas that fail for moisture content only shall be brought to the attention of the on-site COR or the Engineering Division Representative for evaluation prior to adding additional lifts. Based on their direction the contractor may be required to rework the area and re-test.
6 Surveying Requirements.
Surveying is required within levee footprint area and beyond as described in subsequent sections of this SOW. Where required, the following surveying principles shall be adhered to.
Pre and post (as-built) construction surveys. Using the Surveying Requirements outlined below, pre and post construction surveys shall be performed. The pre-construction survey shall be an accumulation of information showing the existing conditions in the field, a topographic map. The post construction survey shall be an accumulation of information showing the as constructed conditions in the field, a topographic map.
a. Pre-Construction Topographic Surveys. Pre-construction topographic survey locations required to support levee construction are 200 feet from both the landward and riverside toe or to the temporary construction easement. Additional surveying may be required in borrow areas once they are identified. (Submittal Required - Pre Construction Topographic Surveys).
b. Post Construction Topographic Surveys (As-Built). Post construction topographic surveys of the levee and landside seepage berms are required. These surveys are also required within borrow areas, including the existing levee location. (Submittal Required - Post Construction Topographic Surveys).

c. Topographic and Utility Mapping. Topographic and utility mapping surveys require a scale of 1” = 30’ with 1-foot contour intervals. Elevations shall be to the nearest 0.01’ for paved areas and 0.1’ for unpaved areas. Density of collected data shall support a scale of 1” = 30’. Mapping shall be accomplished employing appropriate GPS mapping procedures, conventional level routines and electronic total station. All mapping shall conform to current National Mapping Accuracy Standards (NMAS) and applicable CADD standards.
d. Survey Limits. Survey limits are described within, all features shall be mapped within the survey limits. Where practical, features shall be mapped to a logical end point outside of the survey limit to ensure accurate feature type, flow direction, etc.
e. Annotation and Symbology. Include general annotation such as street names, building numbers, feature descriptions, and surface types. Map features shall be shown by the appropriate symbology.
f. Planimetric Features. All surface features within the survey limits shall be shown and identified on mapping. The phrase "planimetric features" is intended to include, but is not necessarily limited to, the following
i. Utilities. Locations of all utilities, above and below ground, within the limits of the survey area shall be shown on the final mapping. It is necessary to show where utilities enter and exit the boundary of the survey limits. Locate and identify all above ground utilities, indicating size, type, material, and direction. Locate all underground utilities, indicating size, type, material, and direction. Collect invert elevation, to include bottom elevations, to the nearest 0.01’. Information for underground utilities that cannot be field located shall be obtained from the most current facility mapping and as-built drawings. The following types of utilities may be encountered.
o Electrical, communication / telephone, water, gas, storm lines, sanitation lines, industrial waste handling features, fuel/petroleum lines, etc.
o For power / telephone / light / camera poles collect all pertinent information including Pole Identification Number, number and type of lines/cables attached to each pole, direction of lines, number of cameras, lights, and transformers attached to each pole.
o For manholes, catch basins, area inlets, curb inlets, pits, vaults, pipes, and other similar features collect top (rim) elevation, bottom elevation, size, direction, material type, and invert elevations.
o For electrical / communication manholes / vaults collect top (rim) and bottom elevations, list number of cables and direction of vault entry / exit.
ii. Roads, Drives, Parking Lots, Walks, and Railroads. All roads shall have elevations at appropriate intervals along centerlines and road edges. When present, gather the top-back of curb and toe-front of gutter elevations. Features requiring labels are back of curb and name of roadway.
iii. Buildings and Structures. All permanent buildings and structures shall be identified and have adjacent ground elevations using the minimum number of points defining the structure's footprint. The footprint shall be a closed line string. Elevations shall be 3D line strings to be used as break lines when developing the DTM for contour processing.

Contours shall not extend into building footprints. Collect finished floor elevations for all permanent buildings.
iv. Fences. All fences, walls and other similar obstructions shall be shown. Indicate type and height of all fences. Use appropriate symbology for type of fence. Collect and indicate all gates within fence.
v. Vegetation. Collect data on individual trees, indicating size (diameter at breast height), drip line, and common name. Shoot individual bushes, indicating drip line and common name. Heavily vegetated areas may be collected by outlining the vegetated area, using appropriate symbology.
vi. Ditches. Show top of ditch (bank) and flow line of ditch.
vii. Other Visible Surface Features. Locate storage tanks, radio antennas or other surface structures visible and located within the area to be mapped. Indication of below ground storage tanks shall also be collected.
viii. Survey Monuments. Points shall be shown on mapping with proper symbology.
Designations, coordinates and descriptions shall be shown by text adjacent to the symbols.
g. Topographic Features. All surface features within the survey limits shall be shown and identified on mapping. Ground points, break lines and/or form lines resulting from DTM processing shall accurately represent the three-dimensional surface of the project area. The collected data shall conform to the most recently issued NMAS.
i. DTM Points. The xyz coordinates of all topographically significant data points shall be recorded. This shall include all high and low points, saddle and slope breakpoints and other points which influence the shape of the mapped surface.
ii. Break Lines. These are non-visible form lines, natural breaks or changes in grade of the terrain such as shelves, depressions, ditches, embankments, road centerlines and other slope break lines. These shall be collected as three-dimensional line strings and are required elements in the MicroStation V8 processing of the DTM surface model and contour generation.
iii. Form Lines. These are visible grade change lines of planimetric features such as drainage centerlines, shorelines, curb lines, pavement edges and other features of surface definition. These lines are required elements in the MicroStation V8 processing of the DTM surface model and contour generation.
iv. Spot Elevations. All spot elevations supporting visual interpretation of terrain elevation shall be provided and shown on final maps. The elevation value shall be displayed as text adjacent to an X placed on the spot elevation position or as text with the decimal point centered over the elevation position. The labeling technique shall be noted in the legend.
Ground elevations on existing breaks or changes in grade shall be indicated, such as tops of hills, bottoms of ditches, saddles and embankment elevations, and any high or low elevations necessary for interpretation. All ground elevations shall be shown to the nearest 0.1’. Mapped features shall be snap-able and shall show proper elevations and coordinates.

• Project Datum and Control. All construction benchmarks established shall be 4’ rebar with cap.
Point identification shall be stamped on the cap. A minimum of two benchmarks shall be established for each repair area location. All benchmarks shall be located in an area that will not be disturbed during construction activities.
a. Horizontal. Provide the appropriate NAD83 state plane zone, U.S. survey feet. The state plane system, zone, and FIPS code shall be clearly indicated on the spreadsheet.
b. Vertical. NAVD88, U.S. Feet, and NGVD29, U.S. Feet. Since NGVD29 is no longer supported by NGS, the NGVD29 elevations can be obtained by conversion through a program such as Corpscon.
c. Data Accuracy. All horizontal and vertical information shall be collected to conform to current NMAS. Horizontal position shall be reported to 0.01’. Elevations shall be reported to 0.01’.
d. Data Reporting. Benchmark data shall be reported in spreadsheet format (Microsoft Excel).
Information on existing control used to establish site-specific benchmarks, if applicable, shall be provided to USACE.
• Deliverables. Topographic mapping products shall be delivered as MicroStation V8 files consisting of design files relevant to production of DTM surface and 1’ contour data. The following shall also be provided.
a. Point file of all data points collected.
b. Excel spreadsheet for all Survey Control Monuments established.
c. Field sketches for all Survey Control Monuments in pdf format.
d. Digital photos of Survey Control Monuments.
e. Field book scanned and provided in pdf format.
7 Access and Haul Routes
The Contractor shall construct haul routes within the project boundaries in a manner to accommodate the equipment utilized to perform the work. Other access routes may also be feasible, provided USACE approval. Prior to the start of construction, the Contractor shall provide all proposed access routes and means/methods of construction that includes equipment type on each access route for approval. The Contractor shall adhere to any and all load restrictions by coordinating with the appropriate State, county, and city officials.
The levee crest and landside berms may be potential haul routes as shown on the drawings but should be avoided where possible. At commencement of construction activities, the Contractor shall place aggregate material as required to minimize damage to the levee crest. Place material at the levee crest such to accommodate equipment necessary to perform the work without damage to the levee. If the levee or berm is degraded due to construction activities, Contractor shall restore the levee to preconstruction conditions.

The Contractor will be responsible for costs associated with any temporary access and haul route improvements.
Contractor shall restore all access routes to the pre-construction condition unless otherwise directed/approved by the CoR. Generally, all routes other than on the levee crest will require removal of all aggregate materials and placement at a stockpile area as designated by the COR.
Access Through Private Property
The Contractor shall coordinate with private entities for access through private property.
Public and Private Road Maintenance and Repair
The contractor shall preserve and protect all existing private access roads, haul roads, and/or right-of-way roads. At completion of the work and prior to the contractor leaving the project/work site, the contractor shall restore to pre-project conditions. Restoration/repair efforts may include replacement of base rock and/or surface treatment as required. All public and private roads utilized to perform work that remain open to the public shall be maintained in a reasonably clean state free of debris and excessive mud and/or soil as directed by the COR.
8 Levee Construction.
Levee construction will be performed at Missouri River Levee Unit R-123 between Station 0+00 and Station 50+50.
• Alignment of Levees. The alignments of the levees are shown on the attached Enclosure 1 – Levee
Alignment.
• Levee Configuration. See cross section and plan views in Enclosure 1.
Prior to levee and landside seepage berm construction, surface vegetation and topsoil shall be stripped from within the footprint of the setback levees and landside seepage berms.
Landside seepage berms shall be constructed on the landside of the setback levee and riverside seepage berms shall be constructed on the riverside of the setback levee.
The landside seepage berms shall be 65 feet wide, measured from the landside toe of the levee to the shoulder of the berms, a minimum of 4.25 feet thick at toe of the levee and 2 feet thick at the shoulder of the berms. Sand Fill Material shall be used to construct the landside seepage berms.
The riverside seepage berm shall be constructed to match Enclosure 1 – Cross Section. Low Pervious Material shall be used to construct the riverside seepage berms.
Prior to demobilization, the Contractor shall verify that finished grades are as indicated on contract drawings, and placing of topsoil, smooth grading, and compaction requirements have been completed per specification requirements. Any deviations from specification requirements shall be corrected prior to demobilization.

Note: Under Executive Order (EO) 13658, an hourly minimum wage of $10.80 for calendar year 2020 applies to all contracts subject to the Davis-Bacon Act for which the contract is awarded (and any solicitation was issued) on or after January 1, 2015. If this contract is covered by the EO, the contractor must pay all workers in any classification listed on this wage determination at least $10.80 per hour (or the applicable wage rate listed on this wage determination, if it is higher) for all hours spent performing on the contract in calendar year 2020. If this contract is covered by the EO and a classification considered necessary for performance of work on the contract does not appear on this wage determination, the contractor must pay workers in that classification at least the wage rate determined through the conformance process set forth in 29 CFR 5.5(a)(1)(ii) (or the EO minimum wage rate,if it is higher than the conformed wage rate). The EO minimum wage rate will be adjusted annually. The body of each wage determination lists the classification and wage rates that have been found to be prevailing for the cited type(s) of construction in the area covered by the wage determination. The classifications are listed in alphabetical order of ""identifiers"" that indicate whether the particular rate is a union rate (current union negotiated rate for local), a survey rate (weighted average rate) or a union average rate (weighted union average rate).
Union Rate Identifiers
A four letter classification abbreviation identifier enclosed in dotted lines beginning with characters other than ""SU"" or ""UAVG"" denotes that the union classification and rate were prevailing for that classification in the survey. Example:
PLUM0198-005 07/01/2014. PLUM is an abbreviation identifier of the union which prevailed in the survey for this classification, which in this example would be Plumbers. 0198 indicates the local union number or district council number where applicable, i.e., Plumbers Local 0198. The next number, 005 in the example, is an internal number used in processing the wage determination. 07/01/2014 is the effective date of the most current negotiated rate, which in this example is July 1, 2014.
Union prevailing wage rates are updated to reflect all rate changes in the collective bargaining agreement (CBA) governing this classification and rate.
Survey Rate Identifiers
Classifications listed under the ""SU"" identifier indicate that no one rate prevailed for this classification in the survey and the published rate is derived by computing a weighted average rate based on all the rates reported in the survey for that classification. As this weighted average rate includes all rates reported in the survey, it may include both union and non-union rates. Example: SULA2012-007 5/13/2014. SU indicates the rates are survey rates based on a weighted average calculation of rates and are not majority rates. LA indicates the State of Louisiana. 2012 is the year of survey on which these classifications and rates are based. The next number, 007 in the example, is an internal number used in producing the wage determination. 5/13/2014 indicates the survey completion date for the classifications and rates under that identifier.

A UAVG rate will be updated once a year, usually in January of each year, to reflect a weighted average of the current negotiated/CBA rate of the union locals from which the rate is based.
WAGE DETERMINATION APPEALS PROCESS
1.) Has there been an initial decision in the matter? This can be:

* a survey underlying a wage determination
* a Wage and Hour Division letter setting forth a position on a wage determination matter
* a conformance (additional classification and rate) ruling
On survey related matters, initial contact, including requests for summaries of surveys, should be with the Wage and Hour Regional Office for the area in which the survey was conducted because those Regional Offices have responsibility for the Davis-Bacon survey program. If the response from this initial contact is not satisfactory, then the process described in 2.)
and 3.) should be followed.
